[Postoperative complications of Bartholin's duct abscess. About two cases].

Bartholinitis is the most common infectious vulvar disease and develops in approximately 2% of all women. The choice concerning treatment may be uneasy between medical or surgical modalities, and we have to be cautious because of the risk of severe complications associated with the procedure. We report two cases: one case of sepsis and the other one of bacterial cellulitis after Bartholin's duct abscess marsupialization. In the light of epidemiological and bacteriological date referring to Bartholinitis, we recommend a safe and effective management (particularly the use of broad-spectrum antibiotics) that we will describe.

[Early postoperative complications of resection anastomosis in colonic and colorectal surgery. An analysis of 624 cases].

Six hundred twenty seven patients have been studied retrospectively to evaluate early complications after sewn or staples colonic anastomosis. Seventy four per cent of the patients underwent surgery because of malignant lesions and seventy seven per cent had an elective operation. Fourty seven per cent of the patients had a sewn anastomosis, 53% a stapled anastomosis. In elective surgery, most of the right hemicolectomies and partial proctocolectomies have been performed using stapler devices (p < 0.05). In emergency surgery, most of the anastomosis after partial and total colectomy have been hand sewn (p < 0.05). Operative mortality was 5.6%. It was significantly higher in older patients (> 80 years old) (p < 0.01) and after emergency operations (p < 0.0001). The rate of anastomotic leak was 3.7% and it was significantly higher after right hemicolectomy and partial proctocolectomies when anastomosis has been hand sewn compared to stapled anastomosis (p < 0.05). With regard to postoperative mortality, intra-abdominal abscess, intestinal obstruction, evisceration, pulmonary embolism and anastomotic stenosis 1.4% (global: after a 6 months follow-up) no significant differences have been observed between sewn and stapled anastomosis.

Postoperative complications and uncorrected acuities after implantation of plate haptic silicone and three-piece silicone intraocular lenses.

Complications after implantation of 503 plate haptic silicone intraocular lenses were compared with those after implantation of 253 silicone three-piece polypropylene loop lenses. The uncorrected visual acuities were also compared. Capsulorhexis and endocapsular phacoemulsification were used in all cases. The incidence of cystoid macular edema and posterior capsule opacification requiring a YAG capsulotomy were significantly higher in the cases with polypropylene three-piece loop lenses. Pigment dispersion and decentration also occurred more frequently in this group. This is probably because the 10.5 mm plate haptic lens is more rigid than the polypropylene loop lens. It also fixates more posteriorly within the capsular bag than the anteriorly located polypropylene lens. Patients implanted with a plate haptic silicone intraocular lens through a 3.5 mm scleral incision had significantly better uncorrected visual acuity at five to six weeks postoperatively than those with a three-piece silicone lens that required a 4.5 mm incision for insertion.

[Abdominoplasty: towards a two procedures surgery? Retrospective study of postoperative complications about 100 cases].

Abdominoplasty has become in France a frequent, appearing to be technically easy but nevertheless source of potentially serious complications clearly deteriorating the aesthetic results and the satisfaction rate of the patients. By studying the results of a hundred abdominoplasties performed between 2001 and 2004, we could count the incidence of the various complications by seeking their possible factors of risk (tobacco or ponderal overload). In spite of the use of a lymphatic preserving technique, the percentage of séroma reaches 12%. The high complications rate or the number of insufficient results explains a rate of surgical reoperation of 31%. Abdominoplasty passed from the statute of repairing surgery to that of aesthetic surgery. The level standard in terms of result became equivalent to the other purely aesthetic interventions. Obtaining an optimal result thus requires often two operational stages, data element that it henceforth seems essential to us to provide to the patients.

[Metabolic and water-electrolyte changes as postoperative complications in aneurysms of the anterior communicating artery].

A study was performed on hydroelectrolytic changes in neurosurgical patients operated for clipping anterior communicating artery aneurysm. The patients were observed during a seven day period in ICU, between the preoperative day and the sixth post-operative day. No statistically significant changes were observed, except for hyponatriemia on the day of surgery. The etiology of this phenomenon is not clear: it could be a change of ADH or "cerebral salt wasting syndrome". A wider number of patients and repeated haematological tests are necessary.

Off-pump surgery decreases postoperative complications and resource utilization in the elderly.

BACKGROUND: Bypass surgery in the elderly (age >70 years) has increased mortality and morbidity, which may be a consequence of cardiopulmonary bypass. We compare the outcomes of a cohort of elderly off-pump coronary artery bypass (OPCAB) patients with elderly conventional coronary artery bypass grafting (CABG) patients. METHODS: Chart and provincial cardiac care registry data were reviewed for 30 consecutive elderly OPCAB patients (age 74.7 +/- 4.2 years) and 60 consecutive CABG patients (age 74.9 +/- 4.1 years, p = 0.82) with similar risk factor profiles: Parsonnet score 17.2 +/- 8.1 (OPCAB) versus 15.6 +/- 6.5 (CABG), p = 0.31; and Ontario provincial acuity index 4.5 +/- 1.9 (OPCAB) versus 4.3 +/- 2.0 (CABG), p = 0.65. RESULTS: Mean hospital stay was 6.3 +/- 1.8 days for OPCAB patients and 7.7 +/- 3.9 days for CABG patients (p < 0.05). Average intensive care unit stay was 24.0 +/- 10.9 h for OPCAB patients versus 36.6 +/- 33.5 h for CABG patients (p < 0.05). Atrial fibrillation occurred in 10.0% of OPCAB patients and 28.3% of CABG patients (p < 0.05). Low output syndrome was observed in 10% of OPCAB patients and 31.7% of CABG patients (p < 0.05). Cost was reduced by $1,082 (Canadian) per patient in the OPCAB group. Postoperative OPCAB graft analysis showed 100% patency. CONCLUSIONS: OPCAB is safe in the geriatric population and significantly reduces postoperative morbidity and cost.

[Comparison of mediansternotomy approach vs left thoracotomy approach for distal aortic arch aneurysm, and their postoperative complications].

We performed surgery for distal arch aneurysm in 22 patients; through a mediansternotomy in 11 patients (group M), and through a left thoracotomy approach in 11 patients (group L). If the distal position of an aneurysm was located at the pulmonary hilus without severe calcification of the ascending aorta, the mediansternotomy approach was chosen, whereas if we suspected that distal anastomosis would be difficult due to severe calcification of the descending aorta or aortic dissection type B, the left thoracotomy approach was selected. The mean cardiopulmonary bypass time (min.) was 229 +/- 56 in group M and 225 +/- 47 in group L, the mean circulation arrest time (min.) was 39 +/- 19 in group M and 31 +/- 5 in group L, and the mean lowest temperature (degree C) was 22 +/- 1 in group M and 20 +/- 2 in group L. No severe cerebral or cardiac complications developed in any patients. The hospital mortality was 9% (1/11) in group M and 9% (1/11) in group L. Following discharge, 8 (73%) patients from group M and 9 (92%) from group L resumed a normal lifestyle. There were no significant differences between the 2 groups and the operative results of both approaches proved satisfactory. Therefore our indications for each surgical approach to treat distal arch aneurysm seemed appropriate.

[Thoracic outlet syndrome: an unusual postoperative complication].

UNLABELLED: Introduction. Neurogenic Thoracic Outlet Syndrome (NTOS) is a chronic lower trunk brachial plexus entrapment caused by a cervical rib or a fibrous band. True NTOS is rare and progresses usually slowly. Case report. A 12-year-old girl complained of numbness and weakness of the right upper limb immediately after an orthopedic surgical procedure for scoliosis. Neurological and neurophysiological features were both consistent with a neurogenic thoracic outlet syndrome (NTOS). CONCLUSION: This observation illustrates the risk of NTOS after certain surgical procedures, especially when a prolonged prone position with abducted shoulders is required.

Aortoesophageal fistula: early postoperative complication at the distal anastomosis of an aortic graft.

Though secondary aortoesophageal fistula and subsequent hemorrhage is rare, the clinician must have a high index of suspicion to establish the correct diagnosis and institute appropriate treatment. We have proposed an algorithm to help prevent a massive hemorrhage such as the one that proved fatal in our patient. In the case of massive upper gastrointestinal hemorrhage, a Sengstaken-Blakemore tube should be done expeditiously. In the case of sentinel hemorrhage, aortography is the initial diagnostic procedure of choice, followed by flexible esophagogastroscopy if necessary.

The value of intravenous heme-albumin and plasmapheresis in reducing postoperative complications of orthotopic liver transplantation for erythropoietic protoporphyria.

Erythropoietic protoporphyria (EPP) is marked by a deficiency of ferrochelatase, which occurs in all cells and tissues, preventing effective conversion of proto porphyrin IX to heme and thereby blocking effective feedback inhibition of heme synthesis. The major source of the excess protoporphyrin is the bone marrow. Protoporphyrin IX may accumulate, with resultant toxicity chiefly of the marrow, skin, nervous system, and liver. Orthotopic liver transplantation (OLT) is, at present, the only adequate intervention for severe liver compromise secondary to protoporphyrin deposition, but it has been complicated by severe photosensitivity and polyneuropathy. Intravenous heme and plasmapheresis have been proposed but not previously reported as means to reduce the protoporphyrin burden before liver transplantation. We report a man with EPP who underwent preoperative heme-albumin administration and plasmaphereses that led to marked reductions in plasma and erythrocyte protoporphyrin levels. His OLT was uneventful, and he developed neither polyneuropathy nor exacerbation of photosensitivity.

[Mathieu's urethroplasty in surgery for hypospadia postoperative complications].

UNLABELLED: The aim of the study was to report the results of meatal based flap urethroplasty called Mathieu urethroplasty, in failed preliminary surgery for hypospadias. PATIENTS AND METHODS: Fourteen boys aged three to 14 years underwent surgery using a meatal-based flap urethroplasty for the correction of prior hypospadia repair complications. The secondary procedure was performed for glandular or coronal urethrocutaneous fistula in ten cases, completely failed urethroplasty in three cases, and for severe meatal retraction in one case. The surgical technique was the same as that used for the primary Mathieu procedures. RESULTS: Only one meatal retraction occurred; the other 13 cases had satisfactory procedure of hypospadias repair failures. Despite the scar skin used for secondary meatal based flap, good results can be achieved specially in two favorite indications: juxta coronal urethrocutaneous fistula and meatal retraction.

[Circadian rhythm in response to surgery and postoperative complications in childhood (author's transl)].

The influence of operation and post-op. course on the circadian periodicity was examined in 27 children. The parameters chosen were heart-rate, respiratory rate, blood-pressure and temperature. Measurements were taken over 6.1 days on average, of which 5.1 days were post-op. 5-minute values were used in the evaluation. It could be proved that the circadian periodicity is disturbed by the operation. Resumption depended on the gravity of the operation, and on the further clinical course. Studies of the circadian periodicity may provide a contribution to research on operation-trauma.

The stapled gastrointestinal tract anastomosis: incidence of postoperative complications compared with the sutured anastomosis.

Performance of gastrointestinal anastomosis by means of surgical stapling devices has achieved popularity in the last decade even though no detailed study has been reported comparing complications following the stapled anastomosis with those following hand sutured procedures performed by the same surgeons. We have reviewed 812 operative procedures on the gastrointestinal tract performed in one hospital over a four year period. Stapled anastomoses were performed in 472 with 13 (2.8%) complications related to the anastomosis; in 296 sutured anastomoses there were nine (3.0%) related complications. Comparison did not disclose any significant difference in the number of complications in these two groups. In 44 instances wherein the anastomosis contained both staples and sutures, there were no related complications. Further analysis of the patients in each group disclosed that stapling procedures were utilized in a much higher percentage of those operations which were performed under emergency conditions or in the presence of intra-abdominal sepsis, intestinal obstruction, and carcinomatosis. If the technical details of surgical stapling are mastered, this technique appears to be as safe as suturing in the performance of anastomoses in the gastrointestinal tract.

Weight loss and postoperative complications in morbidly obese patients with binge eating disorder treated by laparoscopic adjustable gastric banding.

BACKGROUND: The authors investigated the outcome of morbidly obese patients with binge eating disorder (BED) treated surgically with laparoscopic adjustable gastric banding. METHODS: The 5-year outcomes of 130 patients with BED and 249 patients without BED are described. The diagnosis of BED was made preoperatively and all patients with BED were supported with psychological therapy. RESULTS: Patients with and without BED had similar BMI levels before surgery. More patients with than without BED had depressive symptoms and associated minor disturbances of eating behavior (night eating and grazing). Percent excess weight loss (%EWL) in the first 5 years after surgery was similar in patients with and without BED. The percentage of BED patients showing %EWL >50% at the 5-year evaluation was 23.1, and 25.7% in non-BED patients. The percentage of patients showing weight regain in the last 4 years of follow-up was similar in binge eaters (20.8%) and in non-binge eaters (22.5%). The 5-year frequency of gastric pouch and esophageal dilatation was significantly higher in binge eaters than in non-binge eaters (25.4 vs 17.7 %, P<0.05 and 10.0 vs 4.8%, P<0.05, respectively). Binge eaters underwent a higher number of postoperative band adjustments than non-binge eaters (3.0+/-2.1 vs 2.6+/-1.9, P<0.05) and the maximum band fill after surgery was higher in the BED patients than in non-BED patients (3.2+/-1.2 vs 2.8+/-1.3 ml, P<0.01). CONCLUSION: Morbidly obese patients with BED supported by adequate psychological treatment can have good outcomes after gastric banding.

Aorto-enteric fistulas as postoperative complication.

This report is based on 15 own observations and 164 patients collected from the literature. The most frequent location both of primary and secondary aortic-enteric fistulas (AEF) is the end part of the duodenum (60-70%). In the group of secondary AEF the paraprosthetic type represents the most frequent morphological finding. The best diagnostic approach should be seen in a gastroduodenoscopy. The choice of surgical repair is still controversal. Most of the surgeons insist on a radical repair (Type A: restoration of the intestinal tract, complete removal of the vascular prosthesis, blind closure of the infrarenal aortic stump, axillo-bifemoral bypass). Type A repair is loaded with a high operative mortality (52%) and a number of secondary complications (e.g. recurrent AEF; bleeding from the aortic stump). As an alternative surgical approach the author present an in-situ-repair (Type B: closing of the intestinal defect, partial exchange of inserted vascular prosthesis, an encircling omentum sleeve covering the prosthesis and anastomoses). The Type B repair offers a time saving procedure with a reduced operative mortality (26%) and a better outcome for the patients. This method is only contraindicated in the presence of a spread infection of the retroperitoneal space including the vascular prosthesis.

[Acute cholecystitis as a postoperative complication].

The literature data on acute cholecystitis occuring as a complication after operations on the abdominal organs are analysed. The author presents 3 own observations of cholecystitis in early postoperative period among 28,556 laparotomies. Taking into consideration the rarity of this complication, the author believes that the arguments adduced for simultaneous preventive cholecystectomy in operations for other abdominal organs diseases are dubious, especially in emergent abdominal surgery.
