Conceptualization vs reality in periodontal therapy: genetic testing and host modulation.

This article addresses controversial issues associated with two different subjects: reliability of a genetic susceptibility test to identify individuals predisposed to developing periodontitis; use of subantimicrobial-dose doxycycline in the treatment of periodontal diseases. Each subject is assessed with regard to its practical application in the management of patients with chronic periodontitis.

[Entry inhibitors: enfuvirtide, a reality for antiretroviral therapy].

Entry inhibitors make up a new class of antiretroviral agents that block HIV entry into body cells. The recent approval of the fusion inhibitor T-20 or enfuvirtide by both FDA and the European Agency for the Evaluation of Medicinal Products (EMEA) is a first step in the expansion of a therapeutic armamentarium that is expected to increase in at least half a dozen new drugs in oncoming years. T-20 is indicated for the treatment of HIV-1 infection--in combination with other antiretroviral drugs--in patients who are resistant to both protease and reverse transcriptase inhibitors. In phase III studies, T-20 has demonstrated adequate efficacy and safety at 24 weeks after treatment, but long-term effects remain to be established. Until it finally becomes available in Spain, the compassionate use-related Early Access to T-20 Program allows it to be employed in our country for patients who meet criteria specified in the manufacturers users guide.

Myths and realities of electroconvulsive therapy.

With its introduction in 1938 electroconvulsive therapy became a powerful form of treatment for depressive illness and the major psychoses. Its use over the years has declined with the development of effective antidepressant and antipsychotic drugs. It still has a place in psychiatric treatment where urgent resolution of symptoms is indicated in certain conditions. The mechanism by which it exerts its therapeutic effect remains unknown.

BusWorld: designing a virtual environment for post-traumatic stress disorder in Israel: a protocol.

A number of carefully controlled studies have documented the effectiveness of traditional imaginal exposure for the treatment of post-traumatic stress disorder (PTSD). Virtual reality (VR) exposure therapy is based on a similar logic but rather than self-generating imagery, patients wear a VR helmet and go into a three-dimensional (3-D) computer generated virtual world to help them gain access to their memory of the traumatic event. Recent preliminary research has shown that some patients who fail to respond to traditional therapy benefit from virtual reality exposure therapy, presumably because VR helps the patient become emotionally engaged while recollecting/recounting/re-interpreting/emotionally processing what happened during the traumatic event. The present paper presents a brief overview of a new VR World we developed to provide virtual reality therapy for terrorist bus bombing victims in Israel, and a brief description of our research protocol and measures (for details, see www.vrpain.com).
