Haemodynamic response to magnesium administration in mineralocorticoid-salt and two-kidney, one clip renovascular hypertension.

Recent interest has centred on the role of divalent cations in hypertension, particularly in relation to the renin-angiotensin system. This study was undertaken to determine the hypotensive effect of magnesium administration in relation to the state of activation of the renin-angiotensin system. The mean blood pressure (MBP) and heart rate (HR) response to either the acute intravenous administration of a pharmacological dose of MgSO4 or vehicle was determined in conscious mineralocorticoid-salt (DOCA-salt, low-renin) and two-kidney, one clip renovascular, high-renin hypertensive rats. Baseline MBP was higher in the renovascular than in the DOCA-salt rats, while there was no difference in HR or serum Mg concentration between the two. Following administration of MgSO4, serum Mg increased equally in both the DOCA-salt (1.4 +/- 0.8 to 4.9 +/- 0.16 mEq/l; P less than 0.001) and in the renovascular rats (1.8 +/- 0.14 to 4.4 +/- 0.27 mEq/l; P less than 0.001). Magnesium administration significantly lowered MBP over the 1-h infusion in the DOCA-salt (167 +/- 8 to 145 +/- 5 mmHg, P less than 0.001) but not the renovascular hypertensive rats (191 +/- 5 to 183 +/- 4, NS). We conclude that the blood pressure lowering effect of Mg is related, in part, to the state of activation of the renin-angiotensin system. The mechanism of this differential effect remains to be determined.

[Use of a fenestrated aneurysm clip for transposition of the tortuous vertebral artery].

A 69-year-old female was admitted to our clinic on August 19, 1985 with a 5 year history of intermittent left facial pain. She had undergone various types of medical treatment and multiple procedures of nerve blocking which resulted in only a temporary relief of pain. On admission, the intermittent facial pain and hypesthesia were recognized in the 2nd and 3rd divisions of the left trigeminal nerve. CT scan with contrast enhancement showed a shift of the basilar artery to the left side. An elongated and tortuous vertebral artery was recognized at the left cerebello-pontine angle both by CT scan and vertebral angiograms. This tortuous vertebral artery compressed the trigeminal nerve entry zone causing neuralgia. Vascular decompression was carried out by inserting a muscle piece sandwiched in the lyodura between the artery and brain stem. Facial pain disappeared but recurred nine months after this operation. At the 2nd operation, the vertebral artery was passed through the fenestrated aneurysm clip and fixed on the petrous bone. With this procedure the vertebral artery was successfully transposed and the patient was discharged without any episodes of facial pain.

A tubal plug and clip method for female sterilization.

Animal studies using rabbits, stumptailed macaques monkeys, and baboons demonstrate that the tubal plug and clip device is an effective and safe method for female sterilization in these animal models. The devices were placed in 18 baboons that were bred regularly for six to 18 months without conception. Ten of these 18 animals conceived within 12 months after removal of the devices and carried normal pregnancies without any other surgical procedure. Six of the ten animals conceived and carried a second pregnancy for a total of 16 successful pregnancies. The expected pregnancy rate for baboons is 64.9% per year so that the 55.6% success rate of reversal and 16 total pregnancies clearly represents a high degree of reversibility for the method in this animal species.

[The value of Adams and de Weese's technic of cava clipping].

Indications and methods of inferior vena cava interruption in the management of recurrent pulmonary embolism are discussed. The results of a collected series are analyzed with special regard to the partial interruption technique using the Adams and de Weese clip. Five of our own cases, followed up 1 year postoperatively, are presented.

Aneurysms in a branch of the renal artery in the hilus: obliteration by means of surgical clips.

An aneurysm in a branch of the renal artery within the hilus of a 44 year old woman was successfully obliterated without affecting the circulation in the parenchyma. The surgical treatment consisted of applying Scoville clips designed for intracranial aneurysms to the neck of the sac of the aneurysm. Postoperatively the patient has remained free of her previous hypertension for over four years.

[Thrombo-embolic disease and pregnancy. The prevention of pulmonary emboli by placing a clip on the inferior vena cava (author's transl)].

The authors, having had a case of deep pelvic phlebitis occurring in a young primiparous woman who was treated prophylactically against the occurrence of a pulmonary embolus by placing a clip on the inferior vena cava discuss: the etiology of the condition: the difficulties of the diagnosis. They point out the value of phlebo-cavography which is an essential investigation in which no more than 4 films are taken, because it is on this that the indication for surgery rests. They point out the value of anticoagulant treatment, especially that using Calciparine which permits the patient to lead a more or less normal life at home. As far as the surgical procedure is concerned, a median approach seems to them the best way to carry out the operation.

Regression of left ventricular hypertrophy in two-kidney, one clip Goldblatt hypertension.

The effect of regression of left ventricular hypertrophy (LVH) on ventricular performance was studied in two-kidney, one clip Goldblatt hypertensive rats (2K1C) treated with methyldopa or by unclipping. Sham operations were performed in a total of 21 rats; 12 and nine were studied after 4 and 6 weeks, respectively. Hypertension was induced in 38 additional rats. Of these, 11 were studied at 4 weeks. Cardiac index was measured by electromagnetic flowmetry under light ether anesthesia, and ventricular performance was assessed by rapid intravenous saline infusion. Of the remaining 27 hypertensive rats at 4 weeks postclipping, 10 were treated with methyldopa (400 mg/kg/day) and nine were unclipped; eight were left untreated as controls. Two weeks thereafter, ventricular performance was determined as described above. When expressed as the relationship between cardiac index and LV end-diastolic pressure, ventricular performance tended to be depressed in 2K1C. Ventricular performance, mean arterial pressure, and LV-to-body weight ratio returned to control in unclipped rats. Whereas methyldopa resulted in regressed LVH, its effect on mean arterial pressure and total peripheral resistance was not as marked as unclipping, both remaining significantly increased (p less than 0.001). The disparate effects of unclipping and methyldopa on systemic hemodynamics indicate that the improved ventricular performance with methyldopa was related more to its effect on LVH, suggesting that in these animals regression of LVH was associated with improved ventricular performance.

Suppression of the sympathetic nerve activity after surgical reversal of two-kidney, one clip hypertension in rats.

The change in tonic renal efferent sympathetic nerve activity (RSNA) was studied during the rapid fall in mean arterial pressure (MAP) after surgical reversal of two-kidney, one clip hypertension in rats (RHR). Controls used were intact RHR exposed to the same fall in MAP induced by slow infusion of sodium nitroprusside. Within 2 h after declipping, MAP had fallen from 191 +/- 9 mmHg to 106 +/- 8 mmHg. During this decline in pressure, the reflex increase in RSNA, ordinarily following a decrease in pressure, was clearly suppressed; after a short and transient initial increase, RSNA was below the control level. In comparison, upon vascular dilatation with sodium-nitroprusside, the same fall in MAP was accompanied by a 117 +/- 30% increase in RSNA (P less than 0.01). Thus, the rapid normalization of MAP after reversal of renovascular hypertension involves a mechanism that exerts a suppressive action on the efferent sympathetic activity.

[Comparative results of the use of Greenfields' filter, an umbrella filter, and a caval clip for vena cava interruption].

Results of interruption of the vena cava for thromboembolic complications are reported. Greenfields filter was used in 40 cases, its introduction appearing to be a more delicate procedure than when other materials (caval clip in 40 cases and Mobin-Uddin's filter in 40 cases) were used. Results with regard to recurrence of emboli were comparable for the different materials, but Greenfield's filter becomes thrombosed less often and provokes fewer major sequelae.

[Postpartum sterilization with the Filshie titanium silicon rubber clip].

This provisional report on 101 postpartum patients indicates that Filshie clip application to the fallopian tubes is a satisfactory method for postpartum sterilization. Particular advantages are speed and the minimal damage of tissue, with possibly the best present potential for reversal. When compared with the Vienna method of tube sterilization, however, the extra cost militates against its general application. In selective cases, such as patients with two children who request sterilization, it appears to be the best method available today.

[Clipping for prophylactic partial occlusion of the inferior vena cava].

The partial occlusion of the inferior vena cava in the prevention of fatal pulmonary embolism has been used for years in different ways. Over a period of one, and a half year, we put 11 clips of Miles, once for prophylaxis and 10 times after pulmonary embolism. Two patients died during the first 15 postoperative days. Seven patients had a phlebologic control by a Doppler ultrasound, venous plethysmography, isotopic cavography, and grey scale ultrasound. It has been proved that the inferior vena cava was permeable in 6 of the 7 controlled patients. The indications as well as the late results of these operations are discussed.

Computed tomography in the evaluation of patients with surgical clips.

Computed body tomography has proved to be a useful imaging modality in the follow-up study of patients with known malignant conditions undergoing therapy. While technologic improvements have made the number of artifacts few in number, a significant problem still exists from the streak artifacts emanating from surgical clips which degrade the image.

The results of intracapsular cataract extraction with a Binkhorst iris clip lens implant 34 to 40 months after surgery.

A series of 300 consecutive intracapsular cataract extractions with a Binkhorst iris clip lens implant was evaluated 34 to 40 months after surgery, except for those patients who died before this time (30 patients) and those lost to follow-up (13 patients). Eighty-eight percent of the 257 patients achieved 20/40 or better visual acuity. The results were similar to those series previously reported by the authors where the final examination time for each patient was not uniform.

Impairment of endothelium-dependent vasorelaxation in chronic two-kidney, one clip hypertensive rats.

The present study was to investigate a role for endothelium-derived nitric oxide (EDNO) system in the development and maintenance of 2-kidney, 1 clip (2K1C) hypertension. Effects of blocking the synthesis or supplementing the precursor of EDNO on the developmental phase of hypertension were examined in 2K1C rats. Responses of the isolated vasculature to phenylephrine, acetylcholine, sodium nitroprusside, and atrial natriuretic peptide were also examined in chronic 2K1C rats. Ingestion of NG-nitro-L-arginine methyl ester or L-arginine did not affect the development of hypertension in 2K1C rats. Contraction response to phenylephrine was enhanced and relaxation response to acetylcholine was attenuated in the thoracic aortic ring isolated from chronic hypertensive rats, both being more marked in the 12-week hypertensive than in the 7-week hypertensive. Indomethacin did not significantly affect the degree of the attenuated vasorelaxation response to acetylcholine. The vasorelaxation response to sodium nitroprusside and atrial natriuretic peptide remained unaltered in the hypertensives. These results indicate that EDNO does not affect the developmental phase of 2K1C hypertension, whereas an impaired endothelium-dependent vasorelaxation is associated with chronic 2K1C hypertension.

Altered microvascular responses to C5a in rat striated muscle during two-kidney, one clip renovascular hypertension.

OBJECTIVE: To determine how two-kidney, one clip (2-K,1C) renovascular hypertension alters microvascular responses in rat striated muscle to complement C5a, one of the most important inflammatory mediators. METHODS: 2-K,1C hypertension was induced in male Sprague-Dawley rats. Under anesthesia with pentobarbital (50 mg/kg, intraperitoneally) the cremaster muscle microcirculatory preparation with intact neurovascular connections was studied in vivo by closed-circuit videomicroscopy. Recombinant human C5a was applied topically in the tissue bath at concentrations of 10(-12), 10(-10) and 10(-8) mol/l, consecutively. Changes in the microvessel diameters in small arterioles, large arterioles and venules were measured. RESULTS: In normotensive rats complement C5a induces a significant dilation in small arterioles at low bath concentrations (10(-12) or 10(-10) mol/l), but the dilation is attenuated at a higher concentration (10(-8) mol/l). In contrast, in 2-K,1C hypertensive rats C5a constricts small arterioles at low concentrations (< 10(-10) mol/l) but dilates them at a higher concentration (10(-8) mol/l). Large arterioles and venules have minimal responses to C5a in either normotensive or 2-K,1C hypertensive rats. CONCLUSION: 2-K,1C hypertension dramatically alters C5a-induced microvascular responses in small arterioles. The alteration might be attributable to the enhanced vasoconstrictor mechanisms and impaired vasodilator mechanisms during 2-K,1C renovascular hypertension.

[Neck clipping of an aneurysm at the basilar artery-anterior inferior cerebellar artery via presigmoid-transmastoideal approach].

A 51-year-old man presented severe disturbance of consciousness and massive subarachnoid hemorrhage. Angiography disclosed an aneurysm at the ventral bifurcation of the basilar and anterior inferior cerebellar arteries. Neck clipping and additional wrapping/coating were performed via the presigmoid-transmastoideal approach with successful result. The operative procedure was described and advantages of this approach were discussed.
