[Tryptase and fatal anaphylaxic reaction].

OBJECTIVE: To investigate the relationship between tryptase in serum and anaphylaxis. METHODS: The concentrations of tryptase in the sera of heart blood in three persons died from anaphylaxis shock were detected by ELISA. The first sample was obtained from a man, aged 38, died of injecting Amikacin. The second sample was obtained from a man, aged 42, died of injecting Cephradine. The third sample was from a woman, aged 39, died of injecting Lincomycin. All samples were stored in -20 degrees C. RESULTS: The concentrations of tryptase in sera were 52 ng/ml, 121 ng/ml and 0.73 ng/ml. It was unknown why the concentration of tryptase in the third sample was normal. CONCLUSION: In fetal anaphylaxia reaction tryptase measurement is a useful indicator, but the diagnosis is not to be based on the test alone.

[Methicillin resistant Staphylococcus aureus (MRSA) infection in a dog in the Netherlands].

In the Netherlands, methicillin resistant Staphylococcus aureus (MRSA) are regularly isolated from humans. We present the first isolation of MRSA from animal origin in the Netherlands. A coagulase positive staphylococcus was cultured from an infected wound in a Dutch dog that recently underwent surgery abroad. The staphylococcus was resistant to methicillin, ampicillin, amoxycillin + clavulanic acid, cephalexin, erythromycin, lincomycin, tetracycline, gentamicin and enrofloxacin. It was identified as S. aureus by fermentation of mannitol and Martineau-PCR. The presence of mecA was confirmed by PCR.

[Treatment of patients with acute pneumonia].

Efficiency was studied of treatment of acute pneumonia (AP) in 148 patients with a focal croupous form. In the grave course of AP, antibacterial therapy with benzilpenicillin combined with sulphanilamide agents biseptol and nitrofuran was tried, the correcting therapy having been instituted on obtaining findings from assays of the sputum for sensitivity of the microflora to antibiotics. In those cases with Friendländer's bacilli recoverable, methicillin, oxacillin, macrolids, cephalosporins and some other agents were antibiotics of choice. Administered in the atypical course that was related more frequently to mycoplasms or chlamidia, was erythromycin phosphate i.v., lincomycin. In Staphylococcus-associated pneumonia, there have been used antibiotics resistant to penicillinases and endowed with an inhibitory activity toward staphylococci--oxacillin, methicillin, cephalosporins, chlorophenicols or chlorophphyllipt i.v. combined with an antistaphylacoccal plasma, antistaphylococcal gammaglobulin or hyperimmune plasma. The following drugs were prescribed to ensure bronchial patency and liquefaction of the sputum--euphyllin, mucalthin, bromhexin, lasolvan that are known to stimulate production of sulfoctant. The therapeutic complex comprized fibs, aloe, apilac, prodigiosan, solcoseryl, methyluracil known to stimulate bodily reactivity and resolution of inflammatory infiltration, nicotinic acid, heparin that have been shown to improve micorcirculation, tocopherol, unithiol known to regulate lipid oxidation. The data secured suggest to us a sufficient efficacy of the above-outlined therapy of AP.

[Protoplast fusion in Streptomyces fradiae strains producing neomycin and tylosin].

Interspecies fusion of protoplasts of the Streptomyces fradiae strains producing neomycin (an aminoglycoside antibiotic) and tylosin (a macrolide antibiotic) was performed with a view to isolate strains producing novel antibiotics. Fusion of the protoplasts of the neomycin- and tylosin-producing strains labelled by the resistance to monomycin and lincomycin, respectively, caused no formation of stable strains producing antibiotics differing in chromatographic mobility from the antibiotics produced by the initial strains. In fusion of the protoplasts of the unlabelled strains, heat-inactivated protoplasts of the active line of one strain (donor) and native protoplasts of the inactive line of the other strain (recipient) were used. When the neomycin-producing culture was used as a recipient the fusion led to formation of strain 195-34 producing antibiotics of the benzo(a)anthraquinone group. One of these antibiotics, i.e. antibiotic 34-I, proved to be a novel biologically active substance. After regeneration of the protoplasts of the initial strains, no stable strains producing antibiotics differing from neomycin and tylosin were isolated.

[Modifying action of oxytocin on the biological properties of the causative agents of anaerobic non-clostridial infection].

In a number of in vitro experiments the effect of oxytocin on the antilysozyme and anticomplemental activity of Propiobacterium propionicum, Bacteroides fragilis, Prevotella melaninogenica and Peptostreprtococcus anaerobius, isolated from patients with acute pyoinflammatory pleuropulmonary diseases, was studied. Antibiotic resistance dynamics of the infective agents under study to lincomycin, clindamycin, thienam, vancomycin was also detected. The inhibiting activity of oxytocin on the persistence properties of B. fragilis, P. melanogenica and P. anaerobius was noted. Under the influence of the preparations used changes in the sensitivity of the strains to a number of antibiotics of the lincosamide, carbapenem and glycopeptide groups were found to occur. The data thus obtained were indicative of the possible mechanisms of action of oxytocin in the treatment of acute pyoinflammatory pleuropulmonary diseases of anaerobic nonclostridial etiology.

[Drug sensitivity of Ureaplasma urealyticum, persisting in patients with chronic inflammatory diseases of the urogenital tract].

A total of 65 U. urealyticum cultures isolated from patients with chronic inflammatory diseases of the urogenital tract after their prolonged persistence of in the human body, were studied for sensitivity to medicinal preparations of different groups: tetracyclines (tetracycline and doxycycline), macrolids (erythomycin, clarithromycin, midecamycin, josamycin), quinolon (pefloxacin), amino glycoside (gentamicin), lincoamides (lincomycin, clindamycin). The majority of isolated U. urealyticun were highly sensitive to josamycin, clacide, doxycycline (89.2, 84.6, 76/9% respectively), and somewhat lesser number of these organisms were highly sensitive to midecamycin and pefloxacin (51.3 and 44.4% respectively). Among U. urealyticum strains circulating in the Moscow region some strains which persisted in patients with chronic inflammatory diseases of the urogenital tract for a long time were found to be resistant to erythromycin (23.1%), tetracycline (19.5%), and in very rare cases (1.6%) they were found to multiple drug resistance to all preparations under study. In view of the varying sensitivity of the clinical isolates of U. urealyticum to medicines and the presence of resistant forms in their population, the sensitivity of the isolated U. urealyticum should be determined in vitro prior to drug therapy.

[Evaluation of the effect of medicines on biological properties of Clostridium difficile].

The presence of the persistence factors (anti-lysozyme and anti-complement activity) in the vegetative forms of C. difficile was experimentally proved. The effect of different medicines (vitamins B1, B6 and C, prebiotic inulin, probiotics Bifidumbacterin and Enterol) on the persistence factors of C. difficile and microbial resistance to vancomycin, thienam, lincomycin, clindamycin was evaluated. The anti-lysozyme and anti-complement activity of C. difficile was found to decrease under the influence of vitamins B1, B6, C, inulin, exometabolites of bifidobacteria. Under the impact of the preparations used in this study changes in the sensitivity of C. difficile to antibiotics of the lincoamide, carbapenem, glycopeptide groups were found to occur. The data obtained reveal one of the possible mechanisms of the corrective action of the medicines under study on the intestinal microbiocenosis in patients with antibiotic-associated colitis.

[Which antibiotics are helpful in acne? A review of the literature and personal test results with minocyclin (klinomycin)].

All tetracycline preparations seem to be systemically effective in acne. Quality and frequency of their side-effects, however, are different and should be carefully considered in each individual case. In an own study ninety-one patients treated with minocycline showed good response in 69% and moderate or no response in 31+. The main side-effects of the drug were nausea and giddiness, being of short duration in most cases. In patients with no response to a certain preparation change of the preparation may be helpful. Occasionally, acne does not respond to any tetracycline derivative. In these cases the use of non-tetracycline antibiotics, particularly of trimethoprim-sulphamethazol and lincomycines, is clearly effective. As a rule, we start treatment with tetracycline (500-750 mg/d) and reduce the dose to a minimum (ca. 100 mg/d), in order to withdraw the drug. In non-responsive cases another derivative and/or finally non-tetracycline antibiotics are administered for therapy.

In vitro susceptibility of avian Escherichia coli and Pasteurella multocida to danofloxacin and five other antimicrobials.

The in vitro susceptibility of Escherichia coli and Pasteurella multocida isolated from poultry was determined to danofloxacin, a novel fluoroquinolone, and five other commonly used antimicrobials. A total of 1737 E. coli field isolates and 107 P. multocida isolates were tested by veterinary diagnostic laboratories in Europe, Japan, South Africa, and North America during the period 1989-91. The antimicrobial susceptibility of these isolates was determined using the Sensititre broth microdilution technique. The minimum inhibitory concentrations (MIC) of danofloxacin, furaltadone, lincomycin, oxytetracycline, spectinomycin, and trimethoprim:sulfamethoxazole that prevented growth of 90% of the bacteria were 0.25 > 64, > 64, > 64, > 128, and > 16 micrograms/ml, respectively, against E. coli isolates and 0.25, 64, 64, 16, 128, and 8 micrograms/ml, respectively, against P. multocida isolates. Danofloxacin demonstrated considerable in vitro potency against these important poultry pathogens, many of which showed extensive resistance to the other antimicrobials tested.

In vitro susceptibility and resistance of animal staphylococci to macrolide antibiotics and related compounds.

Strains of Staphylococcus aureus isolated from poultry, pigs and cases of bovine mastitis and strains of S. hyicus isolated from pigs were examined for their susceptibility of the macrolides and other related antibiotics. The susceptibility of these strains to erythromycin, oleandomycin, tylosin, spiramycin, lincomycin, clindamycin, virginiamycin, and its components virginiamycin S and M, was tested by the quantitative plate dilution method. With S. aureus, 3 p. 100 of the bovine, 12 p. 100 of the avian and 53 p. 100 of porcine isolated were found to be resistant. With S. hyicus strains 76 p. 100 were found to be resistant. Most resistant strains showed the generalized constitutive macrolide-resistance type. Twelve p. 100 of the porcine S. aureus strains were resistant to the virginiamycin component M.

[The role of antibiotics in the management of verotoxin-associated hemolytic anemia syndromes].

Significant improvements of dialysis techniques and the supportive treatment of the hemolytic uremic syndrome (HUS) during the last three decades do not veil the fact that causal therapy is not yet feasible. This holds true for the classic HUS associated with infections by verotoxin-(VT-)producing E.coli (VTEC) as for the majority of the much less frequent "atypical" forms. The influence of antimicrobial agents on production of verotoxins produced by clinical VTEC isolates was examined. Co-trimoxazole significantly increased the total yield of toxin. In contrast, ciprofloxacin, lincomycin and gentamicin caused significant reduction in toxin yields. The pathogenetic and therapeutic relevance of this finding is not yet known. Analysis of the clinical literature for the postulated relation between antibiotic treatment and development of HUS or its impact on the severity of the disease reveals a controversial picture. Based on the dynamics of the disease, the in vitro findings, and reports on unfavourable outcomes after treatment, it is concluded that antibiotics are not indicated in most cases of the enteropathic HUS or other VTEC infections. Antibiotic prophylaxis has neither been proven to be efficacious nor safe for the prevention of secondary cases during VTEC outbreaks. Apart from the implementations of hygienic measures, prospective studies on the role of antibiotics seem clearly warranted. Possible future strategies may include the design of immunological therapeutic concepts.

[Antibiotic sensitivity of forty-four strains of group EF4 bacteria: study of minimum inhibitory concentrations using the agar dilution method].

EF4 bacteria are found in animal saliva and may contaminate bite wounds. Minimum inhibitory concentrations of 36 antimicrobials against 44 EF4 strains were determined using dilution in Mueller-Hinton agar. EF4 bacteria were susceptible to aminopenicillins, carboxypenicillins, ureidopenicillins, third-generation cephalosporins, fluoroquinolones, rifampicin, and trimethoprime-sulfamethoxazole. Susceptibility was intermediate for penicillin G, low for macrolides and variable for aminoglycosides. EF4 bacteria were resistant to lincomycin and trimethoprime. Routine prophylactic treatment of bite-induced Pasteurella infections using an aminopenicillin or a cycline also protects against EF4 infection of the wound.

The role of opportunistic species of Corynebacterium pseudodiphtheriticum in the pathogenesis of CAP (Community Acquired Pneumonia).

The analysis of eight cases of CAP (Community Acquired Pneumonia) was performed. The clinical samples of sputum were obtained from patients at which C. pseudodiphtheriticum strains were isolated in the quantity indicating the etiologic agent of infection. In two patients, K. pneumoniae and S. aureus were isolated simultaneously. They were considered as coexisting in the infection. C. pseudodiphtheriticum strains were highly susceptible to antibiotics. They were resistant to Erythromycin (87.5%), Clindamycin (87.5%), Lincomycin (75.5%), Trimeth./Sulfam.(37.5%), Chloramfenicol (37.5%). In the examined group of patients (five persons), the infection with C. pneumoniae was detected as recently passed or in progress with chronic character as the high level of specific antibodies (IgG or IgG and IgA) was present. That fact could predispose to infection with the opportunistic species of C. pseudodiphtheriticum. Of all the examined patients, three were infected with C. pseudodiphtheriticum as the only species responsible for infection (CAP).

[Staphylococcal pneumonia].

In January to December 2000, a total of 15 (8.8%) patients with staphylococcal community-acquired pneumonia (CAP), 13 (27.7%) patients with staphyloccocal nosocomial pneumonia (NP) that occurred at general units (GU), and 9 (50%) patients with GU that occurred under artificial ventilation (AV) at intensive care units (ICU) were followed up. Meticillin-resistant S. aureus strains were isolated in 6.7, 38.5, and 55.6% of cases, respectively. As compared with pneumococcal CAP, staphyloccocal CAP were more frequently characterized by the severe course of the disease (46.7% versus 15.4%), bilateral lesion (33.3% versus 5.1%), and the presence of complications (66.7% versus 30.8%). Staphyloccocal NP that had occurred at GU, as compared to that at ICU also showed the severe course of the disease (46.2% versus 2.9%), bilateral lesion (30.7% versus 0%), and developed complications (75% versus 25%). Staphylococcal NP developed under AV at ICU had no specific features as compared with NP of another etiology. Oxacillin and first-second-generation cephalosporins remain to be the drugs of choice when meticillin-sensitive S. aureus strains are isolated; lincomycin and erythromycin being alternative agents against these strains. Glycopeptides are the drugs of choice when meticillin-resistant S. aureus strains are isolated, its alernatives are linesolide or rifampicin. High mortality rates due to staphylococcal pneumonia are preserved. These are 7.1% in CAP, 7.7 and 66.7 in staphylococcal NP occurring at GU and under AV at ICU, respectively.

[Report of a case of Lyell syndrome].

Toxic epidermal necrolysis (TEN) is a rare drug-induced disease characterized by extensive epidermal destruction. We reported here a case of Lyell syndrome which happened few hours later after treatment associating lincomycine chlorhydrate with nonsteroidol anti-inflammatory drugs. The 28-year-old female patient developed many visceral complications with biochemical and haematological disorders. This syndrome is a dermatological emergency whose vital prognosis is displayed.

Isolation of Mycoplasma bovoculi from genitally diseased bovines and its experimental pathogenicity in pregnant guinea pigs.

Thirteen strains of M. bovoculi, 6 from frozen bull-semen (3.5% of 168), 3 from neat bull-semen (3.0% of 100), one each from heart blood and stomach contents of aborted foetus of 85 (1.18%) bovine-abortions, one each from stomach contents and pooled internal organs of 9 (11.1%) stillborn calves, were isolated. All the isolates were resistant to ampicillin and sensitive to spiramycin, vibramycin, demeclocyclin, oxytetracycline, lincomycin and tylosin. However, variation in resistance to tetracycline, erythromycin, neomycin, kanamycin and streptomycin was observed. The gross lesions like congestion of lungs, liver, kidney and spleen were noted only in stillborn calf. However, significant microscopic lesions were encountered in internal tissues of both the aborted bovine fetuses and stillborn calf. Thickened alveolar wall, congestion of blood vessels, mesenchymal cell proliferation along with infiltration of lymphocytes and macrophages were observed in lungs. The liver showed mild infiltration of lymphocytes, macrophages in hepatic triad and necrosis of hepatic cells. The kidney tissues had focal lymphocytic infiltration in the interstitium. One strain of M. bovoculi (isolate # SBC-7/84,IO) isolated from a stillborn calf was found abortigenic upon experimental inoculation in pregnant guinea pigs.

[Drug resistant coagulase-positive staphylococci strains in food].

From the 1572 food samples, examined in Microbiology Department of Frozen Food Industry Research Laboratory in Lódź, 79 (5.0%) coagulase-positive staphylococci strains were isolated. All the strains were sensitive to vancomycin, gentamycin and chloramphenicol. Only individual staphylococci strains were resistant to erythromycin (1.3%), lincomycin (2.5%) and ciprofloxacin (2.5%). 20.3% strains, isolated mainly from raw meat, were resistant to doxycycline and 6.3% to oxacillin. 38.0% of coagulase-positive staphylococci strains had positive results of cefinase test. One strain isolated from minced meat was resistant to methicillin and at the same time it was producing beta-lactamases.

[Interaction of neomycin with other antibiotics on selected bacterial strains].

Antimicrobial combinations are used most frequently to provide broad-spectrum empirical coverage in the treatment of bacterial infections. However, combination of two antibiotics may not influence their activity, may lead to synergy or antagonism in the activity. Neomycin may be combined with one of the following antibiotics: ampicillin, procaine penicillin, gramicidin, bacitracin, polymyxin B, lincomycin, oxytetracycline, and erythromycin in some human and veterinary multiantibiotic drugs distributed in Poland. The checkerboard method has been one of the traditional assays for the measurement of antibiotic interactions. The aim of this study was to analyse the activity interaction of neomycin with second antibiotic in multiantibiotic drugs distributed in Poland on standards and clinical bacterial strains. Checkerboard results for all strains demonstrated synergism for 2.5% of combinations, only for standards strains. In one case Salmonella Enteritidis, in combination of neomycin with bacitracin, inhibition effect was observed. Additive effects were predominant--49%. In 18% neutral effects were shown, but in 26% of combinations FIC indexes were not possible to calculate, because of the resistance of clinical strains to the highest concentration of at least one antibiotic. In combination of aminoglycoside (neomycin) with beta-lactams antibiotics (ampicillin, procaine penicillin) in vitro, no synergy was observed for all examined strains. The best results were achieved for combinations of neomycin with peptide antibiotics (polymyxin, gramicidin and bacitracin)--5 for all 6 synergy effect observed.
