Lower respiratory tract foreign bodies: a retrospective review of morbidity, mortality and first aid management.

OBJECTIVES: The authors describe 28 pediatric cases of foreign body inhalation requiring treatment in intensive care units between 1987 and 1999. The purpose of this study was: (1) to analyze the circumstances, diagnostic difficulties and initial treatment of serious foreign bodies and (2) to compare our series with other literature descriptions and define principles for optimal prevention and initial treatment. METHODS: Twenty-one children presented a penetration syndrome, which was responsible in 13 cases for asphyxia with cardiorespiratory arrest. All these children died, regardless of the initial treatment. Seven children were hospitalized for apparent asthmatic symptoms that did not respond to traditional treatment. RESULTS: The inefficiency of external extraction methods like the Heimlich maneuver and the mean delay between clinical signs and initial treatment lead us to propose a new strategy for the emergency treatment of foreign bodies with asphyxia. CONCLUSIONS: We recommend that emergency teams promote the use of a laryngoscope and Magill forceps. Flexible endoscopy is still recommended as the appropriate diagnostic tool to eliminate doubt in the case of a first severe asthma attack.

Health care and the global community.

Catholic health systems and facilities, to fulfill their commitment to the healing ministry of Jesus Christ, are called to serve society's most poor and vulnerable people. This calling applies to people in need not only locally but also internationally. Individuals and organizations providing aid in foreign lands will face many challenges. Yet if they believe that they can use their expertise to benefit people in need, they can accomplish their goals. Some guiding principles are instructive for those undertaking international outreach efforts: Partners must commit themselves to a common mission; the effort must focus on empowering aid recipients; participants in the effort-donors and recipients alike-can experience transformation; and health and well-being must be central to the work. The Catholic Consortium for International Health Services (CCIHS) has experience in following these principles to bring about change. The consortium can help ministry organizations achieve their international outreach goals.

Integration and immigration pressures in western Europe.

"With xenophobia resurgent in Europe, this article addresses some of the complexities of immigration and integration in EC [European Community] and EFTA [European Free Trade Association] countries. The categories of legal and illegal immigrants (including estimates thereof) are first described, as are the necessary ingredients of an integration policy (a framework law, a secure environment, cultural tolerance, demarginalization in housing and the labour market). The author then considers what types of action would help to ease the present immigration pressure, discussing in turn quota policies, project-tied or training migration and, finally, the use of international aid."

Urbanisation: an economic overview of some of the issues.

"The paper presents a discussion of the economic justification for taking cities seriously, a section outlining the scale of urbanisation and some of the issues raised, a section that discusses the policy approaches, one that looks at the local agencies for the formulation and execution of policy, and a short final part on some of the implications for aid policy." The focus is on the importance of the urban sector for economic development in developing countries.
