Impact of State hospital rate setting on capital formation.

For this article, a new national data base of Medicare's cost reports on more than 2,000 hospitals is used to measure the impact of State prospective rate setting on capital formation. Several investment measures are analyzed, both in nominal and real terms, using a combination of descriptive and multivariate techniques. Results indicate that, over the last decade, State hospital rate-setting programs have had little demonstrable effect on capital formation and they have not caused any significant aging of plant assets. Programs in both New York and Massachusetts were found to be associated with a slowing in the rate of bed growth, however, resulting in significant long-term cost savings.

Anonymous or confidential HIV counseling and voluntary testing in federally funded testing sites--United States, 1995-1997.

Human immunodeficiency virus (HIV) counseling and voluntary testing (CT) programs have been an important part of national HIV prevention efforts since the first HIV antibody tests became available in 1985. In 1995, these programs accounted for approximately 15% of annual HIV antibody testing in the United States, excluding testing for blood donation. CT opportunities are offered to persons at risk for HIV infection at approximately 11,000 sites, including dedicated HIV CT sites, sexually transmitted disease (STD) clinics, drug-treatment centers, hospitals, and prisons. In 39 states, testing can be obtained anonymously, where persons do not have to give their name to get tested. All states provide confidential testing (by name) and have confidentiality laws and regulations to protect this information. This report compares patterns of anonymous and confidential testing in all federally funded CT programs from 1995 through 1997 and documents the importance of both types of testing opportunities.

Transforming healthcare in the Internet Era.

Healthcare services will be transformed in the Internet Era by developments in biotechnology, bioinformatics, health informatics, assimilation of modern business processes, and changing policy expectations. Discoveries in biology and communications technology offer the potential for improvements in health status of individuals and populations. Improved access to information about health and disease will typify early progress. Care in hospitals will shift toward palliation and end-of-life care; curing and prevention will increase in outpatient settings and/or within the home or workplace. Barriers include resistance to change and a lack of a global health information infrastructure that includes financing, standards, and coherent policy.
