Brain and cervical spine injuries occurring during organized sports activities in children and adolescents.

Eighty per cent of severe sports-related central nervous system trauma occurs as a result of collision sports, chiefly American football and rugby union football, followed by wrestling and gymnastics. Although serious head injury is uncommon, episodes of concussion are frequent; repeated concussion should be grounds for suggesting that the athlete give up collision sport. American and rugby union football are the sports mainly responsible for cervical spine injury with resultant quadriplegia.

[Experiences with the application of physiotherapy in peripheral arterial circulatory disorders of the lower extremities (stage I and IIa) under ambulatory conditions].

In a comparative study the authors report on results of series of physical therapy in disturbances of the peripheral arterial blood supply. Examined were the behaviour of blood pressure, the painless walking distance and the peripheral microcirculation with the xenon-133-muscle clearance method before and after treatments lasting 10 to 12 weeks with combined exercises consisting of gymnastics and walking according to the interval principle (twice a week), with sauna visits twice a week as well as before and after diadynamic series of the current. In all therapy groups an improvement of the peripheral haemodynamics could be statistically ascertained. The distance of the intermittent claudication prolonged particularly in the group with interval walking. In the walking group and in the sauna group the values of the systolic blood pressure were significantly reduced. Side-effects were not observed. Therapy effects of the series of physical therapy described are best to be proved by the xenon-133 muscle clearance.

[Development and preservation of locomotor ability in the aged].

For maintaining and developing motor mobility in old age motor activity is essential. We can take from the phylogenesis and ontogenesis of the human being how important physical activity is for personality development and for maintaining physical fitness in old age. Many phenomena, which have so far been thought to be due to natural consequences of the ageing process, can now be traced back to lack of physical activity. These findings are illustrated by examples referring to the most important subsystems of our organism (such as the central nervous system, the cardiovascular system etc.). To keep these subsystems and with them our organism as a whole functioning as well as possible, we must improve their specific adaptability through sports and exercise. Sports and exercise for the elderly as well as gymnastics for senior citizens should therefore adequately improve coordinative skills, the ability of the muscles to relax, joint flexibility, muscle strength, endurance, vegetative adaptability , stress tolerance, controlling body-weight, and resistance to infections. Training plans for the elderly should include as wide a range of abilities and skills and as great a variety of activities as possible. In this way physical activity will be most efficient as a means of prophylaxis against premature deterioration of motor performance in old age, it will maintain motor mobility, which enables us to lead an independent life. By gaining new experience older people will increase their well-being and improve the quality of their life.

[Conservative treatment of lumbar intervertebral disc displacement and its limits].

Non-surgical therapy of lumbar disc protrusions is based on complete differential diagnosis. Drug therapy means mainly muscle relaxants partly combined with analgetics. Physiotherapy is important, especially gymnastics and swimming. Chirotherapy is in most cases useful, but if 3 or 4 therapeutic sessions are without success, operation will be necessary. Indication ofr an operation can be (relatively) persisting disturbances, absolute indication is any kind of weakness or paresis.

The female athlete.

Anatomic considerations are the female athlete's wider pelvis, shorter extremities and lower center of gravity. There is little qualitative difference in the muscle tissue of men and women; differences in strength stem from the amount of muscle mass. Amenorrhea/oligomenorrhea is common in runners, ballet dancers, cyclists, gymnasts, body builders, figure skaters and, to a lesser extent, swimmers. Pregnancy limits activity, but current evidence indicates that exercise during pregnancy is not harmful to either the mother or the fetus.

[Outpatient rehabilitation of postinfarct heart failure].

Patients who had had myocardial infarction complicated by the initial stages of cardiac insufficiency were tested for capacity for work and haemodynamics while undergoing outpatient rehabilitation. A rehabilitation program is offered based on self-training under periodic control, which consists in individual walks and medical gymnastics. Patients undergoing rehabilitation compared to the control groups show statistically significant decrease of symptoms of cardiac insufficiency, enhanced physical capacity for work, improved haemodynamics. This rehabilitation program can be recommended for patients with initial stages of cardiac insufficiency.

[The rank value of sports and movement therapy in patients with breast or pelvic cancer].

In the summer of 1980 fifty women with breast or uterine cancer participating in a health resort measure for cancer aftercare took part in an extended sports and movement therapy programme, which included hiking, swimming, play and dance activities, as well as specific gymnastic exercises to improve shoulder joint function. Only 38 percent of the women had previously received physical therapy during primary clinical treatment. On the basis of a standardized questionnaire submitted at the beginning and at the end of the aftercare measure, information was obtained from 42 women on their holiday and leisure behaviour, on the expected aftercare measure outcome, as well as their attitudes towards sports. In a follow-up interview some six months later 71 percent reported continued sports activities. Exercise programmes must be simple, their effectiveness explained, and they must be felt to be beneficial. As had been the case with the coronary sports groups, the establishment of local "cancer sports groups" needs to be actively pursued.

[Physical activities and sports in asthmatic patients].

Asthma patients are too often advised to refrain from sport. Enforced sedentariness, especially in children, leads to muscle hypotonia, reduced mechanical efficiency, paramorphisms, and adverse psychological consequences. Not all asthmatics develop airway spasm as a result of exercise. On the other hand, there are subjects whose bronchial hyper-reactivity is stimulated solely by muscular effort. The pathogenesis of exercise-induced bronchospasm is not fully understood. In any event, numerous studies have demonstrated the beneficial and even therapeutic effect of physical exercise and sport in cases of asthma. Provided they are practised with judgment and in accordance with a suitable programme, swimming, activities of an alternating aerobic and anaerobic type, cross-country skiing, gymnastics, and fencing are primarily indicated. Some asthmatics have also won Olympic medals. In the light of the studies carried out so far, it is strongly suggested that asthmatics be encouraged to take up sport suitable to their psychophysical characteristics, and not kept wrapped up in cotton wool.

[The effect of respiratory exercise on the Hirtz index and thoracic value in subjects with idiopathic scoliosis].

The effects of prolonged gymnastics on respiratory capacity (assessed by means of the Hirtz index and the chest value) were evaluated in patients aged 10-15 yr with scoliotic curves of not more than 30 degrees. The mean overall Hirtz value, initially well below normal, displayed an average improvement of 0.44 (higher in lumbar forms, and decreasing in dorsolumbar, double and dorsal scoliosis). As expected, the chest value was virtually unchanged.

[Physical therapy in rheumatic diseases].

Physicotherapy is of rather high importance for many rheumatic diseases. Cold as well as warm applications will be positively effecting alleviation of distress and pain if close observation of the various stages is guaranteed. Indications for the different electrotherapeutic proceedings, however, have to some extent not been put up yet. Medical gymnastics is so far representing the only causally active therapy. The present study is demonstrating physicotherapeutic methods in relation to stages of rheumatic disease, as for instance in rheumatoid arthritis, ankylosing spondylitis, during some forms of non-articular rheumatism and--only fleetingly--arthrosis.

[Five years of movement therapy - experiences from health resort work (author's transl)].

Based on the experiences gained with over 10,600 patients affected by attritional, degenerative or other changes of the locomotor system, as well as by insufficiencies of the cardiovascular system, early rehabilitative health resort measures are reported on. Assuming appropriate prior diagnostic as well as monitoring procedures being provided form movement therapy and sports, conceived as integrative components of an active treatment plan comprising natural remedies, are gaining increased importance. In line with sports physiology evidence, adequate improvement, however, presupposes application of these measures at a rate apt of achieving an appreciable, measurable training effect. This is true especially in those cases where a wide spectrum of gymnastic or sports activities - with an emphasis on group activity and expert supervision - are offered that are not based on the principles of increased performance alone but are designed to give some scope to personal inclinations. Along with its purely therapeutical value, health resort early rehabilitation conceived on these lines offers meaningful opportunities for actively experiencing success, an experience that is capable of giving new impulses for a modified life style at home, thereby contributing positively towards the general health of our population.

[Indices of the sympathetic-adrenal and hypophyseal-adrenocortical system reactions in myocardial infarct patients during physical rehabilitation].

Changes in the content of hormones of the cortical and medullary layers of the adrenals in 40 patients with myocardial infarction were studied during physical rehabilitation. Increased activity of the sympathetico-adrenal and hypophyseal-adrenocortical systems after therapeutical gymnastics was revealed. The greatest shifts in all indices were noted in the first and fourth movement regimens. Changes in the blood hormone content were most obvious. The importance of studying hormones during rehabilitation of patients by means of exercise therapy is discussed. Fluorometry was employed in tests for adrenal hormones in blood and urine and for determining their concentration.

[Sport and kinesiotherapy within the framework of health resort clinics for prevention and medical rehabilitation (author's transl)].

In most treatment clinics a considerable portion of the treatment programme, is terms of space as well of time occupied, is dedicated to the kinesiotherapy field. Based on the register of the Verband Deutscher Rentenversicherungsträger (Federation of German Pension Insurance Institutes), 433 health resort clinics were surveyed by mail in 1978/79 in order to obtain information on their sport and kinesiotherapy situations. The present findings are based on the replies of 206 clinics, corresponding to 55 percent (i.e. 39,840 beds) of the initial total of beds. The majority of patients covered suffered from disorders of inner organ function (66 percent) or from musculo-sceletal impairments (22 percent). Actual delivery of the sport and kinesiotherapy programmes lies with massage and balneology personnel in 60 percent, physiotherapists and gymnastics teachers in 32 percent of cases, and only in some 2.5 percent are the measures implemented by qualified sport teachers. The question is discussed whether these percentages are compatible with the health educational goals of present-day concepts for preventive or rehabilitative medical treatment in the health resort clinics. Changes are needed both at the staffing and organisational levels (staffing schedules, space allotment) if the pension insurance institutes' approach of "health do it yourself" is to prove successful in this field.

[Closed drainage of the urinary bladder. Method and results in the rehabilitation of the infected, contracted bladder in stroke patients].

Catheterism of the urinary bladder in the primary therapy after stroke is often the initial condition for the development of an infected contracted bladder. The patient is bearing his catheter for life because there is a disproportion of production of urine (greater than 1500 ml) and capacity of the urinary bladder (less than 100 ml). Infection and restriction of movement hinder his discharge from hospital. There are five conditions for a treatment of contracted bladder using a closed drainage of the bladder: 1. functional training of the capacity, 2. reduction of reinfection and antibiotic therapy, 3. training of sensibility, 4. use of day-and-night-rhythm, 5. gymnastics for pelvic floor. The therapeutic result--200 ml capacity of urinary bladder, control of infection and voluntary miction--can be reached after 4-6 weeks, if patient, nursing staff and physician are cooperative. The experiences of 8 cases demonstrate that a contracted bladder needs not be a final state, but can be rehabilitated.

Body composition and somatotype characteristics of junior Olympic athletes.

Body composition and somatotype were determined in Junior Olympic competitors to evaluate the structural characteristics concomitant to high proficiency in various athletic activities. Underwater weighings and anthropometric determinations of somatotype were performed on 145 male and 133 female adolescent participants in national meet competition in the sports of track and field, gymnastics, diving, and wrestling. The most frequent differences within either the male or female Junior Olympic samples involved the performers in throwing events (shot put, discus, and javelin), who were taller, heavier, fatter, and of unique somatotype when compared to all or most other competitors. Additional structural differences, generally of a lesser magnitude, also existed between other groups of Junior Olympians. Differences in body composition characteristics were also noted when Junior Olympians were compared with other adolescent athletes or non-athletes.

[Scoliosis (author's transl)].

The distinction between the scoliotic posture (transitory and without pathological significance) and the structural scoliosis (for the more progressive and characterized of spinal and costal bones) is fundamental. Facing every deviation it is of great importance, through an accurate clinical and radiographic examination, to establish the characteristic functions of the aetiology, of the evolution and of the therapeutic possibilities. These last consider bloodless treatment (orthopaedic corsets integrated with gymnastics, and physio-kinesi-therapy) and the surgical cure (arthrodesis, frontal and rear). The necessity of a precoce diagnosis and an exact assessment of the evolution for each spinal deviation is stressed out, with the aim of applying the most suitable therapy.

[Fitness and competitive sports in venous pathology].

Since chronic venous insufficiency is a constitutional disorder, the principles of healthy living can be of major importance in its prevention and treatment. Among these, the influence of physical activities has attracted the attention of the author, who has attempted to analyze the mechanisms of each sport in order to study its effects on the peripheral vessels. As a result of long terme comparative studies, he has been able to classify the various sports into two categories, the sports which are beneficial and those which are harmful in subjects with vulnerable veins. Into the first category come those activities which assist the physiological mechanisms assuring the return of venous blood, in particular walking, swimming, cycling and gymnastics. Into the second category come sports which involve risks of constriction, trauma, and jolting of the vertebral column (skating, rugby, tennis, etc.), as well as all competitive sports because of the risks of thromboses, often unrecognized, and of excessive efforts which strain the valvular mechanism.
