Prevalence of diabetes, hypertension and renal disease amongst railway workers in Malaysia.

A survey was done to determine the prevalence of diabetes mellitus, hypertension and renal disease, as well as extent of diabetic control, amongst the workers of Malaysian Railways. The prevalence of diabetes was high at 6.6%, with 3.8% of these being insulin dependent diabetes. The highest prevalence was in Indians (16.0%) followed by Chinese (4.9%) and Malays (3.0%). Using HbA1 measurements, diabetic control was poor in 70.6% of the diabetics. Hypertension was found in 37% and proteinuria in 35%. Renal impairment was present in 30% of the diabetics. This survey shows that diabetes, hypertension and renal disease are high amongst the railway workers in Malaysia.

Preventive effect of aluminium citrate on silicosis.

Aluminium citrate (Al-cit) was shown to be biologically effective in preventing silicosis. A total of 917 cases selected from both engineering corps of railway and coalmines with stage 0 or 0+ by radiographs were studied. Cases were randomly divided into Al-cit treatment group and control group with continuing dust exposures. Workers in the treatment group were injected with Al-cit at a dose of 120 mg Al/year and these in the control group received a consolatant or nothing during the period of five years. The radiographic changes demonstrated that Al-cit showed clear-cut effects in both retardation of the dust-induced fibrosis and decrease of the morbidity of silicosis.

[Fatal accidents at train intersections in Denmark].

Material was collected concerning fatal level-crossing accidents in Denmark during the period 1.1.1980 to 31.12.1987. The epidemiological and traumatic circumstances are illustrated. A total of 51 fatal level-crossing accidents occurred which corresponds to 1% of all the annual traffic deaths in Denmark. The majority of the accidents occurred in good weather conditions and between 12 noon and 3.59 pm. Single-person accidents constituted 75%. The majority of the persons killed belonged to the local population and were familiar with the local conditions. The majority of the accidents were the local conditions. The majority of the accidents were due to poor visual conditions and lack of attention. In level-crossings with a keeper, the commonest causes were blinding by the sun and fog. The majority of the accidents occurred in level-crossings marked by light and sound signals. The risk for accidents in these crossings is six times as great as in crossings with half barriers. Private cars were most frequently involved and the persons killed in these died of head and cerebral injuries because the train buffers hit them at head height. Derailing of the trains was only seen on three occasions where the obstructing vehicles were heavy lorries. The fact that more derailings did not occur must be due to the low rate of the trains in the railway sidings where the majority of accidents occurred. A few proposals are made for prophylactic measures.

[Glomerular erythrocytes, albuminuria, glycosaminoglycans and exposure to hydrocarbons].

As part of a study on the possible nephrotoxicity of hydrocarbons, the urinary erythrocyte concentration and the percentage of glomerular erythrocytes were assessed. Albumin excretion rate, glycosaminoglycan and retinol-binding protein concentration, and N-acetyl-beta-glucosaminidase activity were measured simultaneously. The results show that the erythrocyte concentration and the percentage of glomerular erythrocytes tend to increase with hydrocarbon exposure, and that a nephrotoxic effect of occupational hydrocarbon exposure deserves consideration.

[An expert assessment of the organization of medical care in peacetime catastrophes].

There is given an expert evaluation of medical response and release efforts during the recent disasters in Arzamas, Sverdlovsk, Armenia and Bashkiria. Special attention is paid to the movable medical units activity, to the work of psychiatric, forensic medicine service. The attention is focused on the problem of establishing national disaster medical system.

[An intervention policy for reducing noise pollution in urban areas].

The methodological methods for a reduction of the noise pollution in the urban areas, mainly, can be advised in 8 operative points: 1) reduction of the source's noise; 2) control of the traffic; 3) planning of the urban and regional development; 4) building's shelter against the noise; 5) compensation's and incentivation's interventions for the exposed people; 6) controls activity and restaining of the noise's sources; 7) scientific research; 8) health education. For executive motivations we must, necessarily, divide the intervention turned to a reduction of the noise's emissions caused by fixed source compared to the noise's emissions determined by motor or railway traffic (mobile sources). This operative criteria have been utilized recently (1988) from Italian Department of Ecology for a bill. The controls of fixed source are made by Public Health Services of U.S.L. (Local Sanitary Unit) assisted, in case, by multiarea country's districts. The regulations of traffic can be took in consideration, in short time, only in more simple problems, as the setting up of pedestrian island or the suspensions of the heavy traffic, etc. The traffic's regulation must care same general data: the halved of traffic's flow in same rilevation's point, involve a reduction of only 3 dBA of the sound pressure level previously measured; if the percentage of heavy traffic is higher than 10% of the traffic's flow, the first intervention must be the no thorough fare to heavy lorries. In general, then, the best way for the resolution of vehicular noise pollution is the noisy emission's control of the single vehicle, because only the regulation of traffic's flow doesn't suffice to realise a enough reduction of noise pollution. For the resolution of more complex problems is necessary a preliminary stage to study and to plan those before to can carry out a typology of intervention. This necessity is, of course, more expensive in time and money; in fact, the solution of the problem can be the building of a efficient subway network completed by car parks in the terminal stations. In alternation at this draft, we can pointed out, also, same compromise resolutions like the appoint areas to limited traffic in the town or the break of continuity of the bus-line in the town. The compensation or the incentivation to make defence works against noise is a very expensive way and, moreover, they don't solve the problem for all exposed people in a future.(ABSTRACT TRUNCATED AT 400 WORDS)
