A case-control study of injuries arising from the earthquake in Armenia, 1988.

The study attempts to identify predictors of injuries among persons who were hospitalized following the Armenian earthquake of 7 December 1988. A total of 189 such individuals were identified through neighbourhood polyclinics in the city of Leninakan and 159 noninjured controls were selected from the same neighbourhoods. A standardized interview questionnaire was used. Cases and controls shared many social and demographic characteristics; however, 98% of persons who were hospitalized with injuries were inside a building at the time of the earthquake, compared with 83% of the controls (odds ratio = 12.20, 95% confidence interval (CI) = 3.62-63.79). The odds ratio of injuries for individuals who were in a building that had five or more floors, compared with those in lower buildings, was 3.65 (95% CI = 2.12-6.33). Leaving buildings after the first shock of the earthquake was a protective behaviour. The odds ratio for those staying indoors compared with those who ran out was 4.40 (95% CI = 2.24-8.71).

[The species composition of mosquitoes and ticks in Armenia].

Comprehensive epidemiological, entomological, virological, and parasitological studies were conducted to examine the species composition and size of bloodsucking arthropoda (mosquitoes and ticks). A total of 64,567 mosquitoes and 45,180 Ixodes ticks were collected. Among the mosquitoes, Anopheles maculipennis was a prevalent species (81.6%). In all climatic zones, Dermacentor marginatus was the largest in number and most abundant during flag collections from cattle and plants (62.5% and 95.5%, respectively). Virological studies of the collected field material identified 125 strains of arboviruses belonging to 10 viruses: Tyaginya, Sindbis, Batai, Dkhori, tick-borne encephalitis, West Nile fever, Tamdy, KGL, Geta, and Bkhandzha. The identified arboviruses are environmentally associated with both mosquitoes and ticks. The larger number and diversity of bloodsucking artropoda present a potential risk of outbursts of arbovirus infections on the territory of the republic.

[A clinico-neurophysiological analysis and the treatment problems of the crush syndrome (based on data from the earthquake in Armenia)].

Overall 350 patients with the crush syndrome were examined. It has been shown by the clinico-neurological investigation that in long compression of the limbs followed by the development of the crush syndrome, sensitive fibers and the membrane of the axon are most of all exposed to unfavourable effects. The changes discovered as a result of the clinically intact nerves point to the diffuse impairment of the peripheral neuromotor apparatus in patients with the crush syndrome.

[Investigation of chromosomal anomalies by fluorescence in-situ hybridization (fish) in Armenia].

Prenatal and postnatal molecular-cytogenetic investigations of 75 patients have been performed. 3-stage international standard of patients' examination was used, including general clinical examination, investigation by the methods of conventional cytogenetics and molecular-cytogenetic fluorescence in-situ hybridization (FISH) analysis. On the basis of cytogenetic investigation we can conclude that in 25% of cases of complex chromosomal anomalies the cytogenetic diagnosis, obtained by means of conventional karyotyping, needed further specification. Only in 50% of these cases FISH-investigations confirm the results, received on the basis of conventional cytogenetics. In other cases diagnosis was specified or changed. The investigations were carried out with consultations of Institute of Medical Genetics (University of Zurich). Some cases of chromosomal pathology, representing scientific interest, were included in the European Cytogeneticists Association Register of Unbalanced Chromosomal Aberrations (ECARUCA).

[The outcomes in crush syndrome of the extremities half a year after the earthquake in Armenia].

Squeezed syndrome affecting various segments of limbs has been identified in 245 of 500 victims randomized for examination 6 months after the earthquake. In 119 cases associated with closed fractures of long tubular bones and in 59 cases with open ones. In 124 cases duration of squeezing was up to 12 h, and in 70 cases--1 day and more. Segmental limb amputation was performed in 113 cases. In 95 cases limbs functions were retained and in 81--lost. Perspectives of more successful outcomes depend on the maximal shortening of rescue period, on rational organization of the treatment process and prolongation of recovery management of squeezed syndrome sequelae.
