[Clinico-etiological correlations in children with sciatica].

During a period of 30 years 21 children with sciatic pains were treated at the Developmental Age Neurology, Medical Academy in Gdańsk. The aetiological factors and clinical manifestations were analysed. Nucleus pulposus prolapse was diagnosed in 5 children, in 9 cases neoplasms were found (in 3 cases malignant tumours of the sacral bone, intraspinal tumours in 6 cases); in 4 cases obliterative intraspinal arachnitis, and in 3 cases other causes were diagnosed. With the exception of one child the other ones were in the school-age group. In nearly all cases the onset was insidious. In one-third of children with neoplastic processes the neurological symptoms appeared after injuries and caused initially considerable diagnostic difficulties. In 18 cases disturbances of spinal configuration and mobility were observed. Pain produced by stretching was present in 20 cases. Sphincter disturbances were observed in 6 cases (including 5 with tumours). Motor defects were noticed in all patients, more pronounced in cases of tumours. Patency of the subarachnoid spinal space was without any greater importance for the diagnosis of sciatic pains. Raised protein level in the cerebrospinal fluid was highest in cases of intraspinal tumours, and it was moderately pronounced in obliterative arachnitis, while in nucleus pulposus prolapse the protein level varied. Plain films were of lower diagnostic importance, with the exception of sacral bone tumours and nucleus pulposus prolapse. Of greater importance were contrast medium radiological examinations of fluid spaces.

[Cervical dysplasia].

General characteristics of nucleoli in all layers of stratified squamous epithelium were studied in 234 cases of dysplasia and intraepithelial cancer of cervix uteri. Cytomorphometric measurements were carried out. The nuclear-cytoplasmic ratio was determined. Certain regularities pertinent to neoplastic process progression and aging were established. 2,375 patients with moderate dysplasia and 1,625 patients with marked dysplasia underwent clinical examination. Various schemes of treatment and long-term results were evaluated. Rationale for all-round examination and individually-tailored schemes of treatment is discussed. The data on the end results in 150 untreated cases of dysplasia are presented. The importance of detection and treatment of dysplasia in the control of cervical cancer is suggested.

[Characteristics of the restructuring of the microcirculatory bed of the cervical mucosa in cancer].

Complex colposcopic and macro-microscopic examination of the microcirculatory bed ( MCB ) of the normal uterine cervix and at various stages of the neoplastic process is performed. It is established that MCB undergoes restructuration in the presence of focal epithelial alterations and takes place in the formation of the colposcopic picture. The general view of MCB different parts, their distribution, pleomorphism, peculiarities of their combinations may serve as certain tests in the provisional diagnosis of epithelial displasia, preinvasive and invasive carcinoma. Specific MCB alterations are manifested in marked hemocapillaries proliferation of the subepithelial plexus in precancerous conditions with its enhancement, deformation and atypia in intra-epithelial carcinoma. The restructuration of other MCB components in epithelial displasias and preinvasive carcinoma is stereotype.

[Effect of ionizing radiations on certain parameters of the immune status of cancer patients].

The authors study the action of ionizing radiations upon some aspects of the humoral and cellular immune responses in cancer patients. Both humoral and cellular responses are altered in patients having malignant neoplasms; the therapeutic irradiation diminishes the phagocytic activity of monocytes and granulocytes, the number of lymphocytes, the formation of E rosettes, and the lymphocytes blastic transformation, and produces some changes in immunoglobulins. However, the irradiation-induced changes are such that radiation therapy does not conditionate negatively the neoplastic process evolution.

The fatty liver: pitfalls of the CT and angiographic evaluation of metastatic disease.

Five patients with known primary malignancy demonstrated fatty infiltration of the liver on routine computed tomography (CT) examination. Within the diffuse fatty infiltration were focal areas of relatively increased beam attenuation in four cases. These areas of increased beam attenuation are nonspecific as to etiology. Their differential diagnosis is discussed. Since the true extent of the neoplastic process may be underestimated by conventional CT and celiac angiography, superselective angiography, computed angiotomography, or biopsy is required to accurately assess the etiology and the extent of the disease.

[14C-riboflavin metabolism in mice with Ehrlich's ascitic tumor].

The distribution of 14C-riboflavin in the tissues and elimination of radioactive metabolites with the urine during dynamic development (in 3--5--10 days) of the ascitic Ehrlich's tumour in mice after administration of 10 gamma g per mouse of tagged vitamin in 1--3--24 hr subcutaneously and intragastrically were studied. The development of the neoplastic process was accompanied by a reduced deposition of 14C-riboflavin in the tissues of the macroorganism, an increased elimination of tagged metabolites with the urine, a rising content of the tag in the ascitic fluid. The concentration of the tagged vitamin in the tumour cells remained continuously high in all periods of the test under investigation, its results bearing evidence to an impoverishment of the macroorganism in riboflavial under conditions of the malignant tumour growth.

[Fluorescence angiography in the pathological findings of the tympanic membrane].

Vascularity changes in certain pathological findings of tympanic membranes were examined via a newly developed technique of fluorescence angiography. Characteristic pictures of fluorescence-stained vasculature were found in chronic inflammations or in neoplastic processes. Around the margins of perforations or over sclerotic plaques, there was decreased vascular marking with obviously increased vascular permeability. A lack of vascularisation was observed in cases of atrophic membranes. So-called glomus tumours showed an early onset of strong hyperfluorescence, representing the dense vascularisation of these neoplasms. Our findings in chronic inflammatory lesions such as perforations, sclerotic plaques, and atrophies strongly suggest disturbances of the blood supply. In consequence, areas with such vascularity changes should be excised widely during tympanoplasties and undercovered e.g. with temporal fascia. The high staining ability of the so-called glomus tumours may be of differential diagnostic value in certain cases.

[X-ray evaluation of the results of combined radiation treatment of cancer of the corpus uteri].

Histerography was performed to determine the effect of concomitant radiation therapy provided to 46 patients with corpus uteri cancer. Stages I-II. It should be noted that histerography combined with clinical and pathomorphological data makes it possible to control the time course of a neoplastic process in the uterine cavity following radiation exposure. Histerography performed 3-6 mos. after radiation therapy is an objective method for the evaluation of therapeutic results.

Oncocytic lesions of the larynx.

Eight cases of laryngeal oncocytic lesions treated at Toronto Western Hospital between 1957 and 1977 are reviewed. The typical clinical course was of a benign lesion with prolonged dysphonia. A smaller number with bulkier lesions developed symptoms of airway obstruction. The lesions were supraglottic and cystic. They were excised either endoscopically or via a laryngofissure. The pathologic features support the concept that the lesion represents a metaplasiahyperplasia, and therefore the term 'oncocytic cyst' is preferred to terms which imply a neoplastic process. Three patients with oncocytic cysts had associated squamous dysplasia of the vocal cord. Pathologically this supports the theory of tissue response to chronic irritation, since all three were heavy smokers. Clincially the finding necessitates longterm follow-up with a view to possible squamous cell carcinoma that may arise on the true cords. Follow-up in this series showed no recurrence of symptoms in six of eight cases, two being lost to follow-up.

Acute lymphoblastic leukemia in an adult patient.

A case of acute lymphoblastic leukemia in an adult patient has been presented. Acute lymphoblastic leukemia is a neoplastic process in which the leukocyte precursors have been transformed and no longer respond to the forces that normally regulate their proliferation and maturation. The symptomatology, diagnostic procedures, and treatment modality have been discussed. Unfortunately, although complete remission was obtained for two years, the patient developed recurrent acute lymphoblastic leukemia and subsequently died from a massive, uncontrollable infection.

[Reaction between syngeneic hematopoietic stem cells and lymphocytes in leukemic AKR mice].

The syngeneic donor-recipient model was used to examine the influence of T lymphocytes from AKR mice with spontaneous leukemia on colony-formation and differentiation of hemopoietic stem cells. It was shown that in the stage of the neoplastic process development, the cells of lymph nodes and the thymus inhibited the colony-forming capacity in all the three hemopoietic lines (erythroid, myeloid, megakaryocytic) of the syngeneic normal and autologous bone marrow. In the late stages of leukemia aggravated by generalized infection, T lymphocytes stimulated the exo- and endogenous growth of the granuloid colonies, changing the erythroid-myeloid index to 1:1-1:2.7.

Paraneoplastic syndromes.

Neoplasms can produce a variety of remote effects on the host; these are referred to as paraneoplastic syndromes. The syndromes may affect any of the systems of the body, may precede or follow the diagnosis of the underlying neoplasm, and may or may not parallel the course of the neoplasm in severity. The diagnosis of and therapy for these syndromes can be challenging to a physician, but successful therapy may bring about worthwhile relief for the patient. In addition, the syndromes and the substances that cause them are sometimes useful in diagnosing and in following the course of certain neoplasms. Perhaps of greater importance, study of these remote effects of neoplasia may shed light on the nature of the neoplastic process itself.

[Relationship between immunoglobulin levels in cerebrospinal fluid and serum].

By means of radial immunodiffusion, immunoglobulins A, G, M and albumin were determined in serum and unconcentrated cerebrospinal fluid from 182 controls and 141 patients. Concentrations in cerebrospinal fluid and serum of patients did not correlate, not even in those groups whose elevated immunoglobulin and albumin concentrations in cerebrospinal fluid resulted from damage to the blood-cerebrospinal fluid barrier, as in inflammatory diseases and neoplastic processes of the central nervous system or of the meninges.--Concentrations of immunoglobulins in cerebrospinal fluid should therefore be evaluated independently of the serum concentrations. Cerebrospinal fluid and serum constitute samples from two separate compartments which are capable of independent immune reactions.

Squamous cell carcinoma metastatic to placenta and ovary.

A woman previously treated for squamous cell carcinoma of the neck underwent a cesarean section for fetal distress. Extensive metastases of the placenta and an ovary were histologically compatible with her prior malignancy. Diffuse infiltration of the placenta by this neoplastic process was associated with an infant who was small for gestational age and who had no evidence of metastatic disease.

Phrenic nerve conduction time measurement in pulmonary disorders.

Using surface electrodes over the lower chest wall, we have measured the phrenic nerve conduction time, i.e. the time interval between the stimulation of the phrenic nerve in the neck and the onset of the diaphragmatic muscle action potential, in 31 control subjects (bilaterally in 19 of them) and in patients in whom the phrenic nerve was at risk from intrathoracic disease. The data obtained from the patients showed that 1 degrees this technique, unlike fluoroscopic examination of the diaphragm, provides quantitative information on phrenic nerve function; in particular, it may detect partial impairment of the nerve by a neoplastic process; 2 degrees it allows an accurate assessment of phrenic nerve function even when lung collapse, pleural effusion, or pleural adhesions make it difficult to visualize the movement of the diaphragm; and 3 degrees it avoids the falsepositives seen on fluoroscopy. We suggest that measurement of phrenic nerve conduction time should be preferred to diaphragmatic fluoroscopic screening in the evaluation of phrenic nerve function (Acta neurol, belg, 1982, 82, 91-98).

Riboflavin, vitamin A and zinc status in Chinese subjects in a high-risk area for oesophageal cancer in China.

1. An endoscopic survey was done in Linxian, Henan Province, Northern China on 527 people in May 1980 and histological evidence of oesophageal lesions was obtained. 2. Blood samples were collected from 58 men and 53 women aged 22-71 yr. Measurements of riboflavin, vitamin A and zinc status were made. 3. Most (97 per cent) had biochemical ariboflavinosis on the basis of erythrocyte glutathione reductase activation coefficients equal to or greater than 1.30. Five per cent of subjects had deficient vitamin A status (plasma retinol less than 10 micrograms/dl and one of these subjects reported night blindness) and 37 per cent had low vitamin A status (10-20 micrograms/dl). Approximately 24 per cent of the Chinese had low plasma zinc by comparison with values for subjects in Denver. Plasma retinol binding protein concentration was also low but was only measured in 56 subjects. 4. Concentrations of plasma carotene were relatively high and only one person had a value less than 20 micrograms/dl. The value of plasma carotene as a measure of vitamin A status in this community is discussed. 5. There was no difference between the sexes for any of the measurements made. In addition there was no difference in the biochemical measurements between those with and without oesophageal lesions (oesophagitis 66 per cent, clear cell acanthosis (CCA) 88 per cent) or those with intermittent dysphagia (26 per cent). 6. Multiple linear regression showed no relationship between the biochemical data and either oesophagitis or CCA. There were, however, inverse relationships between oesophagitis and CCA in both sexes and in addition, plasma carotene and riboflavin status tended to fall while plasma retinol tended to increase with age. 7. The role which these nutritional deficiencies may play in facilitating neoplastic processes is discussed.

Actinomycosis. A cause of pulmonary and mediastinal mass lesions in children.

Two patients with intrathoracic actinomycosis were examined. One child was asymptomatic and had a slowly expanding lesion in the left upper lobe. The other child had a chronic illness with back pain, weight loss, amenorrhea, and a posterior mediastinal mass. Establishing the cause of these lesions and making the distinction between a neoplastic process and infection were particularly difficult in both cases. Intrathoracic actinomycosis should be considered in the differential diagnosis of pulmonary and mediastinal mass lesions.
