Ionic flux across canine duodenum. Movement of bicarbonate.

Effect of isosmotic biocarbonate-rich solutions on the Brunner's gland area were studied by means of proximal duodenal explants in eight anesthetized dogs. Volume gain occurred with all instillates and varied indirectly with the concentration of the bicarbonate in the instilled solutions. Net bicarbonate gain occurred with instillate of 40 mEq./1.; net insorption occurred with instillates of higher bicarbonate concentration, the rate of which was dependent on the instillate concentration. Insorption of chloride occurred with the lower bicarbonate solutions and enterosorption occurred at higher concentrations. The modification of intraluminal bicarbonate by the proximal duodenal mucosa is important in the regulation of intraduodenal pH and abnormal function of this mechanism may play a role in the pathogenesis of duodenal ulceration because of incomplete hydrochloric acid neutralization.

[Experimental substantiation of intestinal sorption in acute pancreatitis].

Enterosorption on the carbon material Vaulen was performed in experiments in albino rats with the model of acute pancreatitis. The application of enterosorbents was shown to increase the percentage of survival of the animals, to decrease the activity of pancrease enzymes in the blood, components of the Kallikrein-Kinin system, proteolysis products. The clinical observations support the expediency of using this detoxication method.

[Clinico-immunologic analysis of sorption methods of treatment of rheumatoid arthritis and systemic lupus erythematosus].

The paper is concerned with the results of the application of the sorption methods for the treatment of patients with rheumatoid arthritis and systemic lupus erythematosus. The authors show the advisability of the use of the above methods for different variants of the disease course, define the time of the treatment and complications common to the sorption methods of the treatment. The results of the treatment by hemoperfusion and enterosorption are compared with the results of the patients' treatment with immunodepressants. A study was made of the action of the sorption methods and immunodepressants on the blood content of the main autoantigens and autoantibodies capable of participation in the forming of the immune complexes. The concentration and the qualitative composition of the circulating immune complexes were studied in the course of the treatment.

[Incidence of pulmonary tuberculosis associated with chronic alcoholism and a complex treatment of those diseases].

It was shown that the prevalence of alcoholism among pulmonary tuberculosis patients was high and the outcomes of both the diseases proceeding in the presence of lowered immunity were often unfavourable, including the results of long-term follow-ups. The use of a specially constructed instrument for investigating the neurodynamic characteristics of the cerebral cortex and determination of gamma-glutamyl transpeptidase in the serum promoted diagnosis of prenosological entities of alcoholism. Higher efficacy of alcoholism treatment in the patients with tuberculosis was achieved with narcotic psychotherapy using the mixture of nitrous oxide and oxygen, esperal implantation and application of rifusal, a new preparation, as well as application of extracorporeal hemosorption and enterosorption for eliminating the intoxication. Higher efficacy of tuberculosis treatment in the alcoholic patients was achieved with intravenous drop-wise infusion of antituberculous drugs along with other routes of administration. For increasing the cellular immunity, the patients were treated with chlorophylliptum or T-activin. Continuation of the complex antialcoholic and antituberculous treatment of outpatients along with simultaneous observation by phthisiologists and narcologists provided higher results.

[The structural bases of liver cell aging and age-related characteristics of their reactivity].

The liver of the 8-, 24-, 30- and 38-month-old Wistar male rats were studied morphometrically in the light and electron microscope under normal conditions and after bleeding and enterosorption. Under both conditions--ageing and bleeding--the hepatocyte death (apoptosis and colliquative necrosis) occurs resulting from the progression of granular and hydropic degeneration. Apoptosis develops mainly in the 1st zone and necrosis in the 3rd zone of the liver acinus this being due to the pattern of blood supply. The hepatocyte death through the apoptosis is followed by immunocompetent cell response. Compensatory processes in ageing are manifested in the hepatocyte hypertrophy this being due to the hyperplasia of cytoplasmatic structures, smooth cytoplasmic network particularly.

[Levels of the C 4 component of the complement, lactoferrin and leukocytic thermostabile alpha glycoprotein during the treatment of patients with systemic rheumatic diseases].

The authors studied the dynamics of C4-component of complement (C4), lactoferrin (LF), leukocytic thermostable alpha-glycoprotein (LT alpha G) in the blood serum of 54 patients with rheumatoid arthritis, 32 patients with systemic lupus erythematosus, 8 patients with systemic vasculitis under the effect of hemosorption (HS), enterosorption (ES) and also immunosuppressants (IS). In severe forms of systemic rheumatic diseases the blood serum exhibits a decreased content of C4 but an elevated level of LF and LT alpha G. It has been established that HS is more active than ES or IS, and it influences the mechanisms of humoral immunity. This finds its expression in normalization of C4 level. The favourable clinical effect of HS correlates with a decrease in the level of LF and LT alpha G in the blood serum of patients during treatment.

[Endotoxemia in severe mechanical trauma and the sorption methods of its treatment].

Different methods of sorption detoxication used at the early postshock period in 249 patients with severe mechanical injuries and experimental investigations in 144 rabbits have shown the early sorption detoxication to be a pathogenetically justified method in the complex treatment of critical patients. The application sorption and, under clinical conditions, enterosorption reduce the endogenous intoxication, normalize the parameters of homeostasis, lessen the amount of complications and lethality. The authors do not recommend using hemosorption in critical patients at the early post-shock period.

[Characteristics of barbiturate adsorption on activated charcoal].

Equilibration patterns of barbiturates adsorption on activated charcoals were studied using aqueous solutions of barbital and sodium barbital and commercially available charcoals BAU, AR-3, AR-B, AR-A, SKT-6A. The rate of barbiturates adsorption was described as an additive function of concentrations of amide, ionized and non-ionized imidol forms calculated considering the reactions of dissociation and hydrolysis. Relatively low equilibrium concentrations of barbiturates in the solid phase and simultaneous sorption of the barbital anion as well as of its non-ionized form were related to the considerable rate of the substance ionization due to both tautomeric transformation and dissociation of the acid formed. The procedure for calculation of equilibrium described assumes that the rate of barbital and sodium barbital adsorption on activated charcoals of various brands might be estimated at various pH values of a solution, based on dissociation and hydrolysis reactions, which is of importance under conditions of the barbiturates enterosorption.

[Role of middle mass molecules in the pathogenesis of endotoxicosis in peritonitis].

Under examination there were 107 patients with diffuse peritonitis in its different phases. The relationship of clinical manifestations of endotoxicosis and middle mass molecules in the blood was established. One of the causes of the development of endotoxic shock in peritonitis is found to be an instant release of highly toxic substances (middle mass molecules included) from the microcirculation bed. The most effective means of detoxication of the organism include: intraoperative sanitation of the abdominal cavity with the elimination of the infection source, drainage of the small intestine with enterosorption, forced diuresis, hemo- and lymphosorption.

[Sorption detoxication of the body in a system of complex therapy of cancer patients].

Hemosorption and enterosorption were employed in 83 and 15 patients, respectively, as a component of complex treatment for tumors of various sites (mainly, hemoblastosis). The procedures were intended to control tumor intoxication, cytostatics-induced myelodepression and complications of chemoradiation and operative treatment. Hemosorption showed the best results in combatting chemotherapy- and radiation-induced toxic complications and when used for preparation for surgery. An apparent clinical response and improvement in patients condition as evidenced by laboratory tests permitted to step up antitumor therapy without deviating from standard dosage schedules.

[Enteral detoxication of patients with peritonitis and acute intestinal obstruction].

Detoxicating therapy is necessary to patients with diffuse purulent peritonitis and acute ileus. One of the simplest methods of detoxication is an intraintestinal administration of sorbents, Enterodesis in particular. The application of this drug reduced toxicity of blood serum controlled by protozoa which facilitated rapid recovery of the intestine function. Most pronounced effect of enterosorption was observed in cases with acute ileus. In more severe cases the dosage of the medicine was greater without visible sequellae in the patients. The effect of detoxication was positive as a whole.

Toxicity of cecal filtrates from guinea pigs with penicillin-associated colitis.

Lethal enterocolitis was induced in guinea pigs by intramuscular injection of penicillin. The presence of filterable toxic substance in the cecal contents of penicillin-treated guinea pigs was demonstrated. The toxin was heat-labile and produced enterosorption activity in rabbit intestinal loops and morphologic changes in Y-1 adrenal cell cultures. The cecal filtrates were lethal for mice when injected intraperitoneally and lethal for guinea pigs when injected intraperitoneally or intracecally. Cecal filtrates from saline-treated guinea pigs were not lethal to experimental animals. Filtrates prepared from broth cultures of cecal contents from penicillin or saline-treated guinea pigs also showed the presence of a lethal toxic substance. The study demonstrated that bacteremia is minimal, nonspecific and probably a terminal event in penicillin-associated deaths in guinea pigs.

[Combined treatment of local suppuration in patients with diabetes mellitus].

The results of observation of 132 patients with local purulent inflammation elapsing on the diabetes mellitus background, treated with the help of detoxication (hemosorption, endolymphatic therapy, UV blood irradiation, enterosorption) methods were analyzed. Local treatment using these methods was conducted in the abacterial environment and with the inhibitor trielin application. This gave the possibility to correct effectively the endotoxicosis, secondary immunodeficiency, to prevent the sepsis origin and to improve significantly the results of patients treatment.
