Foot-ground pressure pattern of standing hemiplegic patients. Major characteristics and patterns of improvement.

The study was designed to examine major weight-bearing characteristics of hemiplegic patients receiving physical therapy and to assess the usefulness of the Foot Print apparatus as an objective, clinical measurement tool of these characteristics. Measurements of the foot-ground pressure pattern of 23 patients were taken two times, three weeks apart. The results confirmed the clinical impression of lesser weight bearing by the patient on the affected leg, especially on the heel of that leg, compared with weight bearing on the contralateral leg. Consequently, stance stability was impaired. The difference between the foot-ground pressure of the two measurements indicated improvement in the stance characteristics of these patients after treatment. The Foot Print apparatus seems to be an objective measurement tool of assistance both to the patient and the physical therapist in execution of a treatment.

Stimulus pattern and tolerance development. Exposure of rats to alternating light/darkness patterns in vivo and pilocarpine sensitivity in vitro of the iris sphincter.

Pigmented rats were kept under different light/darkness patterns for 5-12 days. The response of the iris sphincter in enucleated eyes was tested with pilocarpine as a standard stimulus. The eyes were either decorneated or intact. The maximum response (no subsensitivity) was obtained after continuous darkness and the minimum (maximum subsensitivity) after continuous light. Six hours of light and one block of 18 hours of darkness per day caused distinct subsensitivity, but less than if the light was interrupted by six 3-hour darkness periods. Further subdivision of the light into twelve 30-min. periods made no difference. One 18-hour block of light followed by 6 hours of darkness caused less subsensitivity than continuous light. Subdividing the darkness into six 1-hour periods made no significant difference.

Pattern of psychological fitness as related to pattern of physical fitness among older adults.

The purpose of the present study was to investigate psychological fitness as well as physical fitness among late middle-aged normal men and women before, during, and after a 12-wk. physical activity program. Heart-rate of each subject was measured on a graded treadmill four times, and four psychological measures were taken: self-esteem, body-image, trait-anxiety, and well-being. Improvement of fitness was gradual and significant for both sexes, whereas no effects of time or sex were found on the first three psychological measures. Significant differences in pattern of change between women and men on well-being confirm previous findings that women report more depression and less satisfaction than men and thus are more sensitive to change.

Contribution of pattern electroretinogram and pattern visual evoked potential in early diagnosis of primary open angle glaucoma.

Pattern electroretinogram (PERG) combined with visual evoked potential (VEP) has become increasingly useful in clinical assessment of the anterior visual pathways. In regard to the PERG dependence on ganglion cell activity, we studied monocular PERG in 36 patients (71 eyes) with intraocular hypertension (6 eyes under topical beta-blockers) and in 10 patients (18 eyes) with advanced glaucoma, to estimate the potential effect of increased IOP on ganglion cells. We recorded simultaneous VEP to evaluate the retino-cortical conduction time. Referring to normal values obtained from 19 eyes (12 subjects), we found clear-cut PERG abnormalities in 20 eyes among ocular hypertensive patients with increased IOP. They were more altered in the advanced glaucomatous group. Two or three control exams were performed in 10 patients every 6 months: however such a short-time follow-up seems too limited to define the ERG prognosis value in this pathology. Comparing the PERG with the clinical routine assessments, we discuss its place in the diagnosis and in the follow-up of patients with ocular hypertension.

[Distribution pattern of chemical in surface waters as a reflexion of entry pattern--nonvolatile halogenated hydrocarbons].

The concentrations of some nonvolatile halogenated hydrocarbons in surface waters of an industrialized city and its surroundings are given. About 80% of the ballast isomers of hexachlorocyclohexane (alpha, beta and delta BHC) and of DDT and its metabolites are in the range of less than 5 micrograms l-1, whereas ca. 50% of lindane (gamma BHC) were found to be between 5 and 100 micrograms l-1. Starting form the remarks about the differential-diagnostic assessment of environmental concentrations conclusions as to the origin of hexachlorocyclohexane isomers are drawn with the help of their distribution pattern. A high part of the gamma isomer, compared with the ballast isomers means an application (e.g. against mosquitos). This fact may be supported if it coincides with the months typical for such an application. in contrast to this if you have high percentages of alpha, beta and delta hexachlorocyclohexane, it indicates a point source or diffuse inputs of technical BHC. If all four isomers are present on a comparable and very low level this may reflect a remobilization from the sediment.

Regional scalp differences of the androgenic metabolic pattern in subjects affected by male pattern baldness.

Regional differences in the androgen metabolism were established in alopecic and non alopecic areas of patients affected by male pattern baldness (MPB). 5-alpha-reductase (5-alpha-R) activity was measured by the formation of dihydrotestosterone (DHT), using 3H-testosterone as substrate: this activity was higher in the alopecic areas (3.4 pmol/g tissue/h) than in the non alopecic skin (1.5 pmol/g tissue/h). 3-alpha,beta-hydroxysteroid oxoreductase (3-alpha, beta-HO) was studied using 3H-DHT as precursor and measuring the corresponding formed 3-alpha- and 3-beta-androstanediols (alpha DIOL and beta DIOL). The beta DIOL was the predominant metabolite and total 3-alpha, beta-HO activity was higher in alopecic skin (12.4 pmol/g tissue/h) than in non alopecic areas (8.4 pmol/g tissue/h). Also 17, beta-hydroxysteroid oxoreductase was measured using either testosterone or DHT as substrates: androstenedione formed from testosterone was higher in hairy skin (12 pmol/g tissue/h) than in alopecic areas (6 pmol/g tissue/h); androstanedione formed from DHT was also higher in non alopecic areas (8.1 pmol/g tissue/h) than in alopecic skin (2.8 pmol/g tissue/h). The greater formation of beta DIOL in the sebaceous glands-enriched alopecic skin supports the hypothesis for a specific role of this metabolite in the control of the sebaceous activity.

[Prevalence of a type A behavior pattern (coronary prone behavior pattern) in the Vienna population].

The prevalence of the coronary-prone behavior pattern and its interaction with psychosocial and primary coronary risk factors was studied in a Viennese population sample consisting of 1459 people (634 men, 825 women). The sample was representative with respect to sex, age greater than 16 years, socio-economic status and local distribution. A questionnaire-based short rating scale developed by Bortner was used. The distribution of the study population on the A-B axis was unimodal (means = 174.95 +/- 40.76). The upper and the lower third of the A-B dimension was designated to represent type A1 and type B4, respectively. The mean Bortner score and rate of type A1 persons clearly decreased at lower (16 to 20 years) and higher ages (greater than 60 years). Within the group of employed subjects aged 21 to 60 years the mean type A score and the rate of A1 subjects for men (177 +/- 39; 6%) differed significantly from that of their female counterparts (188 +/- 39; 13%). Housewives had a mean score of 178 +/- 41 and 10% of them were scored A1, thus indicating significantly lower mean scores than employed females but the same rate of A1. Subjects with higher educational level showed only a tendency to increased type A. The socio-economic status was positively correlated to the global Bortner score, especially in men, but no relation was found to the rate of Type A1 persons. There was no significant correlation between type A score and systolic or diastolic blood pressure, body mass index or smoking behavior, but a significant correlation between type A score and the subjective feeling of overload. Type A subjects scored themselves significantly more often as continuously overload.

[Analysis of mortality patterns in an area supplying tunnel workers away from home. Part 2. Mortality patterns among tunnel workers].

In order to clarify the long-term effect of tunnel work on the health of its workers, we examined its cause-specific mortality pattern. The subjects were 418 males who died after the age of 30 in 1977 in the southern part of Oita Prefecture, where many men come and work away from home as tunnel workers. Death certificates, 418 in number, presented to the health center were used to examine the causes of death. Occupational careers and life histories of the subjects were obtained by interviewing their families or relatives in 1980. The interviews were successfully carried out with 356 subjects (85.2%). The subjects were divided into two groups. One group was consisted of 73 males who had engaged in the tunnel work (tunnel workers) and the other was of 283 males without experience of tunnel work (others). Proportional mortality ratio (PMR) and proportional mortality classified by age were compared between the two groups. The mean ages at death were also compared among these two groups and all Japanese males over 30 years of age. The results were as follows. The proportion of the deaths of "tunnel workers" among the total deaths in the area surveyed was correlated to the regional rate of tunnel workers and the distribution of pneumoconiosis due to tunnel work. These results show that many tunnel workers suffering from pneumoconiosis had returned to their home area before they died. Excess deaths from the ICD category I; infective and parasitic diseases (000-136), tuberculosis (010-019) and silicotuberculosis (010) were found among the "tunnel workers."(ABSTRACT TRUNCATED AT 250 WORDS)

Regeneration and pattern formation in planarians. I. The pattern of mitosis in anterior and posterior regeneration in Dugesia (G) tigrina, and a new proposal for blastema formation.

Mitotic activity during regeneration in the planarian Dugesia (G) tigrina shows a biphasic pattern, with a first maximum at 4-12 h, a second and higher maximum at 2-4 days, and a relative minimum in between. The first peak is mainly due to pre-existing G2 cells entering mitosis shortly after cutting, whereas the second maximum is due to cells that divide after going through the S period from the onset of regeneration. From a spatial point of view, the highest mitotic values are found in stump (postblastema) regions near the wound (0-300 micron), though regions far from it also show increased mitotic values but always lower overall values. As regeneration continues the postblastema maximum shifts slightly to more proximal regions. In contrast, no mitosis has been found within the blastema, even though the number of blastema cells increases steadily during regeneration. The results suggest that blastema in planarians forms through an early accumulation of undifferentiated cells from the stump to the base of blastema. The results obtained demonstrate that blastema formation in planarians occurs through mechanisms somewhat different to those shown to occur in the classical epimorphic models of regeneration (Annelida, Insecta, Amphibia), and suggest that planarian regeneration could represent an intermediate stage between morphallactic and epimorphic modalities of regeneration.

Prevalence, group pattern, T pattern and seasonal variation of beta haemolytic streptococci in and around Shimla.

One hundred and seventy two strains of beta haemolytic streptococci were isolated from various clinical specimens over a period of one year. Group A was isolated in 69.70% and group C in 16.80 percent. Majority of the strains were isolated during winter season and only 21.00 percent in summer 93.30 percent strains were bacitracin sensitive and all were sensitive to penicillin. The most common T pattern among group A was found to be 5/11/12/27/44 followed by 3/13/B 3264.

Joint exercises in elderly persons with symptomatic osteoarthritis of the hip or knee. Performance patterns, medical support patterns, and the relationship between exercising and medical care.

A total of 110 community-living elderly persons with chronic hip and/or knee pain, all of whom met American College of Rheumatology classification criteria for osteoarthritis (OA), were interviewed to obtain information about their performance of therapeutic joint exercises and exercise-related medical care. Although exercises are considered standard treatment for OA and 96% of subjects had seen a physician for the problem, fewer than half had received medical advice to exercise and very few had received thorough exercise-related teaching and monitoring. Arthritis specialists provided significantly more support for exercising than primary care physicians. Forty percent of the total sample and 63% of those advised by a physician to exercise were making some attempt to do so; however, only 10% were exercising in a manner that might be expected to achieve maximum therapeutic benefit. Supportive medical care was highly related to frequency of exercising.
