[Status of the pituitary-adrenal and cholinergic system of patients with a pain syndrome in spinal osteochondrosis before and after a course of reflexotherapy].

The pituitary-adrenal and cholinergic systems were studied in 57 patients with chronic pain related to vertebral osteochondrosis before and after an acupuncture course. The degree of the therapeutic effect of electroacupuncture (EAP) was found to correlate with the nature of changes in the level of the spontaneous secretion of aldosterone and intensity of the process of acetylcholine (AC) release. Most patients in whom the treatment was followed by the recovery of the physiologic level of the initially depressed secretion of aldosterone and a clear-cut decrease in the initially elevated AC concentrations showed good clinical response. In patients in whom the initially elevated levels of aldosterone and AC failed to decline or in whom there was an even greater increase in AC concentrations, the therapeutic effect of EAP procedures was insignificant and short-lived.

[Dynamics of clinico-electromyographic indices in reflexotherapy of radicular syndromes of lumbar osteochondrosis].

Using electromyography the authors studied the functional status of the segmentary apparatus of the spinal cord in 58 patients with compression of S1-radicle secondary to vertebral osteochondrosis. The findings obtained included changes in the interferential EMG and reciprocal reflexes and decreased reflectory activity of alpha-motoneurons predominantly on the side of radicular involvement with the simultaneous intensification of gamma-loop effect. Acupuncture was followed by pain alleviation and normalization of the functional activity of the segmentary apparatus of the spinal cord mainly at the expense of this activity reduction on the healthy side. Clinical recovery was observed following the compensation of neuroreflectory activity of the segmentary centres on a new functional level. The advisability of the use of acupuncture in the treatment of the radicular syndrome of lumbar osteochondrosis is justified.

[Reflexotherapy of asthenic conditions in neuroses].

The results of clinico-psychological examination of 121 patients have demonstrated that acupuncture is a fairly effective method of treating asthenic states in neurotic patients. Thus, considerable improvement and improvement has been observed in 67% of the patients. The absence of positive effect in the remaining cases is explained by an aggravated history, a stubborn course of the disease, and depletion of the functional activity of the sympathoadrenal system (SAS) and the adrenal cortex. Normalization of EEG rhythm is effected through activation of the synchronizing and inactivation of the desynchronizing cerebral systems; the positive dynamics of the structure of nocturnal sleep is reflected in an increased duration of slow sleep and rapid sleep which were initially reduced, in more regular alternation of the cycles and phases of sleep, and in a shorter period of falling asleep. Biochemical examinations have revealed activation of the mediatory component of the SAS and elevation of the function of the adrenal cortex, while baroalgesimetric studies have shown that acupuncture is characterized by antinociceptive and activizing effects. On the whole the efficacy of acupuncture in controlling asthenic conditions related to neuroses is due to its integral action.

[Various aspects of the pathogenesis of headaches and their reflexotherapy in patients with hypertension].

The authors have studied the mechanisms of action and the results of the use of ethylcloride block (ECB) in 467 hypertensive patients with headaches before and after ECB. A marked analgetic effect was observed in 93.8% of the patients. A myogenic component of pain in the occipital area was verified electromyographically. It has been established that ECB of the sinocarotid zone significantly decreases both regional and total blood pressure.

[Somatosensory evoked potentials and their dynamics among trigeminal neuralgia patients during reflexotherapy].

The study of somatosensory evoked potentials (SSEP) in patients with trigeminal neuralgia made it possible to elucidate their characteristic changes as compared to those in healthy subjects. After a course of acupuncture 75% of the patients showed correlation between some parameters of SSEP and positive clinical results. The authors discuss pathophysiological mechanisms of impairment of cortico-subcortical interactions in patients with trigeminal neuralgia and analyze the possibility of using SSEP for dynamic evaluation of the brain status in the process of treatment.
