Identifying and managing suicide risk in bipolar patients.

Bipolar patients have been shown to be at high risk for suicidal behavior. Therefore, identifying potentially suicidal patients is necessary in the treatment of bipolar patients. A stress-diathesis model for suicidal behavior has been proposed to assist clinicians in determining which patients are at risk. In the model, suicidal behavior is the result of the interaction between an individual's threshold for suicidal acts and the stressors that can lead to suicidal behavior. Suicide risk factors can then be categorized as either diathesis-related or stress-related. In a study applying the model of suicidal behavior to bipolar disorder, bipolar patients who attempted suicide had higher levels of suicidal ideation, lifetime aggression, and substance abuse than the comparison group of nonattempters. Attempters had twice the number of major depressive episodes. Once high-risk patients are identified, their suicide risk can be managed through treatments such as prophylactic lithium treatment and other pharmacologic approaches.

Anaesthetic management of splenectomy in Evan's syndrome during pregnancy with pregnancy induced hypertension.

The management of idiopathic thrombocytopenic purpura (ITP) during pregnancy, especially with ongoing bleeding diathesis, has not been highlighted sufficiently in the literature. Aortocaval compression and reduction in uteroplacental circulation resulting in foetal hypoxia and acidosis, Mendelson's syndrome due to gravid uterus, trauma to airway with resultant haemorrhage and aspiration into lungs, compromised airway due to short neck, anasarca and heavy breast, limitation in using invasive monitoring and regional anaesthesia and uncontrolled bleeding leading to placental hypoperfusion and foetal hypoxia are some of the important risks. In the present case report, anaesthetic management for splenectomy during pregnancy complicated with pregnancy induced hypertension and bleeding diathesis secondary to ITP is described with reference to above risks.

Personality and the genetic risk for alcohol dependence.

The extent to which the genetic risk for alcohol dependence (AD) and conduct disorder (CD) and their common genetic risk overlap with genetic factors contributing to variation in dimensions of personality was examined in a study of 6,453 individuals from 3,383 adult male and female same-sex and unlike-sex twin pairs from the Australian Twin Registry. The associations between the personality dimensions of positive emotionality, negative emotionality, and AD and CD risk were modest, whereas the associations between behavioral undercontrol and AD and CD risk were substantially higher. Genetic influences contributing to variation in behavioral undercontrol accounted for about 40% of the genetic variation in AD and CD risk and about 90% of the common genetic risk for AD and CD. These results suggest that genetic factors contributing to variation in dimensions of personality, particularly behavioral undercontrol, account for a substantial proportion of the genetic diathesis for AD and most of the common genetic diathesis for AD and CD among both men and women.

Six new cases of a caterpillar-induced bleeding syndrome.

We describe six new cases of a hemorrhagic diathesis induced by contact with Lonomia achelous caterpillars. Onset of clinical bleeding varied between a few hours and 10 days post-exposure. Laboratory coagulation tests showed prolonged PT, PTT and ThT; normal platelets and a marked decrease of fibrinogen, factor V, plasminogen and factor XIII (including its subunits A and S). Factors VII, II and alfa 2 anti-plasmin were variably affected. In addition, activation of the fibrinolytic system and the generation of a procoagulant effect could also be demonstrated. Two cases developed severe hemorrhagic diathesis and one of them died of a cerebral hemorrhage. Different aspects of this rare syndrome are discussed in relation to its complex physiopathology and the variability observed in all clinical and laboratory manifestations. Therapeutic recommendations and some possible hazards following replacement transfusions are also considered.

Fibrinogen anomalies and disease. A clinical update.

The precipitous increase in the number of structurally defined fibrinogen defects in recent years has resulted from application of high performance liquid chromatography in combination with peptide mapping and sequencing procedures. More recently, application of DNA sequence of polymerase chain reaction products has accelerated the pace of identification of mutations. Highly frequent defects are Arg substitutions, accounting for eight mutation sites substituted by Cys or His and less frequently by Ser. Amino acid substitutions at different positions on all three chains have pointed to possible structures with polymerization-related functions. Also, substitutions yielding consensus sequences resulted in extra glycosylations of the appropriate Asn in four different mutation sites; the impaired polymerization was reported associated with undue bleeding in two of these. Among informative defects have been those of homozygous probands with A alpha 16Arg----His and A alpha 16Arg----Cys in that failure of release of peptide A (but not of B), as shown with A alpha 16Arg----Cys, resulted in markedly delayed polymerization of such fibrin monomers, in general agreement with conclusions reached in studies of normal fibrin. This dysfunction, as well as the slow rate of release of A shown with A alpha 16Arg----His, was associated with clinically significant hemorrhagic diathesis (in the homozygous probands), consistent with the known physiologic importance of peptide A cleavage in normal hemostasis. Also, defects on the A alpha 17-19 sequence resulting in impaired polymerization are consistent with the known role of this segment in polymerization. Of similar interest have been defects within a B beta chain span encoded its exon 2. Two defects resulting in impaired polymerization and thrombin binding were associated with clinical thrombosis commencing in early life, and this lends strong support to other evidence suggesting a role in polymerization and in noncatalytic thrombin binding by this B beta chain segment. Thrombosis associated with A alpha 554Arg----Cys in a heterozygous proband with impaired tPA interaction is unique and may shed light on this poorly understood but important interaction among fibrin, plasminogen, and tPA. A group of different defects within the gamma 275-375 sequence have pointed to a polymerization role, evidenced by delayed gelation and impaired binding of mutant D to normal fibrin E. An unusual example is a 15 residue insertion between gamma 350 and 351 resulting in impaired polymerization, gamma chain crosslinking, and platelet aggregation support and is associated with hemorrhagic diathesis and poor healing.(ABSTRACT TRUNCATED AT 400 WORDS)

Thrombocytopenia and the risks of intraocular surgery.

BACKGROUND AND OBJECTIVE: In thrombocytopenia, a hemorrhagic diathesis is usually present due to a low platelet count and has been related to the development of cataracts and retinopathy. The concomitant administration of nonsteroidal anti-inflammatory drugs can increase the hemorrhagic diathesis. The purpose of this study was to investigate the impact of thrombocytopenia during and after intraocular surgery. PATIENTS AND METHODS: A retrospective study of medical files of patients who had undergone cataract and glaucoma filtering surgery and were diagnosed as having thrombocytopenia between 1994--1995 and 1998--1999 was conducted. Eight patients with a total of 11 surgical procedures were included in this study. RESULTS: Hemorrhagic complications occurred in 2 of the 11 procedures, for an incidence of 18%. These 2 cases are described in detail. CONCLUSIONS: The current study confirms that thrombocytopenia is a significant risk factor for perioperative bleeding in ocular surgery. A routine questionnaire should be completed before ocular surgery and a complete blood cell count obtained in suspect cases. Consultation with a hematologist is warranted in cases of thrombocytopenia.

[Somato-autonomic symptoms in different types of seasonal affective disorders].

Ninety-seven patients (76 female, mean age 31,2 years and 21 male, mean age 34,6 years) who had a diagnosis (according to ICD-10) of seasonal affective disorder (SAD), depressive episode, have been studied. It is shown that SAD included at least two types of depression with different somato-autonomic symptoms. They were classified as SAD with anesthetic somato-autonomic complex (type 1) and SAD with hyperesthetic somato-autonomic complex (type 2). The authors set up a hypothesis that a particular personality predisposition (constitutional or acquired) in a form of hypochondriac diathesis (type 1) and neuropathic diathesis with weather and climate sensitivity (type 2) can be considered as a causative factor of seasonality of depression and predomination of somato-autonomic symptoms in its structure.

[Intracranial hemorrhage in term newborn].

BACKGROUND: The improvements in perinatal care during last decade have changed clinical presentation of intracranial hemorrhage (ICH) among full-term newborns. New imaging techniques allow for diagnosis of ICH even in asymptomatic babies. AIM: Analysis of prevalence and risk factors of ICH among full-term newborns requiring intensive care. MATERIAL AND METHOD: 397 full-term newborns hospitalized in tertiary Neonatal Intensive Care Unit were analyzed. Detailed neurological evaluation, head ultrasound and/or cerebrospinal fluid were done. RESULTS: ICH was diagnosed in 40 newborns. The most common clinical presentation was subarachnoid hemorrhage (n = 24). Multifocal bleeding was more frequent (28/40) than bleeding only to one brain compartment (12/40). In the first week of life bleeding to different compartments was observed, but in the 2nd week of life there was no bleeding to posterior fossa. Eleven newborns died due to ICH (parenchymal and subarachnoid ICH). Significant, independent ICH risk factors were: delivery complications (OR: 10.4: 95% CI: 3.7-29.6), scull bone fractures (OR: 44.4: 95% CI: 4-495), nuchal cord (OR: 6.4: 95% CI: 2.2-18.8), hemorrhagic diathesis (OR: 4.5: 95% CI: 1.2-17.5). CONCLUSIONS: Significant risk factors of ICH among full-term newborns requiring intensive care are: mechanical (scull bone fractures, labor complications) trauma, nuchal cord, hemorrhagic diathesis. In such cases the detailed neurological evaluation and imaging techniques should be used to exclude bleeding. Multifocal bleeding is the most common form of ICH. Because subarachnoid and subdural space are not easy visible by head ultrasound, the CT or MRI scans are recommended in babies with ICH.

[Post transfusion purpura--a serious underdiagnosed delayed post-transfusion event].

UNLABELLED: Post-transfusion purpura (PTP), a delayed post-transfusion event, is underdiagnosed, not only in Poland. Thrombocytopenia results from the destruction of patients' and transfused platelets by platelet specific antibodies (anti-HPA). The incidence of PTP is not estabilished. THE AIM: Analysis of 10 cases with PTP diagnosed in Poland during last 25 years. MATERIAL AND METHODS: 9 women and 1 man with normal platelet count who developed thrombocytopenia with diathesis haemorrhagica after transfusion of red cells or/and platelets. Platelet specific antibodies (anti-HPA) were examined by the platelet immunofluorescence test and by the monoclonal immobilization of platelet antibodies (MAIPA) assay; leucocyte antibodies by standard lymophocytotoxicity test (LCT). RESULTS: In all 10 patients the PTP was diagnosed because: 1) thrombocytopenia with diathesis haemorrhagia occured 3-10 days after the transfusion (in 5 cases blood was transfused during the operation), 2) in all the patients anti-HPA were detected (in 9- anti-HPA-1a, in 1 case anti-HPA-3a), 3) a primary HPA alloimmunization was very probable. A recovery of platelets count occured in 6 patients within 8-34 days after corticosteroids and/or IVIG, however, therapeutic effect of them was difficult to assess due to a great probability of spontaneous remission up to 1 month. Four patients died due to their basic disease although the impact of the PTP cannot be excluded. CONCLUSION: The diagnosis of PTP was possible because of the detection of anti-HPA antibodies in patients who developed thrombocytopoenia after blood transfusion.

[The protease inhibitor treatment of the disseminated intravascular coagulation syndrome in patients with liver cirrhosis].

The results of the treatment of the syndrome of disseminated intravascular coagulopathy (DIC) in 41 patients with liver cirrhosis are reported. A relation between the severity of the liver impairment and the degree of the increase of the fibrin split products (FSP) was established. FSP above 100 micrograms/ml were found in patients with III class liver failure according to Child-Turcotte-Pugh and rating above 6.5 according to Z. Krŭstev. The administration of 30,000-60,000 U of the protease inhibitor contrykal led to mastering of the hemorrhagic diathesis in 19 of 21 patients (group A). The increased FSP in the serum fell (chi 2 = 5.79; p less than 0.001). In contrast, the administration of PAMBA and styptanon did not affect the hemorrhagic diathesis and DIC syndrome in 16 of 20 other patients (group B--chi 3 = 20.54; p less than 0.001). The authors recommend protease inhibitors as the treatment of choice of DIC syndrome in liver cirrhosis. Only in case of lack of effect should heparin be added.

[Postoperative intracranial hemorrhage due to vitamin K deficiency: report of two cases].

Vitamin K deficient hemorrhagic diathesis is well known as a cause of infantile intracranial hemorrhage. Its occurrence, however, as a post-surgical complication is rare and has never been reported previously. Two cases are presented here which illustrate the existence of such a hazard. Case 1. A 73-year-old woman admitted with subarachnoid hemorrhage (WFNS IV) underwent microsurgical exploration of a left internal carotid aneurysm, and neck clipping of the aneurysm was performed. She had an uneventful postoperative course, but her neurological condition deteriorated suddenly on the fifth postoperative day. CT scan revealed a large epidural hematoma. Case 2. A 6-year-old boy was admitted due to the dysfunction of a ventriculo-peritoneal shunt system that had previously been placed for hydrocephalus. This dysfunction was thought to be caused by meningitis. Twelve days after ventricular drainage and antibiotic therapy, sudden intraventricular hemorrhage occurred. In both cases, PT and APTT were markedly prolonged, FDP slightly increased and fibrinogen slightly decreased. SFMC was positive in case 2. After the administration of vitamin K, PT and APTT were immediately normalized. Recent reports emphasize the adverse effect of antibiotics that leads to vitamin K deficient hemorrhagic diathesis, especially, in patients in a cachectic state. In these two cases, such a cachectic condition was not observed. We presume that the cause of vitamin K deficiency would be, along with the administration of antibiotics, a preliminary condition of disseminated intravascular coagulation which is encountered in some neurological disorders including subarachnoid hemorrhage. We conclude that attention should be paid for these pitfalls in perioperative neurosurgical care.

[The history of the Zollinger-Ellison syndrome].

In 1955, the concept that an ulcerogenic islet cell tumor was responsible for a marked ulcer diathesis that led to gastric hyperacidity was proposed by myself and Ellison. Recently is was suggested that this concept marked the beginning of modern gastrointestinal endocrinology. Although these pancreatic ulcerogenic tumors are relatively uncommon, a survey in 1987 stated that inquiries about the tumor were the most common references requested from surgical journals. The original diagnostic triad proposed included a fulminating ulcer diathesis (12-hr secretion 2-3 liters), rapidly recurring ulceration despite adequate medical, surgical, and radiation therapy, and identification of a non-beta islet cell tumor of the pancreas. The non-beta islet cell was considered to be different from the cells producing insulinoma, which up to that time was thought to be the only type of islet cell tumor produced by the pancreas.

The use of the full thickness skin graft in Dupuytren's contracture.

For those hand surgeons who have experienced early complications associated with limited fasciectomies (those who have not, have not done enough limited fasciectomies) and are frustrated by a high rate of recurrence or extension of the disease, incision of the cord and interposition of a full thickness graft is a technique to seriously consider. The procedure is not difficult to perform, but patience and attention to detail are prerequisites for success. That success is measured by complete or near complete release of the contracture with a minimum of morbidity, a nil recurrence rate, and extension rate of less than 10%. This technique is indicated for patients who have one or more elements of the Dupuytren's diathesis. Usually people older than 65 who develop Dupuytren's contracture do not have the diathesis, and their disease can be managed by limited fasciectomy and Z plasty skin lengthening. Finally, Dupuytren's disease presenting with no contracture can be managed effectively and conservatively by a series of intralesional injections of triamcinolone into the nodules and cords, the treatment of choice for all plantar nodules and knuckle pads.

[Blood aggregation in duodenal ulcer].

Thirty-six children with duodenal ulcer were examined for the coagulation and thrombocytic components of hemostasis. Use was made of the new coagulologic research methods based on administration of the synthetic tri- and tetrapeptides containing a chromoform group. The high sensitivity of the synthetic peptides enables the use of plasma in high dilutions, owing to which the effect of inhibitors is decreased and a small amount of blood is only required, being of paramount importance in carrying out serial tests. In acute ulcer 1/3 of the children show the signs of the DIC syndrome compensated at the expense of the high content of anticoagulants. At the same time 4 patients were discovered to have coagulologic disorders characteristic of Willebrand's disease (3 cases) and thrombasthenia (1 case). The coagulologic alterations should be taken into consideration in treatment administration, since the compensated DIC syndrome untreated by anticoagulant therapy aggravates the course of the ulcerous process and gives rise to hemorrhagic diathesis. Meanwhile in children with hemorrhagic diathesis, peptic ulcer may cause severe, life-threatening hemorrhages.

[Characteristics of immunologic changes and clinical manifestations of allergy in children with impaired thyroid function].

The majority of children with allergic diathesis, living in the Andizhan region of the Uzbek SSR manifest the signs of stable abnormalities of thyroid function, that aggravate immunological alterations and the course of the process. This prompts the use in these children of therapeutic measures aimed at correction of the activity of the thyroid apparatus. With this purpose in view, the children with allergic diathesis associated with increased thyroid function may be administered aqueous solution of potassium iodide whereas those with functional thyroid deficiency may be given thyroidin in proper age-associated doses.

[Pathogenetic characteristics of hemostatic disorders in chronic renal failure].

As many as 45 patients with terminal chronic renal failure (CRF) admitted to the hospital were examined for hemocoagulation and blood plasma content of medium-sized molecules (MSM). Animal (rabbit) experiments were made in vitro and in vivo to study the effect produced on hemocoagulation of MSM isolated from CFR patients' blood by acidic deproteinization, ultrafiltration and gel chromatography. Integrally, the MSM fraction was shown to exert marked anticoagulant and antiaggregation effects, to increase vascular permeability, promoting the development of hemorrhagic diathesis in CRF patients. Hemodialysis (HD) is an effective method for treating hemorrhagic diathesis of the given genesis. At the same time under the conditions of the presence of the cellular template (red blood cell hemolysis, platelet destruction as a result of HD) MSM may give rise to the development of thrombotic complications after HD completion and heparin neutralization. Therefore accumulation of MSM may play an important part in the onset of hemocoagulation disorders in patients with CRF.

A new von Willebrand factor (vWF) defect in a patient with factor VIII (FVIII) deficiency but with normal levels and multimeric patterns of both plasma and platelet vWF. Characterization of abnormal vWF/FVIII interaction.

The patients with inherited bleeding diathesis related to quantitative, structural, and/or functional abnormalities of von Willebrand factor (vWF) are said to have von Willebrand's disease (vWD). We report here the clinical and laboratory features of a 50-year-old woman with a life-long history of excessive bleeding. Her particular laboratory data are factor VIII (FVIII) deficiency, subnormal bleeding time, and the presence of all plasma and platelet vWF multimers in normal amounts. Infused with FVIII/vWF concentrate, she showed a persistent increase in FVIII that led us to discard hemophilia A carrier or "acquired hemophilia" diagnoses. vWF devoid of FVIII purified from normal and patient's plasma by immunoaffinity on anti-vWF monoclonal antibody (MoAb) was immobilized onto polystyrene tubes that were further incubated with purified normal FVIII. The bound FVIII was evidenced using radiolabeled anti-FVIII MoAb. The data showed that the patient's vWF, in contrast to vWF purified from normal plasma, was unable to bind FVIII. Furthermore, no inhibitor of FVIII/vWF interaction was evidenced in incubating purified normal vWF with the patient's plasma before the addition of FVIII and anti-FVIII MoAb. These results support the concept that the bleeding diathesis of this patient appears to be due mainly to her abnormal vWF preventing FVIII/vWF interaction. This abnormality, which is not yet described in present classification of vWD, could be considered as a new variant of vWD.
