Locked twins: a successful outcome.

Twin interlocking is a rare complication that has a high fetal mortality and morbidity rate. We report a successful outcome of locked twins at 32 weeks gestation following spontaneous labor.

Gynecologic and reproductive issues in women with spinal cord injury.

As the treatment of SCI and its secondary medical complications improves, women with SCI are becoming increasingly successful in re-integrating themselves into society. Part of this re-integration involves the resumption of active sexual lives and motherhood. Clinicians who care for patients with SCIs play an important role in facilitating the resumption of these roles. With appropriate follow-up and care, women with SCI should be expected to maintain good gynecologic health and deliver healthy children with minimal complications.

[Clinical experience in diabetes and pregnancy].

We followed patients with pregnancy and diabetes in an outpatient clinic. 240 had gestational diabetes, 16 had type II and 5 type I diabetes. 85% of 110 patients with gestational diabetes had normal glucose tolerance test post partum (AGT). Type I patients were younger (25 years old) than AGT (32) or type II (33) patients. Complications frequently observed among diabetics included hypertension, premature membrane rupture and polyhydroamnios (the latter only among AGT and type II patients). Insulin was required for diabetes control in 14% of cases. Cesarean section was more frequent in diabetics than in a control population (21%): AGT 45%, type II 45% and type I 60%. Larger newborns occurred in 21% of AGT and 22% of type II as compared to 6% in controls. Neonatal mortality was 2.1% in AGT patients (0.8% in controls). Hyperbilirrubinemia, polyglobulia and hypocalcemia were more frequent among newborns of diabetic patients.

[Weight and weight gain in pregnancy].

This article reviews different aspects of maternal weight before and during pregnancy and weight gain in pregnancy, e.g. causes of undernourishment (hyperemesis, anorexia nervosa and bulimia nervosa). Physiological weight gain during pregnancy is normally between 10 and 16 kg, representing 20% of the body weight before pregnancy. The increase in weight is usually lowest during the 1st trimester and greatest between the 17th and the 24th week of pregnancy. Low maternal weight at conception may cause low birthweight. Undernourishment may cause premature delivery or low birthweight, or both. There is an increased risk of gestational diabetes and macrosomia, as well as preterm delivery and hypertension in pregnant women who are overweight. There is also an increased risk of complications arising during general anaesthesia and operative delivery in severely overweight women. These women should be offered heparin or dextran as thrombosis prophylaxis where a caesarean section is to be performed. They should also be given antibiotic prophylaxis. A weight gain of between 7 and 12 kg reduces the risk of complications in overweight patients.

[Birth after two cesarean sections: the role of trial of labor].

OBJECTIVE: To study the management of labor following two cesarean sections, we evaluated maternal and neonatal morbidity subsequent to this attitude. PATIENTS AND METHODS: A retrospective study was conducted over a 6-year period (1-1-1990 to 31-12-1995) in 184 women presenting two uterine scars. Trial of labor was allowed in 96 cases with cephalic presentation and a normal pelvis. RESULTS: The rate of vaginal birth was 65%. Three patients presented a uterine scar dehiscence and in one of them hemostasis hysterectomy for uterine atony was performed. Neonatal outcome was good in all cases. CONCLUSION: Trial of labor after two cesarean sections is possible in the majority of cases. Rate of vaginal birth is high and maternal and fetal morbidity is low.
