Brain heterotopias are rare congenital malformations embryologically related to encephaloceles. They present as a mass in or about the nose (nasal glioma) or in the nasopharynx. We present the clinical and pathological features of 5 cases of heterotopic brain tissue. Four nasal gliomas consisted of mature neuroglial tissue, including neurons in 2 cases, embedded in a fibrovascular stroma. A nasopharyngeal brain heterotopia showed histologic features of mature neuroglial tissue including neurons and ependymal-lined cystic structures. The finding of mature neuroglial tissue in a mass from the head and neck region raises three differential diagnostic possibilities: teratoma, encephalocele, or heterotopic tissue. A teratoma can be ruled out by examination of the entire specimen. Encephaloceles and brain heterotopias can be distinguished only after correlation with the patient's clinical and radiologic findings.

A sufficient body of knowledge has developed about ductal carcinoma in situ of the breast to warrant a separate report on standards of care for women with this disease. This consensus report by these four organizations discusses evaluation of the patient, selection of treatment, technical aspects of diagnostic biopsy and definitive local excision, pathologic evaluation, radiation therapy considerations, follow-up care recommendations, and questions for future research.
Because knowledge has advanced in several fields related to the treatment of early breast cancer, revising the landmark 1992 standards for breast-conservation treatment by these four organizations is appropriate. The current report reviews and summarizes the literature and describes the selection and evaluation of patients, the technical aspects of surgical treatment and irradiation, follow-up care, and areas for further research.

There are currently four principal glycohaemoglobin assay techniques (ion-exchange chromatography, electrophoresis, affinity chromatography and immunoassay) and about 20 different methods that measure different glycated products and report different units. Standardisation will lead to all assays reporting results in a standard unit, the HbA1c percentage of total serum haemoglobin, and should be in place within the next one to three years. In the interim, clinicians using glycohaemoglobin assays should be aware that the ranges indicating good and poor glycaemic control can vary markedly between different assays. The reproducibility of some assays may be insufficient to provide definitive evidence of changes in glycaemic control. Some assays may be so imprecise that they are unable to separate patients with good and poor control. INTERIM RECOMMENDATIONS 1 The terminology to be used for the assay is glycohaemoglobin (GHb) assay (recommendation from the combined meetings of the International Federation of Clinical Chemistry [IFCC] Working Group on HbA1c standardisation and the American Association of Clinical Chemistry [AACC] Subcommittee on Glycohemoglobin). 2 The unit of measurement for GHb assays should be reported as %HbA1c (Diabetes Control and Complications Trial equivalent). 3 Other units, such as % total GHb or %HbA1, should not be used. Assays producing these units should be converted to %HbA1c reporting units. 4 Assays with high precision are highly desirable. The IFCC/AACC are currently recommending between-run coefficients of variation of less than 5% for manufacturers of kits and instruments. However, between-run coefficients of variation of less than 3% are far more clinically useful and therefore desirable.

This issue of the ARCHIVES includes previously unpublished protocols for the examination of specimens removed from patients with cancer of diverse sites. We provide a historical context for the development of these protocols and describe the process of development and approval. General information about the structure and content of the protocols is also provided. Cancer protocol development is an important step in the process of standardized cancer reporting. The value of such standardized reporting is discussed.
