Studies in the epidemiology of prostatic cancer: expanded sampling.
From comparisons of prostatic cancer patients with matched control patients for selected risk variables, patients are characterized by three main trends: (a) delayed sexual drive and development, (b) early repression of sexuality, and (c) premature cessation of sexuality. Excessive numbers of patients reported occupational exposure to fertilizers and auto exhaust fumes. Diets of the patients were higher in animal fats. No differences were found between both groups for frequencies of multiple marriages or sex partners, nor for stressful effects from selected events early or late in life. Trends for circumcision and other variables are presented. The data suggest that early differences are hormonally conditioned, support a provisional endogenous rationale for initiation of prostatic cancer, and oppose a hypothesis favoring transmissible oncogenic agents. If results continue to hold up with increased sampling, limitation upon sexual activity at any time of life may increase risk.

Evaluation of a new silastic seminal fluid collection device.
A new Silastic seminal fluid collection device has been tested and compared in vitro with glass jars, latex condoms, and the Milex polyethylene sheath. In a series of tests utilizing 50 different semen samples and observing the effects on motility over a period of 4 hours, the Silastic seminal fluid collection device demonstrated sperm survival slightly better than that with glass jars, superior to that with the polyethylene Milex sheath, and far greater survival (by several hours) than with the latex condom. The Silastic collection device was found to be more comfortable and more pleasurable to the patients than was the Milex sheath. The accuracy of seminal volume measurements was also found to be better in the Silastic collection device.

[Ejaculation inadequacy in sexual disorders].
Sixteen men with missing ejaculation were observed as outpatients in our department within the past years. Among these, four patients with retrograde ejaculation are included. Possible reasons for the missing ejaculation are operations in the genital region or the pelvis, and spinal cord injuries. Endocrine disorders, diabetes mellitus, drug dependence, and psychogenic alterations have to be discussed as etiologic factors. Secondary lack of ejaculation, which is acquired in later years of life, seems to have a poorer prognosis than the primary disease, which begins with puberty. This group of patients is well responsive to psychotherapy. Other therapeutic approaches are hormonal substitution, care of drug dependence, and treatment with sympathicomimetica in some cases of retrograde ejaculation.
The practical management of the otherwise fully sexually responsive woman who complains of inability to climax during intercourse.
Many women who can climax easily with their partner to direct clitoral stimulation are unable to reach orgasm during intercourse unless they receive supplementary manual clitoral stimulation. Such women frequently consult their physician for advice. This two-part article aims to provide clear practical guidelines to the management of this situation.
