Mortality and morbidity rates after inferior vena caval clipping.

In 112 patients receiving inferior vena caval, serrated, Teflon clips over a 7 year period beginning Jan. 1, 1966, the operative mortality rate was 6 percent (seven patients) and the 1 to 12 month mortality rate was 9 percent (10 patients). Patients with severe heart disease accounted for eight of these 17 deaths. The mortaligy rate was zero in patients who were free of heart disease, malignancy, sepsis, or massive pulmonary embolism. Recurrent pulmonary embolism was suspected, but not lethal, in four patients and was proved in two. Patients who had leg swelling preoperatively were more likely to have swelling postoperatively than those who were initially free of swelling. Postoperative anticoagulation was ineffective in preventing edema and carried the risk of wound hemorrhage. Twelve months after operation, 12 of the 83 patients available for follow-up had severe lower extremity symptoms and 23 had milder, easily controlled edema. Late stasis changes, therefore, appeared at an acceptably low frequency and could reasonably be expected for patients with lower extremity phlebothrombosis. There was no instance of fatal pulmonary embolism in the follow-up period extending from 1 to 6 years.

[The effect of lycium barbarum polysaccharide on vascular tension in two-kidney, one clip model of hypertension].

In the present study, the effects of lycium barbarum polysaccharide (LBP) on endothelial function in the two-kidney, one clip model of hypertension were observed. The results showed that the increase of blood pressure in hypertension rats (HR) could be prevented significantly by treatment with 10% LBP. In isolated aortic rings of LBP-treated rats, the contraction of phenylephrine (PE) was reduced as compared with HR rats. Removal of the endothelium abolished the difference of PE-induced vasoconstriction among groups. In vitro incubation of aortic rings from LBP-treated rats with methyl blue (MB) or N-nitro-L-arginine methyl ester (L-NAME) increased the magnitude of PE-induced contraction. Meanwhile the response to acetylcholine (ACh) was significantly increased in LBP-treated rats, but the response to nitroprusside had no significant difference among groups. Pretreatment with L-arginine partially restored ACh-induced relaxation in RH rats, but no effect in LBP-treated rats. These results suggested that the role of LBP in decreasing vasoconstriction to PE may be mediated by increase of the effects or/and production of endothelium-derived relaxation factor (EDRF). LBP increased formation of EDRF may be related to increase the substrate of EDRF.

[A three-dimensional anisotropic finite element analysis of modified bar-clip implant-borne overdenture in the edentulous mandible under punching loads].

The stress distributions of the modified bar-clip implant-borne overdenture in the edentulous mandible were performed under punching loads with both porcelain and resin chosen as the restorative materials by means of three-dimensional anisotropic finite element method. The purpose of this study was to investigate the patterns of stress distributions and to compare the effect of different restorative materials on the stress distributions under punching loads. The results showed that the value of stress peaks in implants with porcelain restorations seemed higher than those with resin restorations. On the contrary, in the bone-implant interface had little difference, except a comparative uniform stress distribution found in resin restorations. It implies that resin restoration produces a cushioning effect on reducing the stress peaks in implants themselves under punching loads. Both porcelain and resin restorations can be chosen as the restorative materials because of a little influence on the stress peaks in bone-implant interface under punching loads.

Original memoirs: the control of bleeding in operations for brain tumors: with the description of silver "clips" for the occlusion of vessels inaccessible to the ligature. 1911.

One of the chief objects of concern in intracranial surgery should be the avoidance of any unnecessary loss of blood, for at best, in many cases of brain tumor associated with venous stasis, bleeding is likely to be so excessive as to necessitate postponement of the final steps of the procedure until a second or even a third session. The common methods of blood stilling by sponge, clamp, and ligature are largely inapplicable to intracranial surgery, particularly in the presence of bleeding from the nervous tissues themselves, and any device which serves as an aid to hemostasis in these difficult operations will bring a number of them to a safe termination at a single sitting, with less loss of blood and less damage to the brain itself. In addition to the more familiar tourniquet for the scalp, and wax for diploetic and emissary bleeding, suggestions are offered as to the use of gauze pledgets, dry sterile cotton, fragments of raw muscle and other tissues, as well as sections of organizing blood-clots for superficial meningeal bleeding, and silver "clips" for inaccessible individual points ether in dura or brain. The successful consummation of any critical operation often depends upon seeming trifles. It is, however, the scrupulous observance of surgical minutiae that makes possible the safe conduct of major intracranial performances--performances which a few years ago were attended in most cases by a veritable dance Macaber.

[Changes in Galphaq/11 mediated signal transduction pathway in the aorta of two kidney one clip hypertensive rats].

The objective of this study was to investigate the role of signal transduction pathway mediated by Galphaq/11 of the aorta in blood pressure regulation during the development of two-kidney one clip (2K1C) Glodblatt hypertension in the rat. Levels of Galphaq/11-subunit and extracellular signal regulated kinase 1/2 (ERK1/2) of the aorta were measured by Western blot analysis at week 1, 2, 4 or 8 after operation. Phospholipase C (PLC) activity of the aorta was measured by using (3)H phosphatidylinostol (4,5) bisphosphate as a substrate. Both systolic and diastolic blood pressure increased markedly in 2K1C rats at week 2, 4 or 8, whereas, the increase in Galphaq/11 and ERK1/2 expression of the aorta began at week 1 after operation (increased by 57.53% and 40.16%, respectively), and maintained at a high level during week 2 8 compared with time matched controls (P<0.01). PLC activities in the aorta were also increased significantly in 2K1C groups at week 4 and 8 compared with the time matched controls (P<0.05). The results indicate that Galphaq/11-mediated signal transduction pathway of the aorta is activated in 2K1C hypertension, and may contribute to the initiation and maintenance of renal hypertension.

Endoscopic clips -- what are they good for?

Several methods have been developed for the treatment of upper gastrointestinal bleedings, a condition associated with high mortality rates. In recent years new studies have been published reporting the use of endoscopic clips mainly for digestive bleeding. Our paper reviews recent publications, focusing mainly on present indications, such as: digestive bleeding, conservative treatment of perforations of the digestive tract, repair of anastomotic leaks, marking of digestive tumours, endoprosthestesis anchoring and assisting papilla cannulation.

Comparative Study of the Use of a Suturing System and Titanium Clips.

There is a need for secure and easy methods for suturing in laparoscopic surgery, no matter whether vessels are to be ligated, two structures sutured together, or the cystic duct is to be occluded in cholecystectomy. Laparoscopic suturing can be done with a suture, using automatic sewing devices or clips. Improper ligation of a vessel for example can result in bleeding, which is more difficult to treat in a laparoscopic procedure than in an open procedure. By using a Roeder knot in laparoscopic suturing, tying of the knot requires some expertise and makes the procedure more complicated and time-consuming.

[INTRAGASTRIC MIGRATION OF CLIPS. A NEW COMPLICATIONOF LAPAROSCOPIC VAGOTOMY]

Since 1989, when Kathdouda, M. D. and Mouiel, M. D. performed a laparoscopic vagotomy following the Taylor technique, there have been several reports of duodenal peptic ulcer laparoscopic treatment through different methods of vagotomy, from the trouncular to the supraselective or any of their varieties, as Taylor and Hill-Baker procedures. With the appearance of this new form of surgery, new complications are expected. This study reports the intragastrical migration of clips as a complication not yet reported. The possible etiopathogenic factors that could have influenced the appearance of such complication, are here discussed.

Effect of captopril on serotonergic mechanisms in two-kidney, one-clip renal hypertensive rats.

In 2K,1C-RHR (two-kidney, one-clip hypertensive rats) serotonergic mechanisms in blood platelets were studied. The endogenous serotonin (5-HT) concentration in whole blood and in platelets remained unchanged in relation to the sham operated rats. Also the uptake of labelled 5-HT in rats with renal hypertension was not altered. However platelets aggregability was increased in 2K,1C-RHR. Acute administration of captopril (10 mg/kg and 100 mg/kg po) diminished blood pressure but did not change either the concentration of 5-HT in whole blood and in platelets of hypertensive rats or the uptake of this amine. Platelets aggregation and the amplifying effect of 5-HT in hypertensive rats were also unchanged after acute captopril administration. Similar results were observed after its administration in a dose of 30 mg/kg for one week. Our results indicate that captopril did not affect the platelets serotonergic mechanisms in 2K,1C-RHR.

[Clip markings of the uterus and the urinary bladder for the dosimetry of intracavity and percutaneous radiotherapy (author's transl)].

The exact marking of certain regional landmarks of the anatomy permits the determination of the co-ordinates of these points by stereoscopic diagnostic radiology. The mid-point of the upper margin of the symphysis is selected as the origin of the co-ordinates. The markings of the uterus are placed through the laparoscope with silver clips. The markings in the urinary bladder are placed close to the ureteral orifices through the cytoscope. The doses of radiation generated by the sources of radiation (tubes, colpostat, vaginal cylinder, cervical plate) can with this new marking technique not only be measured experimentally in anatomically well-defined points in the body but the dosage (the energy) can be calculated by the computer. In as many points in the body as desired. The maximal permissable dosage in the tumor can be determined with special consideration for the avoidance of radiation lesions to marked adjacent points such as the ureteral orifices.

[A new method of microvascular anastomosis: clips with a soluble stent].

In this study, we investigated a new method of microvascular anastomosis using titanium clips combined with a soluble intraluminal stent. This method was used to perform anastomoses of the common carotid artery in five NZW rabbits and the abdominal aorta in five rabbits. Five rabbits ears were isolated on their central vascular pedicles. The central ear artery was transected and anastomosed using the stent/clip technique. In five carotid arteries, a conventional sutured anastomosis was performed. All anastomoses were patent at POD 1, 7 and 21. The average time to perform stent/clip anastomoses was significantly (p < 0.001) shorter than conventional sutured anastomoses (2.5 min vs. 9 min). There was no observable side effect from the stent material and no evidence of peripheral emboli. The stent/clip technique is an easy, fast and safe method of microvascular anastomosis.

Female sterilization by laparoscopy: a comparative study of tubal occlusion with electrocoagulation and spring-loaded clip with one-year follow-up.

Findings are reported of a study conducted to evaluate and compare the safety and effectiveness of the spring-loaded clip and the electrocoagulation techniques of tubal occlusion via laparoscopy. The study was conducted from June, 1974 to June, 1975 on 300 cases in a Bangkok, Thailand hospital. Only patients who requested sterilization to limit family size were included in the study, and study patients were required to return for 6- and 12-month follow-up visits. To minimize possible patient and physician biases, all sterilization procedures were performed by one author (S. K.). and data were analyzed by the other author (S. P.). The specific operative technique was randomly assigned to the patient immediately prior to surgery, and the patient was unaware of the particular technique used. The findings showed no potentially serious complications for either technique. Neither complication rates nor incidence of pain were significantly different for the two procedures. The presence of adhesions was the most frequent cause of surgical difficulty in both techniques. No mechanical difficulties were reported. No pregnancies were reported within one year of sterilization.

[A case of dissociative stupor with decorticated posture after neck clipping of brain aneurysm].

We experienced a case of dissociative stupor with decorticated posture in a 71-year-old woman after neck clipping of the brain aneurysm. Decorticated posture is observed with severe midbrain disorder caused by brain herniation. In this case, therefore, severe brain stem damage was suspected, although light reflex was observed and respiration was stable. In addition, in this case, the aneurysm was located at the junction of the internal carotid artery and the posterior communicating artery. Therefore, surgical damage to midbrain was not likely to have happened. Postoperative computed tomography showed no abnormality in this region. Two days later, her symptom disappeared, and she could clearly recollect memory of these two days just after surgery. However, she could not follow directions because of a sense of fear. Thus her symptom might be based on a kind of mental disorder. Patients with conversion disorder react abnormally to stimulus or stress. Stupor was diagnosed as due to defect of reaction to stimulus of sound, light and touch. She was diagnosed as dissociative stupor of the conversion disorder type. In this case, decorticated posture was caused by mental disorder. We have to pay attention not only to brain tissue damage but also to psychological damage of patients.

Evaluation of renal functional reserve of contralateral kidney of two-kidney, one clip Goldblatt hypertensive rats.

To evaluate the presence of hyperfiltration in two-kidney, one clip (2K, 1C) Goldblatt hypertensive rats, micropuncture studies of the unclipped kidney were used to examine renal functional reserve (RFR) with glycine (G) infusion in normal (N) rats and 2K, 1C rats. Systemic hypertension in 2K, 1C rats was associated with significantly increased values of single-nephron glomerular filtration rate (SNGFR), glomerular plasma flow (QA) and glomerular capillary hydrostatic pressure (PGC) when compared with N rats. Glycine infusion produced a marked increase in SNGFR, QA and whole-kidney GFR in N rats. In contrast, no changes were observed in these parameters in 2K, 1C rats. Lack of response in 2K, 1C rats was related to the failure of renal vessels to dilate during G infusion. These results suggest that loss of renal functional reserve in systemic hypertension is dependent on an abnormal function of renal vascular resistances.

[Central hyperthermia suspected of malignant hyperthermia in a patient undergoing radical neck clipping for cerebral aneurysm].

A 45-year-old woman underwent radical neck clipping for cerebral aneurysm under isoflurane anesthesia. Her preoperative examination revealed elevated body temperature which had been normal on admission. Her body temperature increased up to 40.3 degrees C during anesthesia and surgery, and it showed a downward trend at the end of surgery. Malignant hyperthermia was excluded because the patient did not have metabolic acidosis, hypercarbia, hyperpotassemia or abnormal sweating anesthesia. The patient received intravenous dantrolene postoperatively since there was a suspicion of malignant hyperthermia on the basis of hyperthermia and increases in serum creatine kinase (CK) and myoglobin (Mb) levels. Her body temperature and serum CK and Mb levels decreased for a while after administration of dantrolene, but they increased again thereafter. The patient was aggressively cooled with a cooling blanket and hyperthermia and increases in serum CK and Mb levels disappeared in postoperative two weeks. She was discharged on foot without any neurological deficit on the forty-third hospital day. According to the diagnostic criteria for malignant hyperthermia by Larach and his colleague, malignant hyperthermia was somewhat less than likely in our case. The clinical course of the patient also suggested that a possibility of malignant hyperthermia was considerably low. The authors conclude that perioperative hyperthermia in our case must have derived from central hyperthermia following subarachnoid hemorrhage, and that postoperative increases in serum CK and Mb levels might have resulted from acceleration of sympathetic nervous system by subarachnoid hemorrhage.

[Retention and rehabilitation of complex maxillofacial defects based on bar-clip and stud attachment].

PURPOSE: To evaluate the application of precise attachment in restoration of complicated maxillofacial defect. METHODS: A framework with bar-clip and stud attachment was designed and fabricated. Maxillofacial prosthesis with this framework was applied to treat a patient with defects of nose, entire upper lip and the corresponding maxillofacial areas. The retention force was tested with Universal Testing Device. RESULTS: After treatment, the patient got a better appearance, normal function of mastication and clear pronunciation. The dislodgement test showed the retention force was 40N. CONCLUSION: Application of precise attachment in complicated maxillofacial defects can get satisfactory result.
