Consortia arrangements and educational telecommunication.

Revolutionary and rapid changes are required in nursing programs and products to fit with the developing health care system. Many present-day nurses and nursing faculty will require changes in their areas of expertise. Nursing colleges that have faculty qualified in the areas of greatest need should explore the use of consortia for delivering distance education. In this way, quality can be controlled, and the conflict of interest involved in schools trying to change the preparation of their own faculty can be reduced or resolved. Organizing consortia is complex, with problem areas in administration, personnel, and resources. Selected television satellite networks are offered as examples of how to manage the problem areas or as possible vehicles for course delivery.

Telehealth: voice therapy using telecommunications technology.

Telehealth offers the potential to meet the needs of underserved populations in remote regions. The purpose of this study was a proof-of-concept to determine whether voice therapy can be delivered effectively remotely. Treatment outcomes were evaluated for a vocal rehabilitation protocol delivered under 2 conditions: with the patient and clinician interacting within the same room (conventional group) and with the patient and clinician in separate rooms, interacting in real time via a hard-wired video camera and monitor (video teleconference group). Seventy-two patients with voice disorders served as participants. Based on evaluation by otolaryngologists, 31 participants were diagnosed with vocal nodules, 29 were diagnosed with edema, 9 were diagnosed with unilateral vocal fold paralysis, and 3 presented with vocal hyperfunction with no laryngeal pathology. Fifty-one participants (71%) completed the vocal rehabilitation protocol. Outcome measures included perceptual judgments of voice quality, acoustic analyses of voice, patient satisfaction ratings, and fiber-optic laryngoscopy. There were no differences in outcome measures between the conventional group and the remote video teleconference group. Participants in both groups showed positive changes on all outcome measures after completing the vocal rehabilitation protocol. Reasons for participants discontinuing therapy prematurely provided support for the telehealth model of service delivery.

An integrated perinatal information system and telecommunications networking.

The integrated perinatal information system in Albany Medical Center has two significant features: that of a flexible user-definable database and ease of networking both locally and regionally. The unified perinatal record is structured in Ansi standard MUMPS and resides on the College Vax 750. A special set of program tools ensures effective multi-split screen display, moving pointers, and corresponding multiple choice entries. Because it is self contained, it has the ability to create complex data structures without programming. A powerful math/statistical library has been incorporated. In house, the use of existing PABX wire allows for simultaneous voice/data traffic. Linkage to the regional system is made by modem connection. The system is compatible with the demands of both clinical and regional management.

The impact of ergonomic intervention on individual health and corporate prosperity in a telecommunications environment.

This paper is a summary of several papers published in different journals and conference proceedings. The contents deal with the incidence of load related musculoskeletal illness of female workers exposed to various workloads. Furthermore, the changes in workload due to improved workplace design and the effect on the incidence of musculoskeletal illness was evaluated. The sick-leave due to musculoskeletal illness was reduced from 5.3% to 3.1% and the reduction in turn-over from 30.1% to 7.6%. The cost and the benefits of this ergonomic intervention were analysed, and showed that an investment of NOK. 350,000 produced total savings of NOK. 3,200,000. Postural load was assessed by recording electromyography (EMG) on the upper part of musculus trapezius, postural angles of the upper arm and flexion/extension of the head and back. A quantitative relationship was found between the static trapezius load and the development of musculoskeletal sick-leave, related to the length of employment. Indications were found that certain factors were associated with reduced incidence of musculoskeletal illness. These were: increasing the number and total duration of trapezius load below 1% to 2% MVC (Maximum Voluntary Contraction); reducing the magnitude of flexion angle of the upper arm in the sagittal plane and distributing the work between flexors and extensors; and a more dynamic work pattern of the upper arm. The relationship between postural load and musculoskeletal injury was studied in comparable groups of female workers with respect to age, working hours per day and time of employment. Psychosocial problems, spare time activities and living habits of the workers did not show any significant differences across the groups. The results from this study indicate that: the static trapezius load must be kept at a minimum; a median arm flexion should be less than 15 degrees and a median arm abduction less than 10 degrees. These values seem roughly to approach an acceptable arm position; and a forward median flexion of the back of less than 20 degrees seems not to lead to a higher rate of low back pain for workers with long periods of employment.

Medical telecommunications--fundamental changes in the art and science of medicine.

The availability of easy to use microcomputers and medical information retrieval systems will result in fundamental and important changes in medical practice. The acquisition of information literacy and fluency will be mandatory techniques for the future practice of medicine and should be taught at the institutional level by faculty sophisticated in the use of information retrieval systems. The resulting improvements in the practice of medicine go beyond simple cost savings to fundamentally alter the ways patients and their disorders are looked upon and handled.

A pilot trial of a telecommunications system in sleep apnea management.

BACKGROUND: Continuous positive airway pressure (CPAP) is an effective therapy for obstructive sleep apnea syndrome (OSAS), although many patients have difficulty adhering to this therapy. The purpose of this study was to investigate the effectiveness of totally automated telephone technology in improving adherence to prescribed CPAP therapy. RESEARCH DESIGN: This pilot study was a randomized clinical trial in 30 patients being started on CPAP therapy for OSAS. Patients were randomly assigned to use of a computer telephone system designed to improve CPAP adherence (telephone-linked communications for CPAP [TLC-CPAP]) in addition to usual care (n = 15) or to usual care alone (n = 15) for a period of 2 months. TLC-CPAP is a computer-based system that monitors patients' self-reported behavior and provides education and reinforcement through a structured dialogue. MEASURES: A sleep symptoms checklist and the Functional Outcomes of Sleep Questionnaire were administered at study entry and at 2-month follow up. Hours of CPAP use at effective mask pressure were measured by the CPAP device, stored in its memory, and retrieved at the 2-month visit. RESULTS: At 2 months, patients randomized to TLC-CPAP had fewer reported sleep-related symptoms (9.4 vs. 13.4, P = 0.047) than those receiving usual care. The average nightly CPAP use in the TLC-CPAP group was 4.4 hours compared with 2.9 hours (P = 0.076) in the usual-care group. CONCLUSIONS: This pilot study suggests that patients with OSAS started on CPAP and a concurrently administered automated education and counseling system had better CPAP adherence and better control of OSAS symptoms.
