State of health of adolescents in eastern regions of Poland. Podlasie region child.

The aim of the study was an attempt to evaluate the state of health of adolescents aged 15 years, living in Podlasie region near the border of Poland and Bielorus. Data from the Schoolchild Development and Health Investigation Charts covering 197 rural and 208 urban adolescents were analyzed. The significance of differences between the two populations were tested by Chi 2 test at the level of p > 0.05. The study showed that nearly 60% of rural and 70% of urban schoolchildren have health problems. The most common of these problems are associated with the body posture. The data on self-reported health indicated that the most often observed complaints in both populations (rural and urban) of adolescents were: headaches, difficulties with falling asleep and lack of appetite. In spite of the high incidence of health problems, the number of schoolchildren being attended by medical specialists in outpatient departments is relatively small, to the disadvantage of the rural population. The laryngology and ophthalmology departments were visited most frequently. The analysis of the results of the studies confirmed that the state of health of the children examined is unsatisfactory. An effective prevention of these negative phenomena requires the creation of efficient systemic mechanisms on the one hand, and on the other, a constant monitoring of objective and self-reported health.

Serum lipid profile and metabolic syndrome occurrence among obese rural women from Lublin Region (Eastern Poland).

Obesity is a multivariate syndrome which can negatively affect whole body functioning. It is most common in highly developed countries, and in recent years a progressive increase in obesity occurrence is noticeable. The aim of the study was to assess serum lipid profile and metabolic syndrome occurrence among obese rural women from Lublin Region in Eastern Poland. The study was conducted in the Institute of Agricultural Medicine in Lublin (IAM). All subjects had a negative history of diseases and treatment that could affect serum lipid profile or glucose measurements. The inclusion criterion for the study group was overweight and obesity, defined as a body mass index above 25 (BMI > 25) and living in a rural area. 44 women participated in the study. There were no women fulfilling the criteria or who had a history of incorrect fasting glucose (IFG) or incorrect glucose tolerance (IGT). In contrast, the prevalence of arterial hypertension (or treatment) was high - 53%. 22.7% women had normal serum TC values. The proportion of those with hyper-LDL-C was 38.6% and with hyper-TG - 18.2%. 20.5% of studied women had incorrect serum HDL-C levels, and in 15.9% hypo- HDL-C was accompanied by high serum TC levels. Analysis of correlation showed that serum TC was positively correlated with both LDL- and HDL-C. 55% of the studied obese or overweight women had at least 2 additional components of the metabolic syndrome.

Allergy to pigeon tick (Argas reflexus) in Upper Silesia, Poland.

The pigeon tick Argas reflexus is avian ectoparasite that typically feeds on pigeons. When devoid of a natural host, the ticks can also attack humans. In Upper Silesia, southern Poland, people living in the vicinity of the birds' nesting sites are frequently bitten by A. reflexus. The bites can provoke serious allergic reactions, including fatal anaphylactic shock. In the present study, residents of 9 apartments invaded by pigeon ticks were invited to undergo medical examination, skin prick tests (SPT) and determination of specific IgE to A. reflexus. The test allergens were prepared of ticks collected on-site. Out of 18 residents living in the infested apartments, 15 accepted the invitation. In this group, 8 persons complained of tick-related health problems, positive SPT to A. reflexus were found in all of them, and specific IgE was detectable in 3 persons (range: 0.38-0.84 kUA/l; CAP class 1-2). Final clinical diagnoses were established of generalised urticaria with asthma in 1 person, generalised urticaria in another 1, and local allergic reactions to tick bites in the remaining 6. Among 7 symptom-free residents, all test results were negative. Besides the study group, the paper also describes the case of a person who developed hypersensitivity to A. reflexus as a child, after a few visits to an abandoned dovecote. 30 years later, positive SPT (++) and specific IgE (0.78 kUA/l; CAP class 2) were still present despite no further re-exposures. The article also discusses available pest control measures against A. reflexus.

[Prevalence of Eimeria and intestinal nematodes in wild boar in north-west Poland].

In the time from September 2003 to March 2004 feacal samples of 147 wild boars from north-west Poland were examined. The prevalence and intensity of the Coccidia and gastro-intestinal nematodes infection were determined by means of Willis-Schlaaf. The species were determined basing on morphology of oocysts (shape, colour, form index, presence or absence of micropyle and its cap, presence or absence of residual, polar and Stied bodies) and time of sporulation. Sporulation was performed in wet chamber at 24-26 degrees C in a 2.5% aqueous solution of potassium dichromate (K2Cr2O7) (Pellerdy, 1974). Oocysts from the genus Eimeria were found in 58.5% wild boars. Four coccidia species occurred in wild boars (E. deblecki, E. suis, E. perminuta, E. scabra). Gastro-intestinal nematodes were found in 83.67%. The following nematode species were found: Oesophagostomum dentatum, Metastrongylus sp., Trichuris suis and Ascaris suum.

Development of renal replacement therapy in Poland.

Different methods of renal replacement therapy (RRT) were introduced in Poland quite early: peritoneal dialysis (1953), hemodialysis (1958), renal transplantation (1966). Unfortunately due to the lack of resources in the health care budget, caused by an inefficient economic system, the further development of this therapy was very slow and not compatible with patients needs. The situation changed in the 1980s and 1990s when a National Board of Specialists in Nephrology chaired by A. Manitius and later by B. Rutkowski created a special Program for the Improvement and Development of Dialysis. Long-term efforts and pressure by a united nephrological community led to the establishment of a special fund in the central budget of the Ministry of Health. Final success was related to the political and economical changes in our country and region. Nowadays all three main RRT methods are available to all patients with end-stage renal disease and the actual incidence rate of ESRD is comparable with those of developed European countries. The Polish model of RRT development was also a good example for other Central and Eastern European countries and developing regions.

[Genetic diversity of Borrelia burgdorferi sensu lato in Ixodes ricinus ticks collected in north-west Poland].

Borrelia burgdorferi sensu lato (s. l.), the etiological agent of Lyme diesease, is transmitted by the bite of Ixodes ricinus. During May and September 1999, field surveys on Lyme disease spirochetes were conducted in three locations of a region of north-west Poland, known as recreational districts visited by many people. The ticks Ixodes ricinus were collected in natural habitats by dragging a flanel cloth over the vegetation. Sex and developmental stage of each tick were determined. Based on a polymerase chain reaction test with primers that recognize a chromosomal gene of all strains, out of the total 1414 specimens collected, 126 (8.9%) were found to be infected. The species B. burgdorferi s. l. comprises at least three pathogenic genomospecies, B. burgdorferi sensu stricto (s. s.), Borrelia garinii, and Borerelia afzelii, witch could be distinguished in nested-PCR tests with species-specific primers. B. burgdorferi s. s. was most prevalent (96% of infected ticks), followed by B. garinii (1.3%), and B. afzelii. was not found. Of the infected ticks, over the 99% were infected with a single species, one specimens was infected with two species. For 4 ticks, the infecting species could not be identied. The difference in rates of prevalence was observed among the tree locations (17%--5.3%--3.2%).

[The influence of different management systems on the infection level of some gastrointestinal parasites in sheep in southern Poland].

On the basis of the fecal egg counts, larval cultures and necropsies the infection of coccidia, tapeworms and gastrointestinal nematodes in dams and lambs under extensive and intensive management systems were estimated. During 1994 and 1995 total of 1740 coproscopic analyses were done. It was state, that in extensive farms the prevalence and EPG of gastrointestinal nematodes was higher in comparison with intensive management system (38.2-86% and 13.9-71.9% and 105-355 EPG and 33-131 EPG respectively). Similarly prevalence of tapeworms 0-18.2% and 1.3-8.3% respectively. Prevalence and intensity of coccidia was lower in extensive farms (20-78,5% and 83-482 OPG) comparing with intensive management system (38.2-88% and 139-2846 OPG). Predominant nematode species in both management systems were those from genes Haemonchus, Teladorsagia, Trichostrongylus, Cooperia and Nematodirus. One species--Nematodirus battus was found new for Poland.

[Helminth fauna of free living European bison Bison bonasus (L.) in Bieszczady Mountains (Karpatian Mountains, Poland)].

Four free living European bison from Białowieza/Caucasus line shot in February 1997 within Lutowiska Forestry District in the Bieszczady Mountains were used in investigations. All examined animals were infected with helminths. There were found 10 species of parasites. Five species of them Ostertagia leptospicularis, O. kolchida, Spiculopteragia boehmi, Cooperia pectinata and Ashworthius sidemi are thypical parasites of Cervides, and from them were infected bison in Bieszczady. In all necropsied animals was found invasion of nematode A. sidemi, with mean intensity 1542 specimens. It is the first registration of A. sidemi in Poland and European bison is a new host for this parasite. It seems that the source of infection A. sidemi in European bison in Bieszczady is the local population of deer, which during their migrations could bring this parasite from neighbouring Ukraine and Slovakia, where this nematode have been introduced together with deer Cervus nippon. There are also presented results of coproscopic examinations of 17 fecal samples from free living European bison in Bieszczady.
