Emergency gastrotomy: treatment of choice for iron bezoar.

The authors believe that gastrotomy and surgical removal of tablets is the treatment of choice in massive iron ingestion with development of an "iron bezoar" unresponsive to gastric lavage. Prompt surgical treatment in such cases may be lifesaving.

Fungal bezoar and bladder rupture secondary to candida tropicalis.

Candidal urinary tract infections typically occur in a host with compromised immune function. Although usually associated with aerobic bacterial infections, emphysematous cystitis occasionally complicates fungal infections of the lower urinary tract, especially in diabetics. Another uncommon occurrence is formation of a "fungus ball" leading to obstructive uropathy. We present a case of bladder rupture in a patient with emphysematous cystitis and obstructing fungal bezoar caused by Candida tropicalis. Various factors predispose to fungal urinary tract infection, including diabetes mellitus, neurogenic bladder, antibiotic usage, and the presence of an indwelling urinary catheter.

Bezoars--analysis of seven cases.

From 1979 to 1986 seven patients were studied in whom bezoars had been diagnosed. The history of five patients revealed that they had eaten a fruit called prunus lauricerasus, grown in the east Black Sea region. This is quite different from persimmons which are well-known as an aetiological agent of phytobezoars. Trichobezoars were present in one case, and phytobezoars in five cases. In one case, trichophytobezoar was removed surgically from the stomach. Our five cases were operated on to treat small bowel obstruction. Surgery was performed in all our cases and prunus lauricerasus should be kept in mind as an aetiological factor besides persimmons.

Case report: bilateral ureteral obstruction secondary to Aspergillus bezoar.

Ureteral obstruction as a result of a primary aspergillus infection is rare. Early clinical suspicion in immunosuppressed patients is essential to diagnosis. We report a case of a 50-year-old diabetic woman presenting with acute renal failure, sepsis, and bilateral ureteral obstruction. Initial management included bilateral percutaneous nephrostomy tubes. Urine culture from both the left and right renal pelvis grew Aspergillus flavus. The left-sided obstruction resolved with antifungal therapy. However, her right ureteral obstruction persisted and was managed with ureteroscopy and removal of the fungal bezoar.

Crohn's disease of the duodenum presented as pancreatitis due to persimmon bezoar.

Pancreatitis due to Crohn's disease of the duodenum is very rare. It is the result of either reflux of duodenal content into the pancreatic duct or stenosis due to direct involvement of the ampullary region with Crohn's disease. The patient described had isolated Crohn's disease of the duodenum; pancreatitis was the result of persimmon bezoar's constricting the ampullary region.

Mechanical intestinal obstruction from pseudocyst bezoar following internal drainage of traumatic pancreatic pseudocyst.

A unique case of intraluminal mechanical small-intestinal obstruction occurring from a bezoar of organized debris that extruded from a post-traumatic pancreatic pseudocyst following Roux-en-Y cystojejunostomy is presented. Although an apparently unusual complication, it is one that should be considered in any patient experiencing incomplete cyst drainage or intestinal obstruction following an internal drainage procedure for pancreatic pseudocyst.

Zenker's bezoar.

Zenker's diverticulum has been associated with a variety of symptoms such as dysphagia, regurgitation, aspiration, halitosis, and occasionally cough. In this case, a large Zenker's diverticulum containing a bezoar caused a persistent, debilitating cough presumably due to compression of adjacent neck structures.

Toxicity associated with a prolonged half-life of phenytoin in a 97-year-old woman: bezoar formation? Case report and clinical pathological conference.

Phenytoin metabolism is best described using nonlinear (Michaelis-Menton) kinetics. When calculated pharmacokinetic values, including half life and clearance, do not correspond to measured variables, alternative reasons must be sought. We present an interesting case of phenytoin toxicity and suggest that formation of a bezoar may be one possible answer to prolonged toxicity.

Acute renal failure in a neonate due to pelviureteric candidal bezoars successfully treated with long-term systemic fluconazole.

Systemic candidiasis with renal involvement is a rare but well-recognized complication during intensive care treatment in very-low-birth-weight infants. We report a term neonate who developed anuria associated with bilateral bezoar formation in the renal pelvis and candidemia. The treatment consisted of placement of a nephrostomy tube in the left kidney, short-term irrigation with amphotericin B and iv, and later, oral administration of fluconazole.

Coconut matting bezoar identified by a combined analytical approach.

A rare type of bezoar composed of coconut matting was found in the stomach of a caucasian man. The exact identity of the fibres was established by scanning electron microscopy, x-ray energy spectroscopy, and microscopic infrared spectroscopy. This report illustrates the importance of these techniques for identifying the nature of foreign material.

Small bowel obstruction with multiple perforations due to enterolith (bezoar) formed without gastrointestinal pathology.

An enterolith (bezoar) usually originates in an intestinal diverticulum or in a segment of bowel loculated by stricture formation. Stasis promotes its formation. This communication describes a case in which a large enterolith caused obstruction and multiple perforations of the terminal ileum in the absence of any predisposing gastrointestinal pathology. The management of this rare occurrence is discussed and the literature reviewed.

A gastric bezoar causing a target lesion as revealed by ultrasound.

A case of a bezoar of the stomach causing a "target lesion" of the gastrointestinal tract is presented. The observance of this mass as water passed through the stomach may, in the future, differentiate the mass from a true stomach-wall lesion as more of these patterns are recognized. Other lesions that may enter into the differential diagnosis are blood clots and pedunculated polyps.

[Bezoars and gigantic stomach ulcers associated with helicobacter pylori].

Available are the results of examination and treatment of 8 patients with bezoars of the stomach (6 of them with gigantic ulcers) associated with Helicobacter pylori. The results showed in all patients a presence of chronic atrophic gastritis of various and hypoacidic condition. Gastritis and ulcer associated with Helicobacter pylori are a suitable background for phytobezoars formation. At the same time phytobezoars favour the progression of these diseases and development of complications. Pathognomonic symptoms of the disease and possible complications are determined, their prophylaxis and treatment are proposed.

[Imaging of urinary Candida bezoars].

OBJECTIVES: To describe the renewed interest in Candida infections in general and urinary tract infections in particular, due to a recrudescence of immunodepressed patients and a better knowledge of their clinical and radiological manifestations. MATERIAL AND METHODS: The authors report 2 cases of Candida bezoars diagnosed by IVU, ultrasonography and CT scan in two insulin-dependent diabetics. RESULTS: The first case presented with right renal involvement, in the form of delayed renal secretion on IVU with the presence of a pelvic filling defect, dilatation of the upper tract cavities on ultrasonography with a dilated renal pelvis occupied by an echogenic formation with no posterior acoustic shadow. CT showed a slightly heterogeneous low-density lesion with no contrast enhancement. The second case presented with right pelvic and vesical involvement. The diagnosis of urinary candidiasis was confirmed, in the first case, by the presence of whitish lumps on catheterization, for which analysis and culture were in favour of candidiasis and, in the second case, by direct examination and urine culture. Treatment with amphotericin B led to improvement followed by disappearance of the signs in both cases. CONCLUSION: In addition to laboratory examinations, noninvasive imaging techniques, mainly ultrasonography and CT, but also percutaneous aspiration, constitute a decisive element in the diagnostic and therapeutic management of urinary candidiasis.

Bezoar: an uncommon cause of intestinal obstruction.

Bezoars or undigested food concretions in the gastrointestinal tract are mostly due to ingestion of a stringent immature fruits following gastric surgery and can lead to the serious complication of acute small bowel obstructions. We are reporting a case of complete jejunal obstruction in a 60 year old female, 15 years following bilateral vagotomy and gastrojejunostomy for pyloric obstruction. Important clinical and pathological features are emphasised to increase the awareness of this rather uncommon cause of intestinal obstruction.

[Unusual cause of ileal occlusion: a coconut bezoar].

A coconut phytobezoar was detected in the distal ileum of a 4-year-old boy, admitted for suspected appendicitis. While persimmon or citrus fruit ingestion has been previously reported as cause of phytobezoar intestinal occlusion, it is the first time that coconut pulp is described to cause ileal obstruction in children. No predisposing factor as previous gastric surgery or Meckel's diverticulum was present. The bezoar was successfully milked past the ileocecal valve into the right colon.

A case report on bezoar.

A nine years old girl was admitted into the paediatric surgery word of Dhaka Medical College Hospital (DMCH) with upper abdominal mass and discomfort, was diagnosed radiologically as a case of gastric bezoar which was further confirmed by endoscopy as trichobezoar (hair ball). The girl was undergone laparotomy and trichobezoar was removed from stomach by gastrostomy. After ten days of operation she became well and was discharged from the hospital.
