Assay of serum immunoreactive trypsin in dried blood spots and the early detection of cystic fibrosis.

Immunoreactive trypsin (IRT) content in blood spots from 17 cystic fibrosis (CF) children born between 1975-80 were assayed with reagents from two commercial radioimmunoassay kits. The manufacturers' procedures were modified to allow assay of blood spot IRT. Half of the CF samples had a significantly raised IRT content compared to normal blood spots. Of the blood spots from five children whose meconium screening gave negative results, three or five (depending on the kit used) had a raised IRT content. On the other hand, less than 50% of children initially detected by meconium screening had raised blood spot IRT values. In our hands, analysis of blood spots for IRT does not consistently identify the newborn with CF and offers no advantage over other screening methods.

Three-year clinical follow-up results of intracoronary radiation therapy using a rhenium-188-diethylene-triamine-penta-acetic-acid-filled balloon system.

BACKGROUND: Intracoronary radiation therapy (IRT) prevents recurrent in-stent restenosis, but its long-term safety and efficacy remain uncertain. In the present study, the long-term clinical outcome of IRT using the rhenium-188 ((188)Re)-filled balloon system was evaluated. METHODS AND RESULTS: After successful catheter-based treatment of either a de novo or restenotic lesion, 187 patients were randomly assigned to either the radiation (N=104) or the control (N=83) group. The (188)Re-filled balloon system was designed to deliver 17.6 Gy to 1.0-mm tissue depth. Angiographic restenosis was significantly reduced with IRT at 9 months (18.9% vs 45.9%, p<0.001), but the incidence of major adverse cardiac events (MACE) including death, myocardial infarction, and target-vessel revascularization (TVR) by 3 years showed no difference. Lack of clinical benefit might be related to TVR caused by geographic miss (6/22, 28.6%), balloon-induced unhealed dissection (3/22, 13.6%) and late thrombosis (2/22, 9.1%). In the restenotic subgroup (N=39), the MACE rate within 3 years was significantly reduced with IRT (14.3% vs 54.5%, p=0.01). CONCLUSIONS: IRT using the (188)Re -filled balloon system is safe and technically feasible. Although IRT failed to show favorable outcomes for de novo lesion, the clinical benefits for restenotic lesions seem durable for 3 years. Furthermore, preventing geographic miss and dissection might improve long-term outcomes.

Isometric contraction and relaxation times of right and left ventricles in normal subjects and in patients with right ventricular overloading measured with bidirectional echocardiography.

With the use of bidirectional echocardiography, the isometric contraction (ICT) and relaxation times (IRT) of both ventricles were measured in 14 normal subjects (N), 6 cases with right ventricular (RV) diastolic overloading (DO), and 5 cases with RV systolic overloading (SO). The RVDO group consisted of patients with atrial septal defect of ostium secundum type who had large left-to-right shunting, and the RVSO group those with pulmonary hypertension of various origins. The mean ICT and IRT in N were 28.5 +/- 4.8 and 43.8 +/- 1.7 msec for RV, and 43.3 +/- 5.6 and 60.9 +/- 9.0 msec for left ventricle (LV), respectively. The RVDO group showed no significant change in the mean ICT and IRT of RV (29.7 +/- 4.6 and 54.3 +/- 11.8 msec, respectively), but significantly greater means of ICT and IRT of LV (58.5 +/- 9.5 and 83.6 +/- 14.1 msec, respectively. In the RVSO group, the mean ICT and IRT were 51.0 +/- 4.1 and 86.8 +/- 8.2 msec for RV, and 72.4 +/- 12.2 and 116.0 +/- 20.4 msec for LV, respectively. These values were all significantly greater than the means for both N and RVDO groups, except that the mean ICT of LV was insignificantly different between the RVDO and RVSO groups. It was noted that the intervals of LV tended to increase with the increasing intervals of RV, suggesting the changes in LV function secondarily due to RV overloading. It was concluded that the measurement of ICT and IRT of both ventricles is of clinical value for evaluation of overall cardiac function in the patients with RV overloading.

Cystic fibrosis newborn screening: impact on reproductive behavior and implications for genetic counseling.

OBJECTIVE: To evaluate the impact of newborn screening for cystic fibrosis (CF) on the reproductive knowledge and behavior of CF families and to determine if heterozygote detection with the immunoreactive trypsinogen (IRT) method in conjunction with DNA analysis (IRT/DNA) influences knowledge and attitudes about reproduction in false-positive families. METHODS: The Wisconsin CF Neonatal Screening Project investigated 650 340 infants from 1985 to 1994 in a comprehensive randomized controlled trial to study both benefits and risks of newborn screening and to determine if early diagnosis would improve the prognosis of children with CF. Assessments of reproductive knowledge, attitudes, and behaviors of 135 families of children diagnosed as having CF in both the early treatment group and control groups were made 3 months after diagnosis using a questionnaire which was completed by 100 families. The same questionnaire was administered 1 year later to evaluate retention of information. It was completed by 71 families. A follow-up assessment tool was also administered in 1994 and responses obtained from 73 families. Knowledge, attitudes, and behavior among false-positive families were also assessed at the time of the sweat test in 206 families who experienced IRT screening and 109 families tested with the IRT/DNA method. Follow-up assessments were completed 1 year later in 106 IRT families and 63 IRT/DNA families. RESULTS: In families with a CF child, 95% initially understood that there was a 1 in 4 risk in subsequent pregnancies, and there was good retention of this information 1 year later. At the 1994 assessment, 52% of families had not yet conceived more children, but 74% of these already had children. In the couples in whom CF was diagnosed in the first child, 70% (95% confidence interval = 54% to 85%) conceived more children. There were 43 subsequent pregnancies in 31 families. Prenatal diagnosis was used by 26% of the families (8/31) for 21% of the pregnancies (9/43). There were 3 pregnancies with CF detected, all of which were carried to term. In the false-positive groups, >95% of families initially understood that their child definitely did not have CF. There was no difference between false-positive IRT and IRT/DNA groups, and the information was retained at 1 year. Follow-up assessment 1 year after negative sweat tests revealed that 7% of the IRT and 10% of the IRT/DNA families still thought about the results often or constantly. When asked whether the experience of screening affected feelings about having more children, an affirmative response was obtained in 4% of IRT families but in 17% of IRT/DNA families. One year later, more than half of the false-positive IRT/DNA families did not understand that they were at increased risk of having a child with CF. CONCLUSIONS: We conclude that CF neonatal screening does not have a significant impact on the reproductive behavior of most families and that prenatal diagnosis is not used by the majority of CF families. IRT/DNA testing experiences seem to affect attitudes about having more children, and some parents are confused about the implications of the results, even with genetic counseling. However, persistent concerns about the sweat test result are limited. Questions raised by this study confirm the need for more research regarding the process of genetic counseling and its impact on reproductive attitudes and behavior in the newborn screening setting.

SEQUENTIAL RESPONSE EFFECTS IN THE WHITE RAT DURING CONDITIONING AND EXTINCTION ON A DRL SCHEDULE.

Sequential IRT data were obtained for three rats on a DRL 60-sec reinforcement schedule. It was found that first-order sequential dependencies exist under this schedule, including the partial dependence of the length of any given IRT on the length of the preceding IRT. The sequential analysis also served to extend the finding in the literature, based on frequency distributions, that the likelihood of a reinforced IRT is greater after a reinforced IRT than a non-reinforced IRT. Rapid extinction and reconditioning were obtained.

The microanalysis of fixed-interval responding.

The fixed-interval schedule of reinforcement is one of the more widely studied schedules in the experimental analysis of behavior and is also a common baseline for behavior pharmacology. Despite many intensive studies, the controlling variables and the pattern of behavior engendered are not well understood. The present study examined the microstructure and superstructure of the behavior engendered by a fixed-interval 5- and a fixed-interval 15-minute schedule of food reinforcement in the pigeon. Analysis of performance typical of fixed-interval responding indicated that the scalloped pattern does not result from smooth acceleration in responding, but, rather, from renewed pausing early in the interval. Individual interresponse-time (IRT) analyses provided no evidence of acceleration. There was a strong indication of alternation in shorter-longer IRTs, but these shorter-longer IRTs did not occur at random, reflecting instead a sequential dependency in successive IRTs. Furthermore, early in the interval there was a high relative frequency of short IRTs. Such a pattern of early pauses and short IRTs does not suggest behavior typical of reinforced responding as exemplified by the pattern found near the end of the interval. Thus, behavior from clearly scalloped performance can be classified into three states: postreinforcement pause, interim behavior, and terminal behavior.

Cardiac function and exercise capacity of diabetic patients.

To evaluate the relationship between cardiac function at rest and the exercise capacity of diabetic patients, left ventricular function and exercise capacity were evaluated in 15 non-insulin-dependent male diabetic patients. Isovolumic relaxation time (IRT) and the PEP/LVET ratio were obtained by simultaneous echophonocardiograms, electrocardiograms, and recordings of the carotid arterial pulse. VO2 at anaerobic threshold was obtained from a cycle ergometer exercise test with expired gas analysis. Patients were divided into two groups: those with IRT less than 90 msec (Group 1) and those with IRT greater than or equal to 90 msec (Group 2). Group 2 patients had a lower VO2 at anaerobic threshold than Group 1 (Group 1: 17.4 +/- 3.6, Group 2: 12.9 +/- 2.5 ml/min/kg; M +/- SD, p less than 0.05). There was a good correlation between the IRT at rest and delta HR, defined as the change in heart rate from rest to anaerobic threshold (r = 0.666, p less than 0.01), and between delta HR and the beat-to-beat variation in R-R interval at rest (r = 0.637, p less than 0.02). There was also a good correlation between VO2 at anaerobic threshold and IRT (r = 0.555, p less than 0.05), and between VO2 at anaerobic threshold and delta HR (r = 0.858, p less than 0.01). Our data suggests that both left ventricular diastolic function and cardiac sympathetic nervous system reflexes, reflected by delta HR, may affect the exercise capacity of diabetic patients. Thus, IRT obtained by a non-invasive method may provide a useful index for cardiovascular response to exercise in diabetic patients without coronary artery disease.

Exocrine pancreatic function in children with systemic lupus erythematosus.

We examined the incidence and spectrum of pancreatic disease in pediatric systemic lupus erythematosus (SLE). We measured serum immunoreactive cationic trypsinogen (IRT) in 185 samples obtained from 35 patients with SLE. Fifteen patients (43%) had elevated IRT levels on at least one occasion. Serum samples were obtained in 20 of 35 patients before start of treatment. Seven of these 20 patients (35%) had elevated IRT levels at diagnosis, which slowly returned to normal as their disease was controlled with treatment. A further 3 of these 20 patients in whom we had sera at diagnosis had elevated levels at some course during their illness. Of the remaining 15 patients in whom sera was not available at diagnosis, 5 patients had increased IRT levels on at least one occasion. We show that elevated IRT levels are common in pediatric SLE, but there was no apparent association with drugs such as prednisone and azathioprine. However, high levels of IRT at the time of diagnosis may be related to an underlying disease component such as vasculitis.

Is the myocardial performance index a reliable parameter in patients with restrictive filling pattern?

OBJECTIVE: The myocardial performance index (MPI) enables noninvasive estimation of combined systolic and diastolic function. The only diastolic function parameter used in the index is the isovolumic relaxation time (IRT). We assessed the impact of shortened IRT in restrictive filling pattern on MPI. METHODS: The MPI was defined as the sum of the isovolumic contraction time (ICT) and the IRT divided by the ejection time (ET), and was calculated in 13 controls (group 1), and 39 patients with ischemic heart disease with left ventricular systolic dysfunction (ejection fraction<50%). The patients were classified into two groups according to mitral early filling deceleration time (DT): group 2 with DT>140 ms (n = 20), and group 3 with DT < or =140 ms (n = 19). RESULTS: The ICT was longer and the ET was shorter in group 2 and group 3 than in group 1. The ICT and ET were not different in group 2 and group 3. The IRT was longer in group 2 (130+/-23 ms) compared to group 1 (82+/-10 ms, p<0.001) and group 3 (85+/-19 ms, p<0.001), but did not differ between group 1 and group 3. The MPI was higher in group 2 and group 3 compared to group 1 (0.79+/-0.25 and 0.65+/-0.19 vs 0.42+/-0.08, p<0.001 and p<0.001; respectively). However, it was lower in group 3 than in group 2 (p=0.03) due to significantly shortened IRT. CONCLUSION: Shortened IRT in patients with restrictive filling pattern results in reduction of the MPI. Therefore, the MPI may not reflect true level of ventricular dysfunction in these patients.

[Early diastolic dysfunction of the left ventricle affected by hypertrophy and abnormal histopathology in hypertrophic cardiomyopathy].

In this study, we investigated correlations of left ventricular hypertrophy and its histopathology with diastolic dysfunction in patients with hypertrophic cardiomyopathy. Nine control subjects and 14 hypertrophic cardiomyopathy (HCM) patients with asymmetrical septal hypertrophy were evaluated. M-mode echocardiography was used to assess fractional shortening (FS), isovolumic relaxation time (IRT), and the left ventricular filling volume index during rapid and slow filling periods and atrial contraction period (RFVI, SFVI and ACVI). End-diastolic thickness of the interventricular septum and posterior wall was determined using biventriculography. Right ventricular endomyocardial biopsies were performed to calculate the diameters of myocytes, the percentage of fibrosis and the eccentricity e which indicates the degree of myocardial disarrangement including disorganization. The FS was normal in the two groups. The IRT of the HCM group was significantly greater and the RFVI significantly less than those of the controls. The left ventricular wall thickness, the diameters of myocytes and the percentage of fibrosis in the HCM group were significantly greater; and the eccentricity e was significantly less, suggesting that myocardial disarrangement was significantly more severe than that in the controls. Significant positive correlations were observed between the IRT and the wall thickness (r = 0.647), between the diameter of myocytes (r = 0.681) and the percentage of fibrosis (r = 0.628), and there was a significantly negative correlation between the IRT and the eccentricity e (r = -0.759). There was a significantly negative correlation between the RFVI and the wall thickness (r = -0.663); and a significantly positive correlation between the RFVI and the eccentricity e (r = 0.579). Multiple regression analyses showed that the diameter of myocytes, the percentage of fibrosis and the eccentricity e all correlated significantly with the IRT (R = 0.821) and the RFVI (R =0.604). The standard regression coefficients of the diameter of myocytes, the percentage of fibrosis and the eccentricity e were 0.253, 0.278 and -0.431 in respect to IRT, and those of the percentage of fibrosis and the eccentricity e were -0.204 and 0.469 in respect to RFVI, respectively. These results indicated that diastolic dysfunction in hypertrophic cardiomyopathy is related not only to the degree of left ventricular hypertrophy, but also to the degree of myocardial hypertrophy, increased interstitial fibrosis, and especially to myocardial disarrangement including disorganization.

Newborn screening for cystic fibrosis is complicated by age-related decline in immunoreactive trypsinogen levels.

Detection of elevated levels of immunoreactive trypsinogen (IRT) in dried neonatal blood spots has been used as a screening test for cystic fibrosis. In other cystic fibrosis newborn-screening studies, a sweat chloride test is generally performed only if an infant has a persistent IRT level above a selected cutoff value on both the initial and subsequent specimens. Neither the timing of the second specimen nor the value of the cutoff point for the second specimen has been comprehensively evaluated. In this randomized, controlled study, 145,024 infants were screened in the neonatal period for cystic fibrosis using the 99.8 percentile (180 ng/mL) as the neonatal cutoff point. A total of 129 infants had elevated neonatal IRT levels and had negative results on sweat tests (false-positive by IRT screening). A total of 54 children with cystic fibrosis were identified in the screened and comparison groups. Excluding patients with meconium ileus, 4 infants with cystic fibrosis had neonatal IRT values less than 180 ng/mL, and an additional 9 infants with cystic fibrosis had values decline to less than 180 ng/mL within the first 2 1/2 months of age. The IRT values of infants with and without cystic fibrosis overlapped considerably beyond 30 days of age. These findings suggest that further refinement of cystic fibrosis screening methodology will be necessary to achieve an acceptable sensitivity and specificity.

The effects of diisopropyl phosphorofluoridate (DFP) on inter-response time and circadian patterns of lever-pressing in rats.

Rats were injected with saline, 1.0 mg/kg or 2.6 mg/kg of diisopropyl phosphorofluoridate (DFP). Three days later the animals were placed in cages in which they could press a lever to obtain their entire daily ration of food. The time of day at which responses occurred and the time between successive responses were recorded over a six day period to determine the circadian pattern of lever-pressing and the distribution of inter-response times (IRTs). The saline injected rats exhibited a normal nocturnal pattern of feeding, while both DFP treated groups exhibited a significantly greater tendency to eat during the day. Analysis of the IRT distributions of the three groups showed a different pattern of results. The saline and 1.0 mg/kg DFP groups produced nearly identical IRT distributions, while the 2.6 mg/kg group produced an IRT distribution which was marked by significant increases in the interquartile range and median IRT. Since the 1.0 mg/kg dose of DFP produced a circadian disruption but did not affect the IRT distribution, it appears that the disruption of circadian activity by DFP which was reported by Raslear and Kaufman cannot be solely explained by simple changes in the motor response.

Serum immunoreactive trypsin after secretin stimulation in chronic pancreatitis.

Serum immunoreactive trypsin (IRT) response to secretin injection was studied in 13 patients with chronic pancreatitis with different degrees of exocrine dysfunction and in 10 control subjects. The maximal increase of serum IRT from basal values and the integrated trypsin output (ITO) after secretin administration were significantly correlated with the output of chymotrypsin into the duodenum during caerulein-secretin infusion (p < 0.01), but not with the output of lipase nor of bicarbonate. Serum IRT response to secretin stimulation was greater in 4 of the 5 patients with chronic pancreatitis with mild to moderate exocrine dysfunction than in the control group, suggesting an increased regurgitation of IRT into the blood stream by the pancreas, probably due to some degree of obstruction to pancreatic secretory flow in absence of severe acinar cell damage. Conversely, the response of serum IRT after secretin administration in 7 of the 8 patients with severe exocrine pancreatic deficiency was lower than in control subjects, probably because of the advanced distruction of the acinar pancreatic tissue. The response of serum IRT to secretin stimulation seems to vary following pancreatic function impairment and might reflect the degree of pancreatic exocrine dysfunction in chronic pancreatitis.

[Screening of newborn infants for cystic fibrosis. A combined analysis of immunoreactive trypsin and delta F508 mutation--a screening without false positive results].

A total of 1081 blood screening cards taken from newborn babies, were anonymously selected for cystic fibrosis (CF) screening by quantitation of immunoreactive trypsin (IRT, Delfia) and by delta F508 mutational analyses using polymerase chain reaction followed by a time resolved fluorescence hybridization assay (Delfia). The IRT values showed a log normal distribution and were significantly higher in girls than boys and in 28 carriers compared with 1052 normals. In 12 newborns, corresponding to 1.02%, an IRT concentration greater than 70 micrograms/l was found. One of these was a delta F508 homozygote with an IRT concentration of 380 micrograms/l. delta F508 mutational analyses showed 1052 normals, 28 heterozygotes, and one homozygote, i.e., a carrier frequency of this mutation for delta F508 of 1:39. In future newborn CF-screening programmes we therefore recommend Delfia IRT followed by Delfia delta F508 analyses for IRT values greater than 70 micrograms/l.

The application of PCR amplification and the polymorphic marker KM.19 to dried blood spots: comparison with deletion 508 for the confirmation of the neonatal screening test for cystic fibrosis.

Cystic fibrosis (CF) screening by measurement of immunoreactive trypsin (IRT) lacks specificity: only 9% of hypertrypsinemic neonates have CF. We have studied retrospectively 114 hypertrypsinemic samples (including 37 CF) for KM.19 polymorphic DNA marker and made risk calculations. If the neonate is homozygous for KM.19 allele 2, the risk of CF rises to 55%; if homozygous for allele 1, the risk is very low (less than 1%) and if heterozygous, the risk is intermediate (4%). In a prospective study including 28,000 IRT tests, 76 neonates with IRT greater than 800 micrograms/L have been identified: 16 were homozygous for allele 2 (8 CF), 30 for allele 1 (1 CF), and 30 were heterozygotes (no CF). Deletion 508 was present in 10 neonates: 4 homozygotes (4 CF) and 6 heterozygotes (3 CF). Two CF did not carry any copy of deletion 508. We have studied 181 (presumably non-CF) neonates with IRT greater than 600 micrograms/L. The KM.19 genotypes distribution is significantly different from the one expected in the French population: homozygotes for allele 2 are more numerous. Furthermore, heterozygotes for deletion 508 are 1 in 15 (expected: 1 in 42). In conclusion, molecular biology in dried blood spots can enhance the specificity of CF neonatal screening, but IRT and genotype may not be independent.

Impaired acquisition of temporal differentiation performance following lesions of the ascending 5-hydroxytryptaminergic pathways.

Nineteen rats received injections of 5,7-dihydroxytryptamine into the dorsal and median raphe nuclei; 16 rats received sham injections. The rats underwent 50 daily training sessions under an interresponse-time-greater-than-15-seconds (IRT greater than 15 s) schedule of sucrose reinforcement. The lesioned group showed impaired acquisition of temporal differentiation, in that their response rates remained significantly higher and their obtained reinforcement frequencies significantly lower than those of the control (sham-lesioned) group. Comparison of the IRT frequency distributions obtained from the two groups during the last 5 days of training showed that the lesioned group produced a significantly higher proportion of very short IRTs (less than 3 s) than the control group; when these short IRTs were disregarded, the lesioned group displayed a significantly lower mean IRT and a significantly higher coefficient of variation than the control group. The levels of 5-hydroxytryptamine (5HT) and 5-hydroxyindoleacetic acid in the parietal cortex, hippocampus, amygdala, nucleus accumbens and hypothalamus were markedly reduced in the lesioned group, but the levels of noradrenaline and dopamine were not significantly affected by the lesion. The results suggest that destruction of the ascending 5HTergic pathways may reduce animals' capacity to inhibit positively reinforced operant behaviour, and may impair temporal discrimination.

Effects of different food-reinforcement histories on cocaine self-administration by rhesus monkeys.

The purpose of the present study was to examine whether a history of responding under food reinforcement schedules that generated either high or low response rates would influence the acquisition and maintenance of cocaine self-administration. Eight experimentally naive rhesus monkeys were initially trained to respond on the right lever under either a fixed-ratio (FR) 50 or interresponse times (IRT) > 30-s schedule of food reinforcement. After 65 sessions of food-maintained responding, monkeys were surgically prepared with indwelling intravenous catheters and cocaine 0.03 mg/kg per injection (IV) was available on the left lever under a fixed-interval (FI) 5-min schedule. After at least 60 consecutive sessions at this dose, a cocaine dose-response curve (saline, 0.01-0.3 mg/kg per injection) was determined. The FR 50 schedule generated high rates of food-maintained responding (90.1 +/- 6.2 responses/min), while response rates under the IRT > 30-s schedule were low (1.9 +/- 0.1 responses/min). Across the 60 consecutive sessions under the FI 5-min schedule, linear changes in response rates and cocaine intake were significantly different between FR- and IRT-history monkeys. FR-history monkeys responded at higher rates than IRT-history subjects, while cocaine intake during the first 15 sessions was lower in FR- compared to IRT-history monkeys. Rates of cocaine-maintained responding after food-reinforcement histories were compared to response rates of monkeys initially trained to self-administer cocaine under an FI 5-min schedule (Nader and Reboussin 1994).(ABSTRACT TRUNCATED AT 250 WORDS)

Trypsinogen expression by two human pancreatic cell lines CFPAC-1 and CAPAN-1. Modulation during spontaneous and induced cell growth.

We previously demonstrated that two human pancreatic adenocarcinoma cell lines, CFPAC-1 (established from a patient with cystic fibrosis) and CAPAN-1, were able to secrete trypsinogens 1 and 2 specifically. In order to analyze the relation of trypsin secretion to differentiation and cell growth, we undertook a comparative study of immunoreactive trypsin 1 (IRT) secretion by the two cell lines during cell growth in the presence and in the absence of various differentiating agents: sodium butyrate (NaBut), dimethylsulfoxide (DMSO), and dexamethasone (DX). In the presence of NaBut, IRT levels in the supernatants of both cell lines were slightly increased, whereas the cellular growth of both cell lines decreased significantly. In the presence of DX, IRT levels in cell culture conditioned media immediately and dramatically decreased, but the cell growth of neither cell line was affected by DX. An important increase in IRT levels was observed when CFPAC-1 cells and CAPAN-1 cells were grown in the presence of DMSO, but for both cell lines the cellular growth decreased in the presence of DMSO. Our data show that neither the IRT secretion level nor the differentiation state of these cell lines correlates with cellular growth, and suggests that the expression of pancreatic proteases by these two tumor cell lines could be either related to a common stem cell with this potential or to a possible acinar origin of pancreatic cancer, as recently proposed by others.
