[Quantitative analysis of recurrence and spontaneous regression of basalioma parts left in situ].

To some extent, parts of basalomas found remaining in situ following tumour excision tend to spontaneous regression. This is a well-known phenomenon and has significance for the recurrence of incompletely excised tumors. The present study involved a quantitative investigation of the relationship between recurrence and spontaneous regression. Following precisely defined excision of basalomas, the entire exterior of the excised material was examined by contrast microscopy in HE-stained paraffin sections (3-dimensional histology). Whenever tumour outgrowths were found, it was possible to document exactly their type, localization, extent, and depth of invasion. In 66 such cases no follow-up operation was performed, but only a follow-up examination after a minimum of 31 and a maximum of 113 months (average: 60 months). Only 50% of these undisturbed tumour outgrowths resulted in a recurrence during the follow-up period. A very high rate of spontaneous regression (71%) was found among the solid tumour outgrowths, but a significantly lower rate (19%) among the fibrosing tumours. Moreover, regression was dependent on the tumour remnant's mass and the clinical diameter of the tumour removed. It was independent of the depth of infiltration. Although the rate of spontaneous regression of tumour outgrowths persisting after therapy is relatively high, it cannot be predicted in individual cases. It is not possible to be certain that tumour removal has been achieved unless micrographic surgery has been continued until complete absence of tumour is proved. In all procedures that are not subsequently monitored, an unacceptably high rate of recurrence must be expected, especially in the case of fibrosing basaloma. This is commented on at length.

[The prognosis of the outcome of the inflammatory process in chronic bronchitis].

The authors relate the data on the leading part fibrosoformation plays in the disease outcome in patients with certain pulmonary chronic illnesses. The levels of free and bound hydroxyproline and hyaluronidase activity served as diagnostic criteria for processes of fibrosoformation. Based on the data obtained a method of predicting chronic bronchitis (CB) was devised. Thus, the two-fold rise of hyaluronidase activity as compared to normal and the index of fibrosing (the ratio of free to bound hydroxyproline) exceeding 1 suggest an unfavourable course of CB accompanied by the enhancement of fibrosoformation. Meanwhile if hyaluronidase activity increases 2-fold and more and the index of fibrosing is less than 1, the prognosis of inflammatory process may be favourable. Based on the clinical data, the significance of prognosis was proved in 93% of cases.

[Role of endothelial dysfunction and coagulation disorders in the development of pulmonary fibrosis in patients with interstitial lung diseases].

Thirty-two patients with different forms of interstitial lung diseases (ILD), such as idiopathic fibrosing alveolitis (IFA) (n = 17) and fibrosing alveolitis concurrent with diffuse connective tissue diseases (FA-DCTD), were examined. Clinical, echocardiographic, computed tomographic, coagulative, and immunological studies were performed. Enzyme immunoassay was used to determine the levels of a complex of thrombin and antithrombin III (TAT) and platelet factor IV (PF-IV). There were significant increases in the levels of PF-IV (4.36 +/- 0.25 mg/l) and TAT (10.87 +/- 3.8 mg/l) in patients with ILD as compared to the control (2.75 +/- 0.47 and 1.8 +/- 0.2 mg/l, respectively; p < 0.05). In patients with early FA-DCTD with the predominance of the milk glass syndrome during high-resolution CT (HRCT), the level of PF-IV was greater than the normal levels (p < 0.05) and decreased with the progression of the disease and with the formation of the honeycomb lung. If there were HRCT signs of active inflammation, the level of TAT was higher than that in the control; this was also in the development of irreversible fibrous changes.

Progression of liver fibrosis in blood donors infected with hepatitis C virus.

BACKGROUND/AIMS: Chronic hepatitis by HCV is progressive towards cirrhosis, with variable rate. We evaluated the rate of fibrosis progression (RFP), risk factors associated with advanced fibrosis (F3 and F4), and estimated the evolution time to cirrhosis. METHODS: We transversely selected 142 blood donors infected only with HCV, with a known route of infection, submitted to liver biopsy at admission. RFP= ratio between stage of fibrosis (METAVIR)/estimated duration of infection in years. Non-parametric tests and logistic regression analysis, with significance level of 5% were used. RESULTS: Median RFP was 0.086 U/year (0.05-0.142). Ten patients had F4 and 25 had F3. Median RFP values were significantly different (p=0.001) from one age group at contamination to the others and ALT and AST levels. There were no differences in the expected evolution to cirrhosis between intermediate fibrosers (F2) and the rapid fibrosers (F3 and F4). The independent variables associated with advanced fibrosis were ALT (OR 7.2) and GGT (OR 6.4) and age at inclusion (OR 1.12). CONCLUSION: This study suggests that RFP is extremely variable, it is exponential with age, and mainly influenced by host characteristics, especially age at contamination and possibly ethnical group. These asymptomatic patients had high percentage of fibrosis F2, F3 and F4.

[The extracellular matrix of peripheral lung cancer in the scar and of pneumosclerosis of different origins (the immunohistochemistry and electron microscopy of the collagens)].

Collagen types and ultrastructural features of the stroma and scar extracellular matrix in the peripheral lung carcinoma, post-tuberculosis and post-pneumonia pneumosclerosis foci, fibrosing alveolitis interstitium were studied on the material of operational and transbronchial lung biopsies. It is established that by the collagen composition the scars in the peripheral carcinoma are identical to the pneumosclerosis foci and are distinguished from the carcinoma stroma by a higher concentration of type IV and V collagens (p less than 0.05). Accumulation of type III collagen in the lung carcinoma stroma reflects anaplasia of the tumour as the domination of type III collagen is characteristic of the embryonal tissue. The decrease of collagen type IV in the cancer stroma correlates with an increase of its malignancy. Pneumosclerosis in the fibrosing alveolitis is distinct from the focal forms of pneumosclerosis by a higher content of collagen I and a lower content of collagen V this being probably due to the character of sclerosis morphogenesis in this disease.

Rapid progression of hepatitis C-induced liver failure in renal allograft recipients.

Early mortality due to hepatitis C virus (HCV)-related liver failure in renal allograft recipients in the absence of fibrosing cholestatic hepatitis is reported infrequently. We report six renal allograft recipients with HCV infection who died of rapid progression to liver failure. Of these, 2 were detected anti-HCV positive at screening prior to kidney transplantation and 4 were diagnosed after transplantation following derangement of liver function (HCV RNA positive in all 4, anti-HCV positive in 2). Median interval between kidney transplantation and derangement of liver function was 11.8 months (range 2 to 25) and median interval between transplant and death was 27 months (range 11 to 53). Liver biopsy performed during the terminal illness in 3 patients and post-mortem liver histology in 2 patients showed chronic hepatitis with mean grade of 10.2 (range 9 to 12) and stage 2.4 (range 2 to 3). None had features of fibrosing cholestatic hepatitis.

Clinical characteristics of patients with fibromyalgia.

Fibromyalgia, also known as fibrositis and muscle rheumatism, is a common, noninflammatory, painful musculoskeletal disorder. It is common between the ages of 30 and 60 years and has a female to male ratio of 5 to 1. Essential symptoms of fibrositis are pain, fatigue, disturbed sleep, morning stiffness and local tenderness. Subjective swelling, paresthesia and numbness sometimes occur. Multiple host and environmental factors seem to contribute to the onset and course of fibromyalgia. Modest improvement follows treatment by antidepressive agents, physical measures and reduction in stress. In this study 60 patients with fibromyalgia were investigated and the clinical characteristics of these patients are described and compared with those in other studies.

The genetic basis of strain-dependent differences in the early phase of radiation injury in mouse lung.

Substantial differences between mouse strains have been reported in the lesions present in the lung during the early phase of radiation injury. Some strains show only classical pneumonitis, while other strains develop substantial fibrosis and hyaline membranes which contribute appreciably to respiratory insufficiency, in addition to pneumonitis. Other strains are intermediate between these extremes. These differences correlate with intrinsic differences in activities of lung plasminogen activator and angiotensin converting enzyme. The genetic basis of these differences was assessed by examining histologically the early reaction in lungs of seven murine hybrids available commercially after whole-thorax irradiation. Crosses between fibrosing and nonfibrosing parents were uniformly nonfibrosing, and crosses between fibrosing and intermediate parents were uniformly intermediate. No evidence of sex linkage was seen. Thus the phenotype in which fibrosis is found is controlled by autosomal recessive determinants. Strains prone to radiation-induced pulmonary fibrosis and hyaline membranes exhibited intrinsically lower activities of lung plasminogen activator and angiotensin converting enzyme than either the nonfibrosing strains or the nonfibrosing hybrid crosses. The median time of death of the hybrids was genetically determined primarily by the longest-lived parent regardless of the types of lesions expressed.

[Superior vena cava syndrome and Riedel's thyroiditis. Report of a case: review of the literature].

The authors present the case of a 42-year-old female patient, initially admitted to hospital for the etiological diagnosis--which remained negative--and the treatment of a superior vena cava syndrome. This patient was seen again two years later as the superior vena cava syndrome recurred, while a thyroid syndrome appeared at the same time. Exeresis followed by a clinicopathological study demonstrated Riedel's chronic fibrous thyroiditis. The authors study the causal relationships between both conditions and their integration into multifocal fibroses. To their knowledge, this is the first case in which the superior vena caval syndrome is the site of occurrence of fibrosis, precedes the thyroid involvement. The new radiological technique at our disposal--CT, MRI--should allow more easily searching for the various sites of the fibrosing disease (biliary ducts, pancreas, duodenum, retroperitoneum, mediastinum, orbits), and immunological studies will make it possible to better classify them and define their pathogenesis.

[The occupational spectrum in alveolitis and pulmonary fibrosis].

The professional spectrum was analysed in 898 patients with ascertained alveolitides and lung fibroses. Most frequent were fibroses in occupations from agriculture and forestry (167 = 18.6%). Then followed workers in metal (119 = 13.3%), commerce and supply (69 = 7.7%), transport (46 = 5.1%), economic management (46 = 5.1%), building trade (42 = 4.7%) and textile workers (34 = 3.8%). The proportion of the other professional branches was lower than 3.5%. Close connections between disease and profession were seen in the employees in agriculture. The group is formed above all by farmer's lungs (exposure against hay, straw, pellets, corn, fruit, vegetables and so on) and bird-fanciers' lungs. In the other professional groups the rate of the etiologically clarified cases is essentially smaller. Particularly the metal branches are a centre for further investigations.

[Therapeutic possibilities in alveolitis and pulmonary fibrosis].

Aetiologically, less than 50% of the alveolitides and pulmonary fibroses are clarified. A successful causal therapy by elimination of the noxae is thus possible only in several acute cases, particularly in forms of the exogenic allergic alveolitis. Usually the clinical pictures are misunderstood for a long time and are diagnosed only in advanced stages. The therapeutically influencible inflammatory changes are in such a case only slightly expressed. The more or less advanced fibrosis is not or only slightly to be influenced. Then the last way out of the difficulty of the lung and the heart-lung transplantation, respectively, the size of which, however, is limited by the small number of suitable donors. In the initial phase of the diseases in toxic and allergic-immunologic the inflammations resulting from this in the pulmonary tissue are partly to be controlled by antiinflammatorily and immunosuppressively, respectively, acting medicaments such as corticosteroids, azathioprin and cyclophosphamide. For the treatment of the lung fibrosis only D-penicillamine is at our disposal. Because of the numerous, partly considerable side effects and due to the effectiveness of the necessary long-term therapy which is only difficult to be measured there are very different opinions about the treatment with penicillamine. The enlargement of the knowledge on the pathogenesis of alveolitides and lung fibroses allows to hope that new beginnings for a therapy are found. No final judgement can be made about the efficacy of cyclosporin A and of antioxidants.

[Methotrexate induced pulmonary disease].

Methotrexate may cause pulmonary side effects. Two cases of methotrexate-induced fibrosing alveolitis are reported. One of the patients died from perforated gastric ulcer, the other was successfully treated with systemic steroids. The pulmonary side effects of methotrexate are reviewed. The most common clinical manifestation is fibrosing alveolitis. Pulmonary fibrosis and acute, non-cardiogenic pulmonary oedema are rare.

[Interstitial lung fibrosis in rheumatoid arthritis--comparison of lung function and bronchoalveolar lavage].

In fibrosing alveolitides that are associated with rheumatoid arthritides, the BAL fluid reveals a characteristic cell pattern. These cellular changes occur at an earlier date than do the radiological signs of pulmonary fibrosis. Together with a measurement of the CO diffusion capacity, BAL is a suitable monitoring parameter for fibrosing alveolitides in rheumatoid arthritis.

[Bronchoalveolar lavage in cytostatic drug-induced lung changes].

Fibrosing alveolitides are associated with a characteristic cellular pattern in the BAL fluid. In contrast, acute interstitial fibrosing pulmonary changes in patients receiving cytostatic drug treatment cannot be detected in the BAL fluid, since the direct influence of cytostatic agents on the cellular constitution of the BAL fluid predominates.

[The effect of fibrin gluing and its important components on fibrosis of nerve anastomoses].

The fibrinous adhesion, although generally adopted and more and more successfully applied in many sectors of surgery, is not yet fully established in the anastomosis of separated nerves. Initially, the adhesion was expected to offer some essential advantages in this domain, especially in the prevention of suture granulomas. However, the blood clots dissolved too early, and this resulted in the formation of dehiscences. So antifibrinolytic substances had to be added to the adhesive, and this led to a frequent appearance of fibroses. The adhesion of nerves is still hampered by this fact. Now we have examined the fibrosis-inducing effect of different factors of the adhesive system. We found that aprotinin and the fibrin clot as an obstacle to supply had no additional fibrotic effect, whereas thrombin, factor XIII and fibronectin promoted the formation of fibroses.

[The margin of safety and depth of excision in surgical treatment of basalioma. Use of 3-dimensional histologic study of 2,016 tumors].

During the treatment of 2016 basal cell carcinomas (BCC), 1757 of which were primary tumours and 259, recurrences, every operation was followed by a check on radicality by means of histological evaluation of the margins of the excised tissue (three-dimensional histology). The average safe margin at first excision was 3.8 mm, and excision normally extended to the lower subcuticular border in depth. After first excisions, tumour tissue was found in 31.6% of histological sections prepared from the marginal sections at the circumference and/or on the underside of the excised material. Tumour material was far more frequent in the marginal area (28.3%) than on the underside (7%). With a 2-mm safe margin around the primary BCC there were still 46.7% tumour-positive marginal sections; with 4 mm, 20.3%; and with 6-8 mm, 14.7%. Fibrosing BCC and tumours with diameters over 20 mm, and recurrent BCC in particular had a significantly larger share of tumour-positive marginal sections and considerably more frequently required two or more reoperations until the final radical excision than did the solid and superficial types of BCC. An average safe margin of 4.5 mm plus standard deviation to give 7 mm (standard deviation 2.5 mm) was necessary for radical excision of primary BCC, but often even larger margins, up to a maximum of 3.2 mm were necessary. Hence, when surgical treatment of BCC does not include three-dimensional histological evaluation generous safe margins are necessary. Surgery with histological monitoring is the only justifiable method of treating tumours of the fibrosing type, recurrent BCC and BCC over 10 mm in diameter.(ABSTRACT TRUNCATED AT 250 WORDS)

Bronchoalveolar lavage fluid (BALF) analysis in interstitial lung diseases--a 7-year experience.

Bronchoalveolar lavage (BAL) was performed in 89 patients with diffuse interstitial lung disorders of varied aetiology and 19 normal control subjects over a period of 7 years. Alveolar macrophage was the predominant cell in BALF in normal control subjects. Increased neutrophils were found in BALF in patients with cryptogenic fibrosing alveolitis (CFA) and fibrosing alveolitis associated with collagen vascular diseases. BALF lymphocytosis was seen in patients with chronic hypersensitivity pneumonitis, sarcoidosis, miliary tuberculosis, silicosis and carcinomatosis. Diagnosis of alveolar microlithiasis was made in one patient with the help of BALF examination. One patient developed anaphylactic reaction to the topical xylocaine solution and there was no mortality with the procedure.

[Allergic alveolitis in agricultural workers, caused by thermophilic bacteria or fungi].

Between 1976-1986 fifty-seven patients with farmer's lung have been diagnosed in Hungary on the basis of data obtained from public institutes for tuberculosis. That are 0.08% of the 744,300 manual workers employed in agriculture and forestry. In the counties of Borsod and Szolnok the rate was 0.5% (referred to 62,900 and 36,000 individuals, respectively). Regional accumulation of the different provoking agent's and variation of disease prevalence seem to be in correlation with geographical, climatic, meteorological, economical and occupational factors, showing a declining tendency in recent years. Antibodies of the Thermoactinomyces vulgaris antigen could be detected most frequently. In the biopsy material of 17 patients obtained by Klassen biopsy were fibrosing and not fibrosing desquamative alveolitis, granulomas similar to sarcoidosis, bronchiolitis with peribronchial fibrosis observed. Mostly focal, rarely subpleural deficiencies were detected by lung-scintigraphy, their dimension in acute cases being greater than the extent of radiological lesions. On the basis of 5 Coombs positive cases authors consider conceivable that sometimes cytotoxic allergic reaction type II participates in the pathogenesis of farmer's lung.
