[Degradation of cereal straw with lye].

Straw is a plentiful agricultural waste product. It cannot be used as the sole feed for ruminants and it cannot even serve to satisfy maintenance requirements. The rumen microbes are not capable of breaking down the cell walls in cereal straw rapidly enough or to a sufficient extent. To improve the utilization of straw, efforts were made to treat this material by physical, chemical and biological methods. Some of these procedures will only improve the intake of straw by the animal, whereas the primary object continues to be the improvement of digestibility. Alkali treatment of straw is the most suitable method for this purpose. The mode of action of alkali on the cell walls of straw is described in the present paper.

Characterization of serum lysosomal enzymatic activities. II. Effect of lumpy skin disease in Egyptian cows.

Clinical findings and lysosomal enzymes (LYE) in eight lumpy skin diseases (LSD) cows and same number of healthy ones were reported in Tal-El Baker village and Tal Alkabir centre, Ismailia province, Egypt. LSD began with fever, anorexia, skin lesions in form of nodules all over the body, which disappeared spontaneously or gathered to form large lumps. It was complicated with respiratory manifestation, corneal opacity, mastitis, dehydration and later on recumbency. It is noteworthy that the level of 3 LYE showed the same trend of significant reduction in acute stage of the disease (5 days after occurrence of LSD) probably due to injection of animals with a therapeutics dose of terramycin. Acid-phosphatase (ACP) enzyme is the sole that behaved very high significant increase in the serum in acute stage of LSD due to the damaged tissues caused by the virus. It underwent insignificant decrease in late stage of the disease (20 days after its occurrence) to restore the normal LYE level in control cows indicating recovery. Alpha-galactosidase (alpha-GAL) decreased perpetually by the progression of LSD because of the decreased bactericidal index which ist in concomitance with the secondary bacterial invader. N-acetyl-beta-glucosaminidase (beta-NAG) and beta-galactosidase (beta-GAL) in LYE had the same fluctuating manner. The activities showed very highly significant decrease in acute stage, followed by highly significant and significant increases (late LSD stage) respectively. The appreciable significant increase of beta-GAL may declare the effect of anorexia on LSD. In view of these findings, it can be postulated that LSD may be diagnosed and prognosed through LYE changes in the serum.

[Experimental study on the treatment of hypertension with combined traditional Chinese and Western medicine].

In order to study the connection of diastolic activity of smooth muscles of blood vessels with development of hypertension, plasma cAMP, cGMP, TXB2, 6-K-PGF1 alpha, ANP, SP were determined with radioimmunoassay, of 173 hypertension patients with Liver Yang exuberance (LYE) 91 cases, and Yin deficiency and Yang exuberance (YDYE) 82 cases. In addition, 228 health subjects served as control. The results showed that the levels of cAMP, cGMP and TXB2 in both LYE and YDYE groups were higher than those in the control group, but the levels of ANP, SP and cAMP/cGMP ratio in LYE and YDYE groups were lower than those in the control. As to the level of 6-K-PGF1 alpha, no significant variance was found between these groups. After TCM-WM treatment, the levels of cAMP, cGMP and TXB2 in LYE and YDYE groups got down, as compared with those in the control, adversely the levels of ANP, SP and 6-K-PGF1 alpha in LYE and YDYE groups turned up significantly. However the cAMP/cGMP ratio had no remarkable change between these groups. The linear regression analyses between the diastolic pressure and ANP or SP both proved negative correlation (r = -0.36, P less than 0.05; r = -0.35, P less than 0.05). The findings indicated that the TCM-WM treatment was the most effective among the therapies employed in the study, and that this therapy affected the diastolic activity of smooth muscles by modulating the above factors existing in the nervous and endocrine systems of the patients with hypertension.

[Clinical study on "liver yang exuberance syndrome"].

In order to study the nature of Liver Yang Exuberance Syndrome (LYES), and find new effective drug prescription to treat diseases with the syndrome, the authors chose 97 cases with LYES of hypertension, climacteric syndrome and hyperthyroidism. A clinical study was carried out on different diseases with the syndrome including the stagnancy of Liver Qi, excess of Liver Wind and rising of Liver Fire with a new prescription according to TCM theory. While another 83 cases with Yin Deficiency Yang Excess Syndrome (YDYES) of the same three diseases were employed as control group. The results showed that their total effective rates were 81.4% (in lowering blood pressure), 100% and 88.9% respectively. The marked effective rate of LYES of hypertension group was significantly higher than that of the control group (P less than 0.05), which showed the special characteristics of the TCM syndrome. Before and after treatment, the patients with LYES of the three diseases were examined in certain laboratory tests including function of autonomic nervous system, urinary MHPG-SO4, CA, plasma cAMP and TXB2, 6-keto-PGF1 alpha etc. The similar changes were found in different diseases with the same syndrome. The laboratory results of the cases got markedly improved with the improvement of their symptoms, which showed that these tests had active significance for comprehending the nature of LYES.

Protection against alkali injury to rabbit esophagus by CO2 inhalation.

Lye ingestion, a poisoning with no known effective treatment, frequently results in esophageal ulceration and healing by stricture formation. Tissue injury by lye is due to its alkalinity, and so therapy is logically directed at neutralization by acid. Here we describe a novel method, the inhalation of CO2, for the rapid delivery of (carbonic) acid capable of neutralizing tissue and luminal alkalinity. We also show that CO2 inhalation in anesthetized rabbits provides protection to the lye-exposed esophagus against transepithelial necrosis. This method has the potential to protect the human esophagus against lye injury, because it is relatively safe, rapidly effective, and can be administered in the field under emergency circumstances.

Caustic ingestion: controversies in management. A review of 214 cases.

Two hundred fourteen patients admitted with a history of caustic ingestion are reviewed. Sixty-five had mucosal penetrating burns. Children five years of age and under accounted for 39% of admissions, but only 8% of burns requiring treatment. Adults accounted for 48% of admissions and 81% of burns requiring treatment. Complications associated with mucosal penetrating burns occurred in 31 patients; all but one were due to lye or acids. A three year prospective study evaluating methylprednisolone in the management of caustic burns is reported. This included 24 patients with mucosal penetrating burns due to lye or acids. The results of this study, and this review as a whole, indicate that methylprednisolone is beneficial in moderately severe burns due to lye, but is not indicated for severe burns from liquid lye, or for acid burns.

Physico-chemical properties of Azizi' green pickled olives as affected by alkali process.

This work was carried out to study the effect of lye treatment at different concentrations (1.0, 2.0 and 2.5%) on some physical-chemical changes of green table olive 'Azizi' cultivar during fermentation process. Results indicated that lye treatment at 1% and 2% using sodium hydroxide were the more suitable concentration for processing green table olive 'Azizi' cultivar under Egyptian condition, where the quality in texture, colour, flavour and appearance was recorded. Statistical analysis revealed that there were significant differences in pH, NaCl content and acidity of green olive during the fermentation period as affected by lye treatment. A positive correlation was found between the sensorial properties of pickled olives.

Alpha/beta- and gamma/delta-T-cell-receptor-positive lymphocytes in healthy and inflamed human conjunctiva.

BACKGROUND: The possible existence and distribution patterns of alpha/beta- and gamma/delta-TCR+ cells, which are important constituents of immune surveillance and act via the CD3+ cell complex have not yet been elucidated in the healthy and inflamed conjunctiva. MATERIALS AND METHODS: Paraffin-embedded conjunctival specimens included 18 from 18 patients with ocular cicatricial pemphigoid (OCP), 20 from 20 healthy controls, 6 from 6 patients with lye burns, and 6 from 2 patients with Stevens-Johnson syndrome; all were worked up by histology and immunohistochemistry. RESULTS: alpha/beta-TCR+ cells were visualized in the conjunctival epithelium and stroma of healthy persons, OCP, lye burns and Stevens-Johnson syndrome. alpha/ beta-TCR+ cells and a small number of gamma/delta-TCR+ cells were observed in the corneal epithelium and stroma of patients who have failing corneal grafts. After ileal mucosa transplantation to the epibulbar conjunctiva, membrane staining changes to nuclear and cytoplasmic staining. Treatment with systemic cytotoxic drugs abolishes all alpha/beta-TCR+ and gamma/delta-TCR+ cells. CONCLUSIONS: alpha/beta-TCR+ cells can be found in the non-infected epithelium and stroma of the healthy and inflamed (OCP, lye burns, and Stevens-Johnson syndrome) conjunctiva, as well as in the corneal epithelium and stroma of failing corneal grafts, whereas gamma/delta-TCR+ cells are absent. A small number of gamma/delta-TCR+ cells are present in the corneal stroma and adjacent conjunctival epithelium of patients with chronic corneal graft rejection or after transplantation of gut tissue. Further investigations may establish the role, if any, of these T-cell subsets in immune surveillance of the non-infected outer eye and in corneal graft rejection.

Perioperative management of paediatric microstomia.

Paediatric microstomia may occur congenitally in the whistling face syndrome but is more often acquired after accidental thermal injuries such as biting an electrical extension cord or ingesting household lye. The surgical correction of microstomia includes separation and cosmetic reconstruction of the fused lips and postoperative oral splinting. Microstomia from lye ingestion may be associated not only with limited mouth opening but also with such severe intraoral scarring that common landmarks guiding either rigid or flexible fibreoptic laryngoscopy are obscured, rendering oral and nasotracheal intubation difficult or impossible. We report a case of paediatric microstomia after lye ingestion in which conventional direct laryngoscopy, flexible fibreoptic laryngoscopy, and multiple blind nasal approaches to tracheal intubation were unsuccessful. However, tracheostomy was avoided and successful tracheal intubation was accomplished using a new rigid tubular pharyngolaryngoscope.

An improved surrogate method for detecting the presence of chaos in gait.

It has been suggested that the intercycle variability present in the time series of biomechanical gait data is of chaotic nature. However, the proper methodology for the correct determination of whether intercycle fluctuations in the data are deterministic chaos or random noise has not been identified. Our goal was to evaluate the pseudoperiodic surrogation (PPS) [Small et al., 2001. Surrogate test for pseudoperiodic time series data. Physical Review Letters 87(18), 188,101-188,104], and the surrogation algorithms of Theiler et al. [1992. Testing for nonlinearity in time series: the method of surrogate data. Physica D 58(1-4), 77-94] and of Theiler and Rapp [1996. Re-examination of the evidence for low-dimensional, nonlinear structure in the human electroencephalogram. Electroencephalography and Clinical Neurophysiology 98, 213-222], to determine which is the more robust procedure for the verification of the presence of chaos in gait time series. The knee angle kinematic time series from six healthy subjects, generated from a 2-min walk, were processed with both algorithms. The Lyapunov exponent (LyE) and the approximate entropy (ApEn) were calculated from the original data and both surrogates. Paired t-tests that compared the LyE and the ApEn values revealed significant differences between both surrogated time series and the original data, indicating the presence of deterministic chaos in the original data. However, the Theiler algorithm affected the intracycle dynamics of the gait time series by changing their overall shape. This resulted in significantly higher LyE and ApEn values for the Theiler-surrogated data when compared with both the original and the PPS-generated data. Thus, the discovery of significant differences was a false positive because it was not based on differences in the intercycle dynamics but rather on the fact that the time series was of a completely different shape. The PPS algorithm, on the other hand, preserved the intracycle dynamics of the original time series, making it more suitable for the investigation of the intercycle dynamics and the identification of the presence of chaos in the gait time series.

[Upper gastrointestinal tract injury caused by ingestion of caustic substances].

BACKGROUND: To determine the distribution, extension and severity of injuries produced by the ingestion of caustic substances in the upper gastrointestinal tract, and to assess the relationship of these factors with the type of caustic substance ingested, whether intake was accidental or deliberate and the appearance of strictures or death. PATIENTS AND METHOD: Between 1993 and 1999, 138 patients (74 males and 84 females) who underwent upper gastrointestinal endoscopy in our unit within the first 24 hours after ingestion of caustic substances were included in the study. Mean age was 47 years (14-97 years). The severity of caustic injury was evaluated using the classification of Showkat et al. RESULTS: The type of caustic substance ingested was as follows: lye in 84 patients (53%), nitric acid in 21 (13%), different caustic mixtures in 43 (27%) and unknown in 10 patients (6%). Lesions were grade I in 51 patients(32%), grade II in 33 (21%) and grade III in 20 (13%). No lesions were seen in 54 patients (34%). Of the 158 patients, gastric injury was found in 91 (57.5%), esophageal burn in 72 (45.5%) and duodenal injury in 28 (18%). Ingestion was accidental in 80 patients (51%) and voluntary in 62 (39%); the reasons for ingestion were unclear in 16 patients (10%). Voluntary ingestion and acid use were associated with greater severity of lesions (p < 0.0005). Eight patients (5.0%) died, 10 patients (6.3%) required emergency surgery, and 7 (4.4%) developed esophageal-gastric strictures that required surgery or endoscopic treatment. CONCLUSIONS: The most common type of caustic substance ingested was lye. In 66% of the patients, lesions were absent or superficial. The more severe injuries, producing strictures and higher mortality, were related to voluntary and nitric acid ingestion. The part of the digestive tract most frequently affected after lye and acid ingestion was the stomach.
