Treatment of chronic otitis externa with Trichophyton, Oidiomycetes, Epidermophyton antigen.

Chronic otitis externa may be a sensitivity (dermatophytid) reaction from a distant pathogenic superficial fungal infection, eg, tinea pedis and monilial vaginitis, which are often difficult to eradicate. Skin testing is done with a combined antigenic extract of TOE (Trichophyton; Oidiomycetes, ie, Monilia or Candida albicans; and Epidermophyton). The serial dilution titration method of skin testing is done, and the end point of reaction is determined. Treatment is begun on a weekly basis, with subcutaneous administration of 0.3 cc of a TOE extract two dilutions weaker than the end point. Symptomatic improvement is usually noted within two to four weeks, with objective improvement within eight weeks. After a variable maintenance period, the injections are spaced at less frequent intervals.

Effect of thymic humoral factor on cellular immune factors of normal children and of pediatric patients with ataxia telangiectasia and Down's syndrome.

Cellular immune functions of nine Down's syndrome patients and of nine was Ataxia telangiectasia vs. nine normal children and nine cord bloods, were evaluated using in vitro assays of peripheral blood lymphocytes. The in vitro assays included E rosette formation, antilymphocytic cytotoxicity by an antithymic antiserum and leukocyte migration inhibition factor (LIF) production. The mitogens and antigens used were phytohemagglutinin, purified protein derivative, and monilia antigen. The effect of a thymic hormone (THF) on these parameters was evaluated and it was administered therapeutically to three Down's syndrome patients and to two patients with Ataxia telangiectasia. Most deficient T-cell functions were reversed to normal after incubation of the lymphocytes with THF, or after THF therapeutic administration. In two Down's syndrome cases, the clinical course was not altered by THF administration, while one seemed to benefit from it markedly. One of the Atactic patients recovered from a severe viral infection, while the other died from intractable bronchopneumonia.

Phagocytosis and cutaneous delayed hypersensitivity in patients with chronic obstructive pulmonary disease.

The objective of this study was to determine if the phagocytic and intracellular killing capacity of peripheral granulocytes or an expression of cellular-mediated immunity, delayed cutaneous reactivity, as measurements of native and acquired immunity, might be risk factors associated with chronic obstructive pulmonary disease (COPD). Over 100 patients with a value for their forced expiratory volume in one second (FEV1) less than or equal to 70 percent of normal were carefully matched with healthy participants having an FEV1 greater than or equal to 86 percent of normal, and together they served as the study group. Phagocytosis and intracellular killing were normal in patients with COPD; however, these patients demonstrated a significant impairment in the ability of their peripheral leukocytes to reduce nitroblue tetrazolium. The delayed-hypertensitivity response rate and the degree of reactivity were similar in the two groups, except for the patients with COPD having a significantly greater degree of reactivity to Monilia albicans extract ("canadin.") This finding is thought to be a consequence of reduced mucociliary clearance rather than a risk factor. The significance of decreased resting and stimulated cells' reduction of nitroblue tetrazolium in patients with COPD is not clear.

Effects of experimental hyper- and hypothyroidism on numbers of blood mononuclear cells and immune function in rats and guinea-pigs.

A possible effect of thyroid hormones on numbers of mononuclear cells and immune reactivity has been studied in hyperthyroid and hypothyroid guinea-pigs and rats. There were no major changes in populations of blood mononuclear cells in hyperthyroid or hypothyroid animals compared with populations in euthyroid animals. Although there was some evidence for depressed cell-mediated responses to an extract of Candida (monilia) albicans in hyperthyroid rats as assessed by skin tests, this was minor, and responses in tuberculin purified protein derivative (PPD) were normal in all groups, whilst production of macrophage migration inhibition factor in response to PPD and Candida was similar in the three groups of animals. Antibody responses to sheep red blood cells, a thymic-dependent antigen, tended to be depressed in hyperthyroid and hypothyroid rats and increased in hyperthyroid and hypothyroid guinea-pigs, although this was significant only for hyper-thyroid guinea-pigs 16 days after immunization. Responses to trinitrophenol-Ficoll, a thymic-independent antigen, were similar to the three groups of guinea-pigs. Thus, a major effect of excess or deficiency of thyroid hormone on immune responses to foreign antigens has not been demonstrated, although it is possible that immune reactions against thyroid antigens may be more sensitive to the effect of thyroid hormones than responses to foreign antigens.

Inhibition of Candida albicans by Lactobacillus acidophilus.

Candida albicans grew at pH 4.6 or above in nutrient broth containing 5% glucose but was retarded at pH 7.7 by filtrates of Lactobacillus acidophilus grown in casitone broth. Vaginal implantation of nonfermented acidophilus milk, yogurt, or low-fat milk for preventing recurrence of monilia vaginitis subsequent to treatment with Nystatin was studied with 30 women. Reinfections within 3 mo according to product received were: no milk product, 3; yogurt, 1; nonfermented acidophilus milk, 1; and low-fat milk, 0.

Gross and histological studies of immediate, Arthus, and delayed skin test responses to schistosome antigen, and of delayed response to ubiquitous antigens in Egyptians.

Gross studies of skin reactions to adult antigen of Schistosoma mansoni were made on 156 hospitalized patients with schistosomiasis and 114 subjects from the nonendemic area of Hurghada in Egypt. Wheal areas equal to or greater than 1.0 cm2 indicated a positive immediate (15-min) reaction to adult worm antigen; the criterion of positivity for both 24-hour and 48-hour delayed reactions was an area of induration equal to or greater than 0.6 cm2. Immediate reactions with adult worm antigen were observed in 99% of the patients with schistosomiasis and 11% of the subjects from Hurghada: the percentages with delayed reaction were 58% and 2%, respectively. Biopsies of skin test sites at various intervals after antigen injection were done on 87 individuals. Eosinophilic and mononuclear infiltrates were characteristic of immediate and delayed skin responses, respectively. Biopsies from 22 patients with marked skin reactions 5 hours after antigen injection showed that a neutrophilic response indicative of Arthus reactivity was present in only 18. Thus, Arthus reactivity could not be determined on gross appearance alone. The studies did not show any evidence of delayed basophilic hypersensitivity to schistosome antigen. Immunofluorescent studies on a small number of biopsies suggested that a late phase (5-hour) reaction due to IgE may occur in some patients. Delayed reactivity to mumps and/or monilia skin test antigens was observed in 91% of Egyptians in a nonendemic area of schistosomiasis. Delayed hypersensitivity to PPD was detected in 44% of the same group.

The diagnostic value of the tine and Mantoux tests in a general hospital.

Tine and Mantoux tuberculin tests were compared in 193 hospitalised White patients with a variety of illnesses. With the Mantoux test 52 (27%) were tuberculin-positive and 19 (9.8%) were positive with the tine test. Had the tine technique alone been used, 33 of 52 tuberculin reactors would have been missed. Additional findings were (i) a 73% positivity for Monilia antigen; (ii) the optimal time for reading the Mantoux tests was on the third, rather than the second day; (iii) tuberculin reactivity was not suppressed by either pyrexia or elevation of the sedimentation rate; and (iv) the use of avian PPD demonstrated very few reactors in this population. It is recommended that the tine test be replaced by the intradermal Mantoux test with stabilised freeze-dried PPD tuberculin supplied in test kits with diluent. This matter should be given high priorty in South African general hospital practice and population surveys.

[Induction of autologous tissue disorders in animals tolerant to cross-reacting microbial antigens].

Injection to newborn C3H mice of streptococcus, group C, or C. albicans vaccines prevented in adult mice induction of the skin autotransplant affection caused by the immunization of these animals with the mentioned microbial vaccines. In response to the immunization with the streptococcus or monilia vaccines the tolerant mice displayed a reduction in the number of cells reacting with the microbial vaccines in the immunoadhesion and blasttransformation reactions; their cytotoxic activity in respect to the syngenous target cells was also diminished.

Enzymatic activity of micro-organisms isolated from cork wine stoppers.

The production of enzymes by micro-organisms which are found on vegetal substrates is important due to their ability to decompose cellulose, lignin and other components, which guarantee the integrity of the vegetal cell. The objective of this study was to determine the enzymatic activity of filamentous fungi, yeasts and bacteria, isolated from natural cork stoppers for bottles of still and sparkling wines. Suspensions of fungal conidia, yeasts and bacterial cells of micro-organisms were established in concentrations of 10(6) CFU/ml. The enzymatic activity of these micro-organisms was evaluated by means of the API ZYM system, with which it was possible to determine and semi-quantify nineteen enzymatic activities simultaneously. The enzymes produced by all of the species were esterase (C1), esterase lipase and naphthol-AS-BI-phosphohydrolase. The micro-organisms with the greatest enzymatic activity were Monilia sitophila, Alternaria alternata, Aspergillus niger and Aeromonas sp.

[127 cases of pulmonary fungal infection].

OBJECTIVE: To find out the clinical features, therapeutic outcome, prognosis and the major predisposing diseases of pulmonary fungal infection(PFI). METHOD: 127 cases with PFI were retrospectively analyzed. RESULTS: Various predisposing diseases were found in 95% of total cases, among them, COPD, systemic lupus erythromatus(SLE), leukemia and chronic renal disease were main predisposing diseases. Primary pulmonary fungal infection was rare. All the clinical manifestations of PFI were not specific, reontgenographic features showed mainly bronchitis type(61%). Monilia (Candida) was ranked the first pathogen (79.5%). The mortality of PIF was high. Chronic renal and hematological diseases might be the risk factors for mortality (RR, relative rate, 5.14). CONCLUSIONS: PFI is an important cause of the secondary infection in many diseases. The clinical features of PFI is non-specific and the morbidity is rising. Clinicians should pay more attention to it.

[Fungal spores in the environment of the asthmatic patient in a semi-desert area of Mexico].

OBJECTIVE: To analyze the frequency and type of fungal spores in the habitat of sensitized asthmatic patients, its relationship with skin test reactivity to fungi and indoor factors favoring the fungi development. MATERIAL AND METHODS: Indoor airborne fungi were collected from the houses of 85 patients using fungal culture methods and adherent cover glasses. Data were analyzed as a cross sectional study. RESULTS: The prevalence of positive fungi skin test reactivity was 22%, the main reactivity was for Aspergillus (54%), Alternaria (51%) and Monilia (48%), and most frequent cultured fungi were: Cladosporium (76%), Alternaria (57.5%) and Aspergillus (51.3%). Another molds (Dreshlera, Rhodotorula) not included in the skin test reactivity panel nor immunotherapy plans were cultured. The fungus spores mean concentration was 1788/m3 +/- 1637 (930 to 10,230/m3) without qualitative or quantitative differences when several house areas were compared. The association of skin reactivity and indoor exposure were significant only for Aspergillus (RR 3.14, 95% CI 1.77-5.58). The use of ventilation-air-conditioning systems (42%), personal humidifier or nebulizers (43%), dampness inside the houses were important characteristics. CONCLUSIONS: Indoor fungus pollution is also important in semi-desert areas, due to high prevalence of many factors that contribute for its development and proliferation.

Potential risks, benefits of progestins in birth control pills outlined.

Progestins in oral contraceptives (OCs) produce potential complications, as well as noncontraceptive benefits, according to Robert A. Hatcher, MD, MPH, professor of gynecology and obstetrics, Emory University Medical School. Hatcher told CTU that lowering the progestin content in an OC may decrease complications, but could also decrease the benefits experienced by women. "The extent to which that will happen remains to be seen," he said. Hatcher cited the following potential complications of progestins in OC: hypertension; decreased levels of high density lipoproteins; acne; oily skin; headaches between pill cycles; dilated leg veins; pelvic congestion syndrome; thrombosis of superficial leg veins; gallstones; Monilia vaginitis; cholestatic jaundice; and depression, fatigue, and decreased libido. Progestins, according to Hatcher, also produce these noncontraceptive benefits: protection against PID; decreased dysmenorrhea; decreased menstrual blood loss, decreased iron deficiency anemia; protection against endometrial cancer; protection against fibrocystic breast disease, and fibroadenomas of the breast; decreased bleeding from fibroids; decreased growth of fibroids. When ovulation is suppressed, Hatcher emphasized, additional benefits that may occur include the following: decreased risk of functional ovarian cysts; elimination of mittleschmerz pain; decreased rick of ovarian cancer; protection against endometriosis.

Ammoniacal dermatitis. Clinical observations on an efficacious, economical and neglected treatment.

Several pediatricians in one community began instructing mothers of infants with ammoniacal diaper rash to use o-benzyl-p-chlorophenol (OBPC) in laundering diapers. This simple, previously reported item of management was prescribed in 87 cases of ammoniacal rash. In almost all uncomplicated cases the rash cleared in an average of four days; when complicated by Staphylococcus aureus or Monilia albicans infection, clearing took a few days longer. The few cases of less than satisfactory results were attributable to improper use of the chemical. Several of the mothers had mild irritation of the hands from use of OBPC.

[Anemophilous fungi: viability and allergenicity].

Three strains of anemophilous fungi (Alternaria, Rhizopus and Monilia) from different genera were studied. We found that the liquid Sabouraud and culture media used in this study lost nutrients after seven days of strain growth, without affecting their viability. This was proven when the fungi were reseeded at 15 and 60 days. Extracts from supposedly sensible patients were developed using intradermoreaction skin tests, finding no significant differences among the results obtained from these tests. We recommended further studies with more prolonged growth periods (90, 120, 180 or more days) to see whether this incides on the viability and allergenicity of the anemophilous fungi.

[Clinical manifestation of AIDS in oral and maxillofacial region (Report of a case)]

The prevalence morbidity of AIDS rises day after day. As the first symptom of this disease usually occurs in maxillofacial region,it must be well attentioned to the dentists. Here introducing a case who was a patient of AIDS. He was first see doctor in july 1995 in our Department, associating with foreign documents, the authors described all kinds of clinic symptoms of AIDS on oral and maxillofacial region in early to late stage.The typical one includes hairy leukoplakia,monilia albicans infection of angular stomatitis and mucous membrane,KS,etc.At last,the authors indicated there are some preventive methods to the disease for the dentists.

[Population studies of Exophiala jeanselmeis by sequence analysis in the ribosomal DNA].

OBJECTIVE: To study the populations of Exophiala jeanselmei strains from diverse sources using rDNA sequences. METHODS: Tested strains included 20 isolates of Exophiala jeanselmei (type culture, reference culture, clinical and environmental isolates), 8 isolates of E. moniliae, 1 isolate of Exophiala werneckii, respectively. DNA was isolated with boiled-iced water method. Ribosomal DNA and its internal transcribed spacer regions were amplified and analyzed with sequencing. RESULTS: The ribosomal genes of E. jeanselmei varied greatly. Twenty strains of this species were divided into 7 types. The genetic distances of some strains in this species were quite long. CONCLUSION: The ribosomal RNA genes and their internal transcript sequences vary more widely than those of the other species in this genus.

Exoantigen test for the rapid identification of Exophiala spinifera.

Exophiala spinifera, one of the etiologic agents of subcutaneous phaeohyphomycosis, may be wrongly identified as Exophiala jeanselmei because of the morphologic similarity of the two species. A reagent containing a precipitin specific for E. spinifera was produced by absorbing antiserum to E. spinifera with E. jeanselmei serotype 1 antigen. Using this absorbed antiserum, we were able to correctly identify isolates of E. spinifera and differentiate them from those of Exophiala alcalophila, E. jeanselmei (serotypes 1, 2 and 3), Exophiala moniliae, Exophiala pisciphila, Exophiala salmonis, Phaeoannellomyces werneckii and Wangiella dermatitidis.

Dysuria in women.

Only 46% of 84 women with dysuria were found to have significant bacteriuria while 8% had gonorrhea, 1% had herpes progenitalis and 17% had monilia and Trichomonas vaginitis. The remainder were considered to have the urethral syndrome. Neither pyuria, microscopic bacteriuria, nor any single subjective or objective datum definitively predicted cystitis at the initial visit. Therefore, we advise formulating the problem as "dysuria" at the initial evaluation even though one treats with an antimicrobial. The initial workup should include a routine urinalysis, gram stain of the urinary sediment, urine culture, an examination of the urethra and labia as well as a urethral culture. Complete pelvic examination is useful in documenting gonorrhea, herpes progenitalis or vaginitis. If the pelvic examination is deferred and the initial urine culture is negative, reevaluation of the dysuria should include a pelvic examination.
