Strapping of joints.

Strapping with adhesive tape plays a significant part in the prevention of joint injuries, particularly at the ankle and hand, and is important during early management of injury and rehabilitation. Recurrence of injury when the athlete recommences training is less likely when supportive strapping is applied.

[Role of the articular immobilization position in prevention of post-traumatic rigidity in humans].

The position in which the articulation is immobilized is in fact the pattern in which capsular fibrosis and of the connective tissue is settled. It will also determine the degree of remanent articular mobility. The author does not recommend immobilization in the so-called "functional" position since this does not prevent the developement of articular rigidity and is even responsible for their developement. In view of maintaining the articular function when the articulation itself is involved or when it is immobilized due to lesions in its vicinity, immobilization should be made in a position allowing for maximal elongation of the connective fibers in the area which depends from the vascular and nervous viewpoint from the traumatic lesion, or that which is involved by surgery, that is in positions which are quite different from those used at present.

Operative lengthening of the femur.

Femoral lengthening with the aid of the apparatus described in this article can be accomplished safely and predictably. However, it is not without considerations and problems which must be fully understood and appreciated. The healing process requires months to years; with a cooperative patient and a diligent attentive orthopedist, the equalization of leg lengths is more than justified.

[Larsen syndrome: multicenter study of 12 new cases. Diagnosis, planning and results of treatment].

Larsen's syndrome is characterized by the association of congenital knee, hip and elbow dislocations, characteristic facial abnormalities, joint hyperlaxity, and other inconstant malformations. A review is made after description of 12 new cases from a multicenter study. A partial modification of diagnostic criteria is proposed, based on the analysis of the frequency and localization of the deformities. Radiological aspects, surgical indications, clinical evolution, complications and late results are presented, with emphasis on lower extremity problems. Good clinical results are more remarkable in the children treated early. Periodic follow-up is obligatory for good therapeutic results.

[Biomechanics and semiology of transient lateral patellar dislocation: reliability of magnetic resonance diagnosis].

Transient patellar dislocation is a rare finding. It is usually due to direct trauma in mediolateral direction or to indirect trauma related to violent quadriceps muscle contraction. The clinical diagnosis of transient forms is usually rather difficult because the associated signs (hemarthros and pain at the joint and/or at the vastus medialis insertion) are not specific. Conventional radiography and CT are very accurate in bone studies, but exhibit major limitations in the detection of capsular injuries with intramedullary and/or cartilaginous bone involvement. We investigated MR diagnostic reliability in the study of the injuries caused by transient patellar dislocation. 526 MR examinations of the knee were reviewed retrospectively; fifteen of them were positive for transient patellar dislocation. We used a .5 T superconductive MR unit with a circumferential extremity coil. Axial, coronal and sagittal T1-weighted spin echo, T2*-weighted gradient echo and axial STIR images were acquired. The following criteria were considered specific for the diagnosis of former lateral patellar dislocation: retinacular changes; patella site; signal intensity changes in femoral and patellar intramedullary bone; joint effusion; cartilage and subchondral bone changes. All the patients with transient patellar dislocation had hemarthros with inner retinaculum involvement. The femoropatellar joint was incongruous in 115 cases; patellar subluxation was external in 8/11 patients and lateral in the remaining 3 patients. Medullary bruises were found in 8/15 cases and cartilage injuries in 13/15. Finally, other joint components, such as the posterior horn of the medial meniscus or the anterior cruciate ligament were involved in 6/13 cases. Arthroscopy was performed in 6 patients with associated injuries and confirmed all MR findings (100% agreement). To conclude, in our experience MRI was a very reliable tool to study the injuries due to transient patellar dislocation because it showed specific changes and possible associated traumas, which helps choose the most suitable treatment.

Augmented repair of avulsion-tear type medial patellofemoral ligament injury in acute patellar dislocation.

This study investigated the results of augmented repair for avulsion-tear type medial patellofemoral ligament (MPFL) injury performed on a total of five knees with initial acute lateral patellar dislocation. Augmented repair was performed using a medial retinaculum slip on all five knees. The average follow-up was 5.9 years. The results were evaluated with the Insall, Aglietti, and Tria grading system. Three knees were classified as excellent, one knee as good, and one knee as fair. The mean Kujala score at follow-up was 97.6 points. Augmented repair of the injured MPFL for avulsion-type MPFL injury may be considered as one good option to prevent subsequent dislocation and subluxation.

Patellofemoral relationships and distal insertion of the vastus medialis muscle: a magnetic resonance imaging study in nonsymptomatic subjects and in patients with patellar dislocation.

The correlation between the insertion level of the vastus medialis muscle and lateral patellofemoral angle (LPA), lateral patellar displacement (LPD) and tilt (LPT), and the height of the patella (LT/LP) was analyzed by magnetic resonance imaging (MRI) in patients with patellar dislocation (n = 10) and in control subjects (n = 10). Images were produced in 0 degrees and 20 degrees of knee flexion. The insertion level to the patella was also analyzed with the knee in extension. No significant correlation was noted between the insertion level and different patellofemoral indexes, but multiple-regression analysis showed that the best predictor for the insertion level was the LPA with the knee in extension. The insertion level was significantly more proximal in patients with patellar dislocations than in normal subjects.
