Barbed needle and inexplicable paresthesias and trismus after dental regional anesthesia.

The tips of 100 needles that had been used clinically for the administration of mandibular block anesthesia (50 by students and 50 by staff) were examined individually under a dissecting microscope. Sixty percent of these needles were found to be barbed. With the use of an animal laboratory simulation, it has been shown that the tips of standard needles as used in general dental practice will barb if allowed to touch bone (medial aspect of the mandibular ramus) during the administration of a mandibular block for dentistry using the direct approach and that a relationship exists between the pattern of this barbing, the disposition of the bevel of the needle at the time of its insertion, and the likelihood of lingual or inferior dental nerve involvement on withdrawal of the barbed needle. Simple precautions are advanced for reducing the possibility of nerve or other tissue damage from this source.

Multifocal idiopathic fibrosis presenting as facial pain and trismus.

An unusual case of idiopathic sclerosing fibrosis is described in which there was involvement of the face, neck, mediastinum and lungs with sparing of the thyroid and retroperitoneal tissues. In contrast to previous cases, facial pain and limitation of jaw movements were the presenting complaints and major causes of disability. Prednisolone treatment failed to arrest the disease.

Comparison of nonsteroidal anti-inflammatory drugs, ibuprofen and flurbiprofen, with methylprednisolone and placebo for acute pain, swelling, and trismus.

Pain, swelling, loss of function, and hyperthermia are acute postoperative sequelae of inflammation due to tissue injury during surgical procedures. Pharmacologic strategies for minimizing the clinical manifestations of surgical trauma are often directed toward blocking the formation or inhibiting the effects of the biochemical mediators of acute inflammation. This study compared two nonsteroidal anti-inflammatory drugs (NSAIDs), flurbiprofen and ibuprofen, with a prototype glucocorticoid, methylprednisolone, in two replicate placebo-controlled studies for suppression of inflammation due to the surgical removal of impacted third molars. The results indicate that NSAIDs produce greater initial analgesia than do steroids, whereas steroids result in greater suppression of swelling and less loss of function. Examination of the pooled data from the two studies indicates that NSAID pretreatment results in a modest suppression of swelling in comparison with placebo. These data suggest that the acute analgesic effects of NSAIDs in the oral surgery model are due to suppression of a nociceptive process, presumably prostaglandin formation, rather than a generalized anti-inflammatory effect.

Trismus pseudocamptodactyly syndrome: Dutch-Kentucky syndrome.

A hand, foot, and mouth combination of anomalies, which is apparent on motion, has been traced through eight generations of a southern Appalachian family. The clinical features from birth to old age are presented, and attention is called to the handicapping aspects. There is a great variability in severity of the trait. Inheritance occurs as an autosomal dominant trait, but there are fewer affected persons than expected, especially females.

Fabrication of a sectional impression tray and sectional complete denture for a patient with microstomia and trismus: a clinical report.

This article describes techniques used to fabricate mandibular and maxillary sectional trays and a folding maxillary complete denture for a patient with limited oral opening caused by scleroderma. For the foldable denture, the anterior teeth had to be arranged on a second base and the hinge fitted at a location higher than the denture base. These 2 factors increased the thickness of the denture and limited the volume of the tongue. Nevertheless, a single-piece denture base provided the patient with ease in placement and removal of the denture.
