Prostatectomy in factor XI (plasma thromboplastin antecedent) deficiency.

A total of 10 patients with factor XI deficiency underwent prostatectomy: 3 who underwent suprapubic prostatectomy and 3 who underwent transurethral resection received fresh frozen plasma preoperatively, while 4 received fresh frozen plasma only in response to bleeding. We conclude that all patients who present with partial or severe factor XI deficiency should receive fresh frozen plasma preoperatively and for at least 10 to 14 days postoperatively. We believe that all Ashkenazi Jews who are about to undergo a urological operation should be screened for factor XI deficiency even when a history of a bleeding tendency is not present.

Isolation and characterization of bovine factor XI (plasma thromboplastin antecedent).

Factor XI (plasma thromboplastic antecedent) has been purified approximately 28 000-fold from bovine plasma with an overall yield of about 30%. The isolation procedure involves barium sulfate adsorption of contaminants, ammonium sulfate precipitation, and chromatography on heparin-agarose, CM-Sephadex, and DEAE-Sephadex. The final product was homogeneous when examined by polyacrylamide gel electrophoresis and immunoelectrophoresis. A minimal mol wt of 124 000 was determined by sedimentation equilibrium. Factor XI is composed of two similar or identical polypeptide chain (mol wt of approximately 55 000), and these two chains are held together by a disulfide bond(s). Factor XI is a glycoprotein which contains approximately 11% carbohydrate including 5.4% heose, 4.7% N-acetylhexosamine, and 1.0% N-acetylneuraminic acid. Other properties of this coagulation factor including its amino acid composition and inhibition by antibodies prepared in rabbits are also reported.

Hereditary plasma thromboplastin antecedent (PTA, FXI) deficiency in a Saudi family.

A rare case of factor XI (PTA) deficiency was discovered in a Saudi family in the Riyadh area. Nine members of the family were studied. Two were found to have a severe PTA deficiency; levels of factor XI clotting activity were 0.01 i.u./ml and 0.02 i.u./ml respectively. Both plasmas were markedly deficient in factor XI antigen and appeared to be negative for cross-reactive material (CRM-). The parents were first cousins and both were found to have a minor PTA deficiency. Factor XI levels were: mother 0.048 i.u./ml and father 0.33 i.u./ml. Another sibling was found to have a FXI level of 0.47 i.u./ml. Menorrhagia and bleeding for 1 day after tooth extraction were the main bleeding manifestations found in one member with severe PTA deficiency. Clinically this member presented with iron deficiency anaemia. Other family members had no significant history of bleeding tendency. This is the first report of a Saudi Arabian family with PTA deficiency.
