A good hospitalist program will more than pay for itself by improving outcomes, reducing length of stay, and enhancing hospital efficiency. Moreover, the program can often attract primary care physicians who prefer to use the service of a hospitalist.
The U.S. Army Aeromedical Evacuation community (MEDEVAC) possesses a long-standing tradition of excellence in the care and transportation of combat casualties. Recent developments in civilian air medical transport and quantitative review of MEDEVAC operations have identified potential areas for improvement, concentrating on enhanced flight medic standards, training, sustainment and medical oversight of the air ambulance system. These proposed changes are discussed in detail, from the perspective of current emergency medicine and aviation medicine standards of practice. If instituted, these changes would facilitate the emergence of a true air medical transport capability comparable with the civilian community standard.

A three-year in-depth evaluation of 12 PCG/Ts shows that much remains to be done in terms of commissioning. PCG/Ts need to be more active in tackling poor performance. Further delegation of responsibilities from health authorities to PCTs will be a challenge, given their reluctance to tackle service changes. GPs tend to dominate board meetings. More resources are needed for management support.

Spanning half of the chiropractic century, the development of the American Board of Chiropractic Orthopedists (ABCO) is a story of an educational bootstrapping that originated from the concerns of practicing field chirporactors. In the post World War II era, a need was determined to develop various principles and procedures of orthopedics in relation to chiropractic practice. Innovative chiropractors sought to promote greater levels of diagnostic precision within the profession by creating a post graduate program to teach advanced methods of physical and neuromusculoskeletal examination and treatment. Various specialty societies emerging after the 1947 creation of the professionally owned, non-profit Los Angeles College of Chiropractic provided a humble beginning for the task. Eventually, with its roots in the National Chiropractic Association, the ABCO and its predecessors were successful in enhancing the education of practicing doctors, instituting similar programs in chiropractic college curricula, contributing papers to chiropractic literature and providing groundwork for early chiropractic research pertaining to musculoskeletal disorders.
Ambulatory surgery in France was under constraining rules since 1992. In 1998, there were 7,600 ambulatory stations which allowed 2,700,000 ambulatory procedures in a year. French Association for Ambulatory Surgery and International Association for Ambulatory Surgery adopted a limited list of 18 reference procedures to launch a survey on the prevalence of ambulatory surgery. The prevalence rate of ambulatory procedures among this list raised from 35.2% to 39.3% in 1999. This rate was the same in public and private health institutions. Ambulatory practice was increasing more than the inpatient procedures. Intercountry comparisons pointed out France as the tenth country for ambulatory prevalence among 13 members of Organization for Economic Cooperation and Development. This underlined the fact that there was probably a growth potential for ambulatory surgery in France.

Quality assurance of the data generating processes in epidemiologic studies is a prerequisite for the internal validity of study results. This paper presents practical aspects of such a quality assurance system pertaining to the planning, data gathering, data entry and data processing phase of a study. It is concerned with data obtained in the framework of a project rather than with data accumulating continuously in private practices, research institutes or veterinary faculties. During the planning phase of a project, standard operating protocols should be developed that assure a reliable performance of observation, coding and data entry. The data base structure, consisting of tables, input validation rules and queries, should be predefined and well documented. A data safety concept will provide the necessary integrity, physical safety and availability of the data. The paper presents technical solutions to common data processing problems with emphasis on re-coding and relational data base facilities (Microsoft-ACCESS) using a hypothetical study on risk factors for mastitis.
