In an earlier study using the visually guided pointing task (VGPT) the authors showed that the timing of imagined movement sequences in children with developmental coordination disorder (DCD) does not conform to the conventional speed-for-accuracy trade-off (or Fitts' law [P.M. Fitts, Journal of Experimental Psychology 47 (1954) 381-391]) that occurs when the distance and accuracy requirements of movements are varied [P. Maruff, P.H. Wilson, M. Trebilcock, J. Currie, Neuropsychologia 37 (1999b) 1317-1324]. The present study sought to replicate this earlier finding and to examine (using a weight manipulation) whether this deficit was also attributable to inaccurate programming of relative force. The chronometry of real and imagined movements was investigated in a group of 20 children with DCD aged between 8 and 12 years and a group of controls matched on age and verbal IQ (VIQ). Movement duration was tested for real and imagined movements using the preferred hand, with the VGPT performed under two load conditions: with and without the addition of a weight attached to a pen. Group means of each subjects' mean movement duration were calculated and plotted against target width for each of the four conditions [Movement type (2) x Load (2)] and a logarithmic curve was fitted to the data points. In the control group, the speed-for-accuracy trade-off for both real and imagined performance conformed to Fitts' law under each load condition. In the DCD group only real movements conformed to Fitts' law. Moreover, the effect of load differed between groups--for real movements, movement duration did not differ between load and no-load conditions for either group, while for imagined movements, movement duration increased under the load condition for the control group only. These results replicate and extend the results of our earlier study. This pattern of performance suggests that children with DCD have an impairment in the ability to generate internal representations of volitional movements which may reflect an impaired ability to process efference copy signals. The ability to programme both relative force and timing appears to underly this difficulty. Results have implications for the use of (guided) motor imagery training in order to facilitate the development of motor skill in children with DCD.
This study proposes a strategy to generate new anticancer therapy using hydrogel-based drug delivery systems to improve drug bioavailability and increase the therapeutic efficacy. We have synthesized biodegradable hydrogels based on N-(2-hydroxypropyl)methacrylamide (HPMA) with prolonged drug release. Pharmacokinetic data from in vitro studies showed that the in vitro release of hydrophilic drugs (doxorubicin, vinblastine) from HPMA-hydrogels is affected mainly by drug diffusion and only partially by hydrogel degradation. The release of hydrophobic drugs (cyclosporine A, CsA) actually copies the process of degradation and therefore it is slower. Hydrogels with degradation time of 50 h released the doxorubicin over a period of at least 96 h after s.c. implantation. Drug concentration at pharmacologically active levels was maintained in the bloodstream over a period of at least 4 days, ranging between 0.1 and 1 microg/ml. The therapeutic potential of HPMA-hydrogels in vivo was studied in Bcl1 leukemia. HPMA-hydrogels containing DOX were significantly more effective in inhibition of Bcl1 leukemia in comparison with free DOX or non-targeted polymeric drug (PK1). The efficacy of therapeutic combination using unspecific, hydrogel-based therapy with specific, antibody-targeted therapy at late stages of Bcl1 leukemia was also tested. In contrast to application of DOX alone, a cocktail of DOX with CsA as a blocker of P-glycoprotein (Pgp) incorporated into HPMA-hydrogel blocked the proliferation of Pgp-overexpressing multidrug resistant cell lines in vitro by induction of apoptosis.
The National Health Service (NHS) Plan for England has directed that from April 2004 clinicians will offer patients the opportunity to receive copies of letters that are written about them. Patients like to have more information and patients who have received copies of letters have found them useful. It is hoped that copying letters will improve relationships between doctors and patients, encourage patients to be better informed, and improve the quality of information provided to patients. Relatively little empirical research has been performed in this area but what exists is generally supportive. Attention will need to be paid to issues of confidentiality, the language and content of letters, and individuals who may have difficulty obtaining information from letters. This initiative is one of many that the NHS has introduced to enhance openness, honesty and the quality of information provided to patients.

Stability of the mandible was analysed with a Xerox copy of the dental cast in 35 cases of mandibular prognathism corrected by ramus osteotomies. The occlusal relationship in the Xerox copy was illustrated by superimposing two contact points on the upper and lower dental arches and the change in the position of the lower dental arch in relation to the upper dental arch was obtained by superimposing the latter on preoperative, predicted postoperative, and six months postoperative occlusions. The stability of the mandible was analysed by measuring the movement of five landmarks (two posterior, two molar and one incisor points) set around the lower dental arch to represent the movement of the mandible. The relapse was estimated by the movements of the landmarks from the predicted postoperative occlusion to the six months postoperative occlusion. The mean estimated anterior relapses at the posterior and molar points with the larger predicted movement and the incisor point were between 0.9 mm. and 2.0 mm. at six months postoperatively, whereas on the side with the smaller predicted movement, the points moved posteriorly by 0.6 mm. and 0.5 mm. Despite the fact that the amount of the predicted lateral movement was much smaller than that of the predicted posterior movement at operation, the posterior points were estimated to relapse laterally by 2.0 mm. and 1.7 mm.(ABSTRACT TRUNCATED AT 250 WORDS)

Description and evaluation of a drug information centre's (DIC) professional activities one year after establishment are summarized. The aims of the centre are: to evaluate which problem areas are frequently requested, how frequently and by whom; which information sources are the most appropriate for the service and the approximate cost of such a service. We received 80 questions during the first 8 months. The questions were mainly asked by hospital physicians (32.5%) and pharmacists (32.5%), other health-care professionals were a minority; the scopes of most frequently asked questions include general properties of active substances (12.2%), contents of particular preparations (21%), dosage (11%), side effects (21%) and indications/contraindications (15%). Cost of the service included variable (mainly phone and mail charges, copy and print of the documents) and fixed costs (cost of software and salary of the staff).
Imitation was studied experimentally by allowing chimpanzees (Pan troglodytes) to observe alternative patterns of actions for opening a specially designed "artificial fruit." Like problematic foods primates deal with naturally, with the test fruit several defenses had to be removed to gain access to an edible core, but the sequential order and method of defense removal could be systematically varied. Each subject repeatedly observed 1 of 2 alternative techniques for removing each defense and 1 of 2 alternative sequential patterns of defense removal. Imitation of sequential organization emerged after repeated cycles of demonstration and attempts at opening the fruit. Imitation in chimpanzees may thus have some power to produce cultural convergence, counter to the supposition that individual learning processes corrupt copied actions. Imitation of sequential organization was accompanied by imitation of some aspects of the techniques that made up the sequence.

The duplication technique is a well recognised method of replacing complete dentures. Advantages claimed for this technique are that patients adapt more easily to their new complete dentures and the numbers of treatment visits are reduced. The case notes of 100 patients were examined retrospectively, 50 of whom had had dentures constructed by undergraduates using a duplication technique and 50 with a conventional technique. In addition, a questionnaire was sent to the patients to obtain their views on the replacement dentures. The 'duplication group' required fewer visits for the delivery of the dentures, but there was no statistical difference between the numbers of post-insertion visits, for the two groups, needed to make the dentures comfortable. The only identifiable reason for selecting a duplication technique was the failure of construction of new dentures by a conventional method in 15 patients. Apart from this the two groups appeared remarkably similar when comparing age of dentures being replaced, period of edentulousness, number of previous complete dentures and medical histories. As judged by the questionnaire more patients in the duplication group continued to wear their new dentures compared to the conventional group.
