We examined whether voice-activated word processors provide an acceptable means for emergency physicians to create medical records. Our study addressed three areas of inquiry: whether physicians can be induced to try this new technology, whether they will continue to use it after outside technical support is withdrawn, and the factors contributing to adoption and substantial use of voice-activated computers by practicing emergency physicians. This paper presents findings from the first half of the study, reflecting physicians' reported experiences while receiving onsite training followed by technical support for three months after system installation. Based on preliminary assessments, the keys to successful use appear to include physician and group commitment, acceptance of a steep learning curve, and flexibility in adapting the computer software and/or practice habits.

Automating the production of routine reports does not necessarily require programming in a traditional computer language. If you are conversant with Microsoft Word, its macro facility will allow you to develop a customized report generator in a familiar environment with only a modest amount of programming. Furthermore, Word will take care of many important details, including the length of lines, automatic centering of text, and printer support. While the programming requires some work, the resulting report generator will save considerable time and can be mastered by a novice.
A literature review is presented regarding keyboarding for school students experiencing handwriting difficulties. Despite the overall dearth of research, some general conclusions appear warranted. Students need to be able to keyboard at least as fast as they can handwrite and should learn the touch-keyboarding method if possible. Appropriate instruction appears critical for the development of keyboarding competency. The upper elementary age is an appropriate time to start teaching keyboarding, with students possibly requiring 25-30 total hours of instruction. Students experiencing handwriting difficulties might need customized goals and strategies. Although the existing literature regarding the role of performance components in keyboarding provides some direction to clinicians, further investigation is required.

We are developing GenIE, a prototype intelligent system to create first drafts of genetic counseling patient letters. GenIE will apply natural language generation techniques to construct the first draft of a letter for subsequent review and editing, if needed, by the genetic counselor. For purposes of knowledge acquisition, we have been analyzing a corpus of patient letters. Based on the corpus analysis we are developing a knowledge base and text generation strategies.
BACKGROUND: The hypnotic zolpidem and the hormone melatonin were evaluated and directly compared for their effects on performance when subjects sleeping under their influence were prematurely awakened from daytime sleep. METHOD: Non-sleep deprived volunteers (eight men and five women) received single oral doses of 5 or 10 mg melatonin (Mel-5; Mel-10), 10 or 20 mg zolpidem (Zol-10; Zol-20), or placebo immediately before retiring at 13:00. Performance testing and subjective evaluations occurred prior to dosing and following forced awakening at 15:00, 2 h after dosing. RESULTS: Compared with placebo, on being awakened under Zol-20, significant performance decrements were prevalent on 9 of 10 cognitive tasks, including grammatical reasoning, mathematical processing, and word memory. Recovery required up to 6 h post-awakening for the more complex tasks. Loss of coordination and nausea were also present on awakening under Zol-20. On being awakened under Zol-10, significant but relatively less severe and shorter duration performance decrements occurred for 4 of the 10 tasks and recovered by 4 h post-awakening. Under Mel-5 or Mel-10, performance decrements seldom occurred and were considerably less severe, briefer, and less systematic than for zolpidem. CONCLUSION: Findings indicated that when operational personnel sleeping with the aid of either 10 or 20 mg zolpidem are prematurely awakened, it would be prudent to evaluate their general well-being and possible need for assistance prior to their being permitted to depart crew-rest or to perform tasks and duties. In contrast, we found little to no evidence of deteriorated well-being or need for assistance when awakened while sleeping under the influence of melatonin.
Showcase is an instructional authoring and case presentation system for veterinary medicine. It is intended to permit instructors and students to deal with clinical case information in a dynamic and interactive way. The programs are distributed as "adware;" and, it is hoped, the case files will be freely shared and distributed.
