Nebulizer therapy with antibiotics in chronic suppurative lung disease.

Aerosolized antibiotics have been shown to be a useful modality of treatment in patients with cystic fibrosis. In this investigation we examined the utility of this treatment in patients with other chronic suppurative lung disorders. These included forty patients, thirty men and ten women with chronic airway infection (27 with bronchiectasis, 6 with chronic abscess and 7 with chronic suppurative bronchitis). Pathogenic organisms were isolated from the affected part of the lung by a fiberoptic bronchoscopy using a sterile disposable bronchial microbiology brush. Cultures from these specimens were used to determine the appropriate antibiotic. A second control group of 20 patients was treated with systemic antibiotics alone. Both systemic and aerosolized antibiotics were administered in 20 patients. A statistically significant improvement in clinical, and ventilatory functions was recorded in the first group compared to the second. Nebulized antibiotics used as adjunctive therapy in association with systemic antibiotics may offer a therapeutic advantage in chronic suppurative lung diseases.

Acute necrotizing pancreatitis and acute suppurative pancreatitis in the cat. A retrospective study of 40 cases (1976-1989).

Medical records and histologic sections of 40 cats with acute pancreatitis were reviewed. Two distinct groups of cats with pancreatitis were established by histologic analysis of tissue. Group 1 (32 cats) had acute pancreatic necrosis (APN). Group 2 (8 cats) had suppurative pancreatitis. Ages of affected cats ranged from 3 weeks to 16 years. The majority consisted of indoor cats of the Domestic Short-Haired breed but Siamese cats were over-represented relative to the general population (P < 0.05). Twenty-two percent of cats were obese and 57% were underweight. Thirty-eight percent of cats had acute disease. In the other cats, two stages in the progression of the disease were evident: (1) anorexia, weight loss, and lethargy, followed by (2) acute deterioration, development of shock, and a moribund state, despite fluid therapy. The most common clinical signs were severe lethargy (100%), reduced appetite (97%), dehydration (92%), and hypothermia (68%). The initial hemogram occasionally showed a neutrophilia (30%) and anemia (26%) but packed cell volume (PCV) decreased markedly to the extent that 55% of cats were anemic terminally. Serum biochemical abnormalities included increased activities of ALT (68%) and ALP (50%), and increased concentrations of bilirubin (64%) and cholesterol (64%). Cats with APN were hyperglycemic (64%), glycosuric (60%) and ketonuric (20%), whereas cats with suppurative pancreatitis tended to be hypoglycemic (75%). Renal failure and electrolyte abnormalities were mild or infrequent except for hypokalemia (56%). This study characterizes a severe necrotizing pancreatitis in the cat similar to that reported in other species, and a histologically distinct suppurative pancreatitis.

Etiological agent and primary source of infection in 42 cases of focal intracranial suppuration.

The microbiological findings for 42 patients with focal intracranial suppuration were analyzed and correlated with the different sources of primary infection. Streptococcus spp. were identified in focal intracranial suppuration of all origins except postcraniotomy. Microaerophilic streptococci were important in cases secondary to respiratory tract infection and in those of unknown origin. Streptococcus faecalis, Proteus spp., and Bacteroides fragilis were the organisms most commonly found in polymicrobial otogenic abscesses. Clostridium sp. was the main microorganism implicated in postcraniotomy suppurations.

Characteristic clinical features as an aid to the diagnosis of suppurative keratitis caused by filamentous fungi.

AIM: To assess whether the presence of characteristic clinical features can be used as a diagnostic aid for suppurative keratitis caused by filamentous fungi. METHODS: Patients presenting with suppurative keratitis in India underwent detailed clinical examination followed by microbiological investigation of corneal scrapes. A partial diagnostic score based upon the strength of the association, as estimated by the odds ratio, between reported clinical features and laboratory confirmed diagnoses was devised and subsequently tested using a case series from Ghana. RESULTS: Serrated margins, raised slough, dry texture, satellite lesions and coloration other than yellow occurred more frequently in cases of filamentous fungal keratitis than bacterial keratitis (p<0.05). Hypopyon and fibrinous exudate were observed more frequently in bacterial keratitis (p<0.05). When incorporated into a backwards stepwise logisitic regression model only serrated margins, raised slough, and colour were independently associated with fungal keratitis; these features were used in the scoring system. The probability of fungal infection if one clinical feature was present was 63%, increasing to 83% if all three features were present. CONCLUSIONS: Microbiological investigations should be performed whenever possible; however, where facilities are not available, a rapid presumptive diagnosis of suppurative keratitis may be possible by scoring clinical features.

Suppurative, nonseptic polyarthropathy in dogs.

The goals of this study were to determine the historical, physical examination, and clinicopathologic findings in dogs with suppurative, nonseptic polyarthropathy and to identify concurrent disorders associated with this syndrome. Medical records of 52 dogs with cytologic evidence of suppurative inflammation in two or more joints were examined retrospectively. Age of dogs was 4.8 years (median, range: 0.5-12 years). There was no clear breed or sex predilection, but most were large-breed dogs (body weight > or = 20 kg [44.4 lbs] in 40/52). Body temperature was 103.0 degrees F (39.4 degrees C) (median, range: 100.0-105.9 degrees F), with 29 of 52 dogs having a body temperature > or = 103 degrees F (39.4 degrees C). Lameness was identified in 42 of 52 dogs. Erosive changes were found in only 1 of 37 dogs that had radiography performed. A clear underlying disease process was not identified in 34 of 52 dogs. Seven dogs had evidence of infectious or inflammatory processes at extra-articular sites; 4 dogs were diagnosed with systemic lupus erythematosus (SLE); 2 dogs had gastrointestinal disease; 2 dogs had been vaccinated within 1 month before onset of polyarthritis; 1 dog had cancer; 1 dog had polyarthritis and meningitis; and 1 dog had erosive polyarthritis. Of the 44 dogs tested, 25 had antibodies to Borrelia burgdorferi, detected by an ELISA assay, which was significantly greater than the general hospital population (P = .007). Antibodies against Rickettsia rickettsiae and Ehrlichia canis were not definitively identified in the sera of any dog tested in this study (45 and 44 dogs, respectively). We conclude that an underlying disease process is not identified in most cases of suppurative polyarthropathy in dogs and that intestinal disease, neoplasia, and SLE are uncommon causes of polyarthritis. While seropositivity against the causative agent of Lyme disease was common and possibly a cause of polyarthritis in some dogs of our study, evidence of other vector-borne infection was not identified.

Suppurative extensor tenosynovitis of the extensor carpi ulnalis tendon sheath.

Suppurative tenosynovitis of the flexor tendons has been reported frequently, and its diagnosis and treatment are well established.(1-6) However, suppurative tenosynovitis of the extensor tendons is a rare entity that has been infrequently reported in the English literature. Reported cases were usually nonlocalizedand atraumatic and occurred in immunocompromised patients. In this article, we report a case of isolated suppurative extensor tenosynovitis of the extensor carpi ulnaris tendon sheath in a healthy individual and review the literature, anatomy, and treatment.

[Vitrectomy combined with intravitreal injection of drugs for the management of advanced suppurative endophthalmitis].

Sixty cases (60 eyes) of advanced suppurative endophthalmitis treated by a combination of vitrectomy and intravitreal injection of drugs (antibiotics or/and corticosteroids) were investigated perspectively. Of the 60 cases, 43 were infected after trauma, 12 were infected after surgical procedures, while the others were of unknown causes. During the follow-up period of 1/2-5 years, 57 cases (95%) retained functioning eyes, the postoperative visual acuity of 34 eyes (57%) being 0.05 or better and the postoperative visual acuity of 17 eyes (28%) 0.4 to 1.0. The advantages of the therapeutic regimen are analyzed. We believe that it is the most effective therapy for suppurative endophthalmitis. The timing of the operation and the factors influencing the results are discussed. This report also presents our clinical experiences in the management of advanced suppurative endophthalmitis.

The possible role of Chlamydia trachomatis in perineal suppurative hidradenitis.

OBJECTIVE: To find out if there is an association between perineal suppurative hidradenitis and Chlamydia trachomatis infection. DESIGN: Open study. SUBJECTS: Seven consecutive patients treated for perineal suppurative hidradenitis during the past three years, and 10 control subjects who were being treated for acute cryptogenic perianal abscesses. MAIN OUTCOME MEASURE: Presence of C. trachomatis detected by direct immunofluorescent staining. RESULTS: All but one patient had serological evidence of C. trachomatis infection. All 10 control subjects failed to react to IgA antibodies to C. trachomatis, and two reacted to IgG antibodies. CONCLUSION: There may be a link between C. trachomatis infection and suppurative hidradenitis, but it is uncertain whether it is a direct cause or a predisposing factor.

Acute suppurative parotitis.

The parotid is the salivary gland most affected by inflammatory process. The authors reviewed the records of 36 patients with acute suppurative parotitis admitted between July 1996 and June 2002. All patients had unilateral swelling of the cheek that extended to the angle of the jaw; 23 had fever (64%) and 6 diabetes (17%). Even though Staphylococcus aureus is the usual pathogen in acute suppurative parotitis, in the present study, alpha-hemolytic streptococci predominated. Adequate hydration and administration of parenteral antimicrobials are essential managements of suppurative parotitis. In the event a well formed abscess, surgical drainage was necessary. The choice of antibiotics depends on the causal microbes.

[Clinico-morphological characteristics of cancer of the gallbladder complicated by suppurative cholecystitis].

Suppurative inflammatory complications were reported in 40 patients with gallbladder carcinoma. It was stated that the type of the complication and its clinical manifestation depended mainly on the site of the tumor in the gallbladder, the type of the tumor growth and its invasiveness. Development of suppurative complications was observed most frequently when the tumor was located in the cervical area of the gallbladder and in the endophytic type of its growth. Depending on these parameters, the clinical picture may have acute or torpid pattern with the corresponding pathologic anatomic changes in the abdomen due to suppurative complication. Thirty-five patients were operated on. Lethal outcomes made up 62.8%.

Microbiological diagnosis of suppurative-inflammatory processes of the E. N. T. organs and the maxillo-mandibulo-facial region under suspicion of anaerobic non-clostridial infection.

The etiological role of non-sporulating anaerobic bacteria as causative agents of suppurative-inflammatory diseases (SID) of the maxillo-mandibulo-facial region and the E. N. T. organs as studied. Express diagnosis of anaerobic infection was carried out by means of gas-liquid chromatography. The species-specific composition of the microflora of the suppurative focus was investigated. It was established that only obligate anaerobes in monoculture or in association were isolated from patients with so-called "sterile" inoculations when strictly anaerobic bacteriological technique was used. The use of anaerobic bacteriological technique of investigation enlarged the spectrum of the microflora isolated from the suppurative focus from 5 to 26 species. Results of chromatographic and bacteriological examinations were compared; the main causes of obtaining false-positive and false-negative results of chromatography were analysed. Statistical processing using factor analysis has shown that the information power of chromatographic examination of the metabolites of anaerobic bacteria is higher in comparison with the main clinical-laboratory indices, but statistical processing using cluster analysis and correlation analysis has revealed that an index like metabolic activity reflects the degree of real participation of anaerobic microflora in the development of the pathological process, and can be used in the clinic for the evaluation of the degree of severity of the course of the SID and of the effectiveness of treatment.

[Perimaxillary suppurations in the stomatological practice].

Perimaxillary suppurations hold an important place in the field of buccomaxillo-facial pathology, by their frequency (which is almost 12%), and by severity. Antibiotic therapy, which has modified for a period of time the evolution and the frequency of these affections, appears to be less successful of late, and, by contrast, we have noted the development of new types of suppuration much more severe, and more difficult to control from the therapeutical viewpoint. The main etiology is represented by dento-parodontal lesions, and the complex treatment of the initial forms of peri-radicular septic processes, and of the parodontal ones is recommended strongly. Evaluations are made concerning the antibiotic therapy, even in those conditions when there is no available facility for making antibiograms. With regard to the severe forms of suppuration various problems are discussed, with the aim to define diagnostic, surgical and general therapy aspects.

[The prevention of postoperative suppurative complications in stomach cancer].

Bacteriologic studies of gastric juice and purulent drainage fluids in the event of complications were carried out in 80 patients who had undergone gastric resections and had been on conventional parenteral antibiotic treatment (control group). Another (experimental) group of 37 patient, having undergone the same operations, received prophylaxis of the suppurative complications with 5-nitrox and Flagyl solutions through nasogastric probe once in 3 hours for 6 to 8 days. The suppurative complications in the experimental group were reduced from 47.5 to 8.1 per cent and of the postoperative lethality due to suppurative complications from 22.5 to 0 per cent.

[Hearing loss in suppurative meningitis and encephalitis in our cases].

Seventeen cases of the suppurative meningitis complicated by hearing loss have been analysed clinically. Emphasis is on the possibility of such a complication in any case of the suppurative infection of CNS. The authors have shown the necessity of otolaryngological examinations in each patient, basing on their own experience and available literature. An early diagnosis and appropriate treatment produces complete recover or decreases the degree of hearing loss being a complication of the suppurative neuroinfections.

Acute experimental suppurative pancreatitis in the rat.

Acute pancreatitis was induced in rats by infusing sodium taurodeoxycholate with or without Escherichia coli and Bacteroides fragilis into the bile-pancreatic duct. Survival did not differ between the 'noninfected bile group' (NIB) and the 'infected bile group' (IB). At standardized macroscopic evaluation, pancreatitis was more severe in the IB group (p less than 0.05). Histologic examination on day 7 showed suppuration of pancreatic tissue in 6/7 IB and 3/14 NIB rats (p less than 0.05). Bacteriologic culture of pancreatic tissue was positive in 6/8 IB and 3/17 NIB rats (p less than 0.01). Bacterial culture of blood, peritoneal fluid of pulmonary tissue was seldom positive. Concordance between microscopically observed suppuration and positive bacterial culture was almost total. Recall antigen skin testing indicated anergy in the IB group, while the NIB group showed moderately diminished reaction (p less than 0.001). Similar increase of S-fibrinogen was found on day 3 in both groups, but complement factor C3 was reduced only in the IB group. This experimental model, with suppurative pancreatitis induced by intraductal infusion of an infected bile salt, may be useful for studies of systemic complications in acute pancreatitis.

Acute suppurative parotitis caused by anaerobic bacteria: report of two cases.

Staphylococcus aureus is the predominant pathogen in acute suppurative parotitis. Although anaerobic bacteria are prevalent in the normal oral and upper respiratory tract flora and frequently are involved in oral and facial infections, they have seldom been isolated from patients with suppurative parotitis. It may be that a role for anaerobes in parotitis has not been documented because of inadequate specimen transport or anaerobic culture techniques. We describe two cases of acute suppurative parotitis in which cultures yielded anaerobic bacteria. In one case, no aerobes were isolated; in the other, the anaerobe was the predominant organism numerically.

[Laboratory studies of the urine in suppurative surgical infection in cattle].

One hundred and eight cattle (20 controls, 14 with an experimentally induced and 74 with spontaneously occurred suppurative surgery infection) were studied in terms of their general condition and some of the urine indices--pH, specific weight, proteins, urobillinogen, acetone, sugar, blood, urea and sediment. It was established that when the general status deteriorated and death occurred the urine pH value rose (by 1--1.5), and the specific weight was 0.005--0.010 higher, while the level of urina urea dropped by 0.5--0.6 g%. In 89 per cent of the investigated animals there were 0.1 per percent proteins, and the sediment consisted of 20--30 erythrocytes, from sporadic to 10-12 leucocytes, and from single to 5--6 kidney epithelium cells per field. In 38.8 per cent of the cattle affected with suppurative processes there were tripelphosphates in the sediment. Urobillinogen was found in 63.6 per cent of the cases. Suppurative infections caused by Corynebacterium pyogenes are accompanied by greater changes in the sediment. In almost all of the recovered animals the urine indices came back to normal.

Bacteriophage treatment of suppurative skin infections.

The study material comprised 31 patients with chronic suppurative infections of the skin caused by Pseudomonas, Staphylococcus, Klebsiella, Proteus and Escherichia. Within 2-16 weeks of the treatment, an improvement of the general state was observed as well as suppression of the local inflammation, purification of a wound from the suppurative and necrotic content, faster healing of the ulcers and fully negative results of the bacteriologic tests. In 16 cases, an outstanding therapeutic effect was obtained, in 7 cases marked improvement was reported and in 2 a transitory improvement was reported. In 7 patients the treatment was abandoned due to the lack of improvement (1 case) or development of side effects (6 cases). The results obtained provide evidence for the high effectiveness of phage therapy in the treatment of suppurative skin infections.
