[Noninvasive diagnosis of pulmonary hypertension in aged and elderly patients with chronic pulmonary heart disease].
Pulmonary hypertension was diagnosed in 97 elderly patients (age 60-79) using electrokymography of the heart and vessels, ECG and rheography of the lungs. All the patients suffered from chronic cor pulmonale (CCP). Electrokymography proved an effective diagnostic tool in detection of pulmonary hypertension in CCP patients, whereas quantitation of the hypertension was successful at electrokymography with application of L. Burstin nomogram and electrocardiography according to S. A. Dushanin. ECG according to R. Bernar et al. appeared poorly sensitive, especially at early CCP stages. A. M. Novikov's rheopulmonography technique can provide only an approximate value of mean pressure in the pulmonary artery of elderly CCP patients.
Degenerative progressive myoclonic epilepsy electrokymographic observations.
Clinical and pathological findings in two cases of degenerative progressive myoclonic epilepsy (PME) are described. The clinically difficult task of differentiating a "cerebellar" tremor from an action myoclonus is emphasized. Simultaneous electroencephalography and electrokymography was done, using capacity to ground transients for recording hand movements. This method was found useful in corroborating the cerebellar nature of the remaining disorder, after successful treatment of the myoclonic element with anticonvulsants.
Assessment of left ventricular function by noninvasive methods.
The possibility of evaluating left ventricular function by noninvasive methods is discussed in detail. The methods that are considered are electrocardiograph, phonocardiography, apex cardiography, sphygmography, impedance cardiography, electrokymography, and echocardiography. Following a brief section of 'definitions', each method is described in detail including technical problems, difficulties, and results. The systolic time intervals and the stress tests are briefly discussed. Based on modern experimental studies, the stress test should include both an electro- and a phonocardiogram. In the latter, one would measure the amplitude of the first heart sound as an index of contractility. The conclusion is that combined methods give the best results. They are electrocardiography, phonocardiography, impedance cardiography, and echocardiography. An alternative, dictated by technical problems, is to use at first phonocardiography and impedance plus electrocardiography; then echocardiography plus electrocardiography; and then, if indicated, a stress test might complete the study; the latter should include both an electrocardiogram and a phonocardiogram.
[Changes in microcirculation and respiration in acute intra-abdominal pathology according to electrokymographic data].
Electrokymography was applied for studies of pulmonary circulation and ventilation in 31 patients. A decrease of pulmonary circulation by 23.3% and lung ventilation reduction by 15.6% were found. The disturbance of circulation and ventilation ensues from the changes of microcirculation in acute intraabdominal pathology.
[Age-related changes in the elastic properties of large arterial vessels (author's transl)].
228 healthy subjects aged 20 to 100 were examined. The elasticity of the large arterial vessels was assessed by measuring the pulse rate. Methods of arterial sphygmography were applied for studying the vessels of the systemic circulation, while electrokymography was used for the pulmonary circulation. The examination results showed that the elasticity of the large arterial vessels diminishes in the process of ageing while their rigidity increases. This is primarily due to morphological changes in the vascular wall - i.e. the development of sclerosis. The age-dependent changes are unevenly distributed. They are more pronounced in the arteries of the systemic circulation than in the system of the pulmonary artery. The reduced elasticity of the arterial vessels greatly affect cardiac output and also reduces the general adaptability of the circulatory system.
[Unit for the registration of cinekymograms].
The principle underlying the action of basic subassemblies of the appliance for registration of kymographic curves from roentgenocinemafilms is described. A comparison of the cinemakymographic method with that of electrokymography is given and advantages of the proposed method are indicated.
[Determination of the systolic time interval in normal subjects from electrokymographic studies of vessels near the heart].
By means of electrokymography curves of the motion of the vascular margin of the large vessels near the heart, aorta and pulmonary artery, may be recorded noninvasively. Since these curves of motion widely correspond to the central curves of the arterial pulse it is possible to state systolic time intervals separately for the right and left heart. Analogically to the presphygmic index after Rentsch with global evidence on the left-ventricular function of the heart these are possible for the outflow way of the right ventricle, also by formation of the quotient of the electrokymographically established STI. Comparisons between right-ventricular and left-ventricular STI and relations to the at the same time electrokymographically stated dynamic parameters of the heart wall are the result. In the present paper on the basis of electrokymographical investigations on 24 test persons with healthy heart the concerning the contents reasonable STI with relatively slight statistical distribution and their quotient are determined as starting-point to further investigations.
[Effect of treatment with cardiac glycosides and biologically active substances on the state of the lesser circulation according to the electrokymographic data].
Electrokymography was used to assess by pulmonary densograms and pulmonary artery pulse curves treatment-induced changes in cardiac contractility and lesser circulation in 455 patients with rheumatic heart disease and stage IIA, IIB, and III circulatory insufficiency. Treatment with cardiac glycosides in combination with metabolic preparations (ATP methionine, vitamin B12, inosine-F, and extract of the heart muscle) leads to a more conspicuous reduction in the symptoms of congestion in the lesser circulation than that encountered in treatment with cardiac glycosides alone. The reduction in these symptoms following treatment with ATP and heart-muscle extract may be explained not only by improved myocardial contractility but also by the direct vasodilating effect of adenyl compounds.
