Interaction between histamine H3 receptors and other prejunctional receptor systems in the isolated guinea pig duodenum.

The hypothesis that prejunctional histamine H3 receptors and alpha-2 adrenoceptors interact with each other was assessed on the cholinergic transmission of the guinea pig duodenum. Specific agonists acting at histamine H3 receptors, alpha-2 adrenoceptors and adenosine A1 receptors, (R)-alpha-methylhistamine (1 nM-1 microM), UK 14,304 (1 nM-1 microM) and N6-cyclopentyladenosine (0.1 nM-0.1 microM), respectively, inhibited muscle contractions evoked by electrical stimulation, the effect being antagonized by specific receptor blockers, thioperamide and clobenpropit (H3 receptors), idazoxan and yohimbine alpha-2 adrenoceptors) and 8-cyclopentyl-1,3-dimethylxanthine (A1 receptors). The simultaneous activation of H3 receptors and alpha-2 adrenoceptors, using EC50 values of the specific agonists (UK 14,304: 30 nM; (R)-alpha-methylhistamine: 20 nM), produced a combined effect that did not differ from the sum of the individual effects, a result that excluded the occurrence of interactions between these receptors. Conversely, the inhibition evoked by the coadministration of N6-cyclopentyladenosine (EC50: 2.5 nm) and (R)-alpha-methylhistamine or of N6-cyclopentyladenosine and UK 14,304 was significantly lower than the sum of the individual effects, which suggests that the corresponding prejunctional receptors interact with each other. No interaction could be detected when threshold concentrations (EC(10-15)) of the different agonists were simultaneously applied. These data show a negative cooperativity between H3 and A1 receptors and between A1 and alpha-2 receptors. Conversely, no evidence of positive cooperativity emerged, even when the different agonists were applied at low or maximum concentrations. The lack of cross-reactivity between the respective agonists excludes an interaction at the recognition sites of the receptor moyeties. Therefore, these phenomena are more likely to reflect interplays between second messengers or effectors involved in modulating the ACh release.

Investigation of microcirculatory changes in the duodenum of dogs caused by surgical suture materials.

Surgical suture materials play an important role in the safe performance of surgical interventions. In our experiments we made an attempt to investigate what microcirculatory changes result from pulling the thread through the wall of the duodenum in Lembert stitches by including 3 kinds of absorbable and non-absorbable suture materials each (Catgut, Dexon, PDS, Silk, Ethibond, Ethilon). Research is still in the pre-experimental stage. In the long run, we hope to enrich the description of these suture materials by some new details. It could help prevent suture insufficiency, facilitate wound healing and thus, improved surgical safety.

[Primary adenocarcinoma of the duodenum].

Adenocarcinoma of the duodenum represents a rare neoplasia characterized by an indefinite symptomatology, at least early, and deceitful, that, therefore, arrives at surgeon, almost always, in advanced stage. The Authors, taking from a clinical case, recently observed, as a starting point, review the literature, referring the etiopathogenetic hypothesis that explain the relative rarity of this neoplasia. They therefore report the diagnostic and therapeutic procedure to carry out in these patients.

[Juxtapapillary diverticula of the duodenum and bilio-pancreatic symptoms (author's transl)].

Juxtapapillary diverticula of the duodenum are more frequent than has been assumed up to now. Among 2100 patients undergoing duodenoscopy 135 duodenal diverticula have been found. These diverticula may cause symptoms of diverculitis as well as of bilio-pancreatic disease. Such diverticula ought to be treated by surgery. In our department diverticula are turned inward, or a papillo-diverticulotomy is done depending upon the localization of the diverticula.

Effect of 2,4-dichlorophenoxy acetic acid on rabbit duodenum.

2,4-dichlorophenoxy acetic acid (2,4-D) produces myotonia in healthy animals. The action of this drug was studied on smooth muscles in vitro using isolated strips of rabbit's duodenum. The drug was found to have a stimulant action on the smooth muscle. The action seems to be a direct one.

[An inhibitor of enteropeptidases and trypsin from the bovine duodenum].

Enteropeptidase inhibitor (DI) was isolated from bovine duodenum during purification of this enzyme. DI was purified by affinity chromatography on immobilised trypsin. DI preparations contain two main components: DI-9 (9 kD) and DI-20 (20 kD). The N-terminal amino acid sequence 1-19 of DI-9 is highly homologous to the Kunitz inhibitor (BPI). Molecular weights of DI-9 and BPI are the same (gel electrophoresis data). Fragment 1-19 of DI-9 differs from the corresponding region of BPI only at the position 17: DI-9 contains Ala-17 instead of Arg in BPI. The homology of N-terminal amino acid sequence 1-25 of DI-20 with the corresponding regions of some phospholipases A2 suggests that this protein is a new intestinal phospholipase A2. Inhibitor DI-9 and phospholipase DI-20 are probably isolated in a common lipoprotein complex. The only earlier known in vitro inhibitor of enteropeptidase, BPI, was localised in vivo in different tissues with this enzyme. In our opinion the Kunitz-type inhibitor DI-9 is, a physiological inhibitor of enteropeptidase.

Primary malignant lymphoma of the duodenum: report of a case.

We present a case of long-term survival in primary malignant lymphoma of the duodenum treated with a curative surgical resection and post-operative chemotherapy. A 72 year-old man was admitted to our hospital complaining of epigastric discomfort. Endoscopy revealed an ulcerative tumor in the duodenal bulb. The tumor was diagnosed histologically as malignant lymphoma by endoscopic biopsy. A distal gastrectomy and duodenal bulb resection were performed with lymph nodes dissection. The tumor was histologically classified as B-cell phenotype, large cell, immunoblastic lymphoma of high grade, according to the Working Formulation. The patient received CHOP (cyclophosphamide, doxorubicin, vincristine and prednisone) chemotherapy after the operation, and he has no sign of recurrence during a follow-up period of 6 years.

Massive bleeding from a ruptured superior mesenteric artery aneurysm duodenum.

A traumatic false aneurysm of the superior mesenteric artery resulting from a gunshot wound five years previously, caused massive hemorrhage into the second portion of the duodenum. Dissection, resection, and end-to-end anastomosis was successful in controlling bleeding into the gastrointestinal tract, but immediate postoperative angiography showed a reappearance of the aneurysm, and three months later, it bled into the retroperitoneum. The patient underwent further resection and two subsequent reconstructions of the superior mesenteric artery, the first with a Dacron graft, which clotted, and the second with autologous saphenous vein, which was successful. Three years later, the patient is well. The case shows that adequate reconstruction of an arterial injury is best performed immediately after it is diagnosed.

Adenocarcinoma of duodenum and ampulla of Vater: clinicopathology study and expression of p53, c-neu, TGF-alpha, CEA, and EMA.

Oncogenes, tumor suppressor genes, and growth factors are being explored as to their role in the initiation and progression of most neoplasms, but little information exists on the expression of oncoproteins or growth factors in adenocarcinoma of the duodenum or ampulla of Vater. This report covers expressions of p53, c-neu, TGF-alpha, CEA, and EMA in duodenal adenocarcinoma and ampullary adenocarcinoma, as well as correlations between expressions and tumor stage, histological grade and patient survival. The expression of p53, c-neu, TGF-alpha, CEA, and EMA has been studied in 15 duodenal adenocarcinomas and in eight ampullary adenocarcinomas by avidin-biotin-peroxidase complex indirect immunoperoxidase technique. The positive reaction for p53, c-neu, TGF-alpha, CEA, and EMA in duodenal adenocarcinoma was 20%, 60%, 60%, 73%, and 100%, respectively, and in ampullary adenocarcinoma, 13%, 100%, 50%, 63%, and 100%. Among the duodenal tumors, C-neu and p53 expression was noted more frequently in groups with high histological grades. Patients with c-neu positive duodenal adenocarcinoma had a shorter survival than the patients with c-neu negative duodenal adenocarcinoma (P < 0.01). C-neu product may serve as an unfavorable prognostic indicator in duodenal adenocarcinoma. No statistically significant correlation was found between the expressions of CEA, EMA, p53, and TGF-alpha and patient survival, tumor stage, or histological grade in either duodenal or ampullary adenocarcinomas.

Villous tumors of the duodenum: a clinicopathologic study.

Eight villous tumors of the duodenum were found and studied in a review of surgical and autopsy material collected over 30 years. Although the number of cases is small, it is nevertheless one of the largest single series on this lesion that has been reported. The purposes of the study were (1) to describe the histopathology of the lesions and (2) to test the diagnostic criteria of malignancy previously proposed in the literature. Of the large number of criteria studied, none, alone or in combination, proved reliable indicators of malignancy; even in this small series exceptions could be found. Of all the criteria tested, however, secondary gland formation was found to be the most reliable. The value of endoscopic biopsies in establishing the preoperative diagnosis of malignancy in this lesion seems to be analogous to that in similar colonic lesions. Prognostic and therapeutic criteria are discussed. In addition, the pertinent literature is reviewed.

Gastrin-secreting tumor of the duodenum (G-cell apudoma) associated with secondary biliary cirrhosis.

A middle-aged woman with evidence of chronic cholestasis of several years and no previous abdominal surgery was initially thought to have primary biliary cirrhosis. Clinical evaluation disclosed a well-developed secondary biliary cirrhosis apparently caused by extrahepatic obstruction due to a 1 X 2 cm neoplasm of the periampullary duodenum. Electron microscopy and immunofluorescent studies showed the neoplasm to be a G-cell adenoma. Wide local excision has resolved the biliary obstruction. Bening or slow-growing duodenal tumors, if they involve the ampulla of Vater, may produce prolonged partial extrahepatic obstruction and secondary biliary cirrhosis.

Primary non-Hodgkin's lymphoma of the duodenum. Case report and literature review.

A case of primary non-Hodgkin's lymphoma of the duodenum is presented. A review of the English literature documented 95 cases, and the presenting signs, symptoms, and radiographic findings have been abstracted. Attention is also drawn to the treatment of 27 cases, 11 of which (41%) survived for 2 or more years. Ten of the 11 long-term survivors were treated with radiotherapy with or without resection, whereas only one long-term survivor was treated by surgery alone.

Villous adenomas in the duodenum.

Five patients with villous adenomas in the duodenum are described. In one patient malignant degeneration had occurred at the time of diagnosis. This patient was treated with a pancreaticoduodenal resection and is doing well 2 years postoperatively. The other four patients all had a duodenotomy and a local excision of the tumour. One patient had a recurrence with malignant degeneration within 1 year. The other three patients are doing well without signs of recurrence 1-4 years postoperatively. It is concluded that duodenal villous adenomas are potentially malignant. The strategy of surgical treatment is discussed.
