A thrombin ointment that achieves rapid haemostasis of split thickness donor wounds, particularly on the scalp.

A procedure is described to achieve rapid haemostasis of split thickness donor wounds by using an ointment containing thrombin (10,000 units mixed in 10 g of petroleum jelly-based gentamicin ointment). The ointment, prepared just before the removal of the skin graft, is immediately spread over the wound after the graft has been taken. With this technique, a steady haemostasis is promptly and simply achieved without the recurrence of bleeding compared to the technique using only a thrombin solution (bleeding time: a mean of 11.5 s for the thrombin ointment and 25.5 s for the thrombin solution, P less than 0.01). Thus, particularly when many skin grafts are needed, further skin grafts from adjacent areas can be readily taken without interference by bleeding. This procedure has proved to be extremely useful in achieving rapid haemostasis of split thickness skin donor wounds, especially when taking skin grafts from the scalp.

Biological monitoring of mercury vapour exposure by scalp hair analysis in comparison to blood and urine.

The reliability of human scalp hair as an indicator of mercury vapour exposure is contentious. In this study mercury concentrations in hair were compared with those in blood and urine of 20 dental students during their first "occupational" exposure to mercury vapour. Samples were collected before, at the end of the technical course of operating dentistry which lasted 6 weeks, and 3 months later. Mercury was measured by cold vapour atomic absorption spectrometry. In all biological media studied, mercury levels significantly (P < 0.05) reflected exposure to mercury vapour. After the time period without exposure mercury content decreased. Hair mercury levels were correlated to those in erythrocytes at sampling times 1 and 3 (r = 0.686 and r = 0.492) and to the frequency of fish consumption at sampling time 1. It is concluded that hair may be used as an indicator of internal uptake of mercury provided that it was not externally exposed to mercury vapour. In cases of occupational exposure to mercury vapour, hair is an useful tool for monitoring external exposure.

The envelope following response: scalp potentials elicited in the Mongolian gerbil using sinusoidally AM acoustic signals.

Scalp potentials which follow the low frequency envelope of a sinusoidally amplitude modulated stimulus waveform were evoked and recorded in anesthetized gerbils. This envelope following response (EFR) is presumably due to the synchronized discharge of populations of neurons in the auditory pathway. The magnitude of the EFR increased and the latency decreased in a near monotonic fashion with increased stimulus intensity and modulation depth. The modulation rate transfer function (MRTF) was determined for modulation frequencies between 10 and 920 Hz imposed on carrier frequencies ranging from 1 to 7 kHz. The MRTF was low pass in character having a corner frequency of 100-120 Hz. Measurements of the group delay, determined from the phase of the response relative to the stimulus phase, indicate that the response is generated in at least three distinct regions within the auditory pathway.

LAT (lidocaine-adrenaline-tetracaine) versus TAC (tetracaine-adrenaline-cocaine) for topical anesthesia in face and scalp lacerations.

The study objective was to compare the topical anesthetic LAT (4% lidocaine, 1:2,000 adrenaline, 1% tetracaine) to TAC (0.5% tetracaine, 1:2,000 adrenaline, 11.8% cocaine) for efficacy, adverse effects, and costs. The study design was a randomized, prospective, double blind clinical trial set in an inner-city emergency department with an emergency medicine residency program. Adults with linear lacerations of the face or scalp were eligible for inclusion in the study. Patients had lacerations anesthetized with topical TAC or LAT according to a random numbers table. A total of 95 patients were included in the study with 47 receiving TAC and 48 receiving LAT. Patients stated the number of sutures causing pain and patients and physicians rated the overall pain of suturing using a standard visual analog scale (VAS). The power of the study to determine a ranked sum difference of 15 was 0.8. Visual analog scale results and number and percentage of sutures causing pain were compared using Wilcoxon's Rank Sum Test. According to patients, the percentage of sutures causing pain was significantly fewer for LAT than TAC (P = .036, Interquartile Range 0.13 to 0.0 for LAT, 0.25 to 0 for TAC). Physicians found LAT statistically more effective than TAC (P = .0093, Interquartile Range 1 to 0 for LAT, 2 to 0 for TAC) but patients did not report a difference (P = .266, Interquartile Range 1 to 0 for both LAT and TAC). Our cost per application was $3.00 for LAT compared to $35.00 for TAC. Follow-up was accomplished in 91 of 95 patients (95%) with no reported complications for either medication.(ABSTRACT TRUNCATED AT 250 WORDS)

Prosthetic valve endocarditis developing after an infected scalp laceration.

This case report describes a 21-year-old man who developed prosthetic valve endocarditis (PVE) from an infected scalp laceration. Despite intravenous antibiotic therapy, the infectious process could not be controlled and the patient required replacement of the aortic valve. Our patient is the fourth case of late PVE occurring after a subcutaneous infection. In each case, antibiotic prophylaxis of infective endocarditis was not practiced. Current antibiotic regimens for prophylaxis of PVE are reviewed.

Electron field within an electron field treatment of the scalp or other curved surfaces.

The treatment of a curved surface is a challenge for radiation therapy. This patient had previous treatment with electrons to the "skull cap" area for malignant melanoma. The current task was to design a treatment schema to include the rest of the scalp while avoiding the skull cap. Opposing lateral 6-MeV electron fields were used, as well as two anterior 6-MeV electron fields, one field within the other, to produce an acceptable radiation dose to the tumor volume.

Design of an FECG scalp electrode fetal heart rate monitor.

The design of a fetal heart rate (FHR) monitor using fetal electrocardiogram (FECG) scalp electrodes is described. It is shown that the design approach followed two stages: generation of FHR pulses at R-R intervals and FHR computation. The former uses a simple hardware approach for QRS detection and R-wave enhancement, while the latter requires a software implementation in order to produce FHR traces on a beat to beat basis. The QRS detection is based on bandpass filtering using switched mode capacitor technique; the R-wave enhancement and amplitude information are achieved by differentiation followed by fullwave rectification and peak detection. An adaptive threshold together with a comparator circuit are used to generate FHR pulses at R-R intervals. Beat to beat variations of FHR traces are produced by hardware and software implementation on a Z80 microprocessor board. Results obtained by the FHR monitor are evaluated and contrasted to other commercial FHR monitors.

Scalp reconstruction with free flaps using the external carotid artery and internal jugular vein as the recipient vessels without the need for interpositional vein grafts or arterio-venous loops.

The technique of lengthening of the pedicles of the rectus abdominus and latissimus dorsi free flaps are described in five patients. Lengthening of these pedicles allowed scalp reconstruction using the external carotid artery and internal jugular vein as the recipient vessels without the need for interpositional vein grafts or arterio-venous loops. Post-operative recovery was uneventful in all cases.

Letrozole for brain and scalp metastases from breast cancer--a case report.

Brain metastases from breast cancer have a poor prognosis. There have been reports of patients with breast cancer and brain metastases responding well to tamoxifen therapy. We report a very unusual case of intact breast carcinoma with brain as well as scalp metastasis responding well to letrozole (aromatase inhibitor) therapy for a prolonged period of time.

A system for automatic artifact removal in ictal scalp EEG based on independent component analysis and Bayesian classification.

OBJECTIVE: To devise an automated system to remove artifacts from ictal scalp EEG, using independent component analysis (ICA). METHODS: A Bayesian classifier was used to determine the probability that 2s epochs of seizure segments decomposed by ICA represented EEG activity, as opposed to artifact. The classifier was trained using numerous statistical, spectral, and spatial features. The system's performance was then assessed using separate validation data. RESULTS: The classifier identified epochs representing EEG activity in the validation dataset with a sensitivity of 82.4% and a specificity of 83.3%. An ICA component was considered to represent EEG activity if the sum of the probabilities that its epochs represented EEG exceeded a threshold predetermined using the training data. Otherwise, the component represented artifact. Using this threshold on the validation set, the identification of EEG components was performed with a sensitivity of 87.6% and a specificity of 70.2%. Most misclassified components were a mixture of EEG and artifactual activity. CONCLUSIONS: The automated system successfully rejected a good proportion of artifactual components extracted by ICA, while preserving almost all EEG components. The misclassification rate was comparable to the variability observed in human classification. SIGNIFICANCE: Current ICA methods of artifact removal require a tedious visual classification of the components. The proposed system automates this process and removes simultaneously multiple types of artifacts.

Seizures and adverse events during routine scalp electroencephalography: a clinical and EEG analysis of 1000 records.

OBJECTIVE: To quantify the incidence of seizures and adverse events during standard electroencephalography (EEG). METHODS: A retrospective random sample of 1000 of a total of 3391 reports of standard scalp EEG recordings during 2002 at Kings College Hospital were studied, and adverse events during standard EEG were recorded. Photic induced seizures and epileptiform activity were compared with the resting, hyperventilation and sleep EEG. RESULTS: Adverse events occurred in 131 records (13.1%), including seizures in 60 records (45 electro-clinical and 15 non-epileptic seizures). The overall incidence of electro-clinical seizures was not statistically different during the resting EEG (2.8%), sleep EEG (2%), hyperventilation EEG (2.1%) and during photic stimulation EEG (1.4%). There was a higher frequency of electro-clinical seizures during hyperventilation and sleep in those with a diagnosis of idiopathic generalised epilepsy (31.5%) and during photic stimulation in photosensitive patients (31%). The incidence of electro-clinical seizures was significantly less during activation procedures in focal epilepsies (2.6%). Activation techniques made a unique diagnostic contribution when routine resting EEG was normal or equivocal in 11% of cases. CONCLUSIONS: Adverse events occurred in 13.1% of records, and most were minor. Sixty of the adverse events were seizures. Those generated during the EEG were brief and safety precautions operated successfully. In those without a prior diagnosis, the chance of seizures is the same during both resting and activation EEG. In those patients with generalised epilepsy or photosensitivity, activation procedures have a higher rate of seizure induction. SIGNIFICANCE: This study has implications for informed consent for EEG.

The persistence of human scalp hair on clothing fabrics.

This study reports the persistence behaviour of human scalp hairs under a number of different circumstances. The effects of artificial dyeing of hairs, the presence or absence of roots and different types of fabrics on the persistence of hair on a variety of garments were investigated. The garments were made from cotton, polycotton, cotton/acrylic, polyester and wool. The results indicated that neither artificial dyes nor the presence or absence of roots had statistically significant effects on the persistence of hair. In contrast, the type of fabric had a major impact and it was found that, generally, hairs persist longer on rougher fabrics. The rate of loss of hairs from non-woollen fabrics during normal wear was found to follow an exponential decay curve. In contrast, the rate of loss from the woollen garments was quite linear, indicating a constant, even loss, and thus suggests that a different process is involved in the persistence of hairs on woollen garments from that on non-woollen garments. The speed at which hair was lost from fabrics decreased in the order polyester, cotton/acrylic, polycotton, cotton, smooth wool, rough wool, so that wool gives the best chance of recovering samples of hair. Due to the uniqueness of each case, it is advised that caution be used when making any interpretations and before drawing any conclusions.

Scalp recordings of mid-latency AEP and auditory gating in the Göttingen minipig: a new animal model in information processing research.

Early central information processing, measured in humans by the gating of the middle latency auditory evoked potential (AEP) P50 and the effect of attention on AEP N100, is affected in schizophrenia. Exploring the possibility of using miniature pigs in longitudinal studies of chronic neuropsychiatric disorders, we present a method for recording seven channel surface middle latency AEP in the awake animal. The AEP and the AEP gating measured in a paired stimulus paradigm similar to the P50 gating paradigm used in humans, were recorded in six adult male minipigs in two sessions. The AEP had a stable N40/P60/N120/P200 configuration and in the gating paradigm the difference between stimulus one (S1) and two (S2) P60 and N120 amplitudes were significant. Mean AEP P60 gating ratio (S2/S1) at the posterior central electrode was 0.66 (std 0.29) range 0.21-1.08 and corresponding N120 was 0.60 (std 0.19) range 0.28-0.76. The method presented is feasible for scalp recordings of middle latency evoked potentials in the awake animal, but further studies of interval sensitivity and the effect of arousal manipulation are needed to assess the equivalence of the pig components to those of the human at similar latencies.

Complete remission of Merkel cell carcinoma of the scalp with local and regional metastases after topical treatment with dinitrochlorbenzol.

Merkel cell carcinoma (MCC) is a highly aggressive tumor with a high percentage of recurrence, metastatic spread, and mortality. Treatment of metastasized MCC is not standardized and prognosis of metastasized MCC is often poor. Current protocols recommend surgery, adjuvant radiation therapy, and often lymph node dissection to prevent recurrences. A few sporadic reports of spontaneous regression of MCC suggest a so far not yet characterized role and potential of the immune system in controlling this tumor. We describe a 69-year-old man with extended inoperable MCC of the scalp including multiple local and regional metastases who responded with complete remission to 4 weekly treatments of topically applied immune-modulating dinitrochlorbenzol. Together with subsequent irradiation, remission has now lasted for more than 1 year.

Primary cutaneous paraganglioma of the scalp.

Paragangliomas are neuroendocrine tumors that arise in relation to the sympathetic or parasympathetic ganglia of the autonomic nervous system. Rarely, they are reported in unusual locations such as the urinary bladder, prostate, and uterus. Herein, we report a case of primary cutaneous paraganglioma of the scalp in a child. The tumor was studied histologically, immunohistochemically, and ultrastructurally. This case might represent an aberrant migration of neural crest cells in their descent toward the branchial arches. In addition, it highlights the complex interaction of the neural crest and branchial arches/clefts during embryogenesis.
