The effect of splanchnic viscera removal upon canine liver regeneration.

The influence of portal blood factors on canine liver regeneration was studied with graded nonhepatic splanchnic evisceration, coupled with 44 and 72 per cent hepatectomies. In one type of experiment, the pancreas was retained while the rest of the intra-abdominal gastrointestinal tract was removed. In a second variety, total pancreatectomy was performed with preservation of the intra-abdominal organs. In a third kind of experiment, total nonhepatic splanchnic evisceration was performed. Liver regeneration after hepatectomy was decreased by all three kinds of viscera removed as judged by deoxyribonucleic acid synthesis, autoradiography and mitotic index. Pancreatectomy and nonpancreatic splanchnic evisceration caused almost equal decreases in the regenerative response. Total nonhepatic splanchnic evisceration essentially halted regeneration during the first three postoperative days and intraportal infusions of insulin or glucagon, or both together, did not reverse this effect. The decrease in liver membrane bound adenyl cyclase activity and biphasic change in liver cyclic 3', 5' -adenosine monophosphate concentrations normally seen after partial hepatectomy were disrupted after the various eviscerations. Adenyl cyclase activity and cyclic 3', 5' -adenosine monophosphate concentrations tended to be higher than normal in the eviscerated dogs. These observations provide more support for our previously proposed hypothesis that control of liver regeneration is by multiple factors. Pancreatic hormones are important modifiers of this response but, by no means, exercise exclusive control. Other substances of gastrointestinal origin, presumably including hormones and nutrient supply apparently play important specific roles. The volume of portal flow is a secondary and nonspecific, but possibly significant, factor.

The topography of the thoraco-abdominal viscera in the ostrich (Struthio camelus).

The topography of the thoraco-abdominal viscera in the ostrich was studied in 20 birds varying in age from 2 weeks to 12 months. The lungs occupied the dorsal third of the thorax, and the heart lay in the cranioventral thorax perpendicular to the long axis of the body. There was no pleural cavity. The liver was situated in the caudoventral part of the thorax, and the proventriculus occupied the left cranial part of the abdomen between the 7th vertebral rib and the acetabulum. The gizzard lay in the cranioventral part of the abdomen, resting on the sternum and abdominal floor. The duodenum formed a loop from right to left, with the pancreas lying between the 2 limbs of the loop. The coiled jejunum and ileum occupied the ventral part of the abdomen between the gizzard and pelvis. The two caeca lay on either side of the terminal ileum with their apices in the pelvis. The rectum was the longest part of the intestine and could be divided into a thick proximal segment situated in the right dorsal part of the abdomen, and a thin distal part that occupied the left caudodorsal part of the abdomen. The trilobed kidneys lay along the ventral surface of the synsacrum, with the adrenal glands at their cranioventral poles. The testes lay ventrally to the cranial divisions of the kidneys, whereas the left ovary was situated ventrally to the cranial division of the left kidney. The spleen lay wedged in between the right kidney, caudal vena cava and proventriculus. The thyroid glands were situated at the cranial borders of the subclavian arteries, and the thymus lay at the base of the neck.

Aflatoxin and encephalopathy with fatty degeneration of viscera (Reye).

In the past 5 years we have observed 27 children at the age from 3 days to 8 years who died on the encephalic syndrome with fatty degeneration of the viscera (Reye). According to the morphological changes in the liver and to the clinical course, we have divided our cases into 3 groups. In the first group there were 20 children who died within 2-10 days after the first symptoms of the disease appeared. In their liver diffuse fatty degeneration was found. In the second group there were 3 children who died within 1-2 months after the acute onset of the disease. In their liver fibrosis with bile duct proliferation and steatosis were found. In the third group there were 4 children who died within 2-4 months after the first symptoms. Their liver showed cirrhosis. The bacteriological and virological investigations in all cases were negative, but in the most of them direct contacts with viral infections were proved. In the liver specimens of the children in all 3 groups the presence of aflatoxin B1 chromatographically and spectrophotometrically was found. The source of the intoxication in 5 cases was aflatoxin contaminated milk food. Our observations of 2 - 3 - days old newborns with the morphological changes similar to that of Reye's syndrome seems to support our hypothesis about the possibility of the intrauterine intoxication. We suggest that aflatoxin represents an important factor in the etiology of this syndrome and a high risk for human health.

[Possibility of a method for dynamic study of abdominal viscera contractility from echographic images. 1st results].

Brilliance variations in selected areas of echographic display are recorded in real time thanks to photoconductive integrations from the screen. The areas concerned are: 1) gastroduodenal or colonic wall; 2) intraluminal medium in the same portion of the digestive tract. Simultaneously the pneumogram is recorded with these photometric variations in long period of time (1/2 h-2 h). Visceral displacements or medium echographic modulations give us informations about motricity and hydraulic changes. Several components of frequency are shown: pulse, respiratory interactions and several components of parietal contractions; we can notice the concordance between frequency variations obtained with others technics such as electrosplanchnography or manometry. This possibility of atraumatic confrontations with a visual control allows us new perspectives in physiopathological studies concerning hemodynamic, biomechanic interactions between viscera.

[Radical surgery for advanced bowel cancer with the involvement to adjacent viscera or distant metastasis--combined resection of involved organs].

Since 1962, 1422 patients with large bowel cancer have been treated surgically in our hospital and 373 patients among them have received the extended radical combined resection of the tumor and its invaded adjacent viscera. The result is much encouraging as follows: (table: see text) Even though there are still some problems in concerning to scope of resection for invaded organ, the extended radical operation must be taken actively for the advanced cancer of large bowel.

Ammonia and amino acid metabolism y the portal-vein-drained viscera in chronic renal insufficiency.

Arterial portal-venous differences (A-PV) of ammonia (NH3) and amino acids (AA) were measured in patients with chronic renal insufficiency (CRI) and in controls. In CRI, NH3 release by the portal-vein-drained viscera (PDV) is not different from controls, Gln uptake decreases, Glu extraction occurs, Pro and Thr output increases and Ser release becomes evident; one third of N released as NH3 and AA can be accounted for by Gln and Glu uptake. The N exchange across the PDV is similar in CRI and in controls.

[Study of the methods for detecting Salmonella in meat and viscera].

Studied were a toal 415 samples of viscera (musculature, liver, spleen, kidneys and a lymph node from skeletal musculature) taken from 83 emergency slaughtered pigs and cattle, and liver samples of 10 experimentally infected guinea pigs using direct immunofluorescent microscopy. All samples were preliminary kept in the tetrathionate-broth of Müller and Kaufmann, the broth for cultivation of Gramnegative bacteria, and in paraffin blocks, for 24 hours, in order to obtain more abundant growth of bacteria. The cultures were studied by the standard method of reseeding from liquid nutrient media at the 24th, 48th and 72nd hour. Highest per cent of correlation (89.4%) between the results of the immunofluorescence method and the cultural method was shown by enriching the growth in the tetrathionate-broth of Müller and Kaufmann. This suggests the use of immunofluorescence microscopy as a quick helpful method to detect Salmonella bacteria in the microbiologic inspection of meat.

[Clinical analysis of spontaneous perforation of abdominal viscera in patients with severe viral hepatitis].

Eight out of 246 patients with severe viral hepatitis were complicated with spontaneous abdominal viscera perforation. The eight patients all had serious digestive symptoms, orange green colour in skin, swollen abdomen with ascites, abdominal pain. All cases were diagnosed by liver function examination, colour ultrasonography, x-ray examination, abdominal puncture and one case by operation. Among these 8 cases, 3 cases had perfornation of stomach, 3 cases had perforation of gall bladder and 2 cases had perforation of small intestine. Though emergancy treatment had been applied, only one case was saved.
