Intraorbital wood. Detection by magnetic resonance imaging.

The authors present two cases in which intraorbital wooden foreign bodies remained undetected after initial ophthalmologic examination and radiologic investigation which included plain orbital x-rays, orbital computed tomography (CT) scans, and, in one case, orbital ultrasound. In each case, subsequent magnetic resonance imaging (MRI) showed a well-delineated low-intensity lesion suggestive of a retained foreign body. Investigation of a case of suspected wooden foreign body in the orbit should include an MRI scan if there is no contraindication, and no foreign body has been defined on CT scan, ultrasound, or plain orbital films.

Pulmonary surfactant protein A-mediated uptake of phosphatidylcholine by alveolar type II cells.

Pulmonary surfactant protein A (SP-A)-mediated uptake of phosphatidylcholine (PC) by alveolar type II cells was investigated. SP-A enhanced the uptake of liposomes containing dipalmitoylphosphatidylcholine (DPPC), 1-palmitoyl-2-linoleoyl phosphatidylcholine (PLPC), or 1,2-dihexadecyl-sn-glycero-3-phosphocholine (DPPC-ether), a diether analogue of DPPC, but about twice as much DPPC was taken up by type II cells as PLPC or DPPC-ether. When subcellular distribution was analyzed, 51.3 +/- 2.9% (mean +/- SD, n = 3) of cell-associated radiolabeled DPPC was recovered in the lamellar body-rich fraction in the presence of SP-A, whereas only 19.3 +/- 1.9% (mean +/- SD, n = 3) was found to this fraction in the absence of SP-A. When type II cells were incubated either with DPPC at 0 degree C or with DPPC-ether at 37 degrees C, or no cells were included, low proportions of the cell-associated lipids were present in the fractions corresponding to lamellar bodies even in the presence of SP-A. Anti-SP-A antibody significantly reduced the radioactivity incorporated into the lamellar body fraction. Phosphatidylcholine that had been incorporated into lamellar bodies remained largely intact when SP-A was present. Subcellular fractionations of type II cells with radiolabeled SP-A and DPPC revealed that the sedimentation characteristics of cell-associated SP-A are different from those of DPPC, although a small broad peak of radiolabeled SP-A was found in the lamellar body fraction.(ABSTRACT TRUNCATED AT 250 WORDS)

Serum ferritin and Hodgkin's disease.

The haemoglobin, serum iron, transferrin saturation, serum ferritin, erythrocyte sedimentation rate (ESR), splenic weight and non-haem iron concentration in the marrow, liver and spleen were measured prior to treatment in 35 patients with Hodgkin's disease who underwent staging laparatomy. The Hb, serum iron and transferrin saturation showed a significant decrease with increasing stage of the disease. In contrast, there was a significant increase in the serum ferritin, ESR, splenic weight and in all the tissue non-haem iron concentrations. The calculated total iron content of the body remained relatively constant throughout at about 2 g but with increasing stage there was an internal redistribution of iron, with a progressive drop in Hb iron and a reciprocal rise in storage iron, especially in the liver. Serum ferritin concentrations, which rose with progression of the disease, were inappropriately high in relation to the size of body stores at all stages but especially in patients with 4B disease and hepatic involvement. It was concluded that the serum ferritin concentrations are raised for several reasons in Hodgkin's disease. They reflect an increase in body iron stores, ferritin's role as an 'acute phase' protein in the inflammatory response and hepatic damage in patients with advanced disease.

Rice allergenic proteins, 14-16 kDa albumin and alpha-globulin, remain insoluble in rice grains recovered from rice miso (rice-containing fermented soybean paste).

The rice grains (RG) and rice seed proteins remaining in rice miso were investigated with a view point to the potential allergenicity of rice miso. RG ranging from 36 to 180 mg dry weight per g dry miso were separated from several samples of commercially available rice miso. Scanning electron microscopy of the recovered RG indicated that starch granules disappeared almost completely while protein bodies remained intact in RG. Most of the major seed proteins were extracted from RG by heating with 1% SDS/2% 2-mercaptoethanol and detected by SDS-polyacrylamide gel electrophoresis. Major rice allergenic proteins, 14-16 kDa albumin (Alb14-16) and alpha-globulin (alpha-Glb) were also detected by immunoblotting using the specific antisera, and their contents were estimated to be 1.7 to 9.0 and 1 to 7 mg protein per g dry RG respectively. However, the major rice proteins, including glutelin and prolamin, in RG were insoluble in salt, alcohol, and urea solutions, but soluble in 6 M guanidine hydrochloride (Gu-HCl). By immunoblotting and ELISA, no Alb14-16 and only a slight amount of alpha-Glb were detected even in the 6 M Gu-HCl fraction, indicating that these major allergenic proteins are denatured and are present in an insoluble form in rice miso.

Radiopharmacokinetics and radiation absorbed dose calculations from 131I-meta-iodobenzylguanidine (131I-MIBG).

In connection with clinical 131I-MIBG studies of patients with suspected pheochromocytoma and adrenomedullary hyperplasia quantitative biokinetic data have been collected in order to improve the present estimations of absorbed dose to various organs and tissues. Whole-body profiles as a function of time were measured with a whole-body counter. The retention in the total body and in the thyroid gland could be derived from the measured whole-body profiles by summing up the corresponding values. The retention of 131I-MIBG could not be exactly measured for further organs from the whole-body profiles in man. For this reason animal studies were performed with mice. The biokinetic animal data were transferred to man in form of the cumulative activity for the various organs. The mean absorbed dose for selected organs per injected activity unit was calculated using the concept of absorbed fractions (MIRD method) taking into account the radioactivity within the remaining body. Except for both the adrenal medulla and the thyroid gland the absorbed doses for all the other selected organs are in a range from 0.108 mGy MBq-1 for the testes to 0.176 mGy MBq-1 for the lungs. The absorbed dose to the thyroid gland amounts to the considerable value of 5.69 mGy MBq-1 although the thyroid gland was blocked. The greatest absorbed dose was estimated for the normal adrenal medulla with 18.67 mGy MBq-1.

[More detailed criteria for distinguishing lymphatic and reticular cells using electron microscopy].

About 100 illustrative cases of malignant lymphoma and non-tumorous lymph node processes studied by electron microscopy were revised. Some distinguishing features were stressed as about nuclear patterns and intercellular relations of basic lymphatic elements. The centrocyte is unique in a fine chromatin pattern of the cleft nuclei and in varied superficial qualities. It tends to form junctions with dendritic cells and bilateral dense bodies remaining from severed cellular projections. The centroblast and immunoblast represent a different structural line related to plasmacytes and macrophages or epitheloid cells. The T-line tends to form characteristic junctions with interdigitating cells as well as dense invaginations of cytoplasmic membrane on the both sides of junctions. There is a limited number of similar junctions with pericytes. Dense bodies produced at the junction of lymphatic and stromal reticular cells are discussed.

Fatal reducing body myopathy. Ultrastructural and immunohistochemical observations.

Two female infants who developed normally during infancy began to have progressive muscle hypotonia and weakness from 2 years 10 months and 2 years 3 months of ages, respectively. Both patients had rapidly progressive muscle weakness with death from respiratory failure at 4 years 11 months and 3 years 9 months, respectively. In addition to mild inflammation in their muscle biopsies, the most striking finding was the presence of numerous reducing bodies (RB) in almost all degenerating fibers. By electron microscopy, these bodies consisted of fine granular material, usually located around the degenerating nucleus. These bodies showed no immunohistochemical reaction to antibodies against structural, cytoskeletal and membrane proteins and a histone-specific antibody against nuclei and chromosomes. They were occasionally positively stained with a ubiquitin antibody. Although the origin of these bodies remains unknown, they appeared to be related to active myofibrillar degeneration, probably resulting from primary nuclear degeneration.

Eight years' experience with foreign-body aspiration in children: what is really important for a timely diagnosis?

BACKGROUND/PURPOSE: Aspiration of foreign bodies remains a major cause of morbidity and mortality in childhood. The aim of this study was to evaluate the predictive diagnostic value of clinical signs and symptoms, the history, and the radiology to perform early diagnosis and therapy. METHODS: From January 1990 to March 1998, 87 children were admitted to the Pediatric Surgery Department of Universita di Padova because of suspected foreign body aspiration. Sensitivity and specificity of the considered diagnostic tools were evaluated. RESULTS: Neither clinical signs and symptoms nor radiology have sufficient diagnostic sensitivity, and especially specificity, on which to rely for the diagnosis. Only the choking crisis, when present in the history, has good sensitivity and specificity (respectively, 96% and 76% in this series). CONCLUSIONS: A choking crisis in the child's history should alert physicians to the possibility of a foreign body aspiration. In the present series, complications always were related to the diagnostic delay.

Clinical experiences of removing foreign bodies in the airway and esophagus with a rigid endoscope: a series of 3217 cases from 1970 to 1996.

This study examined 11,333 rigid endoscopy procedures performed in the Department of Otolaryngology, National Taiwan University Hospital, during a 27-year period from 1970 to 1996. Among these cases, 3217 were performed to remove foreign bodies from the airway (459 cases, 14.3%) and esophagus (2758 cases, 85.7%). Retrospective analysis of these data revealed that peanuts (217 cases) and animal bones (1184 cases) were the most frequent foreign bodies encountered in the airway and esophagus, respectively. The successful rate of removal of these foreign bodies was 99.9% (3213/3217). The complication rate was only 0.2% (8/3217), and the mortality rate was less than 0.1% (2/3217). On the basis of these results, we conclude that foreign bodies in the airway and esophagus can be removed safely under direct visualization through rigid endoscopy with relatively few complications. A significant finding in this study is the declining trend in the number of cases in recent years. Despite the decline in the number of procedures, endoscopic removal of foreign bodies remains as a vital skill of the aerodigestive tract surgeon.

Expression of growth-associated protein B-50 (GAP43) in dorsal root ganglia and sciatic nerve during regenerative sprouting.

Recently it has been shown that B-50 is identical to the neuron-specific, growth-associated protein GAP43. The present study reports on the fate of B-50/GAP43 mRNA and B-50/GAP43 protein, determined by radioimmunoassay, in a rat model of peripheral nerve regeneration (sciatic nerve crush) over a period of 37 and 312 d, respectively. Moreover, the effects of repeated subcutaneous injection of the neurotrophic peptide Org.2766 (an ACTH4-9 analog) and of a conditioning lesion on B-50/GAP43 protein levels in the regenerating nerve and dorsal root ganglia (DRG) were investigated. Both treatments enhanced the functional recovery as evidenced by a foot-flick withdrawal test. Immunocytochemical analysis using antineurofilament antibodies revealed a peptide-induced increase in the number of outgrowing sprouts in the sciatic nerve. Both the peptide and the conditioning lesion amplified the crush lesion-induced increase in B-50 protein content in the nerve as determined by radioimmunoassay. B-50 protein levels seem to correlate proportionally with the number of sprouts. In the DRG of the crushed sciatic nerve, the time course of B-50 expression was studied. B-50 mRNA was quantified from Northern blots. A linear increase up to 10 times the basal level of B-50 mRNA was observed 2 d postsurgery, followed by a gradual decline to normal levels at day 37. The first significant rise in B-50 mRNA level became apparent between 8 and 16 hr after placement of the crush lesion. The first significant rise in B-50 protein level occurred 40 hr after the crush lesion, reaching a plateau of 3 times the basal level between day 6 and 20. B-50 protein levels in DRG cell bodies remained elevated up to 60 d after crush, a period much longer than that observed for B-50 mRNA. Thus, during a later phase of peripheral axonal regeneration, the presence of B-50 appears to be prolonged, probably by an increase in half-life and not so much by enhanced transcription. Treatment with Org.2766 did not affect the B-50/GAP43 levels in DRG cell bodies during the first 6 d following crush. Conditioning lesion resulted in a DRG B-50/GAP43 protein amount at the same level as in rats 14 d after the test lesion. B-50/GAP43 levels in DRG are probably influenced by the rapid axonal transport of the protein, as has been reported by others.

Bacterial contamination of the biliary tree after choledochostomy.

Secondary exogenous contamination of bile after choledochostomy could be an important cause of long-term morbidity if infection persists after removal of the T tube. Surprisingly, documentation of the frequency of conversion of sterile to infected bile after choledochostomy has rarely been recorded. Patients undergoing exploration and T-tube drainage of the common bile duct between July 1966 and January 1975, in whom intraoperative and postoperative cultures of bile were available, were studied. Postoperative cultures were obtained from five to forty days after operation. Of ninety-five patients available for study, contamination developed postoperatively in previously sterile bile in 44 per cent. The common duct bile contained bacteria at operation in 42 per cent and remained sterile throughout in 14 per cent. The most common secondary contaminants were klebsiella group and Escherichia coli. The fate of the contaminating organisms in a biliary tree without anatomic abnormalities is unknown. Two cases are presented which suggest that persistent infection may linger in the biliary tree for many years and give rise to pigment calculi. Since bacterial cannot be eradicated so long as a foreign body remains in the common duct and because such bacteria may cause symptoms even in an anatomically normal biliary tree, we suggest that a closed system of biliary drainage be employed and that appropriate antibiotic therapy be instituted for seven to ten days after removal of the T tubes.

The Bristol Children's Hospital experience of tracheobronchial foreign bodies 1977-87.

In the 11 years 1977 to 1987, 36 confirmed cases of tracheobronchial foreign body were seen in the Bristol Children's Hospital. The distribution of ages, sites, and natures of the objects inhaled are similar to those in other reviews. Complications of removal of the foreign body once diagnosed were few, and have been absent since the Storz rod lens system was introduced in 1982. Eleven patients experienced delay of at least 7 days between the beginning of the symptoms and diagnosis, of whom 3 subsequently required open thoracotomy. Six of these had sought medical advice during this period, of whom 4 had a positive history of choking or inhalation. Inhaled foreign bodies remain a source of diagnostic difficulty, although this should be improved by greater attention to history and appropriate investigation. Technical advances have ensured their safe bronchoscopic removal in most cases.

Foreign body in the airways: a review of 90 cases.

Despite the progress made in the fields of endoscopy and anesthesia for children, as well as the accumulated experience, the inhalation of foreign bodies remains a serious and often dramatic problem. Every case varies to some extent, and only general conclusions may be drawn. The retroactive analysis of the documents from every case that was treated at the Ear, Nose, and Throat Clinic of the Aristotelian University from 1970 to 1975, as well as the comparative statistical study, show common characteristics shared by all 90 cases under consideration. In these cases, the foreign body was of organic origin (mainly seeds and dry fruits) and particularly pumpkin seeds (25% of all cases). Half of the patients sought hospitalization, with a delay varying from two days to four months, while localization varied (larynx, 11 cases; trachea, 10; bronchus, 69). Localization in the left bronchus was more frequent than in the right one, especially in children. The diagnosis was based on the case history, clinical examination, and radiologic findings, while in the majority of the cases, the treatment involved bronchoscopy, the removal of the foreign body, and treatment of lung problems in delayed cases.

[Intra-orbital explosion of a rocket].

Orbito-palpebral ballistic wounds are relatively common, most often due to firearms, and raise the problem of their surgical management. Excision of the intra-orbital foreign bodies remains difficult, above all when there is impaction of the anterior portion. The authors report a model case of orbital trauma by rocket splinter, excision of which was possible via the entry orifice.

The time course of silver accumulation in rainbow trout during static exposure to silver nitrate: physiological regulation or an artifact of the exposure conditions?

The pattern of gill silver accumulation in rainbow trout during waterborne silver exposure has been reported to be unusual, reaching a peak in the first few hours of silver exposure followed by a marked decline with continued exposure. The potential causes of the pattern were investigated. Rainbow trout (1-5g) were exposed in a static system to 110mAg labeled AgNO(3) at a total concentration of 1.92microg Agl(-1) for 24h in synthetic soft water. Periodically throughout the exposure, gill and body 110mAg accumulation, gill and body 24Na uptake (from which whole body Na(+) uptake was calculated), gill Na(+)K(+)-ATPase activity, plus water silver (total and dissolved), Cl(-) and total organic carbon (TOC) concentrations were measured. Gill silver levels rapidly increased, peaked at 3h of exposure and then decreased until a plateau was reached at 12h of exposure. Body (minus gills) silver levels increased steadily over the exposure period until 18h of exposure. Whole body Na(+) uptake decreased, was maximally inhibited by 3h of exposure but recovered by 12h despite continued silver exposure. Gill Na(+)K(+)-ATPase activity was not inhibited until 5h of exposure. The water dissolved silver concentration declined by approximately 70% over the 24h exposure period and the TOC content of the water increased over three-fold during the first 2h of exposure. There was a decrease in the calculated contribution of Ag(+) (from 20.9 to 2.5%) and an increase in the calculated contribution of Ag-TOC complexes (from 77 to 97.3%) to the total water silver concentration over the first 2h of exposure. Apical silver uptake into the gills decreased over the initial 2.5h of exposure while basolateral silver export out of the gills to the body remained constant throughout the exposure. The results of this study suggest that: (1) physiological regulation of silver movement may explain the pattern of gill silver accumulation observed in rainbow trout, although not by a mechanism coupled to Na(+)K(+)-ATPase inhibition as originally proposed; (2) alternatively or additionally, a decreased bioavailability of silver, due to the static exposure conditions, may explain the pattern of gill accumulation; (3) the early inhibition of whole body Na(+) uptake observed during silver exposure occurs via a mechanism other than Na(+)K(+)-ATPase inhibition; and (4) gill silver accumulation may be an appropriate endpoint for biotic ligand modeling.

[Study of spondylodiscitis with magnetic resonance. Use of gadolinium-DTPA].

Eighteen patients suffering from spondylodiscitis were examined with MRI and 22 pathologic levels were identified: 1 cervical, 6 thoracic and 15 lumbosacral. As for etiology, 4 cases were tubercular, 3 strepto-staphylococcal, 3 brucellar 1 strepto-bacteroides and 11 of unknown origin. Five patients underwent MR follow-up 4-9 months after the clinical onset. SE T1- and T2-weighed sequences were always performed and 11 patients, with 13 pathologic levels, also underwent SE T1-weighted sequences with i.v. Gd-DTPA administration. No correlation could be made between the signal intensity of the involved vertebral bodies and disks and etiology. On SE T1-weighted sequences 91% of the vertebral bodies were hypointense, whereas the intervertebral disks were hypointense in 55% of cases and isointense in 45%. On SE T2-weighted sequences, the vertebrae appeared hyperintense in 65% of the cases while the disks were hyperintense in 55% and isointense in 41%. After Gd-DTPA injection signal intensity increased in 100% of cases, both at the body and at the disk. In 6 cases an inflammatory infiltration of the intracanalar structures was found, whose identifications was possible, in 4 cases, only after contrast medium injection. In the follow-up of 5 cases at 4-9 months the vertebral bodies remained hypointense in 3 cases on T1-weighted sequences, while in 2 cases signal intensity tended to normalize. Signal normalized in 4 cases on T2-weighted sequences. In 2 follow-up examinations disk structures were never depicted, neither on T1- nor on T2-weighted sequences. To conclude, T1-weighted sequences appear to be more sensitive than T2-weighted sequences to demonstrate inflammatory processes in the vertebral bodies, while the two sequences are equivalent to evaluate the disks. Gadolinium-enhanced T1-weighted sequences were more sensitive than the T1- and T2-weighted ones, especially to assess the extent of inflammatory processes into the spinal canal and to evaluate their persistence. The authors suggest the use of unenhanced and enhanced SE T1-weighted sequences only at least in the follow-up of spondylodiscitis.

Two-dimensional electrophoretic analysis of Aedes aegypti mosquito fat body proteins during a gonotropic cycle.

We have used two-dimensional polyacrylamide gel electrophoresis to examine changes in the relative abundance and diversity of non-secreted proteins in Aedes aegypti fat body preparations during a reproductive cycle. Electrophoretic profiles were evaluated at four time points after eclosion, and at five time points after a blood meal. In contrast to the dramatic changes in abundance of specific secreted proteins such as vitellogenin, our results show that the complement of proteins internal to the fat body remains relatively constant during the mosquito reproductive cycle. Of the approximately 5-10% of proteins that do change in abundance, the majority undergo a dramatic decrease within 24 hours after eclosion.

Analysis of the human herpesvirus-6 immediate-early 1 protein.

Herpesvirus immediate-early (IE) gene products play key roles in establishing productive infections, regulating reactivation from latency and evading immune recognition. Analyses of HHV-6 IE gene expression have revealed that the IE1 gene of the HHV-6A and HHV-6B variants exhibits a higher degree of sequence variation than other regions of the genome and no obvious similarity to its positional analogue in HCMV. We have analysed expression of the HHV-6 U1102 (HHV-6A) and Z29 (HHV-6B) IE1 gene products using transient expression vectors, stable cell lines and in the context of lytic virus infection. The IE1 transcripts from both variants demonstrate a similar pattern of splice usage within their translated regions. The HHV-6 IE1 proteins from both variants traffic to, and form a stable interaction with, PML-bodies (also known as ND10 or PODS). Remarkably, PML-bodies remained structurally intact and associated with the IE1 protein throughout lytic HHV-6 infection. Immunoprecipitation studies demonstrated that HHV-6 IE1 from both variants is covalently modified by conjugation to the small ubiquitin-like protein SUMO-1. Overexpression of SUMO-1 in cell lines resulted in substantially enhanced levels of IE1 expression; thus sumoylation may bestow stability to the protein. These results indicate that the HHV-6 IE1 protein interacts with PML-bodies yet, unlike other herpesviruses, HHV-6 appears to have no requirement or mechanism to induce PML-body dispersal during lytic replication.
