Results from 7-mm bilateral recessions of the medial rectus muscles for congenital esotropia.

We reviewed the surgical records of 77 patients who had undergone 7-mm bilateral medial rectus recessions for large-angle congenital esotropia. The mean age of onset of esotropia in these patients was 3.5 months, and the mean age at the time of surgery was 12.9 months. The mean preoperative deviation was 69 delta of esotropia. This procedure resulted in satisfactory horizontal alignment in 47 patients (61%) at their latest follow-up examination (mean, 27 months; range, 6 months to 6 years). The esotropia was undercorrected in 21 patients (27%) and overcorrected in nine (12%). Amblyopia was noted in 30%, oblique muscle dysfunction in 44%, and dissociated vertical deviation in 36% of the patients. Regression analysis of multiple variables showed alignment at 6 weeks to be the only predictor of the final outcome (R = .83). Extramacular fusion was achieved in 10 of 11 patients for whom reliable sensory data were available. The mean age at surgery of these patients was 9.7 months. Seven-millimeter bilateral medial rectus recessions are an effective alternative to three- and four-muscle procedures in the initial treatment of large-angle congenital esotropia during infancy.

[New reconstructive surgery in traumatic detachment of the ciliary body with recession of the angle of the anterior chamber].

Penetrating wounds of the corneoscleral area are remarkable for particularly severe course of posttraumatic period, this being explained by direct injury to the ciliary body, impaired anatomic relationships in the anterior chamber corner, formation of this corner recession, this resulting in detachment of the ciliary body and development of the hypotonic syndrome. The authors suggest a new surgical technique of reconstructing the anterior chamber corner in its recession and detachment of the ciliary body with the aim of correcting the hypotension and fortifying the ciliary body at the site of its anatomic localization. This method was used in the treatment of 32 patients with subatrophy, detachment of the ciliary body, and hypotension; positive effect was achieved in 28.

Experts offer differing views on the impact of a recession.

What are the risks of a prolonged recession for hospitals? Although health care would probably not be hurt as much as other industries by a lengthy downturn, financial experts are saying a continuation of the current soft economy would add to hospitals' already long list of financial woes. The coming recession "will exacerbate the problems for hospitals already facing fiscal stress," says Glenn Wagner, an investment banking consultant based in New York City.

Hemihang-back recession: description of the technique and review of the literature.

We describe a modification of the hang-back and loop techniques that we call the hemihang-back recession, in which the muscle is reattached to the sclera posterior to the original insertion with a single, absorbable suture. We recommend it for difficult strabismus cases, specifically when recessions of 7 mm or more are required.

[The ependymal lining of the infundibular recess in males and hermaphrodites in small ruminants in scanning electron microscopy].

A scanning electron microscope was used to study the surface of the ependymal epithelium of the lower part of the third cerebral ventricle of rams, he-goats, and in one goat with a developmental sexual disorder (testicular hermaphroditism). During mating, the apical membranes of the ependymal tanycytes of the rams and billy goats were found to be covered by numerous blebs (ependymosecretion). It was only in the billy goats in the mating season (not in the rams) that individual supraependymal cells with protrusions forming "ruffled membranes" were present on the surface of the infundibular recession. Although the finding on the sexual organs of the hermaphrodite was closer to the male, the presence of the large number of supraependymal cells was rather a female trait. As suggested by the results, it is also in the males of small ruminants that the activity of the sexual organs is associated with the structure and function of the ependymal epithelium in the infundibular recess.

[Recession of the inferior rectus muscle in superficial anesthesia in the treatment of thyroid orbitopathy].

Recession of the inferior rectus in surface anaesthesia by 5% cocaine drops was performed in 8 patients with signs of hypotropia in the course of thyroid orbitopathia and with diplopia persisting after conservatory treatment and not corrected by prisms. This method enables us an accurate intraoperative settlement of the distance of recession and it may be recommended in cases metabolically normalized, with not too much advanced exophthalmos, with a stability of the motor disturbances for at least 6 months and without any signs of inflammation.

Acitretin in the symptomatic therapy for severe recessive x-linked ichthyosis.

Eight men with severe recessive x-linked ichthyosis were treated with acitretin, the main metabolite of etretinate, during four months. All of the patients showed marked clinical improvement of scaling during therapy. Hypervitaminosis A-type adverse reactions were observed in all patients. Although the overall tolerance was good, therapy was interrupted in one atopic patient because of pruritus. There were no undesirable laboratory changes in values. Thirty-five milligrams of acitretin daily provided the best efficacy, with minimal side effects. The beneficial effect of this retinoid lasted between four and six weeks after therapy was stopped. These results suggest that acitretin is a useful agent in the symptomatic therapy of severe recessive x-linked ichthyosis resistant to topical therapeutic modalities. Good results with this agent can be achieved with interval therapy adjusted to seasonal variations of the skin symptoms.

Recession vs marginal myotomy surgery for strabismus: effects on spatial localization.

Palisade endings (PEs) are found at the musculotendinous junction in eye muscles and this is the site of much strabismus surgery. To assess the role of PEs in supplying proprioceptive information about eye position, we compared the effects of marginal myotomy and recession surgeries on tests of eye-hand coordination (open-loop pointing responses) taken before and after surgery in two groups of children having medial rectus weakening procedures for the first time. The results indicate that the myotomy is more proprioceptively "de-afferenting" than the recession, and we assume this is due to the greater disruption of PEs in this procedure.

Trochlear recession for correction of luxating patella in the dog.

Trochlear recession is a surgical technique for stabilizing luxating patellas by recessing the trochlear sulcus 1 to 2 mm. The smooth hyaline cartilage of the articulating trochlear surface is preserved. This technique has provided excellent results in 41 stifles of 28 dogs, as determined by a 6-month to 6-year follow-up, with an average follow-up period of 3.2 years.

Lumbar spinal-lateral recess stenosis.

The syndrome of a lumbar spinal and lateral recess stenosis is characterized by pain and a variety of paresthetic symptoms occurring principally when the patient stands or walks. Sitting or lying down alleviate the symptoms promptly. The neurologic examination is characterized by a negative straight leg-raising test and a paucity of abnormal neurologic findings. The diagnosis is confirmed by a high-resolution CT scan. If conservative treatment fails, a myelogram is in order to establish a definitive diagnosis and assess the severity of neural compression prior to placing the indication for a surgical decompression of the stenotic spinal canal. The surgical procedure consists of a laminectomy and a partial facetectomy of the hypertrophied portion of the facet joint that compresses the adjacent lumbar nerve root from a dorsal direction. It is important to recognize all associated pathologic processes that must be dealt with accordingly at the same time in order to assure success of the operative procedure. The results of a surgical decompression for a lumbar spinal and lateral recess stenosis are excellent.

[Differential diagnosis of contrast medium recesses of the renal pelvis or the renal calyx system (author's transl)].

Contrast medium recesses in the renal pelvis and calyx system of a retrograde pyelogram or urogram are due to many widely different pathologico-anatomical changes in the renal region. It is the aim of the present study to show up this multitude of differential diagnostic possibilities which lead to contrast medium recesses in the renal pelvis and renal calyx system of a pyelogram.

Recession: what can hospitals expect?

Although not all the experts are agreed, it seems likely that there will be a recession beginning in 1979. This article takes a look at what people who are concerned with hospital planning can expect as a result of a recession.

Alterations in osmotic but not pressor responses to ACTH by optic recess lesions in sheep.

This study examines whether neural structures in the region of the optic recess of the third ventricle may be involved in the genesis of adrenocorticotrophic hormone (ACTH)-induced hypertension in sheep. Five sheep were prepared with lesions in an area of the forebrain that included the organum vasculosum of the lamina terminalis (OVLT) and surrounding periventricular tissue. In these animals the dipsogenic response to systemically infused hypertonic sodium chloride (NaCl) was abolished. ACTH treatment (20 micrograms/kg/day) for 5 days caused an increase in mean arterial pressure (MAP) of 19 mm Hg, a response identical to that seen in normal sheep. With ACTH treatment, increases in plasma osmolality were greater than normal, but polydipsia did not occur in the lesioned sheep. In six other sheep with lesions either lateral or anterior to the optic recess of the third ventricle, the dipsogenic response to hypertonic NaCl and pressor response to ACTH were normal. These studies establish that in ACTH-treated sheep the integrity of the anterior ventral part of the third ventricle is not essential for the development of the hypertension. This is in contrast to the finding in other models of experimental hypertension in the rat.

Selective intestinal malabsorption of vitamin B12 displays recessive mendelian inheritance: assignment of a locus to chromosome 10 by linkage.

Juvenile megaloblastic anemia caused by selective intestinal malabsorption of vitamin B12 has been considered a distinct condition displaying autosomal recessive inheritance. It appears to have a worldwide distribution, and comparatively high incidences were reported 30 years ago in Finland and Norway. More recently, the Mendelian inheritance of the condition has been questioned because almost no new cases have occurred in these populations. Here we report linkage studies assigning a recessive-gene locus for the disease to chromosome 10 in previously diagnosed multiplex families from Finland and Norway, proving the Mendelian mode of inheritance. The locus is tentatively assigned to the 6-cM interval between markers D10S548 and D10S466, with a multipoint maximum lod score (Zmax) of 5.36 near marker D10S1477. By haplotype analysis, the healthy sibs in these families did not appear to constitute any examples of nonpenetrance. We hypothesize that the paucity of new cases in these populations is due either to a dietary effect on the gene penetrance that has changed with time, or to a drop in the birth rate in subpopulations showing enrichment of the mutation, or to both of these causes.

[The effectiveness of the Faden operation on the superior rectus muscle combined with recession of the muscle for the treatment of dissociated vertical deviation].

We made a retrospective study of the effectiveness of the Faden operation on the superior rectus muscle in combination with recession of the muscle for the treatment of dissociated vertical deviation (DVD). A follow-up period of at least one year was required for inclusion in this study. Fourteen patients met this criterion, and two of them had received bilateral surgery. The average preoperative DVD was 31.2 +/- 9.5 delta (range 14 to 45 delta). The surgical procedure was the Faden operation on the superior rectus muscle 11 to 14 mm posterior to its initial insertion in combination with a 2-to 5-mm recession of the muscle, except in one case. The average of the muscle, except in one case. The average hyperdeviation of the operated eyes was 5.6 +/- 11.4 delta (range 0 to 16 delta), and 12 eyes (75%) had satisfactory results (0 to 9 delta) at the last visit. The average reduction of hyperdeviation was 26.5 +/- 11.4 delta (range 8 to 45 delta), The long-term effect of the Faden operation was relatively stable. The hyperdeviation of operated eyes before surgery was clearly larger than that of unoperated eyes at the last visit (p < 0.001).

Recession of the superior oblique tendon for inferior oblique palsy and Brown's syndrome.

Superior oblique tendon recession of 12 to 14 mm was performed in five patients with inferior oblique muscle palsy and three patients with Brown's syndrome. The angle of deviation was decreased by an average of 18 prism dioptres in the worst field of gaze for the former group and by 17.3 delta for the latter group. As a secondary benefit of the procedure for the patients with inferior oblique palsy, any associated preoperative A-pattern was collapsed an average of 10 delta postoperatively. The procedure eliminated all abnormal head postures and improved versions and ductions for both groups. Symptoms of diplopia and torsion were resolved, and full fusional ability was maintained at preoperative levels in all fields of gaze for all patients. After an average follow-up period of 18 months none of the patients had manifested superior oblique palsy or other notable postoperative surgical complications. Theoretical advantages of superior oblique tendon recession include the potential for reversibility and reoperation if required, the possibility of asymmetric surgery and the potential to perform the procedure with an adjustable suture. The technique is an advantageous approach in the surgical treatment of these complicated cases.

Angle recession.

Angle recession is a relatively common result of ocular injury, which, if extensive, creates a noteworthy risk of elevated intraocular pressure (IOP) and glaucoma. The unilateral elevation in IOP is usually seen long after the injury has occurred. The diagnosis is often made based on clinical suspicion, either because of information obtained during the history-taking or from signs of ocular trauma observed during the examination. The treatment of angle-recession glaucoma is in many ways similar to that of primary open-angle glaucoma, although miotic drugs and argon laser trabeculoplasty are controversial therapies for this condition.
