Jejunal atresia with "apple peel" deformity: report of one case.

A case of jejunal atresia with "apple peel" deformity is presented. A 27-year-old woman, gravida 3, para 2, suffered from premature labor pain and vaginal bleeding at 32 weeks of gestation. Prenatal ultrasound examination revealed presence of three areas of cystic dilatation in the fetal abdomen and marked polyhydramnios, indicating a high bowel obstruction. A female baby was delivered by Cesarean section with Apgar scores of 7 and 8 at one and five minutes, respectively. Physical examination showed abdominal distention. Nasogastric tube aspiration got more than 50 ml of blood-tinged fluid. Roentgenography demonstrated a high jejunal obstruction as well as a normally rotated colon. Laparotomy disclosed proximal jejunal atresia with typical "apple peel" appearance of the distal small bowel. An end-to-end anastomosis was performed smoothly after resection of both the proximal and distal small bowels. A reduction tapering antimesenteric jejunoplasty was also done. Partial parenteral nutrition had been administered for 10 days followed by enteric feeding. The postoperative course was uneventful. Her chromosomal analysis revealed a normal karyotype of 46,XX.

[A case of Sjögren's syndrome with a clinical course similar to diffuse panbronchiolitis].

A 64-year-old man was admitted because of some infection of the airway. Chest roentgenography showed a bilateral reticulonodular shadow which was similar to the shadow of DPB. Pathological findings obtained by lung biopsy indicated bronchiolitis. This case was diagnosed as Sjögren's syndrome because of the increase of titers of anti-ssA and anti-ssB antibody, lachrymal dysfunction, pathological findings and sialography. The relationship between DPB and lung lesions of Sjögren's syndrome was investigated.

[Value of contrast arthrography in the diagnosis of hemophilic arthropathy].

X-ray semiotics of changes in the roentgen-negative components of the knee joint at different stages of hemophilic arthropathy has been described basing on the study of 46 contrast arthrography ++ of the joint, and their correlation with the results of macroscopic and histological investigations of the tissues removed during surgery. Identification of the character of the damage in the soft-tissue and cartilage components of the joint is particularly important at the early stages of arthropathy, as it enables determining the optimum therapeutic tactics when the data of routine roentgenography give insufficient information, and changes in the bones are limited and, as a rule, reversible.

[X-ray assessment of the size of the nasopharynx].

A new method for x-ray diagnosis of nasopharyngeal size is based on the use of long-distance roentgenography in the lateral projection and of skeletal markers to estimate the cephalometric parameters. The authors draw attention to the usefulness of measuring the size of bone base, soft-tissue structures, and nasopharyngeal air column when assessing the function of the nasopharynx and throat.

[A case of pulmonary thromboembolism due to circulating lupus anticoagulant].

A 22-year-old man was admitted because of hemosputum and progressive dyspnea with 3 attacks of chest pain and dyspnea over the previous 4 months. Chest roentgenography showed pulmonary infarction of the left lower lobe, and the diagnosis was confirmed by pulmonary perfusion and inhalation scintigraphy and pulmonary arteriography. Thrombolytic therapy was performed, but no significant effect could be obtained and anticoagulant therapy was performed continuously. No deep-vein thrombosis could be seen. He was considered to have autoimmune hemolytic anemia with lupus anticoagulant on the basis of auto-antibody data. Lupus anticoagulant is an antibody to phospholipid, and it is suggested that a decrease in the production of prostanoid in the endothelium causes thrombosis. In this case, as the patient showed a low level of 6-keto-PGF1 alpha in the blood, it is suggested that one of the etiological factors of pulmonary thromboembolism is a disorder of prostacyclin production in the endothelium, causing thrombosis by lupus anticoagulant.

[An experimental study on the composite graft of ceramic xenogenic bone and bone morphogenetic protein].

The ceramic bovine bones combined with bone morphogenetic protein (BMP) were implanted in the mandibular artificial defects of dogs. Observed by roentgenography, histology (Scanning electronic microscope and light microscope), and radioisotope scanning. The results confirmed that the composite bone possessed superior biocompatibility, osteoinductive and no obvious immunogenic reaction. One year later, the grafted bone and the new bone fused into each other with the histologic structure similar to the host bone. This is a new xenogenic bone graft material which may be used in clinic.

[Survey and analysis of incidence and relevant factors of osteoporosis in the elderly (with a report of 2041 cases)].

This paper reports the results of roentgenography of calcaneum in 2,041 persons without any underlying disease causing osteoporosis. The trabecular patterns of calcaneum were divided into 5 grades according to Jhamaria method. The incidence of osteoporosis is 18.18%, 25.96%, 54.80%, 67.33%, 85.45% and 90% and the average grading of calcaneum were 2.98, 2.96, 2.53, 2.11 and 2.14 in male and 3.50, 2.89, 2.50, 2.06, 1.60 and 1.61 in female in these aged groups respectively Serum Ca, P, Mg and Vitamin D were determined in 1,798 persons over 60 years of age. There is no significant difference among them Bone density was determined in 381 cases, showing 0.507 +/- 0.003, 0.598 +/- 0.095, 0.673 +/- 0.052, and 0.0645 +/- 0.095 among Jhamaria grading index I, II, III, IV. There is significant difference between 1-2 and 3-4 grading index, suggesting Jhamaria method gives a reliable, simple and applicable criterion for diagnosis of osteoporosis. The relevant factors influencing osteoporosis are discussed.

[Persistent left superior vena cava to left atrial communication detected with radionuclide angiography: a case report].

A persistent left superior vena cava (PLSVC) is the common clinical congenital abnormality of the greater central veins. This anomaly may occur as an isolated lesion, but rarely it is significantly associated with other congenital cardiac lesions, particularly it drain into the left atrium. We recently encountered a patient in whom PLSVC to left atrial communication with atrial septal defect was diagnosed on the basis of radionuclide angiography with Tc-99m pertechnetate. Radionuclide angiography might be the noninvasive study of choice in patients suspected of having PLSVC by the clinical findings or chest roentgenography.

Surveillance strategies after resection of carcinoma of the colon and rectum.

The primary aim of postoperative surveillance of patients with carcinoma of the colon and rectum is to detect recurrent tumor when cure is still possible. Most recurrences are detected within 30 months after the initial operation. Patients who have had carcinoma of the colon and rectum must be observed not only because of the risk of recurrence or metastatic disease but also because of the increased risk of subsequent primary carcinomas of the colon and rectum as well as of other sites. Careful history-taking and thorough physical examination provide the first indication of tumor recurrence in as many as 48 per cent of instances. Although the liver is the most common site of metastases from carcinoma of the colon, liver chemistry tests are seldom the first to indicate recurrent disease. Fecal occult blood testing, roentgenography with barium enema and colonoscopy are useful surveillance tools, not for detecting recurrences but for detecting second primary carcinomas. Imaging techniques, such as intravenous pyelography, CT and scintigraphy of liver and spleen are generally not cost-effective in surveillance, but MRI and ultrasonography have shown some promise in detection of recurrence without exposing patients to ionizing radiation. The most effective indicator of recurrent disease is a progressive increase in serial levels of CEA. When CEA levels rise and other methods of imaging cannot account for the change, second-look operation is generally appropriate.

[Gynecomastia in children].

The experience with treatment of 34 children with gynecomastia is summarized. The examination included roentgenography of the Turkish saddle, definition of bone age, investigation of buccal sex chromatin, excretion of 17-ketosteroids, excretory urography against the background of retropneumoperitoneum, ultrasound investigation of the mammary glands. In juvenile gynecomastia, the mammary glands were removed, the expectant tactic being considered unwarranted. The long-term results of the surgical treatment are good.

[Organizational principles of the outpatient examination of patients suspected of malignant tumors].

Out-patient diagnosis of cancer is carried out in three stages which involve activities of different physicians. Cancer is first suspected by a general practitioner. Prophylactic measures other than examination by specialists and chest photo-roentgenography proved insufficient, and the authors give reasons for this. During the second stage, the patient undergoes further thorough examination which should be well organized to succeed. The third stage, viz. making the final diagnosis, should be performed in an oncologic establishment only.

[Application possibilities of human bone-collagen substance as a transplant in skeletal surgery].

We present first results of our clinical trial with a demineralized-bone-matrix (DBM) not only as a transplant to fill up a defect of bone but above all to induce local osteogenesis. Indications and practical applications of the material of transplants are described. By help of the clinical progresses and the reports of laboratory examinations the biological compatibility is regarded in this paper. Roentgen morphologically fine structures of the transplants and their transplant beds are described in serial roentgenography and used as evaluation of processes of transformation and building in of bones. The in a relative good time appearing markings of trajectories of the fine structures in the sphere of the transplant as a sign of accelerated regeneration of "organic bony substance" allows the expectation on being released osteogenetic potencies by DBM for the induction of local bone regeneration.

[Zonographic study of pleural lesions due to asbestos dust].

Results of zonographic examination of the chest were compared with roentgenographic data in 34 patients with asbestos hyalinosis of different severity. The zonographic substrate of pleural hyalinosis are echo-poor intermediate spaces that are visualized bilaterally in different parts of the pleura. It was established that zonographic examination is more sensitive than roentgenography for revealing pleura hardening. Chest zonography is recommended to reveal of exclude pleura hyalinosis induced by asbestos in the absence of typical asbestos-induced changes on the pulmonary x-ray picture.

[Mesothelioma and mesothelial hyperplasia].

Twenty cases of pleural mesothelioma (2 localized and 18 diffuse malignant pleural mesothelioma), 6 cases of diffuse malignant peritoneal mesothelioma and 1 case of mesothelial hyperplasia were reported. The prominent clinical manifestations of malignant mesothelioma was serosal effusion. The exudate was bloody, mucinous and of high specific gravity. It contained numerous mesothelial cells. The diagnosis is difficult because mesothelial hyperplasia and other malignant tumors may mimic mesothelioma in some clinical aspects. Roentgenography, CT scan, cytology of serosal fluid and biopsy world help to establish the diagnosis of mesothelioma. Immunohistochemical features of 4 cases of mesothelioma is negative immunostaining for carcino embryonic antigen (CEA) and positive for keratin antigen. In one case ultrastructural examination showed characteristic features of epithelial mesothelioma cells. The average survival period in case of chemotherapy and/or surgical intervention in patients of this series was 31.3 months. Mesothelial hyperplasia has rather good prognosis; but the strategy of therapy may be conservative.

[Respiratory biomechanics and the state of capillary blood flow in patients with fibrous-cavernous pulmonary tuberculosis before and after partial lung resections].

The article discusses the results of studying the ventilation function of the lungs, biomechanics of respiration, and condition of the capillary blood flow in 36 patients with fibrous-cavernous tuberculosis before and after partial resection. In 69.4% of cases obstructive signs were attended by restrictive signs of deficient ventilation. The disorders of capillary blood flow corresponded to or were more extensive than the zone of affection demonstrated by roentgenography, which made it possible to determine the volume of the surgical intervention objectively. Signs of rigidity of the bronchopulmonary system prevailed in the early period after partial lung resection and deterioration of microcirculation was encountered. Restoration of the functional parameters and improvement of the capillary blood flow in the late-term follow-up period confirmed the efficacy of partial resections conducted in the early stage of complex treatment of patients with fibrous-cavernous pulmonary tuberculosis.
