Physiologic adaptation changes of the "Bering Bridge" expedition participants.

Cardiorespiratory measurements were repeatedly performed on indigenous and Caucasian Soviet and American male team members of the Bering Bridge expedition which covered approximately 1280 km over a 61 day period. Significant baseline differences in cardiorespiratory functions at rest and during submaximal cycle ergometer exercise were noted between racial groups. Indigenous members had significantly lower mean vital lung capacity and oxygen consumption levels during submaximal exercise, but a greater systolic blood pressure response to exercise. The energy cost during submaximal work decreased during the trek in both groups, but was marked in the Caucasians, suggesting that the indigenous subjects were better adapted at baseline to the Arctic environment. Overall, there was stabilization of functional responses in four of seven parameters studied, although differences between racial groups remained the same.

Psychological functioning among members of a small polar expedition.

BACKGROUND: While depressed mood, insomnia, irritability and impaired cognition represent common responses to the physical and psychosocial stressors associated with polar environments, wide variations exist in their expression and the degree to which they adversely affect the health and performance of polar expeditioners. In particular, the process of successful adaptation to polar environments and the psychosocial characteristics associated with this process remains poorly understood. HYPOTHESIS: Psychosocial characteristics associated with successful coping with typical stressors are also associated with successful adaptation in polar environments. METHODS: The 4 men and 3 women participating in a 3-week scientific expedition in the Canadian High Arctic completed a battery of psychological questionnaires, including the Profile of Mood States (POMS), prior to their departure to Isachsen, N.W.T. In Isachsen, subjects completed the POMS and the Structured Interview Guide for the Hamilton Depression Rating Scale--Seasonal Affective Disorders Version (SIGH-SAD) each week. RESULTS: Good psychological adjustment was demonstrated by a significant decline in POMS factor scores for tension-anxiety (p = 0.005), fatigue (p < 0.0001), and confusion (p = 0.024) from baseline to Week 3, and a significant decline in SIGH-SAD depressive symptoms (p < 0.0001) during Weeks 1-3. This is attributed to high levels of paratelic dominance and low levels of neuroticism, and use of planful problem-solving as a coping strategy more frequently than other coping strategies. CONCLUSIONS: Improved psychological functioning among polar expeditioners reflects a combination of psychosocial characteristics that facilitate successful adaptation to any stressful experience, as well as characteristics specifically adaptive for living in polar environments.

The effect of prolonged isolation from environmental allergens on the clinical and laboratory manifestations of the allergic state. Observation on members of the South African Antarctic expedition.

The 1975 South African Antarctic expedition included 8 men with a history of allergy, 7 who showed cutaneous hypersensitivity to common allergens, but denied having experienced symptoms, and 6 who had neither symptoms nor positive skin tests. During a year of residence in the essentially allergen-free, barren environment of Antarctica, allergic subjects were entirely sypmtom-free. Prolonged isolation from environmental allergens did not consistently diminish the serum concentration of total or specific IgE or the intensity of positive skin test reactions, which indicates that the cells responsible for the synthesis and secretion of reaginic antibodies have a prolonged existence and do not require repeated allergenic stimulation for maintenance of function.

[Expedition, experiment and expertise reflected in Robert Koch's assets].

Basic techniques in bacteriology provided the foundation for the success of experimental medicine. The combination of laboratory experiments and field studies consolidated bacteriology as a paradigm for the development of preventive strategies. Robert Koch's (1843-1910) contribution to this development is crucial in that he integrated the technical facilities of his era into microbiological research. The present study introduces various sources of Koch's assets which, in addition to manuscripts and letters, include also bacteriological slides and photographs. These relicts allow to reconstruct an example of the scientific process as a whole, to investigate his style and purpose as a scientist, and to follow Koch's strategy of visualisation with contemporary technical equipment. The example chosen is Koch's last expedition when the German government sent him to investigate the spreading of sleeping-sickness in Eastern Africa and to test "Atoxyl" as a specific remedy. Most case-studies and laboratory-research took place on the Sese-Islands in Lake Victoria and illustrate the extreme conditions in tropical Africa. The experiments searching for the etiological agent of sleeping-sickness--a typical disease only for central Africa--involved bacteriological slides which were to be the basis for further experimental results and the expertise expected of him.

Comb-type cationic copolymer expedites DNA strand exchange while stabilizing DNA duplex.

The accelerating effect of cationic substances on the DNA strand exchange reaction between a 20 bp DNA duplex and its complementary single strand was studied. A polycationic comb-type copolymer, that consists of a poly(L-lysine) backbone and a dextran graft chain (PLL-g-Dex) and known to stabilize triplex DNA, expedites the strand exchange reaction under physiological relevant conditions. Electrostatically a small excess of the copolymer let to a 300-1500-fold increase in the DNA strand exchange while large excess of spermine or cetyltrimethylammonium bromide, a cationic detergent known to promote markedly hybridization of complementary DNA strands, shows only a slight effect. The efficacy of the copolymer was not affected by a 10 mM Mg2+ concentration. Notably the copolymer promotes the strand exchange reaction while it stabilizes double-stranded DNA. The stabilization of strand exchange intermediates consisting of the parent duplex and the single strand by the copolymer is believed to be responsible for the observed acceleration behavior.

A biodegradable copolymer for the slow release of growth hormone expedites scarring in diabetic rats.

In many diseases wound healing is impaired. This study was designed to establish whether the healing process in diabetes could be improved using a site-specific polymer delivery system containing hGH. The system was first optimized in in vitro experiments performed on cultured fibroblasts taken from healthy and diabetic rats and then tested in an incisional wound model created in the diabetic Wistar rat. In the in vitro experiments using cultured fibroblasts, cell viability, growth, and proliferation were determined, along with polymer degradation, hormone release rates and the expression of TGFbeta1 in the culture medium. For the in vivo experiments, polymer discs with/without GH were inserted through 3 cm incisions made on the backs of the animals. Wound specimens were obtained 7 and 30 days after surgery to evaluate inflammatory/apoptotic cells, metalloprotease expression and neoangiogenesis using microscopy and immunohistochemical techniques. The local administration of GH using a polymer delivery system did not affect the normal wound healing process. Conversely, when used in diabetic animals, epidermal and dermal repair was expedited. Our findings indicate that GH induces cell proliferation, enhances CD4(+) infiltration; increases extracellular matrix protein deposition; stimulates angiogenesis; and diminishes apoptosis at the diabetic wound site. These effects give rise to a comparable wound healing process to that observed in healthy animals.

Onset of pro-thyrotropin-releasing hormone gene expression in cultured rat anterior pituitary cells is expedited by dexamethasone.

The early onset of proTRH gene expression in anterior pituitary (AP) cells in culture and its regulation by dexamethasone (DEX) were investigated. AP cells derived from 15-day-old rats were cultured for up to 4 days in the presence or absence of 10(-7) M DEX. TRH peptide levels, which could be detected only after 3 days of culture in control cells, were detectable after 1 day in DEX-treated cells. Levels rose from undetectable (< 35 fmol/well/0.2 x 10(6) cells) to 121 +/- 11 fmol/well in control cells and from 59 +/- 3 to 2978 +/- 88 fmol/well in DEX-treated cells (Day 1 to Day 4; means +/- SEM, n = 6). ProTRH mRNA levels as analyzed by in situ hybridization showed an excellent correlation with TRH peptide levels: mRNA was already detectable on Day 1 in DEX-treated cells and on Days 2-3 in control cells. DEX stimulated proTRH mRNA levels as determined by Northern blot analysis within 4 h. The half-life of proTRH mRNA was calculated based on a first-order decay model by measuring mRNA levels after addition of 5 micrograms/ml actinomycin D with or without DEX. The t1/2 of proTRH mRNA in control cells was 13.1 +/- 2.8 h and was not influenced by DEX treatment (12.5 +/- 2.8 h). Since DEX stimulated proTRH mRNA levels acutely without any increase in mRNA stability, we propose that DEX expedites proTRH gene expression in our AP cell culture system by acting at the transcriptional level.

Changes in plasma lipids and lipoprotein cholesterol during a high altitude mountaineering expedition (4800 m).

Effects of high altitude exposure on plasma lipids and lipoprotein cholesterol were studied in 8 mountaineers who spent 3 weeks at the Annapurna IV base camp (4800 m) after a 12 day trek. In spite of the moderate physical exertion at the camp, the loss of body weight was more pronounced during the stay at high altitude than during the trekking period. Compared with baseline values observed at sea level, marked reductions in plasma cholesterol (-27%) and phospholipids (-19%) were found 3 days after arrival at the camp and persisted during the next 17 days. A less marked fall in plasma triglycerides occurred, weakly significant at the end of the stay. Because there were no relevant changes in very low density lipoproteins or in high density lipoprotein (HDL)-cholesterol, the low plasma cholesterol levels at the high altitude resulted mainly from the reduction in low density lipoprotein (LDL)-cholesterol: the mean HDL/LDL cholesterol ratio changed from 0.39 at sea level to 0.63 at the end of the stay at 4800 m. Fluctuations in LDL-cholesterol were not concomitant with those in body weight and were independent of the exercise training during the expedition. This study shows moreover that the early drop in LDL-cholesterol was associated with an opposite change in plasma levels of catecholamines and thyroid hormones. Taking into account that such hormonal responses are classically observed at high altitude, the concomitant decrease in LDL-cholesterol might be interpreted as being a relevant adaptative response to hypoxic conditions at high altitude.

Extracellular pH defense against lactic acid in normoxia and hypoxia before and after a Himalayan expedition.

The extracellular pH defense against the lactic acidosis resulting from exercise can be estimated from the ratios -delta[La].delta pH-1 (where delta[La] is change in lactic acid concentration and delta pH is change in pH) and delta[HCO3-].delta pH-1 (where delta[HCO3-] is change in bicarbonate concentration) in blood plasma. The difference between -delta[La].delta pH-1 and delta[HCO3-].delta pH-1 yields the capacity of available non-bicarbonate buffers (mainly hemoglobin). In turn, delta[HCO3-].delta pH-1 can be separated into a pure bicarbonate buffering (as calculated at constant carbon dioxide tension) and a hyperventilation effect. These quantities were measured in 12 mountaineers during incremental exercise tests before, and 7-8 days (group 1) or 11-12 days (group 2) after their return from a Himalayan expedition (2800-7600 m altitude) under conditions of normoxia and acute hypoxia. In normoxia -delta[La].delta pH-1 amounted to [mean (SEM)] 92 (6) mmol.l-1 before altitude, of which 19 (4), 48 (1) and 25 (3) mmol.l-1 were due to hyperventilation, bicarbonate and non-bicarbonate buffering, respectively. After altitude -delta[La].delta pH-1 was increased to 128 (12) mmol.l-1 (P < 0.01) in group 1 and decreased to 72 (5) mmol.l-1 in group 2 (P < 0.05), resulting mainly from apparent large changes of non-bicarbonate buffer capacity, which amounted to 49 (14) mmol.l-1 in group 1 and to 10 (2) mmol.l-1 in group 2. In acute hypoxia the apparent increase in non-bicarbonate buffers of group 1 was even larger [140 (18) mmol.l-1]. Since the hemoglobin mass was only modestly elevated after descent, other factors must play a role. It is proposed here that the transport of La- and H+ across cell membranes is differently influenced by high-altitude acclimatization.

Carbon dioxide storage and nonbicarbonate buffering in the human body before and after an Himalayan expedition.

Before and 7-12 days after an Himalayan expedition CO2 equilibration curves were determined in the blood plasma of 12 mountaineers by in vitro and in vivo CO2 titration; in vivo osmolality changes (delta Osm x deltaPCO2(-1), deltaOsm x delta pH(-1), where PCO2 is the partial pressure of CO2) during the latter experiments yielded estimates of whole body CO2 storage. In vitro -delta[HCO3-] x delta pH(-1) [nonbicarbonate buffer capacity (beta) of blood] was increased 7 days after descent [before 31.3 (SEM 0.4) mmol x kgH2O(-1), after 38.3 (SEM 3.9) mmol x kgH2O(-1); P<0.05] resulting from an increased proportion of young erythrocytes; in additional experiments an augmented beta was found in young (low density cells) compared to old cells [<1.097 g x ml(-1): 0.216 (SEM 0.028) mmol x gHb(-1), >1.100 g x ml(-1): 0.145 (SEM 0.013) mmol x gHb(-1), where Hb is haemoglobin; P < 0.02]. In spite of increased Hb mass in vivo delta[CO2total] x deltaPCO2(-1) [0.192 (SEM 0.010) mmol x kgH2O(-1) x mmHg(-1)] and -delta[HCO3-] x delta pH(-1) [17.9 (SEM 1.0) mmol x kgH2O(-1)] as indicators of extracellular beta rose only slightly after altitude (7 days +16%, P<0.02; +7%, NS) because of haemodilution. The deltaOsm x deltaPCO2(-1) [0.230 (SEM 0.015) mosmol x kgH2O(-1) x mmHg(-1)] remained unchanged. Prealtitude differences in deltaOsm x delta pH(-1) between hypercapnia [-41 (SEM 5) mosmol x kgH2O(-1)] and hypocapnia [-20 (SEM 3) mosmol x kgH2O(-1); P<0.01] disappeared temporarily after return since the former slope was reduced. The high value during hypercapnia before ascent probably resulted from mechanisms stabilizing intracellular pH during moderate hypercapnia which were attenuated after descent.

Determination of PAH in atmospheric particulates in the area of the Italian base in Antarctica: report on monitoring activities during the last three scientific expeditions.

Concentrations of polycyclic aromatic hydrocarbons (PAH) in atmospheric particulates have been determined in the frame of the control activities for environmental protection during the VI (1990-1991), VII (1991-1992) and VIII (1992-1993) Italian expeditions in Antarctica. Samples were collected at the four cardinal points at about 200 m from the central area of the Italian scientific base. Data show low levels of the individual PAH (95% in the range 1-50 pg m(-3)) confirming the absence, up to now, of significant contamination from the base. These results could be used as a reference for the assessment of the air pollution due to combustion sources present in the base. Any possible variation in the base's activities in the future should be analysed and evaluated with respect to these findings.

Radiation risk of the crew members of the expeditions on the "MIR" station during the 22nd solar activity cycle.

The radiation risk at the end of the flight was calculated for the members of the main expeditions on the "Mir" station. It was based on the absorbed dose dynamics data measured by the board dosimeter. The radiation damage models created for standards of the radiation safety of the space flights were used in the calculations. The analysis of the obtained values of the risk and its dynamics for some cosmonauts are presented in the topic. The risk values delta P are close to the limited levels given by equation of delta P = 0.6 x 10 x T(-4), [this equation appears also as delta RHrad = 0.6 x 10(-4) x T later in the text] where T--is flight duration in months.

Vascular endothelial growth factor and matrix metalloproteinase-2 expedite formation of endometriosis in the early stage ICR mouse model.

OBJECTIVE: To establish a mouse model for endometriosis and to evaluate roles of vascular endothelial growth factor (VEGF) and matrix metalloproteinase-2 (MMP-2) in the formation of disease. DESIGN: Experimental laboratory study. SETTING: A women's hospital in China. PATIENT(S) AND ANIMAL(S): Ten women with endometriosis and 10 control women, as well as ICR mice. INTERVENTION(S): Endometrial fragments were transplanted in the peritoneal cavities of mice at minilaparotomy. Transplants were observed and then removed for the assessment of morphology and immunohistochemical staining of VEGF and MMP-2. MAIN OUTCOME MEASURE(S): Observation of transplants, expression of VEGF and MMP-2. RESULT(S): On days 1 and 2, glandular and stromal cells were viable at the margins of transplants. On day 3, the transplants were surrounded by mesothelial cells, and the endometrial glands and stromal cells were clearly viable at the interface. The scores of VEGF and MMP-2 of viable glandular cells of transplants were increased compared with the ones before transplantation. The scores of VEGF and MMP-2 of transplants from women with endometriosis were higher than those of control women. CONCLUSION(S): Endometrial transplants from the patients with endometriosis express more VEGF and MMP-2 than endometrium in control women, suggesting that VEGF and MMP-2 may expedite the formation of endometriosis in its early stage.

Radioactive contamination from dumped nuclear waste in the Kara Sea--results from the joint Russian-Norwegian expeditions in 1992-1994.

Russian-Norwegian expeditions to the Kara Sea and to dumping sites in the fjords of Novaya Zemlya have taken place annually since 1992. In the fjords, dumped objects were localised with sonar and ROV equipped with underwater camera. Enhanced levels of 137Cs, 60Co, 90Sr and 239,240Pu in sediments close to dumped containers in the Abrosimov and Stepovogo fjords demonstrated that leaching from dumped material has taken place. The contamination was inhomogeneously distributed and radioactive particles were identified in the upper 10 cm of the sediments. 137Cs was strongly associated with sediments, while 90Sr was more mobile. The contamination was less pronounced in the areas where objects presumed to be reactor compartments were located. The enhanced level of radionuclides observed in sediments close to the submarine in Stepovogo fjord in 1993 could, however, not be confirmed in 1994. Otherwise, traces of 60Co in sediments were observed in the close vicinity of all localised objects. Thus, the general level of radionuclides in waters, sediments and biota in the fjords is, somewhat higher or similar to that of the open Kara Sea, i.e. significantly lower than in other adjacent marine systems (e.g. Irish Sea, Baltic Sea, North Sea). The main sources contributing to radioactive contamination were global fallout from atmospheric nuclear weapon tests, river transport from Ob and Yenisey, marine transport of discharges from Sellafield, UK and fallout from Chernobyl. Thus, the radiological impact to man and the arctic environment of the observed leakages from dumped radioactive waste today, is considered to be low. Assuming all radionuclides are released from the waste, preliminary assessments indicate a collective dose to the world population of less than 50 man Sv.

[Analysis of logistical and organizational aspects of a cruise for patients with chronic respiratory insufficiency. The RESpIRA Expedition and the COPD Cruise].

Patients with chronic respiratory insufficiency who are receiving domiciliary oxygen therapy and mechanical ventilation report great difficulty in taking complex trips involving several destinations and prolonged stays away from home. Such patients share a common need for home equipment whose technology is relatively sophisticated, a condition that limits their freedom of movement. We are referring to systems for delivering oxygen therapy and mechanical respirators. Given that such patients have problems traveling by air, we hypothesized that a cruise would be an ideal alternative, given that travel would take place in the hotel itself. A cruise would facilitate the logistics of the journey, given that the equipment would have to be set up at only one setting. Working with these assumptions, we have thus far organized two cruises for chronic respiratory insufficiency patients: the "RESpIRA Expedition" and the "COPD Cruise". Our experience shows that the organizational problems to be coped with are patient recruitment, financing and choice of itinerary. With those aspects clear, organizers must then obtain the authorization of the cruise operator, including the approval of the medical and safety personnel on board. After obtaining permission for the cruise and as soon as the organizers know how many patients will travel, a list of oxygen therapy equipment (respirators and disposable supplies) must be compiled. Finally, equipment suppliers must be found. Afterwards, all that remains is to enjoy the trip. The participation of physicians responsible for domiciliary oxygen therapy and mechanical ventilation programs is essential for making patients feel safe and for assuring solutions for technical and medical problems that might arise.

Dosimetry and microdosimetry characteristics measured on board the MIR station during the 28th basic expedition.

Three types of detectors were used onboard the MIR station during the 28th base expeditions to characterise the radiation field: a linear energy transfer (LET) spectrometer was used to establish the LET spectrum between 7 and 700 keV/micrometers corresponding mostly to secondary charged particles; a set of thermoluminescent detectors was used to characterise the low LET component of the onboard radiation field; and Si-diodes were installed to determine the contribution to the exposure due to fast neutrons. It was found out that the LET spectrum from secondary particles between 7 and 700 KeV/micrometers does not depend on the external radiator; the average quality factors for the region mentioned are about 6.0 with ICRP 26 quality factors and about 6.8 with ICRP 60 quality factors. Both differential and integral LET spectra are presented for some typical cases, not only for particle number but also for the dose characteristics like dose and dose equivalent. The spectra obtained also permitted us to calculate the total doses and dose equivalents due to secondary particles with the LET values between 7 and 700 keV/micrometers. It was found out that these quantities are higher for the case of detectors placed in the less shielded area, both for the LET spectrometer (high LET part) as well as for TLDs measuring the low LET component. Total dosimetric characteristics obtained as a sum of both components mentioned are a little lower than previously reported.
