Early posttraumatic pulmonary platelet trapping and its potentiation by oral pretreatment with alcohol.

Soft tissue trauma is associated with platelet aggregation and sequestration in the lungs. This is believed to be an early step in the later development of adult respiratory distress syndrome. In the present experiment using a new method for in vivo dynamic studies of platelet sequestration, we wanted to evaluate the effect of soft tissue trauma on pulmonary platelet trapping in pigs and the influence of acute alcohol intoxication. The results show that significant pulmonary platelet trapping is registered within minutes of trauma and that alcohol significantly increases platelet sequestration in the lungs. This indicates an increased risk for posttraumatic pulmonary problems in alcohol-intoxicated trauma victims.

The prevalence of alcohol-related mortality in both sexes: variation between indicators, Stockholm, 1987.

OBJECTIVES: The purpose of this study was to analyze the prevalence rates of alcohol-related mortality--according to various indicators--in both sexes in Stockholm, Sweden. METHODS: A study of alcohol involvement at death was undertaken for all 668 deceased persons aged 15 through 54 years in 1987 in Stockholm. Death certificates, autopsy information, police records, and information about earlier conviction were analyzed. RESULTS: When different measures of estimation were compared, there were great differences in the prevalence rates of alcohol involvement. According to the death certificates, 9.2% of the males and 11.2% of the females had alcoholism, alcohol intoxication, pancreatitis or liver cirrhosis as underlying cause of death. When all accessible information was used, potential alcohol involvement was found in 57.5% of the male and in 32.2% of the female deaths. There was a marked association between earlier drunken driving and alcohol involvement. After reevaluation of the diagnoses with autopsy findings, the number of cases with cardiac enlargement and suspected cardiomyopathy increased from 10 to 62. CONCLUSION: The results point to the serious underdiagnosis of alcohol involvement in death certificates and the misclassification of important causes of death (i.e., liver cirrhosis and cardiac disease); they also call for increased efforts regarding prevention.

Increased MB-creatine kinase isoenzymes in an alcoholic population.

Using agarose gel electrophoresis MB-creatine kinase (MB-CK) activity was examined in the serum of 120 patients with acute alcohol intoxication admitted to a detoxification ward. Total CK activity was increased in 67 percent of patients and MB-CK activity was increased in 8.3 percent. Alcoholic patients also were studied by Sephadex chromatography and, in seven cases, MB-CK was greater than three standard deviations from the normal. Thus, this study demonstrates that acute alcohol intoxication is associated with increased MB-CK activity. These findings raise the possibility that excessive alcohol ingestion may lead to acute myocardial injury.

[Fatal accidents in house fires. The most significant causes, such as smoking and alcohol abuse, multiplied by four the incidence during the last 40 years].

A population-based descriptive investigation of housefire accidents in Denmark was carried out for the two five year periods 1953-58 and 1988-93, based on death certificates, police reports and autopsy reports. The number of deaths due to housefire accidents in Denmark has increased (1953-58: 136 (66 men and 70 women), 1988-93: 363 (212 men and 150 women), mostly due to an increase in tobacco-smoking related fire accidents. In 1988-93 the three common causes of housefire deaths were tobacco-smoking, often in combination with alcohol intoxication or handicap (51%), cooking-accidents (10%) and accidents with candles (9%). The largest risk groups were chronic alcoholics, handicapped and elderly people. In conclusion, warnings should be issued against smoking in bed and use of loose-fitting clothing while cooking on an open fire. Protective aprons and devices for use while smoking, self-extinguishing cigarettes and use of fireproof materials in furniture and clothing may prevent ignition. Smoke-alarms may secure early warning.

New insights into positional alcohol nystagmus using three-dimensional eye-movement analysis.

The semicircular canals selectively transduce angular velocity and are normally insensitive to gravity and linear acceleration. In acute alcohol intoxication, however, the cupula becomes lighter than the endolymph, rendering it sensitive to gravity (buoyancy hypothesis). This results in positional alcohol nystagmus (PAN) and rotatory vertigo. We evaluated PAN in 8 normal subjects by means of three-dimensional eye-movement analysis in an attempt to clarify if the buoyancy mechanism is sufficient to explain PAN. Forty minutes after intake of 0.8 g of alcohol/kg of body weight, the subjects were positioned such that the lateral canals were earth vertical. They were then rotated in the plane of the lateral canals about an earth-horizontal axis to either 45 degrees or 90 degrees , right or left ear down, and eye movements were recorded for 40 seconds in each position. The spatial analysis of the responses showed that in addition to the nystagmus induced by the buoyancy of all six cupulae, alcohol intoxication also causes a vertical velocity offset (in all subjects, slow phase down) that is independent of the orientation of the subject in space. The offset may represent a toxic effect on central vestibular pathways, producing a tone imbalance of the vertical vestibulo-ocular reflex.

[Splenopentin--modulator of immunological and behavioral reactions in secondary immunodeficiency state induced by experimental alcoholism].

Effect of splenopentin on some patterns of immunity was studied in mice with chronic alcoholic intoxication. Splenopentin was administrated into animals once intraperitoneally (250 micrograms/kg). Administration of splenopentin was found to normalize several immunological patterns in animals with chronic alcoholic intoxication: the immune response to the thymus-dependent antigen sheep red blood cells and phagocytic activity of peritoneal macrophages. Also observations over C57B1/6 mice characterized by high level of alcoholic motivation showed that alcohol consumption in mice decreased after administration of splenopentin at a dose of 250 micrograms/kg during two weeks.

[Metabolism of glycolipids in the rat brain during acute and chronic ethanol administration].

Increase in content of sulfatides I and II and a decrease in cerebrosides was found in brain of rats after acute alcoholization with 25% ethanol, 4 g/kg, intragastric administration. In this case incorporation of 1,2-14C-ethanol into cerebrosides II and III was decreased. Content of sulfatides I was increased, while specific radioactivity of cerebrosides III--decreased as a result of chronic alcohol intoxication (25% ethanol, intragastric administration, 7 g/kg daily within 21 days).

[Intoxication at the wheel].

A large body of experimental and statistical evidence demonstrates that even mild alcohol intoxication with blood alcohol concentrations above 0,5% is associated with a significantly increase in the susceptibility for an accident. The lowering of the critical concentration limit is indicated from a medical, social-political and economical point of view.

[Latent myocardiopathy in chronic alcoholic patients with or without hepatic involvement].

Infraclinical myocardial lesions were searched for in patients with various types of liver disease due to chronic alcoholic intoxication. During a single procedure, a transjugular liver and right endoventricular biopsy and hemodynamic evaluation were performed in 26 patients without clinical evidence of cardiac involvement. Patients were classified into 5 groups: I, no liver disease (n = 4); II, fatty liver (n = 7); III, acute alcoholic hepatitis (n = 3); IV, cirrhosis (n = 7); V, cirrhosis with alcoholic hepatitis (n = 5). The study also included the determination of the serum thiamine level, a 24 h non-stop EKG recording and a M mode echocardiography. The cardiac-thoracic ratio, the EKG and Holter monitoring were normal. The serum thiamine levels decreased regularly from group I to group V, but there was no significant difference between patients with cirrhosis (group IV and V) and the others (group I to III). The same findings applied to the echocardiographic data. At rest, hemodynamic data were normal in all patients. Various degrees of myocardial lesions were present in 86 p. 100 of the cases. They included: cellular hypertrophy, contraction bands, interstitial fibrosis, fibroblastic infiltrate, perinuclear, cellular and or interstitial edema. Although frequent, these lesions were moderate and not specific. No correlation was found between the myocardial lesions and the type of liver disease. Myocardial lesions without cardiac manifestations have therefore been observed in vivo in nearly all patients with chronic alcoholic intoxication. These lesions were not correlated with the stage of alcoholic liver disease.

[Metabolic disturbances in blood from patients with senile cataracts accompanied by somatic diseases].

The results of metabolic parameters determination, which characterize the state of protein's metabolism, membranes stability, cell s system of antioxidant deficiency at the patients with senile cataract are presented with the purpose to make clear the differences between biochemical mechanisms of lenses clouding progress at the elderly persons without and on the background of different somatic pathology. Our results have shown that disturbances of membranes structural-functional parameters and the lowering of activity glutathione metabolic system is more expressed at the patients with accompanying diseases of cardio-vascular system, gastro-intestinal tract, liver and chronic alcohol intoxication. The exhaustion of antioxidant system is observed at the patients with senile cataract in the cases of accompanying disease of liver and chronic alcohol intoxication unlike the patients without accompanying somatic diseases. Diseases of cardio-vascular's system, gastrointestinal tract and liver effort the metabolic disturbances in blood of the patients with senile cataract.

[Alcohol as a provoking factor of suicidal behavior].

A study was made of the time-course of changes in the rate of suicides in some cities of South Kazakhstan. Hard drinking and chronic alcoholism were established to play a prominent role in the occurrence of suicidal situations. The positive changes in the social life that occurred over the recent years, realization of measures aimed at the control of hard drinking and alcoholism favoured a 35.4-percent reduction in the number of suicides. However, in spite of such a reduction, the share among the suicides of persons who resorted to self-aggression in a state of alcoholic intoxication remains in fact at the former level. This dictates the necessity of applying more efforts to the control of hard drinking and alcoholism.

Changes in myocardial metabolism induced by ethanol administration.

The utilization of ethyl alcohol in the myocardium and the reactions connected with its metabolism were investigated in rabbits which were given over a period of two months a 30% ethanol solution per os in a dosage of 1 g/kg body weight, and in a control group. Alcohol intoxication led to a reduction of the reserve of oxidized nicotinamide coenzymes, to an intensification of glycolytic processes, to slowed down utilization of substrates in the citric acid cycle and to an accumulation of less-oxidized metabolism products (dihydroxyacetone phosphate, lactate, pyruvate, malate, oxaloacetate, 2-oxoglutarate). A change was also observed in the myocardial microstructure. Metabolic changes are evidently one of the links of the pathogenetic mechanism of alcohol intoxication, which cause disorders in cardiac activity diagnosed as alcoholic myocardial dystrophy.

A critical involvement of oxidative stress in acute alcohol-induced hepatic TNF-alpha production.

Tumor necrosis factor-alpha (TNF-alpha) production is a critical factor in the pathogenesis of alcoholic liver injury. Both oxidative stress and endotoxin have been implicated in the process of alcohol-induced TNF-alpha production. However, a cause-and-effect relationship between these factors has not been fully defined. The present study was undertaken to determine the mediators of acute alcohol-induced TNF-alpha production using a mouse model of acute alcohol hepatotoxicity. Alcohol administration via gavage at a dose of 6 g/kg to 129/Sv mice induced hepatic TNF-alpha production in Kupffer cells as demonstrated by measuring protein levels, immunohistochemical localization, and mRNA expression. Alcohol intoxication caused liver injury in association with increases in plasma endotoxin and hepatic lipid peroxidation. Treatment with an endotoxin neutralizing protein significantly suppressed alcohol-induced elevation of plasma endotoxin, hepatic lipid peroxidation, and inhibited TNF-alpha production. Treatment with antioxidants, N-ACETYL-L-CYSTEINE, or dimethylsulfoxide, failed to attenuate plasma endotoxin elevation, but significantly inhibited alcohol-induced hepatic lipid peroxidation, TNF-alpha production and steatosis. All treatments prevented alcohol-induced necrotic cell death in the liver. This study thus systemically dissected the relationship among plasma endotoxin elevation, hepatic oxidative stress, and TNF-alpha production following acute alcohol administration, and the results demonstrate that oxidative stress mediates endotoxin-induced hepatic TNF-alpha production in acute alcohol intoxication.

Evaluation of colorimetric dipstick test to detect alcohol in saliva: a pilot study.

The Alco Screen Saliva Dipstick is an inexpensive, easy-to-use, colorimetric test that gives a semiquantitative estimation of the blood alcohol value by measuring the relative concentration of salivary alcohol. To evaluate its accuracy, we compared the results from tests with the Alco dipstick with values from simultaneously measured blood alcohol tests in 53 patients who were suspected of having ingested alcohol. The correlation between Alco dipstick results and blood alcohol values was strong (r [Spearman's rho], + .91). When the blood alcohol concentration was 0.1 g/dL or more, the Alco dipstick test was 90.9% sensitive, 71.4% specific, and 92% efficient. At Alco dipstick values of 0.02 and 0.05 g/dL, however, semiquantitative concordance was unsatisfactory. Nevertheless, even at the 0.05-g/dL value of salivary alcohol, the test was still valuable as a screen of de facto alcohol ingestion. Definitive diagnosis of relative alcohol intoxication requires confirmatory breath or blood alcohol concentrations by standard methodologies.

Alcohol, sexual arousal, and the transfer of excitation.

Investigations of the alcohol-related disinhibition of responses to deviant sexual stimuli suggest that the pharmacological actions of ethanol have little influence on the disinhibition process. The mere belief that alcohol is consumed is sufficient to induce increased sexual arousal. Studies with conventional stimuli, however, suggest that interactions occur between the pharmacological presence of ethanol and the psychological expectations of its presence. Thus, this article examines the contribution of pharmacological, cognitive, and environmental variables to perceived sexual arousal. A balanced-placebo design varied drink instruction and drink content independently. Pictures that elicited either a low or moderate level of self-reported sexual arousal were viewed and evaluated by men (n = 64) and women (n = 64) after completing their drinks. The evaluations and arousal measures suggested significant Instruction X Content X Arousal interactions. The strongest perceptions of arousal occurred among individuals who did not know they were drinking alcohol (i.e., subjects who were told that their alcoholic drinks did not contain alcohol). Apparently, when drinkers were unaware of the alcohol intoxication, the pharmacological excitation induced by alcohol transferred to the perception and evaluation of the slides.

[Psychiatric emergency cases in Hamm/Westfalen].

PURPOSE: Our study examines the frequency and further characteristics of psychiatric emergencies in the ambulance service of doctors on call in the city of Hamm. METHODS/PATIENTS: All emergency documentations of the year 2000 were retrospectively examined and evaluated with regard to psychiatric disorders according to the ICD-10. In Hamm (181, 197 inhabitants) there were 3812 emergency calls for doctors on call in the examined period. RESULTS: Due to internal emergencies, psychiatric diseases were diagnosed twice as often. Alcoholic intoxications (32.8 %), suicide attempts (16.8 %) and alcoholic withdrawal-syndromes were mostly diagnosed in psychiatric emergencies. Most patients were male. The average age was 41.7 years. CONCLUSIONS: In the education for doctors on call, the training in the subject psychiatry is very low. Therefore the education and training of the diagnostic and therapy of psychiatric disorders must be intensified in the acquiring of the certification for ambulance service. acquiring
