[Late results of stomatologic and ophthalmologic treatment of injuries involving the central part of the face].

The authors present an analysis of the results of treatment of patients with injury of the central fragment of the face, who have been hospitalized in the 2nd Department of the Maxillofacial surgery in Silesian Academy of Medicine. In the period 1980-1987 there were 447 patients with an injury of the central fragment of the face. Among this group 60 (13.4 p.c.) patients showed disturbances of the visual system. The most frequent causes of the injury were: assault (61.5 p.c.), traffic accidents (20.1 p.c.), occupational injuries (8.8 p.c.). Deformation of the face was the most frequent pathology observed in the course of a control stomatological check-up (32.0 p.c.). The most important ocular pathology were the posttraumatic atrophy of the eye (3.3 p.c.) and optic atrophy. Palpebral scars (33 p.c.) and diplopia (7.6 p.c.) were the most frequent ocular signs.

[Development of gingival inflammations in schoolchildren instructed in proper oral hygiene in a seven-year longitudinal study conducted under clinically controlled conditions and as part of the Preventive Stomatology research project (author's transl)].

Instruction of six- and seven-year-old children (N = 1167) at the beginning of this longitudinal study was given by a stomatological nurse at weekly intervals and, later, at fortnightly and monthly intervals, respectively. Compared with a group of untreated controls were not only those children who were instructed in proper oral hygiene but children receiving additional doses of amine fluorides as well. During the seven-year period of observation there was noted a distinct improvement in oral hygiene (OHI) as well as a reduction of gingival inflammations (PM index). Whereas additional administration of amine fluorides had no appreciable effect upon both oral hygiene and the condition of health of the gingiva there was a direct relation to the frequency of instructions in oral hygiene at school.

[Antibiotic prophylaxis of infective endocarditis in stomatology--a survey of cardiologists].

According to epidemiological reports; 14-20% of infectious endocarditis (IE) are associated with previous stomatological procedures (oral surgery, procedures during periodontal and conservative endodontic therapy and intra-ligament anesthesia) which cause transitory bacteriemia for 10-15 minutes. In patients at risk, such short lasting bacteriemia is sufficient for the development of infectious endocarditis. In everyday practice we encounter patients at risk for IE referred to oral surgery (most often tooth extraction) at the Oral Surgery Division, Department of Facial, Maxillary and Oral Surgery, Dubrava University Hospital. Majority of these patients are given IE prophylaxis according to the latest AHA recommendations. For some patients we first consult a cardiologist due to the underlying disease, and as a rule these patients return with cardiologist's advice on IE prophylaxis. We noticed that these recommendations differ between cardiologists. Some cardiologists have for years been recommending the same "prophylaxis" which is not in accord with AHA guidelines. From the available cardiac records we analyzed antibiotic IE prophylaxis before oral surgery in cardiac patients at risk recommended by cardiologists in four Zagreb hospitals in 2003. We collected in total 17 recommendations prescribed by 12 cardiologists. Regretfully, none was in accord with valid AHA guidelines from 1997. Only 6 out of 17 recommendations are in agreement with the outdated guidelines from 1994, which also raises concern. Therefore, we thought it was worthwhile to present these cases, hoping this will stimulate discussion on common guidelines for the benefit of patients at risk for IE.

[Kasabach-Merritt syndrome as viewed from a surgical and stomatological standpoint (author's transl)].

The Kasaach-Merritt syndrome involves the very infrequent symptom complex of "giant hemangioma--thrombocytopenia--coagulation disorder". Hemorrhagic diathesis is here based on consumption coagulopathy. This paper reports upon secondary bleeding after extraction of a tooth in a 25-year-old male affected with this syndrome. Continuation or initiation of anticoagulant therapy is recommended for surgical stomatological treatment on an inpatient basis, and measures to be taken in connection with wound treatment are described.

Hereditary familial polyposis and Gardner's syndrome: contribution of the odonto-stomatology examination in its diagnosis and a case description.

Familial Adenomatous Polyposis (FAP) and its phenotype variant, Gardner's syndrome, constitute a rare autosomal dominant inherited disorder. They are characterised by the development, generally during the second and third decades of life, of multiple adenomatous polyps in the colon and rectum. These polyps have a high risk of subsequently becoming malignant, which normally occurs in the third and fourth decades of life. The phenotypical features of FAP can be very variable. As well as colorectal polyps, these individuals can present with extra-colonic symptoms, among which are particularly: gastro-duodenal polyps, dermoid and epidermoid cysts, desmoid tumours, congenital hypertrophy of the retinal pigment epithelium, disorders of the maxillary and skeletal bones and dental anomalies. In this paper the most important aspects of this syndrome are reviewed, showing an example based on a well documented clinical case. The importance of odonto-stomatological examinations should be pointed out, among others, as a means of reaching a presumptive diagnosis, whose confirmation is vital to the patient.

[Functional characteristics of periodontium sensing apparatus and its significance in prosthetic stomatologic clinic].

Using developed by us method and instrument Periosensomer the sensor function of the periodontium on the initial stages of masticatory act (test of holding and splitting of food object) in persons with different functional periodontium status was explored. Evaluation of the sensor function of the periodontium and dynamics of the sensor reaction during holding and splitting of food object can be used as the method of functional diagnostics in the prosthetic stomatological clinic.

[Caries morbidity in the odonto-stomatologic services in Yaounde, Cameroon].

In our regions, it is believed that dental decay is an illness without prestige or danger for the life. It's the reason of teeth loss and many complications. The objective of this study was to valuate the morbidity of this pathology in odonto-stomatology services. With an average of 4.5 teeth decays per person and a morbidity index of 0.15; dental carie concern all socio-professional levels, without distinction of age or sex. Women seem more vulnerable to teeth decay, and they consult services more than men. We are potential edentulous because of dental decay, if preventive and training measures in mouth and tooth healths are not taken in our country.

Behavior of individuals with mental disorders during stomatological treatment.

The paper deals with the types of stomatological treatment of patients with mental deficiency aged from 3 to 23 years attending daily the Institute of Social Care. Interesting enough was that nearly 75% of patients with serious intellectual deficiency could be examined and treated under normal conditions. Few patients had to be given premedication and only 7% of patients were treated under general anesthesia.

[Evaluation of morbidity through dental decay in students by school stomatologic monitoring].

Starting from the fact that stomatologic dispensarization in schools is the best method for evaluating morbidity by bucco-dental disorders in students, the authors present the results of a longitudinal study which consisted in the permanent monitoring of 48 students for a period of 8 years (1980-1988). The children were from the I-st to the VIII-th class. It is stressed that the most relevant indexes of morbidity are those of incidence (for the evaluation of the present potential of developing a disorder), and that of severity (for the assessment of the morbidity status). The authors intend to take up again this study, with the same methodology, in a group of students to which a local fluorination procedure was applied from the I-st scholar cycle.

[Contribution to the treatment of children in the first aid medical service of the stomatological department].

At the first aid medical service of the stomatological department of a type II hospital with policlinic in the course of the calendar year a total of 1,761 children from 2-15 years were treated. During this period the staff treater 1,718 teeth and 43 times they treater soft tissues of the oral cavity. Most frequently periodontitis of the decidual teeth was treated, resorption of the root of a decidual tooth, pulpitis and caries. As to therapy, extractions were most frequent, followed by trepanation of the pulp cavity and treatment of the tooth by a provisional filling. The author draws attention to unsuitable therapeutic procedures such as trepanation of the pulp cavity in case of periodonititis of a decidual tooth and application of cobalt in pulpitis of a decidual tooth after the age of seven years. She emphasizes also the necessity to use local anaesthesia in extraction of decidual teeth.

[Effect of psychotherapy on problem behavior of children and adolescents in stomatological office].

The group of 269 "untreatable patients" aged 3-18 years was followed up for the effectiveness of psychotherapeutical management of problem behavior (in %). The used psychotherapeutical methods appeared to be most effective in individuals attending primary school (81.5%), satisfactory were in patients attending the school with special education (44.8%). No effects of psychotherapy were observed in oligophrenic patients who refused cooperation during the first stomatological treatment.

[History of the Dental (Stomatologic) Clinic through 1920 (József Arkövy)].

The Dental Clinic of the Budapest University was established 100 years ago. The prehistory and the history of the first three decades of the clinic is closely attached to the course of life of Professor József Arkövy (1952 to 1922). Arkövy acquired a medical and dental master certificate in 1876. Between 1881 and 1890 he directs a private dental clinic. This became the basis of the University Dental Clinic opened in 1890 which disposed of a treating hall with 10 dental chairs, a narcosis chamber and a class room. In 1908, the clinic moves into a new building planned for such purpose and its name becomes Clinic of Stomatology. In the new clinic the departments of surgery, conserving dentistry and prosthetic dentistry became separated and there was a bedpatient department as well. Arkövy retired in 1920.
