Pitfalls and pratfalls in consultation-liaison psychiatry in a general hospital.

As Consultation-Liaison Services continue to develop and expand in general hospitals, psychiatrists must be aware of pitfalls and pratfalls inherent in dealing with medical colleagues and other allied health professionals, as well as with the patients. Practical considerations in answering consultation requests are discussed with respect to "hidden agendas" of the consultee, role and expectations of the psychiatrist, and problem referrals. It is only through mutual respect and collaboration that Consultation-Liaison Services can truly be effective.

Evaluation of isokinetic muscle performance.

The development and availability of isokinetic testing equipment, recording systems, and systems for computer analysis have improved the quality and quantity of information about specific skeletal muscle performance. The value of both isokinetic exercise and instrumentation designed to objectively record muscle performance characteristics is becoming more widely recognized, especially by professional members of the sports medicine community. The present demand for appropriate equipment is high and new instrumentation for this purpose is being developed. Current instrumentation continues to be modified and improved. The information gained from carefully designed testing protocols is useful and necessary for understanding the role of specific muscle groups in athletic performance and for determining the readiness of the athlete to resume training or competition following injury or deconditioning. With expanded profile data about the muscle performance characteristics of various categories of athletes, training or rehabilitation programs may be adapted to meet the strength, speed, and endurance requirements of a given sport, which may enhance the quality of participation and prevent the occurrence of injury or reinjury.

Action research and the process of continual quality improvement in a cancer center.

The development of a sound quality assurance program is key to the simultaneous development of effective and appropriate standards of care, which should not only promote safe care, but also should reflect current knowledge. To adopt and apply a standard of care to practice, the professional nurse must have a personal investment in each standard's purpose and intent and must clearly support the desired patient outcomes. At Ellis Fischel State Cancer Center, the nursing quality improvement process allows the professional staff to build a standard of care within a shared governance environment. Guided by the Oncology Nursing Society's outcome standards, this action research-oriented quality improvement approach led to the development of standards of care reflecting the complex nature of the cancer nursing specialty. Because of a decentralized approach, the staff nurse is accountable for conducting research that has a direct impact on each clinical area's care delivery system. In addition, the standards of care that are produced serve as the framework for the new-staff orientation program. In this way, the standard of care serves to bring about cultural change by continuously directing caregivers' attention toward methods to improve patient outcomes.

[Systematic aspects of problem-based, case-related, practice-oriented, professional continuing education].

Traditional CME using lectures has not been shown to improve patient care, which is the purpose of all learning in medicine. As knowledge taught mainly by lecturing is not useful for dealing with clinical problems, problem-based courses were developed to integrate general principles of basic and clinical sciences in undergraduate education. Case-based teaching is used for effective teaching of patient care. Six components were defined as a case system for clinical education: 1. Demonstration of patients; 2. free access to patients or data of patients, 3. responsible care of the patients, 4. acquisition of basic knowledge integrated in clinical problem-solving, 5. evaluation of documented work and 6. participation in patient related research. Surveys of general physicians showed that they regard a multitude of the components of the case system as desirable for learning in quality circles. Small group work can be implemented using computer supported work.

Developing leaders for the health service of the future: the experience in Ireland.

How can we identify and then develop the people who will provide the leadership in our health services in the next ten to fifteen years? That was the question the Office of Health Management addressed in 1997. The paper outlines the leadership competencies used to select the participants and indicate how they were developed. It will also describe the programmes content and design, the issues that arose in the course of the programme and the evaluation framework. It concludes with an assessement of the obstacles the potential young leaders are likely to encounter and gives an indication of how it is proposed to continue to support these young people in the future.

Boyer's model of scholarly nursing applied to professional development.

Scholarship in nursing often is difficult to define and identify, but it is an important element in nursing. Scholars are needed in every practice setting to question the status quo and pursue fresh approaches to issues. This article examines Boyer's revised model of scholarship, which uses paradigms of discovery, integration, application, and teaching. Knowledge dissemination is emphasized as an important component for sharing information with fellow nurses and the public. Boyer's model of nursing scholarship provides institutions and OR nurse managers with a scholarly method for evaluating employee efforts. Strategies offered include professional development plans and employee portfolios. These applications encourage scholarly activities.

The choice of practice location.

A ten per cent sample survey of all general practitioners in England and Wales in 1969-70 included two questions about the choice of practice location. The most common reasons given were the absence of any real alternatives (in the immediate post-war period), the influence of family or friends, the existence of medical contacts in the area, and favourable points about the practice itself. In considering possible future moves, general practitioners would pay closest attention to the educational facilities of an area, its rural or coastal location, its social and cultural amenities, and the practice conditions. The conclusion is drawn that financial incentives are unlikely to contribute much towards a more equal distribution of general-practitioner manpower. More thought should be given to recruitment to the medical profession in under-doctored areas through the development of the highest professional standards and facilities in such places.

Supporting clinical nursing leadership and professional practice at the unit level.

Professional practice models (PPMs) are innovative approaches for improving health care delivery. Implementation of these new practice models needs to be well planned and supported to ensure their success in producing desired outcomes. This article describes two programs used to implement a PPM in a health care organization, focusing mainly on a program design intended to support the implementation of a new PPM on a medical-surgical hospital unit. The program blends the concepts of the organization's PPM with the leadership concepts from the AACN's Clinical Nurse Leader role to help nurses further develop their professional behaviors and clinical leadership skills.

Professional holistic care of the person with a stoma: online learning.

Stoma care nurse specialists are valued for their diverse expertise, but it is essential that all practitioners who regularly care for people with stomas have the opportunity to develop professionally and influence this important area of practice. The vision of a clinical and educational team from Suffolk and south Norfolk led to the development of innovative web-based learning material. The aim is to inspire nurses to engage actively with people who have a stoma. 'Professional care of the person with a stoma' is about caring for the whole person, physically and emotionally, from the period before surgery, to their continuing care in the community. An overview of four aspects of stoma care is presented here as a way of sharing with a wider audience the expert practice harnessed during the development of this web-based module.

Australian public health policy in 2003 - 2004.

In Australia, compared with other developed countries the many and varied programs which comprise public health have continued to be funded poorly and unsystematically, particularly given the amount of publicly voiced political support. In 2003, the major public health policy developments in communicable disease control were in the fields of SARS, and vaccine funding, whilst the TGA was focused on the Pan Pharmaceutical crisis. Programs directed to health maintenance and healthy ageing were approved. The tertiary education sector was involved in the development of programs for training the public health workforce and new professional qualifications and competencies. The Abelson Report received support from overseas experts, providing a potential platform for calls to improve national funding for future Australian preventive programs; however, inconsistencies continued across all jurisdictions in their approaches to tackling national health priorities. Despite 2004 being an election year, public health policy was not visible, with the bulk of the public health funding available in the 2004/05 federal budget allocated to managing such emerging risks as avian flu. We conclude by suggesting several implications for the future.

Active euthanasia in The Netherlands.

Active euthanasia in the Netherlands remains a topic for both professional and public debate. However, many aspects of the medical practice of active euthanasia remain unclear, and no figures on the actual incidence of this practice exist. Legally, active euthanasia is a criminal offense, but a pattern of jurisprudence has developed since the first court case in 1973 that has allowed physicians to practice euthanasia under certain strict conditions. Two proposals, one from the Royal Dutch Medical Association and one from a government-appointed state commission, have advised that the current law be changed. While the debate continues, a number of Dutch institutions have developed procedures and policies to enable physicians and health care providers to participate in active euthanasia in an acceptable and controllable manner. However, many Dutch physicians remain uncomfortable with the professional and public tolerance of this practice.

Questioning the claims from Kaiser.

BACKGROUND: The article by Feachem et al, published in the BMJ in 2002, claimed to show that, compared with the United Kingdom (UK) National Health Service (NHS), the Kaiser Permanente healthcare system in the United States (US) has similar healthcare costs per capita, and performance that is considerably better in certain respects. AIM: To assess the accuracy of Feachem et al's comparison and conclusions. METHOD: Detailed re-examination of the data and methods used and consideration of the 82 letters responding to the article. RESULTS: Analyses revealed four main areas in which Feachem et al's methodology was flawed. Firstly, the populations of patients served by Kaiser Permanente and by the NHS are fundamentally different. Kaiser's patients are mainly employed, significantly younger, and significantly less socially deprived and so are healthier. Feachem et al fail to adjust adequately for these factors. Secondly, Feachem et al have wrongly inflated NHS costs by omitting substantial user charges payable by Kaiser members for care, excluding the costs of marketing and administration, and deducting the surplus from Kaiser's costs while underestimating the capital charge element of the NHS budget and other costs. They also used two methods of converting currency, the currency rate and a health purchasing power parity conversion. This is double counting. Feachem et al reported that NHS costs were 10% less per head than Kaiser. Correcting for the double currency conversion gives the NHS a 40% cost advantage such that per capita costs are 1161 dollars and 1951 dollars for the NHS and Kaiser, respectively. Thirdly, Feachem et al use non-standardised data for NHS bed days from the Organisation for Economic Cooperation and Development, rather than official Department of Health bed availability and activity statistics for England. Leaving aside the non-comparability of the population and lack of standardisation of the data, the result is to inflate NHS acute bed use and underestimate the efficiency of performance by at least 10%. Similar criticisms apply to their selective use of performance measures. Finally, Feachem et al claim that Kaiser is a more integrated system than the NHS. The NHS provides health care to around 60 million people free at the point of delivery, long-term and psychiatric care, and continuing care after 100 days whereas Kaiser provides care to 6 million people, mainly employed and privately insured. Important functions, such as health protection, education and training of healthcare professionals, and research and development are not included or properly costed in Feachem et al's integrated model. CONCLUSION: We have re-examined the statements made by Feachem et al and show that the claims are unsupported by the evidence. The NHS is not similar to Kaiser in coverage, costs or performance.

Fiber glass and rock/slag wool exposure of professional and do-it-yourself installers.

The fiber glass (FG) and rock/slag wool (RSW) manufacturers have developed a Health and Safety Partnership Program (HSPP) with the participation and oversight of the Occupational Safety and Health Administration (OSHA). Among its many provisions the HSPP includes the continuing study of FG and RSW workplace concentrations in manufacturing facilities operated by FG/RSW producers and among their customers and end users. This analysis estimates the probable cumulative lifetime exposure (fiber-months/cubic centimeter [f-months/cc]) to those who install FG and RSW insulation in residential, commercial, and industrial buildings in Canada and the United States. Both professional and do-it-yourself (DIY) cohorts are studied and the estimated working lifetime exposures are compared with benchmark values derived from an analysis of the epidemiological studies of FG and RSW manufacturing cohorts. The key finding of this analysis is that both of these end-user cohorts are likely to have substantially lower cumulative lifetime exposures than the manufacturing cohorts. As the most recent updates of the epidemiological studies concluded that there was no significant increase in respiratory system cancer among the manufacturing cohorts, there is likely to be even less risk for the installer cohorts. This analysis also underscores the wisdom of stewardship activities in the HSPP, particularly those directed at measuring and controlling exposure.

Evergreen staff: building a tree of knowledge for continuous learning.

Healthcare institutions breathed a collective sigh of relief on January 1, when efforts made for remediation, testing, and contingency planning for the year 2000 finally paid off. Now that the technology has been improved to ensure compatibility, it is important to keep the momentum going to improve efficiency and increase productivity and patient satisfaction levels. One way to incorporate organizational priorities, goals, and strategy at the departmental level is to develop an education plan that stresses mastering fundamental skills. This article explores the components and role of an education plan and identifies the types of efforts that result in the greatest return. It concludes with a case study.
