[Incidence and significance of the gastric mucosal constellation in pernicious anemia].

1977 cases with gastric complains but without localized findings like ulcer or cancer were investigated by gastroscopy with biopsies of corpus and antrum ventriculi. We found in 5.5% an isolated mucosal atrophy of the gastric corpus (mean age 67.2 years compared to 59.4 years in cases with normal mucosa). 0.7% showed an atrophic gastritis of the antral mucosa, 0.9% an atrophic gastritis of the body and the antrum. Most of the cases with an isolated atrophy of the gastric body gave findings of hematologic disturbances. In all cases with isolated atrophy of the gastric body, including localized findings in the stomach, we found in 10.8% neoplastic processes, in another 3.9% hyperplasiogenic polyps. It seems to us of clinical importance to evaluate the gastric histology of the antral and body mucosa routinely because of the high incidence of pernicious anemia-like gastric atrophy and its relation to gastric neoplasias.

Mixed hyperplastic and adenomatous polyp arising from ectopic gastric mucosa of the duodenum.

We report an unusual case of a polyp arising from ectopic gastric mucosa in the duodenum. The polyp demonstrated both hyperplastic and adenomatous proliferation. Adenomatous proliferation in a duodenal polyp arising from ectopic gastric mucosa is, to our knowledge, unique. The areas of adenomatous change were morphologically indistinguishable from those of adenomatous polyps arising in the stomach; hence, the adenomatous change observed in this polyp was regarded as a neoplastic process.

Organ and tumor specificity of colon mucoprotein antigen in a rat model.

We purified a high-molecular-weight colon mucoprotein antigen (CMA) from normal F344 rat colon and from a transplantable dimethylhydrazine-induced colon carcinoma. Chemical analysis of the mucins showed similar amino acid and carbohydrate compositions. This finding was in contrast to the major differences that occurred in the composition of human CMA as a consequence of neoplasia. Immunohistochemical techniques were used in an examination of organ and tumor specificities. A New Zealand White rabbit antiserum against purified normal rat CMA, appropriately absorbed, detected a normal colon-specific determinant(s). The organ-specific determinant was lost as a consequence of neoplasia. However, a new tumor-specific determinant was then detected. CMA may have a potential role as an organ-specific marker of the neoplastic process.

Analysis of paraprotein transport into the saliva by using anti-idiotype antibodies.

To determine the extent of clonal involvement of the secretory immune system and the origin of salivary immunoglobulins (Ig) in monoclonal gammopathy patients, saliva and serum samples were collected from five affected individuals (two IgA myelomas, one IgG myeloma, one IgG benign monoclonal gammopathy, and one IgM lymphoma) and were assayed for the presence of monoclonal Ig. Purified polyclonal or monoclonal anti-idiotype (Id) antibodies were prepared against each of the isolated serum paraproteins. In all five individuals, the patient saliva samples inhibited the binding of 125I-labeled homologous Ig to the corresponding anti-Id antibodies, but normal saliva did not. The concentration of Id in patients' saliva varied from 1 to 400 micrograms/ml; i.e., 0.004 to 1.0% of the corresponding serum values. Saliva of a lymphoma patient whose IgM kappa protein exhibited rheumatoid factor (RF) activity also contained RF. The salivary Id-bearing molecules were found to have the same Ig isotype as the serum paraproteins. The myeloma IgA represented a minor component (0.4 and 3.9%) of the total salivary IgA. The salivary IgA myeloma proteins were associated at least in part with secretory component, but the salivary IgG paraproteins were not. In an IgA myeloma patient, a minority (17%) of the IgA+ plasma cells found in the lacrymal gland biopsy specimen were Id+, whereas the great majority (98%) of bone marrow IgA plasma cells were Id+. The results suggest active transport rather than passive transudation of myeloma IgA into the patients' saliva, and the integrity of the secretory immune system was not compromised by the neoplastic process.

[Biological markers in cancer of the ovary].

The organism produces substances whose presence or amount indicate the existence of a neoplastic process, called biological tumor markers. They can be used in the clinical laboratory for diagnostic purposes, and also to orient about the evolution of the tumoral mass. There are about 90 tumor markers, 33 of them have studied in relation to cancer of the ovary, that are described in this work. Eight of them are the most important: CSAp, OCA, OCAA, IAP, Nagao's isoenzyme, hCG/hCG beta, hPL, and AFP.

[Current approach to the treatment of early forms of cervix cancer].

The effectiveness of treatment of early cancer of the uterine cervix (TIS, Tla and Tlb) is subject to evaluation of tumor stage, concomitant diseases and patient's age as well as the degree of neoplastic process and immunologic vigor. Complex treatment should be unoffensive for immunity. Immunodepressive agents should be used in conjunction with immunostimulants.

[Langerhans cells--their structure, function and pathological role].

Data on the structure and function of Langerhans' cells (LC) are presented. LC are defined as dendritic cells of the epidermis and mucous membrane epithelium having the mesenchymal origin and relating to the antigen-representing macrophages. Morphological, enzymatic and antigenic LC markers are characterized. The crucial role of LC in the development of the skin delayed type hypersensitivity is shown: contact hypersensitivity, specific inflammation, transplant rejection, neoplastic process.

A protein cross-reacting immunohistochemically with rat glutathione S-transferase placental form as a marker for preneoplasia in Syrian hamster pancreatic and hepatocarcinogenesis.

Immunohistochemical staining using antirat glutathione S-transferase placental form (GST-P) rabbit antibody demonstrated marked binding to cells of putative preneoplastic lesions induced in both the liver and pancreas of Syrian hamsters by dihydroxy-di-n-propylnitrosamine treatment, whereas background normal-appearing tissue was negative. Thus, a protein showing immunological cross-reaction with rat GST-P is also elevated during pancreatic carcinogenesis and hepatocarcinogenesis in the hamster, suggesting its usefulness as a marker with potential relevance to the mechanisms underlying the neoplastic process.

Peripheral blood lymphocyte nucleoi of rats with Yoshida ascitic sarcoma and the effect of endotoxin.

Nucleoli of lymphocytes were studied in the peripheral blood of male Wistar rats bearing Yoshida ascitic tumor untreated and treated with Escherichia coli endotoxin. Both the neoplastic process and the endotoxin produced an increase of peripheral lymphocytes with "active" nucleoli in number. The treatment of experimental animals bearing the tumor with the endotoxin decreased their mortality and enhanced the increase of the lymphocytes with "active" nucleoli in their peripheral blood.

Biologic considerations regarding the one and two step procedures in the management of patients with invasive carcinoma of the breast.

Investigation of the biologic significance of delay between biopsy and mastectomy was performed upon women with invasive carcinoma of the breast in protocol four of the NSABP. Since the period of delay was two weeks or less in approximately 75 per cent, no comment concerning the possible effects of longer periods can be made. Life table analyses failed to reveal any difference in ten year survival rates between patients undergoing radical mastectomy management by the one and two step procedures. Similarly, no difference in adjusted ten year survival rate was observed between women managed by the two step procedure who did or did not have residual tumor identified in the mastectomy specimen after the first step or biopsy. Importantly, the clinical or pathologic stages, sizes of tumor or histologic grades were similar in women managed by the one and two step procedures minimizing selection bias. The material used also allowed for study of the possible causative role of biopsy of the breast on the development of sinus histiocytosis in regional axillary lymph nodes. No difference in degree or types of this nodal reaction could be discerned in the lymph nodes of the mastectomy specimens obtained from patients who had undergone the one and two step procedures. This finding indicates that nodal sinus histiocytosis is indeed related to the neoplastic process, albeit in an undefined manner, rather than the trauma of biopsy per se as has been suggested. These results do not invalidate the use of the one step procedure in the management of patients with carcinoma of the breast. Indeed, it is highly likely that it will be commonly used now that breast-conserving operations appear to represent a viable alternative modality for the primary surgical treatment of carcinoma of the breast. Yet, it is apparent that the one step procedure will be performed for technical and practical rather than biologic reasons.

[Functional activity of the lymphocytes of mouse lymph nodes in a system of viral carcinogenesis].

A study was made of the PHA-induced lymphocyte blastransformation (BT), and lymphocyte antitumor activity, using a model of progressive and regressive neoplastic process induced by the Moloney virus in mice BALB/c. The antitumor acitvity of lymphocytes was estimated by their ability to inhibit spheroid production by tumor cells. A lymphocyte BT and their antitumor activity was registered during a progressive growth of the tumor and its restoration as the tumor is regressing.

[Comparative ultrastructural study of the epithelial stromal junction in the prostatic gland].

In this paper, the authors carry out a comparative study of the structures adjacent to the alveolar periphery of the prostate-glands of men, dogs and bulls. In the three cases an E.S.J. (Epithelial-Stromal-Junction) similar to that described in the mammary gland and characterized in the first two cases by the existence of four, ultrastruct strata, consisting of the basal lamina, the somas of the adjacent fibroblasts and their cytoplasmic expansions as well as the arrangement of the intercellular, collagen microfibrils, was found to exist in this gland. In the case of the bovine prostate gland, the structural organization of the E.S.J. was basically characterized by the structural arrangement of the collagen microfibrils. The structures described constitute models on which to base future research on the hyperplastic and neoplastic processes of the prostate gland.

[Progressive multifocal encephalitis (PML)].

In four cases of progressive multifocal leucoencephalitis the basic disease was always a malignant lymphogranuloma. Changes in the brain were multifocal, however the frontoparietal area of the white matter was affected most frequently and foci in this part were oldest. In these foci demyelination with atypical astrogliosis predominated, and there was a very small number of scavenger cells. At the periphery of foci always proliferation and enlargement of homogenous oligodendroglia nuclei were found sometimes with inclusions which in immunoassay sometimes had a common antigen with papovirus SV 40 and on electronoptic examination always contained many virions of the papovirus. No direct relationship was found between the duration of the neoplastic process, its type and changes in the CNS.

[Complications of surgery of cancer of the colon].

In the paper a systematic analysis is carried out of postoperative complications of colon surgery, 118 patients had complications, out of a total of 400 interventions on the colon. The complications are studied in relation with the time of their occurence and the localization of the neoplastic process, as well as from the point of view of the influence exerted by age and the "background" of the patients. A review is also made of the therapeutic attitudes required by the various complications, of the results obtained and finally a detailed analysis is presented of the causes that led to death of operated patients.

Serological and epidemiological considerations of the role of herpes simplex virus type 2 in cervical cancer.

To assess the possible biological significance of the observations that women with cervical cancer tend to be younger at first intercourse than control women, data from 1823 women were analyzed for the relationship between age at 1st intercourse and number of sex partners. Women who were younger at first intercourse had more sex partners than did women who were older at first intercourse. The interdependence of age at first intercourse and number of sex partners does not exlude the possibility that intercourse at an early age represents a biologically significant event in which the neoplastic process is initiated. However, it is equally possible that younger women at first intercourse may have multiple sex partners and be at greater risk of coming in contact with a putative oncogenic agent later in life. In addition, sera from patients with herpesvirus infections were assayed for cross-reacting and type-specific antibodies. Approximately 80% of the total antibody activity was to the cross-reacting antigen and only 20% was to the type-specific antigens in the sera of patients infected with either type 1 or type 2 virus. Among patients infected with both types of virus, less antibody activity to the type-specific antigens and more antibody activity to the cross-reacting antigens were found. These observations are discussed with respect to case-control seroepidemiological studies.

Cutaneous tuberculosis and malignancy.

This is a report of three cases of associated tuberculous and neoplastic processes. The first is a case of lymphosarcoma complicated by cutaneous tuberculosis. The remaining two are cases of cutaneous tuberculosis complicated by squamouse cell carcinoma. Due to clinical similarity, the diagnosis of the secondary process in these cases was evoked by its failure to respond to treatment of the primary disease.

Atypical vascular proliferation in benign soft tissue tumors.

Atypical endothelial proliferation and continuing growth of small vessels were found to be the main features of two benign soft tissue tumors. Lack of history of injury and insidious onset suggested neoplasia, but the morphology was that of a reactive rather than neoplastic process. Ultrastructural study of the relationship of the endothelial cells to one another and to perithelial cells was helpful in making this distinction. The process appeared to have begun in an organizing thrombus or hematoma, in one case in relation to an occluded artery, and in the other in an abnormally vascular area of skeletal muscle. It is known that endothelial hyperplasia can occur in reparative reaction of this type. The prominent muscular development of small vessels in the cases reported here suggests that abnormal flow or intraluminal pressure could have been a stimulant to atypical growth.

[Renal carcinoma with argyrophil cells].

A case of an unusual type of renal carcinoma is reported. The tumor had a poorly differentiated appearance with focal endocrine differentiation. Scattered argyrophilic cells were revealed by the Grimelius procedure. The relationship of this tumor to other neoplastic processes displaying endocrine differentiation is considered. The present case must be segregated from the classic carcinoid group. It belongs to the heterogeneous class of carcinoma with argyrophil cells and bears some resemblance to the atypical carcinoid.
