Treatment of perforations of the esophagus associated with stricture.

Most strictures associated with perforation of the esophagus are anatomically suitable for this repair. The merit of this procedure is threefold: first, a two layer closure of the perforation is obtained; second, the stricture is obliterated, and third, the underlying cause of the disease--reflux--is adequately addressed.

[Caustic burns of the esophagus in childhood].

Acute burns of the upper gastrointestinal tract require an immediate diagnostic and therapeutic approach. Early endoscopy has been shown to assist greatly in determining the further treatment of corrosive injury. The main task of radiology is to detect perforations. Early corticosteroid treatment seems to be effective in preventing stricture development in second degree burns.

Endoscopic clipping of perforation following pneumatic dilation of esophagojejunal anastomotic strictures.

Two patients with small perforations occurring after endoscopic balloon dilation of esophagojejunal anastomotic strictures were treated conservatively using metal clips. Closure of the perforation was achieved in both cases, using one and two clips in a single session. There was no procedure-related morbidity and no patient developed complications. Patients were discharged from hospital on days 4 and 5, respectively. In patients with esophagojejunal anastomotic strictures, endoscopic treatment of postdilation perforation by metal clips is safe and effective.

Routine chest radiography following endoscopic oesophageal dilatation for benign peptic oesophageal strictures.

Endoscopic oesophageal dilatation is a well-established procedure for management of patients with benign oesophageal strictures. The most important complication of the procedure is perforation with a recorded incidence of approximately 1%. Patients undergoing the procedure require close observation after dilatation. Routine chest radiographs are advisable following the procedure.
