Very young adolescent women in Georgia: has abortion or contraception lowered their fertility?

Despite a state law enacted in 1972 which allowed minors to obtain contraceptive services without parental consent, births to very young women in Georgia (age 14 and less) have risen in recent years. Beginning in 1974, however, this trend has reversed. Increased access to induced abortion following the 1973 U.S. Supreme Court decision appears to have been responsible for the decline. Supporting this assertion are: 1) the temporal relationship between increased access to abortion and the decline in births, 2) the geographic evidence that the decline in births occurred first in Atlanta where abortion utilization is the highest and then followed in areas with somewhat more limited utilization, and 3) a similar observation that the decline occurred earlier and more markedly among young white teenagers whose abortion utilization is higher. Although abortion appears to have had the most visible impact on births, most people would probably agree that efforts toward providing contraception to these young women remain worth the challenge. The ratio of young teenegers accepting contraceptives to young teenegers getting pregnant is suggested as a useful intermediator of the success of family planning programs.

Study of the differentiation of secretory cells in the golden hamster oviductal epithelium by use of a monoclonal antibody.

The ontogeny of an oviductal zona pellucida glycoprotein, designated ZP-0, in the golden hamster oviductal epithelium was investigated by means of light and electron microscopic immunolabeling studies using a monoclonal antibody. Light microscopic immunohistochemistry showed that ZP-0 appeared in the oviductal epithelium from 9.5 to 10.5 days after birth. At first, ZP-0 appeared in the Golgi region of the epithelial cells, and then increased in amount as development progressed to fill the supranuclear cytoplasm. Electron microscopic observations showed that rough endoplasmic reticulum and Golgi apparatus were well developed in non-ciliated cells of the ampullar and isthmic segments at 10.5 days after birth. Secretory granules appeared near the Golgi apparatus and markedly increased in number until 15.5 days after birth. Ultrastructural immunocytochemistry showed that ZP-0 appeared in the well-developed Golgi apparatus and then was stored within the secretory granules of developing secretory cells. ZP-0 was never present in the ciliated cells. These results indicate a coincidence between the beginning of ZP-0 biosynthesis and the morphological cytodifferentiation of secretory cells in the golden hamster oviduct during postnatal development. This leads us to the conclusion that ZP-0 is a possible differentiation marker of the golden hamster oviductal secretory cell.

Population growth rates decline in five provinces.

Natural population growth rates in Shandong, Zhejiang, Shaanxi, Jiangxi, and Henan provinces last year were much lower than the 1980 national average of less than 11/1000, according to provincial authorities. The population growth rate in Shandong province, which has 73 million people, dropped to 7.03/1000 last year, 3.76/1000 less than in 1979. Some 100,000 fewer babies were born in Zhejiang province last year, compared with the figure for 1979. That birth rate was the lowest achieved in the past 30 years. In Shaanxi province, the population growth rate in 1980 was 7.19/1000. More than 458,000 couples in the province who have 1 child have decided to bear no more children, the authorities said. The population growth rate in Jiangxi and Henan provinces fell from 13.74/1000 in 1979 to 9.11/1000 in 1980 and from 12.88/1000 in 1979 to 9.53/1000 last year. The decline of the population growth rate is attributed to late marriage, late childbearing, family planning education, and encouragement of 1-child families.

The safety and effectiveness of a new hysteroscopic method for permanent birth control: results of the first Essure pbc clinical study.

BACKGROUND: Current methods of female surgical sterilisation require incisional surgery, general anaesthesia and a prolonged recovery time. We studied the safety and effectiveness of Essure pbc, a minimally invasive, transcervically placed micro-insert that occludes the Fallopian tubes, resulting in permanent female contraception. Device under study: The Essure pbc implant is a dynamically expanding micro-insert which is placed in the proximal section of the Fallopian tube using a modified minimal access technology for cannulating the tube. STUDY POPULATION: Women aged 21-43 seeking permanent birth control. METHODOLOGY: Essure pbc micro-inserts were inserted into the proximal portion of the Fallopian tubes under hysteroscopic visualisation with intravenous sedation or paracervical block. RESULTS: Bilateral device placement was achieved in 111 of 130 (85%) women who underwent device placement attempts. Women found the device placement procedure to be highly acceptable. Of women wearing the device for up to two years rate, 97% rated it to be very good to excellent. There have been no pregnancies reported in 1894 woman-months of effectiveness. Adverse events preventing women from relying on Essure pbc were < 5%. DISCUSSION: This first clinical trial showed the Essure pbc method of permanent contraception to be safe and highly acceptable to women. Experience and improvements to the delivery system should increase overall micro-insert placement rates. CONCLUSION: The Essure pbc method of permanent contraception is an exciting alternative to vasectomy or laparoscopic sterilisation that does not require general anaesthesia or incisions.

The relationship between age of mother and child health and development.

This study investigates the relationship between age of mother and children's health and development at birth and at approximately three years of age. The sample is composed of Black and Hispanic women and their firstborn children who were delivered on the wards of a large New York City hospital in 1975. There were no differences between children of teenage and older mothers in terms of prematurity or birthweight, but the children of younger mothers had higher Apgar scores than those of older mothers. Age of mother was not significantly related to hospitalizations, the need to see a physician regularly, or abnormal weight. Although the number of injurious conditions and the incidence of burns were higher among the children of adolescent mothers, the effect of age of mother was not independent of other factors. The children of teenage mothers scored better than those of older mothers on the total Denver Developmental Screening Test, as well as on the Fine Motor sector. These findings thus suggest that when relevant background characteristics are controlled, children of teenage mothers are as healthy and develop as well as children of older mothers.

Reproductive events and family history as risk factors for breast cancer in northern Alberta.

Reproductive events and family history as risk factors for breast cancer in northern Alberta were investigated with the use of data from a computerized population-based registry. Women aged 30 to 79 years attending diagnostic breast clinics at the Cross Cancer Institute from 1971 through 1975 constituted the two study groups; 1232 women had diagnosed breast cancer (malignant disease group) and 602 women were clinically free of all types of breast disease (control group). An increased relative risk of breast cancer was found in women with a family history of breast cancer, those who gave birth to their first term infant at age 30 years or older, those in whom more than 15 years elapsed between menarche and that birth, and those with a late natural menopause. There was a decreased risk, relative to nulliparity, in the postmenopausal women who first gave birth to a term infant 5 years or less after menarche. Artificial menopause (bilateral oophorectomy), parity and age at menarche had no apparent effect on the risk. The pattern of risk factors in northern Alberta differed from that reported for other geographic areas, including other provinces of Canada, thus emphasizing the need for local studies in the planning of screening programs.

Measuring the childbearing motivation of couples referred to the Shiraz Health Center for premarital examinations.

Millions of children who were born during the first decade after the Islamic revolution in Iran are now reaching the age of marriage and childbearing. Short spacing between marriage and the birth of the first child has the potential to cause an excessive and costly increase in the growth of population in Iran. Research into the motivations for the birth of first child among newly married couples can create a knowledge base that will enable health centres to help these couples make better decisions about the timing of their first pregnancy. Using a consecutive sampling technique and administering Miller's Childbearing Questionnaire, data were gathered regarding the childbearing motivations and desires of 300 couples who had been referred to the Shiraz Health Center for premarital counselling. The Childbearing Questionnaire, with some minor modifications, was found to be a valid and reliable instrument for measuring the childbearing motivations of newly married couples of Shiraz County, Fars Province, Iran. The utility of these findings for counselling in health centres is discussed. Based on the results, a longitudinal study is being designed that will allow the development of models for predicting the time of first pregnancy after marriage.

The somatic growth of a regional birth cohort of 351 preterm infants during the first two years of life.

The somatic growth of a regional birth cohort of 351 preterm infants was followed during their first two years. The measurements of weight, height, head and thorax circumferences took place at the well-baby clinics run by public health nurses at 3, 6, 12, 18 and 24 months of conceptional age. The growth of the preterm infants was compared with that of their term peers with uncomplicated course of pregnancy, labour, delivery and neonatal period. The preterm infants without intrauterine growth retardation caught up the term control infants by three months of conceptional age. The preterm small-for-gestational age infants remained smaller than the preterm appropriate-for-gestational age or the control infants in all measures throughout their first two years of life. Birth weight rather than gestational age predicted the growth outcome in preterm infants. Moderate or severe respiratory distress had no impact on the future growth of the infants in the present material. Prematurity as such does not seem to influence the growth of preterm infants.

Fetal structural anomaly screening at 11-14 weeks of gestation at Maharaj Nakorn Chiang Mai Hospital.

OBJECTIVE: To determine the detection rate by ultrasound scanning of fetal anomaly by first trimester (11-14 weeks of gestation). MATERIAL AND METHOD: A prospective descriptive study of 597 pregnant women undergoing Nuchal Translucency (NT) measurement at 11-14 weeks of gestation at Maharaj Nakorn Chiang Mai Hospital. The sonographic examinations focused on NT thickness and fetal structural survey. The final diagnoses were based on neonatal outcomes assessed by the pediatricians and abortuses evaluated by the pathologists. The main outcome measure was the detection rate of fetal anomaly using ultrasonographic examination. RESULTS: Of 597 pregnant women recruited into the present study, the mean age was 29.41 +/- 5.8 years, the incidence of fetal anomaly was about 4% (24 from 597 cases). The detection rate by first ultrasound scans was 58% (14 from 24 cases) and the most common detected structural anomaly was cystic hygroma and exencephaly. The rate of undetected fetal anomalies was 42% (10 from 24 cases). Abnormal NT was found in 16 from a total of 597 cases (2.7%), most of them, however, had normal karyotype and no gross anomaly at birth. CONCLUSION: First trimester (11-14 weeks) ultrasound scan is probably a useful method for detection of fetal structural anomalies with a relatively high detection rate, and may be a good adjunct to the conventional examination.

First Australian trial of the birth-training device Epi-No: a highly significantly increased chance of an intact perineum.

BACKGROUND: A German report suggested significantly better outcomes in terms of perineal care, second stage length and neonatal outcome for users of Epi-No. OBJECTIVE: To carry out a pilot study of the first use of the Epi-No birth training device in Australia for women having their first baby. STUDY POPULATION AND METHODS: Forty-eight primigravidae having their confinement at Birralee Birthing Unit who used the device compared to all other primigravida who delivered during the same period. RESULTS: The study shows a highly significantly improved outcome for the perineum when users are compared to primigravid non-user controls. We could not demonstrate decreased instrumental delivery rates nor a better outcome in term of Apgar scores. DISCUSSION: The Epi-No device should be offered as an option to all primigravidae to use during the late third trimester.

Body measurements of crossbred calves sired by Simmental bulls divergently selected for progeny first-calf calving ease in relation to birth weight.

Simmental bulls (n = 27) were divergently selected on linear model first-calf calving ease expected progeny difference (CEPD) relative to birth weight expected progeny difference (BEPD) so that body measures of calves from sires whose progeny tended to be born either with more or less dystocia than expected from BEPD could be obtained. At birth, calf weight, head circumference, shoulder width, hip width, heart girth, cannon bone circumference and length, and body length were recorded for 204 calves. These measures had also been obtained from the Polled Hereford X Angus dams of the calves at their births. Sire differences (P less than .05) existed for calf cannon bone circumference and length before and after adjustment for gestation length and birth weight of the calf. Sire BEPD was positively associated with cannon and head circumferences independent of phenotypic birth weight and gestation length. No relationship existed between CEPD or threshold model first-calf calving ease expected progeny difference and any calf measure, either before or after adjustment for birth weight. Multivariate factor analysis was used to describe independent components of skeletal width, frame, and thickness after removal of birth weight effects; factors were not related to genetic merit for calving ease or observed calving performance. Independent of weight, newborn calf measures were not highly related to body measures at weaning or to dam birth measures. Body shape differences at constant weight existed in crossbred calves, but they were not related to sire genetic merit for calving ease or measured dystocia. Selection for calf body shape, independent of birth weight, would not reduce dystocia.
