[Mucocele of the appendix in highly differentiated adenocarcinoma].

A patient 69 years old had been admitted at the University Women's Hospital Berlin because of a suspected ovarian tumour. Outside a computed tomography was done. In the small pelvis a tumour partly cystic and partly solid at the right side could be confirmed sonographically. Because this an operation was indicated. After opening the abdomen the internal female genital organs were in conspicuous with the exception of extensive adhesions. Histologically we found a mucocele of the vermiform appendix with a highly differentiated adenocarcinoma. All tissue layers were infiltrated and perforated. This combination of findings is very seldom, because carcinomas of the appendix could be proved only in 0.01 per cent of all appendectomies.

Adenocarcinoma

Appendix as continent urinary reservoir outlet.

The submucosally embedded in situ appendix offers an ideal continence mechanism in patients with ileocaecal urinary reservoir. Ninety-two patients who underwent this procedure at three urology centres (48 patients in Mainz, 20 in Wuppertal-Barmen and 24 in Marburg) are reviewed. The appendicocutaneous stoma was placed in the lower right abdominal quadrant in 12 patients (most of whom previously had bladder exstrophy) and into an umbilical or, in some exstrophy cases, a neoumbilical tunnel in 80 patients. Urodynamic investigations of the submucosally embedded appendix showed maximum closure pressure greater than 80 cm H2O.

Appendix

Mucocele of the appendix: computed tomographic, endoscopic, and pathologic correlation.

Mucoceles of the appendix are a rare group of lesions thought to occur as a sequelae of luminal obstruction as well as certain benign and malignant neoplasms of the appendix. Early, preoperative diagnosis is rare. We report a case of an appendiceal mucocele in which the diagnosis was suggested by computed tomographic imaging and colonoscopy, and confirmed at surgery. Radiologic, endoscopic, and pathologic correlations of this rare entity are discussed.

Aged

Primary adenocarcinoma of the appendix. A clinicopathological study of 11 cases.

Of eleven cases of adenocarcinoma of the appendix that were studied, five were examples of mucinous cystadenocarcinoma and four were examples of colonic-type adenocarcinoma. Two lesions were very distinctive, with thick, cord-like transformation of the appendix. The term linitis plastica carcinoma is proposed to describe this rare variant of appendiceal carcinoma. Justification for the separation of linitis plastica carcinoma from the other two types is based on morphological appearance and on outcome. Both our linitis plastica patients died of metastases within six months of surgery, whereas, in the other two groups, death was directly related to spread of the carcinoma in only two of nine patients.

Adenocarcinoma

[Preoperatively as adnexa tumor diagnosed mucocele of the appendix].

In a 51 year old woman a cystic in the right lower abdomen was diagnosed by ultrasound. The patient underwent laparotomy assuming the tumor to be a right adnexal mass. Intraoperatively, a pathological appendix was found. Histologic evaluation of the appendix showed a mucinous cystadenoma with mucocele.

Cryptosporidiosis of the appendix vermiformis: a case report.

The present case report describes a 30-year-old man with AIDS who developed cryptosporidiosis of the appendix vermiformis. The patient had been admitted to hospital with all the symptoms of appendicitis, and an appendectomy was performed. The histological work-up of the surgical specimen revealed an acute phlegmonous appendicitis, and also a welldeveloped cryptosporidiosis, which was confirmed by electron-microscopic examination. Two years later, the patient died of pneumonia contracted during a generalised CMV infection. The postmortem examination revealed cryptosporidial organisms in the biliary tract. As far as we know, this is the first ever report of cryptosporidiosis of the appendix vermiformis.

Acquired Immunodeficiency Syndrome

[Rosai-Dorfman disease of the mesenteric lymph nodes and appendix. Secondary to salmonellosis?].

A 28 year-old female patient was operated for acute abdominal pain. At the surgical intervention the mesenteric nodes turned out to be enlarged and the appendix was apparently normal. Microscopically, the changes observed were of the Rosai-Dorfman disease type in the appendix as well as in the nodes. A salmonella D was cultivated in the mesenteric nodes. This association, which has not been described in the literature, permits us to comment clinicopathological aspects, poorly defined, with special reference to the possibility that some instances of Rosai-Dorfman disease could be secondary to an infectious process.

[The immunomorphological characteristics of the appendix in a viral-bacterial lesion in children with appendicitis].

The characteristics of viral-bacterial lesion in the appendix of children with appendicitis are given by means of histologic, histochemical, immunomorphological and morphometric methods. Antigens of influenza viruses A, B, C, entero-, adeno- and paramyxoviruses were observed among which influenza C was found significantly more frequently (64.1%). Viral infection in the majority of cases (79.5%) was found in the association with opportunistic flora, more frequently with E. coli, and less R. aeruginosa and Klebsiella. Analysis of the immunomorphological shifts in the appendix indicated a peculiar dynamics of the antibody production in acute appendicitis: the enhancement of the IgG, IgD and IgE production and the decrease of the production of IgA and IgM. The viral-bacterial effect, the development of the immune responses with IgE hyperproduction and participation in this process of labrocytes and endocrine cells suggest the integration of the two pathogenetic mechanisms-infectious and allergic.

[Carcinoid of the appendix. Presentation of a case].

The paper describes a recent case of carcinoid of the appendix in a 15 year old patient, operated for appendicitis. The diagnosis was found incidentally by histologic examination. Following a review of the literature the Authors discuss the epidemiology and clinical aspects of the appendiceal carcinoid, the most common tumour of the appendix.

Necrotizing vasculitis of the gallbladder and the appendix. Similarity in the morphology of rheumatoid arteritis and Polyarteritis nodosa.

Two patients with rheumatoid arthritis (RA) and polyarteritis nodosa (PN) presented with abdominal pain. Both revealed necrotizing vasculitis of the gallbladder and the appendix respectively. The controversial role of corticosteroids in the pathogenesis of rheumatoid arteritis, the rarity of rheumatoid arteritis of the gallbladder and the morphologic similarity of the vascular lesion in the gallbladder and appendix are discussed.

[Mucocele of the appendix. Sonographic, computed tomographic and radiologic characteristics].

The knowledge of the diagnostic imaging methods features of the mucocele of the appendix is important for a correct preoperative diagnosis, especially to avoid the formation of pseudomyxoma peritonei. A case of a mucocele of the appendix with a special sonographic appearance by a patient with an additional carcinoma of the left kidney is reported. The computer-tomographic and radiological features of the mucocele (plain abdominal film, i.v. urography, barium enema) are presented and discussed with a review of the literature. A sonographically-guided fine-needle puncture of the mucocele was performed. Its importance for the diagnosis is discussed.

[A mucocele of the appendix--a case report].

Mucoceles are rare lesions in the region of the appendix and occur in only 0.4% of all tumours of the appendix. Although their radiological-morphological pattern varies, a correct preoperative diagnosis is nevertheless possible in most cases via imaging methods and taking the pattern of clinical findings into consideration.

[Lectin histochemical analysis of the epithelial lining of the appendix in rabbits].

The binding of PSA, PNA, HPA, WGA, Con A, LCL, RCA, SBA, and PHA lectins to epithelial structures of the normal rabbit appendix was studied. Differences were observed in the affinity of some lectins to the epithelium of the intercryptal lining of the rabbit appendix, to the epithelium hemming the glandules and crypts of the domes of Peyer's patches. The obtained results document the difference in the affinity of individual batches of anti-WGA antibodies to this lectin after its binding to the epithelial structures studied. This implies the possibility that differently reacting antibodies may develop to different commercially available batches of WGA. Precipitation of WGA at sites of alkaline phosphatase occurrence demonstrates the relationship of this enzyme to WGA binding sites in tissues.

Animals

[Volvulus of the appendix].

The authors describe the case-history of a volvulus of the appendix which developed as a result of an inborn defect in the appendiceal mesenteriolume. The surgical finding did not suggest primary appendicitis but rather strangulation of the aboral part of the appendix.

[Hypotrophic and duplicated appendix. A case in a child].

A case of hypotrophic double appendix with inflammation is reported. Duplication of the appendix is rare, and fewer than 80 cases have been reported in the literature. The classification of these cases and possible aetiology are discussed.

[Torsion of the appendix in a child].

Torsion of the vermiform appendix is a rare condition that simulates acute appendicitis. An 8-year-old girl with symptoms suggestive of acute appendicitis is reported. At operation the appendix was found to be twisted 72 degrees and was resected.

Blood supply and position of the vermiform appendix in Zambians.

The arterial blood supply, position, and length of the appendix were studied 103 Zambian cadavers. In 39.8%, more than one appendicular artery was found. In position, 43.6% were pelvic, and 20.3% retro-cecal. The average length of the appendix was 12.0 cm in males, and 11.4 cm in females. The prevalence of the dual blood supply and pelvic position, may partly explain the recorded rarity of appendicitis among Africans.

Adolescent

Fistulisation of an iliac pseudoaneurysm into the appendix. Presentation of a case and a review of the literature.

A case of ilioappendiceal fistula is presented. The patient had previously been operated on for a ruptured aneurysm of the common iliac artery. 21 years later he developed occult gastrointestinal bleeding without signs of infection. Colonoscopy revealed bloody faeces and an isotope scan haemorrhage in the ascending colon. Laparotomy and right hemicolectomy was performed without identifying the fistula. The head of the appendix was left attached to the scarred peritoneal wall. As the bleeding continued, a second laparotomy was performed revealing an iliac pseudoaneurysm with fistulisation into the head of the appendix. Vascular reconstruction was attempted, but the patient succumbed to massive bleeding on the operating table.
