[Marijuana--2000].

Marihuana (Cannabis sativa, the hemp plant) is one of the most widely used illicit drugs all over the world. Cannabis products are usually smoked. The plant contains chemicals called cannabinoids. One of these, 1-delta-9-tetrahydro-cannabinol (THC) is believed be responsible for most of the characteristic psychoactive (euphoria) and cardiovascular (tachycardia, conjuctivitis) effects. Although some clinical studies suggest the medical utility of marihuana (i.e. on the basis of its antiemetic, anticonvulsive and analgesic effect)--the scientific evidence is weak. Therefore the complete legalization of the drug is strongly opposed.

Evaluation of risk from space radiation with high-energy heavy ion beams.

The most challenging radiation in space consists of fully ionized atomic elements with high energy for which only the few lowest energy ions can be stopped in shielding materials. The health risk from exposure to these ions and their secondary radiations generated in shield materials is poorly understood since there are few human data and a systematic study in relevant animal model systems has not been made. The accuracy of risk prediction is described as the major limiting factor in the management of space radiation risk. The expected impact of systematic studies is examined using the limited available biological data and models. Given the limitations of current predictions, models must be developed that are able to incorporate the required fundamental scientific data into accurate risk estimates. The important radiation components that can be provided for laboratory testing are identified. The use of ground-based accelerator beams to simulate space radiation is explained and quantitative scientific constraints on such facilities are derived. Three facilities, one each in the United States, in Germany and in Japan, currently have the partial capability to satisfy these constraints. A facility has been proposed using the Brookhaven National Laboratory Booster Synchrotron in the United States; in conjuction with other on-site accelerators, it will be able to provide the full range of heavy ion beams and energies required.

Asymptomatic bacteriuria, bacteremia, and other infections due to NSU corynebacteria.

By means of the new medium, nonspecific urethritis (NSU) chocolate agar, NSU corymebacteria were isolated from patients with asymptomatic bacteriuria, bacteremia, cervicitis, conjuctivitis, and pericarditis, and also with bone marrow, wound, and cul-de-sac infections. The NSU corynebacteria were considered the etiologic agents. On the basis of biochemical reactions, antibiotic sensitivity, and complement fixation tests some isolates were the same microorganisms. Both patients with conjunctivitis were infected with the same NSU corynebacteria. A second isolate was cultured from patients with osteomyelitis and cervicitis, while a third was recovered from an infected leg wound and from a patient with pericarditis. Seven of the isolates, when injected into rabbits hypersensitive to four NSU corynebacteria isolated from the inflamed epididymis of patients with epididymitis, elicited delayed hypersensitivity reactions, which indicated that they also were related antigenically. It is suggested that nonspecific urethritis and eididymitis may represent an infection with NSU corynebacteria, or may be an extension of bacteriuria due to these microorganisms, with a delayed hypersensitivity reaction as a possible additional complication. Colony counts on NSU chocolate agar of the bacteria in urines from male and female patients were higher than those obtained on conventional agar media. NSU chocolate agar is superior to other agar media for the isolation of pathogenic and saprophytic bacteria not only from the urogenital tract but also from other foci of infection. It is easily prepared from commercial blood agar plates and its use should be considered when a selective medium is not required.

Cytokines in the generation and maturation of dendritic cells: recent advances.

Dendritic cells (DCs) are extremely efficient antigen presenting cells (APCs) that are potent stimulators of both T and B cell-mediated immune responses. Although DCs are normally present in very small numbers in the peripheral blood (PB), recent advances have made it possible to generate relatively large numbers of cells in culture. DCs can be differentiated in vitro from various cellular sources, including bone marrow (BM), cord blood (CB) and PB mononuclear cells (PBMCs). Although a wide variety of conditions have been reported to be able to support DC generation, the majority of research and clinical protocols to date differentiate DCs from precursors using granulocyte-macrophage colony stimulating factor (GM-CSF) in combination with either tumor necrosis factor-(TNF-)alpha or interleukin (IL)-4. However, a diverse array of cytokines has been shown to be able to induce DC differentiation under a variety of conditions. According to recent reports, cytokines such as IL-2, IL-6, IL-7, IL-13, IL-15 and hepatocyte growth factor (HGF), in combination or even, in some cases, alone, can contribute to the generation of DCs from either monocytes or CD34+ cells. Although the majority of cytokine combinations include GM-CSF, some do not. For example, Flt3 ligand (FL), in conjuction with IL-6 (in the absence of GM-CSF), has been reported to be able to induce DC differentiation from BM cells in a murine system. Other agents can play a dual role in DC activity. CD40 ligand (CD40L), as a single agent, has been shown to be able to generate DCs from PB monocytes, while numerous other reports have also demonstrated its role as a potent maturation factor. In contrast, for other cytokines such as IL-16 or IL-17, although there is no data for a role in DC generation, they have been reported to be involved in promoting DC maturation in vitro as defined by upregulation of costimulatory molecules, major histocompatibility complex (MHC) antigens and antigen presenting/T lymphocyte stimulatory capacity. Furthermore, cytokines such as stem cell factor (SCF) and FL have been shown to dramatically enhance in vivo DC recovery. The wide variety of cytokines and conditions that have been shown to be able to influence DC differentiation and activity to amply demonstrate the extreme heterogeneity found in the DC population, something that is reflected in the diverse phenotypes, functions and ontogeny displayed by DCs. This diversity may account for the large number of roles that have been attributed to DCs in the development and function of the immune system and, in turn, emphasizes the potential as well as the challenges of modifying specific aspects of the immune response system by manipulating specific DC subpopulations.

[Psychiatric manifestations of systemic sclerosis].

Systemic sclerosis (SSc) is a rare conjuctive tissue disorder that is characterized by fibrosis of the skin and internal organs as well as vascular obliteration phenomenons. SSc is responsible for increased morbidity and mortality, and skin fibrosis may be responsible for major body changes. These phenomenons may contribute to the occurrence of psychological disturbances such as anxiety and depression, whereas the incidence of psychotic symptoms is very low in SSc patients. However, the prevalence of psychiatric symptoms during the course of SSc has not been evaluated yet with standardized evaluation scales, and an important breakthrough can be made in understanding the underlying mechanisms of psychiatric manifestations, in order to improve therapeutic management and quality of life.

The structural basis of the philtrum and the contour of the vermilion border: a study of the musculature of the upper lip.

The course and insertion pattern of muscle fibres in the philtrum of the human upper lip were studied in seven post-mortem specimens using serial histological sections and a reconstruction method using sheets of Plexiglas. The fibres of the musculus orbicularis oris entering the upper lip from one side were observed to decussate in the midline and proceed to insert into the skin of the opposite side lateral to the philtral groove. The philtral ridges appeared to represent the medial borders of bilateral muscle insertion zones of the lip in which the fibres of the m. orbicularis oris raised the level of the skin by splaying out and inserting into it. Contributions to philtral form also came from the musculus levator labii superioris as it descended as far medially as the philtral ridge to insert into the vermilion border lateral to the median groove, and from fibres of the musculus nasalis which inserted into the philtral ridges superiorly. The philtral groove corresponded to the more compact median decussation of the orbicularis oris where lip thickness was also reduced by the relative absence of muscle fibre insertions into the skin. The displacment of the vermilion border giving rise to Cupid's bow appeared to result from the lifting action of the m. levator labii superioris lateral to the median groove in conjuction with a depressor action by the m. orbicularis oris on the median tubercle.

[Characteristics of modern diagnosis of diseases caused by Chlamydia bacteria].

Chlamidia psitacci, Chlamidia trachomatis, Chlamidia pneumoniae are intracellular bacterial parasites causing ornithosis, trachoma, conjuctivitis, urogenital, respiratory infections, arthritis, Reiter's disease. The role is discussed of the above microorganisms in the development of cardiological, neurological disorders. The past years have seen further progress in microbiological diagnosis but information now available on, in the first place, Chlamidia pneum., is scarce. In most clinical studies they use a serological diagnosis which is not without faults, with the cultural method and polymerization chain reaction being most informative diagnostic tools. The diagnostic value of the above-mentioned reaction will, we believe, increase with its improvement. In the preliminary diagnosis, cytomorphological methods can be made use of.

[Particular aspects of rhinitis and rhino-conjunctivitis].

The authors report their experience of 28 years of practice in the opthalmology service of the CHRU at Rennes. The describe certain particular aspects of rhinitis and rhino-conjuctivitis where the danger resides in an extensive risk to the uvula or the bronchi. They insist on the importance of understanding polyvalents in internal medicine (rôle of the seat of gingivodentary "ideas", value of research into indicative markers of protection A2 and B40 in the HLA system. In the region of nutrition, they emphasize the importance of the rôle of zinc, of vitamin C and of magnesium. They base their hopes on study of the local markers of sensitivity, in order to assess the elements of preventative action for an eventual ocular and bronchial extension.

Ocular manifestations in paediatric HIV/AIDS patients in Mulago Hospital, Uganda.

BACKGROUND: In Uganda the prevalence of HIV averages 12% as was reported to the STD/AIDS control surveillance unit. In Uganda there are approximately 30,000 HIV infected infants per year. The burden of HIV disease is high in Uganda and patients present with ocular complications. However, there is paucity of information and knowledge concerning ocular manifestations in the paediatric HIV/AIDS population and how they may differ from those of adults. OBJECTIVES: To describe the ocular manifestations of HIV/AIDS infection in an African paediatric population. Generally the study will record the external ocular manifestations seen but specifically to document the intra-ocular lesions, in particular the retinal changes associated with paediatric HIV/AIDS. DESIGN: A cross-sectional hospital based study. SETTING: The study was conducted at the Paediatric Infectious Disease Clinic at Upper Mulago Hospital, in Kampala, Uganda. PATIENTS: Patients are those with positive HIV sero status, with or without symptoms and signs of AIDS. Parents/caretakers of the children were interviewed to obtain the socio-demographic data of the patients and a general physical as well as an ophthalmic examination were conducted to document any ocular problems. RESULTS: A total of 158 HIV -infected children were examined. The overall rate of ophthalmic involvement was 35%. The most common finding was a non-purulent conjuctivitis, observed in 12% of the patients, followed by perivasculitis of the peripheral retinal vessels, in 12% of patients and molluscum contagiosum.

Prophylaxis of ophthalmia neonatorum: comparison of silver nitrate, tetracycline, erythromycin and no prophylaxis.

From November, 1989, to October, 1991, 4544 neonates were born at our hospital. Neonatal ocular prophylaxis immediately after birth was used with 1% tetracycline ophthalmic ointment in 1156 neonates, 0.5% erythromycin ophthalmic ointment in 1163 neonates and 1% silver nitrate drops in 1082 neonates. No prophylaxis for neonatal conjunctivitis was given to 1143 neonates. A total of 302 infants (6.7%) developed conjunctivitis during the first 4 weeks of life. Between December, 1991, and January, 1992, 425 neonates were born at our hospital and all were given 0.5% erythromycin ophthalmic ointment twice in the first 24 hours after birth for ocular prophylaxis. Thirty-one (7.3%) infants developed conjunctivitis during the neonatal period. The incidence rates of neonatal chlamydial conjuctivitis in the tetracycline, erythromycin, silver nitrate, no prophylaxis and erythromycin twice groups were 1.3, 1.5, 1.7, 1.6 and 1.4%, respectively. We conclude that neonatal ocular prophylaxis with erythromycin (one or two doses) or tetracycline or silver nitrate does not significantly reduce the incidence of neonatal chlamydial conjunctivitis compared with that in those given no prophylaxis.

The disturbance of oxidative phosphorylation in rat liver mitochondria by the carcinogens 12-hydroxystearic acid and its methyl ester.

In a search to determine common molecular features in potential carcinogenic dietary fatty acids, Swern et al. (1970) found that 12-hydroxystearic acid and its methyl ester were carcinogenic. This was surprising to Swern et al. (1970) and Van Duuren et al. (1972), as it was not readily apparent how such agents could be converted to alkylating agents. We have observed that the carcinogens 12-hydroxystearic acid and its methyl ester disrupted oxidative phosphorylation in rat liver mitochondria. The in vitro mitochondrial effects induced by the agents included: a) uncoupled respiration, b) ATPase activity, c) energized volume changes linked either to respiration or ATP. Mitochondria mediated the enzymic conversion of the ester to the acid. Conversion was retarded by the thiol reagent showdomycin. These data in conjuction with our previous reports dealing with other carcinogens and their derivatives contribute to an experimental confluence between oxidative phosphorylation and chemical carcinogenesis.

[Neutrophins in biology and medicine].

Nerve growth factor NGF was the first neurotrophic substance to be identified, isolated, and characterized. NGF is member of a class of substances which has been termed the neurotrophins familly. This family of NGF-related trophic factors includes also brain-derived neurotrophic factor--BDNF, neurotrophin-3, neurotrophin-4/5 and neurotrophin-6 and 7. Each neurotrophin is able to bind to the p75NTR and a specyfic Trk tyrosine kinase receptor. Traditional view is that trophic factors are relesed from target cells, retrogradely transported along their axons, and rapidly degraded upon arrival in cell bodies. But several trophic factors can move anterogradely along axons. NGF-responsive cells are known to belong to the hemopoetic-immune system and to populations in the brain involved in neuroendocrine function. The concentration of NGF is elevated in a number of inflammatory and autoimmune states in conjuction with increased accumulation of mast cells. Neurotrophins can also regulate adult nervous system plasticity by promoting neuronal survival and stimulating nerve regrowth following injury. The potential exists for these neurotrophic factors to be used as therapeutic agents for the treatment neurodegenerative disorders.

Carotid-cavernous sinus fistula after trauma to the head.

The similarities between the various conditions described are remarkable. The orbital or frontal bruit and pulsating exophthalmos are common only to the carotid-cavernous fistula. A recent history of dental treatment, of facial infection with an elevated white blood cell count, and leukocytosis suggests cavernous sinus thrombosis. The superior orbital fissure syndrome and orbital apex syndrome occur most frequently in conjuction with Le Fort II and III fractures, which can involve the optic foramen and superior orbital fissure, therefore disrupting the sensory and motor innervation to the orbit and adjacent structures (Fig 4). Direct orbital trauma without evidence of fracture and with all symptoms mentioned, except infection, bruit, and pulsating exophthalmos, suggests orbital hematoma. A thorough differential diagnosis with complete data is essential for a correct diagnosis, thereby reducing morbidity by institution of the appropriate treatment. Treatments for carotid cavernous fistula have varied over the years--all aiming toward abolition of the fistula. Currently, the preferred treatment is the supraclinoid clamping of the internal carotid artery, followed by muscle embolization of the internal carotid artery distal to the bifurcation, and clamping of the internal carotid artery in the neck.

[Action of various surface-active substances on the genetic transmission of R-plasmids in E. coli].

The effect of 4 groups of surface active-substances (SAS) on conjugation and transduction transfer of plasmid RIdrd from the donor of E. coli J 5-3 to the recipient of E. coli C600 was studied. It was shown that the anionic SAS (8 compounds) had the highest inhibitory effect. They completely inhibited both the conjugation and the transduction transfer of R-plasmids. The amphoteric SAS, i. e. tego-51 and ampholan had the same effect only on the transduction R-transfer. The conjugation R-transfer was inhibited by the above compounds to a significant extent but not completely, i. e. by 70.8 and 361.9 times respectively. The effect of the non-ionogenic preparations, i. e. sintanol DT-7 was even less (the inhibition coefficients of 54.8 and 22.8 on conjuction and transduction respectively). The cationic SAS, i. e. catamine AB and HIPC-200 had no effect on the genetic R-transfer. It was shown that the anionic SAS had a phagocytic effect on phage Plks. It is of interest to note that the anionic SAS, i. e. sintanol NP-3 effectively suppressed the conjugation transfer even in a concentration of 100 gamma/ml which was 1000 times lower than the subbacteriostatic one. The effectiveness of the anionic SAS did not depend on the length of the dydrocarbon chain and was the same in separate homologues and their mixtures. The high inhibitory activity of most of the SAS and especially the anionic ones with respect to both the conjugation and the transduction R-transfer was of interest.

Scanning electron microscopy of vertebrate cerebellar cortex.

In this study, the Golgi method for light microscopy, transmission and conventional scanning electron microscopy, the ethanol-cryofracturing technique, the freeze-fracture method for SEM, and the freeze-etching method have been used in conjuction to analyze the three-dimensional cytoarchitectonic arrangement and intracortical circuits of vertebrate cerebellar cortex. Approximately more than 100 specimens of mice, rat, teleost fishes and human cerebelli were processed by the above mentioned techniques. A chronological review of other methods for studying hidden surfaces of cerebellar nerve cell has been also described. The three-dimensional morphology, outer and inner surfaces of granule, Golgi, Purkinje and stellate cells were reviewed by means of a correlative and comparative study. The cerebellar circuits formed by mossy and climbing fibers with granule cell dendrites and Purkinje cell-granule cell synapses have been traced by ethanol-cryofracturing technique, freeze-fracture method for SEM and freeze-etching technique. These findings have been correlated with previous light and electron microscope findings published in the last century. Some advantages and limitations of each method are pointed out. The review emphasizes the paramount importance of correlating light microscope Golgi method with ethanol-cryofracturing and slicing techniques for SEM. The correlation between freeze-fracture method for SEM and freeze-etching technique provides a new approach for studying three-dimensional morphology of nerve cells at cellular and macromolecular levels. This modern methodology of three-dimensional analysis offers new potential areas for future experimental investigation in embryology and pathology of the central nervous system.

Perhexiline maleate in the treatment of angina pectoris. Five years of personal clinical experience.

A five-year personal experience of the use of perhexiline maleate (Pexid) in the treatment of severe angina pectoris is presented. Ninety-four patients, all severely incapacitated by cardiac pain, received perhexiline maleate for an average period of 12.2 months. Perhexiline maleate was used either alone or, more commonly, in conjuction with other antianginal therapy, such as beta-adrenergic receptor blocking agents. The results demonstrate that perhexiline maleate is a very effective agent which appears to be safe for long-term usage. Side effects have been frequent, and occasionally bothersome, but all have been transient and dose-dependent. The possibility that the regimens of treatment may materially improve long-term prognosis is raised.

A rapid screening technique for the detection of spermatozoa.

Phase contrast microscopy has been used for some time to search for and identify spermatozoa. An enhancement to the technique using xylene in conjuction with phase contrast microscopy is discussed. The method has been found to save time by allowing identification of spermatozoa with xylene-enhanced phase contrast microscopy in many cases that would have been unsuccessful using the normal "dry" phase contrast microscopy techniques. This eliminates a staining and reexamination step.
