[Outpatient coronary groups. 1st experiences with high-risk infarct patients after a year's model experiment].

One year's experience with patients after myocardial infarction with low physical capacity (high risk-patients) and their participation in coronary clubs. After satisfactory results have been recorded with coronary training-groups at Hamburg first results can be reported from a trial with a coronary exercising-group at the patient's community. Patients after myocardial infarction with low physical capacity (high risk-patients) have participated regularly in coronary club meetings. One year's observation with medical controls showed no hazards from special adapted exercises combined with health education. Cardiac complications did not occur. The exercise program aims on increasing coordination and flexibility; general aerobic endurance is not to be improved. Thus a better and more economical functioning of the cardiovascular system is reached; life quality is improved. Further propagation of the coronary exercising-program is recommended if the described precautions are observed. This way many coronary patients could benefit also from long-time participation in comprehensive care in coronary clubs, which were up to now excluded from coronary training groups because of high risk.

[Laparoscopic colon surgery--from experiment to initial clinical experience].

The technique of Laparoscopic resection of the colon sigmoideum was practised in an experimental model before starting a clinical program of laparoscopic colonic surgery. Until now, two resections of the colon sigmoideum, one ileocecal resection, one right colectomy, one Hartmann's procedure and one transversostomy have been performed laparoscopically. Three patients had colonic carcinoma. One sigmoid resection had to be converted to open surgery and this patient underwent relaparotomy because of postoperative ileus. The postoperative course of the other patients was uncomplicated. The promising initial experience demonstrates that laparoscopic colonic surgery is feasible, although further studies have to prove advantages and indications, especially with respect to oncology.

[Goal attaining scale. Reliability and practical experiences in 397 psychiatric treatment courses. 2: Practical experiences with a simplified version of Goal Attainment Scaling in clinical practice and intermediate length rehabilitational treatment].

Goal attainment scaling (GAS) measures outcome dependent on the degree, to which patients attain, what was thought to be their potential. Can this method be simplified by giving up to detail in advance, what would be "much more" and "much less" than the expected outcome? In 339 psychiatric treatment courses of inpatients, GAS was compared with a traditional clining rating. The patients did least agree to goals, their therapists wrote down concerning "social contacts" and "coping". The simplified form of GAS turned out to be more vulnerable to wrong prognoses. The influence of the vocational experience of those, who constructed the scales was tested as well. Despite methodological shortcomings GAS has evident therapeutic qualities: how else can we evaluate, how much of their potential our patients realize?

New device for wire and catheter capturing: in vivo experiments and first clinical experience.

PURPOSE: A new self-expanding tulip sheath was tested for percutaneous wife capturing in a femoro-femoral pull-through maneuver. METHODS: In six sheep, a self-expanding tulip sheath was introduced into the right iliac artery. A 5 Fr catheter and an 0.035-inch guidewire were guided into the iliac artery from a left femoral approach. Thirty-six tests were performed to guide the guidewire out of the right groin using the tulip sheath, by catching the catheter first in 18 tests and just by catching the guidewire in the other 18. As a control, six tests were performed using a conventional sheath. We performed one clinical arterial application. RESULTS: There was prompt capture of the wire in 35 of 36 tests using a self-expanding sheath. Five of six tests using a conventional sheath failed. The clinical application of the tulip principle was successful. CONCLUSION: Using the self-expanding tulip sheath to facilitate wire capture during pull-through maneuvers, even in difficult situations, is promising.

[Experiments in vitro with Litomosoides carinii (Nematoda: Filarioidea) II. Influence of different sera on the release of microfilariae and experiments on the readiness for copulation of the worms (author's transl)].

When fetal calf serum or human serum is added to the culture medium Tc 199, adult females of Litomosoides carinii release the same amount of microfilariae in vitro as after addition of cotton rat serum. In contrast, in media with serum of Meriones unguiculatus or Mastomys natalensis only a few microfilariae are released; moreover, addition of Mastomys serum immobilizes them. When males and females are kept together, the same amount of microfilariae is released as compared with single females. Worms 4 weeks of age (i.e., about 2 weeks before copulation) maintained in medium Tc 199 with 33% cotton rat serum do not mate (no insemination of the females). However, the females live 18 days (harmonic mean) in vitro and grow on average 5 mm in length (28%) and the males roll up their tails in characteristic manner. Worms 5 or 6 weeks of age kept in pairs for 45 to 6 days in vitro were implanted into cotton rats. In vivo they grew to normal length, copulated and delivered microfilariae into the circulating blood. Females implanted solely into the recipient grew as well but remained sterile; this means that no insemination took place in vitro.
