[Role of the argon laser in the treatment of varicose veins of the legs. Apropos of 17 cases].

The Argon is not just a fashionable sophisticated toy; it has definite indications in the field of phlebology: varicosities which can not be catheterised, flat angiomas, angiokeratoses and pigmented lesions. Some of its major advantages are the possibility of treating a large number of angiectases at the one session; the low risk of recurrence of the treated lesions in comparison with other methods and the excellent healing (respect of the pilosebaceous adnexae) which, however, is slower than for acne rosacea. Red angiectases respond better than blue lesions which require higher intensity laser. The major disadvantage is the cost, which justifies the creation of multi-disciplinary units (Ophtalmology, Gastro-enterology...). The techniques of laser coagulation still have to be defined (continuous or intermittent, intensity, time of irradiation). Finally, the skin of the legs acts as a shield by the thickness of the horny layer which absorbs or reflects the radiation.

Treatment of blepharitis and blepharoconjunctivitis: comparison of gentamicin-betamethasone, gentamicin alone and placebo.

At two university centres 46 patients presenting with symptomatic infective blepharitis or blepharoconjunctivitis participated in a double-blind study of a new antibiotic-steroid ointment containing gentamicin and betamethasone. There was no significant difference in response to 2 weeks of therapy between the patients treated with this ointment and those treated with either an ointment containing gentamicin alone or a placebo ointment. There was also no significant correlation between the clinical response and the results of the bacterial cultures of swabs obtained at the beginning and the end of the study. However, the patients treated with the placebo had a much higher rate of recurrence of their symptoms over a 6-week follow-up period. The patients who before treatment had associated rosacea, dry eyes and a long duration of symptoms showed a poor response to therapy.

[The specificity of the lupus band test. Personal findings and literature data (author's transl)].

The lupus band test is usually considered as a valuable aid for the diagnosis of lupus erythematous. Involved skin shows positive results in about 900 p. 100 of cases of discoid LE and systemic LE. However, the specificity of this test has been challenged by reports of positive immunofluorescent findings in other cutaneous diseases, like rosacea, in non-cutaneous diseases, and in healthy individuals. Discrepant data from the literature prompted us to report our own experience : direct immunofluorescent examination of skin biopsies was performed in 550 patients from a dermatologic department. Concerning diseased skin, a dermal-epidermal band was observed only in 18 cases of lupus erythematosus, in 4 cases of lepromatous leprosy, in one case of vasculitis, one case of prurigo, and one case of dermatomyositis. Concerning healthy skin, a band was seen only in 8 cases of systemic lupus erythematosus. These results confirm other reports establishing the specificity of the lupus band test.

Sacroiliac joints in chronic dermatoses.

In 34 patients with different chronic skin diseases (including eczemas, lichen ruber, erythrodermia, pyodermia, pemphigus erythematosus, vasculitis, parapsoriasis varioliformis, scleroderma adultorum Buschke, prophyria cutanea tarda, rosacea, dermatomyositis, erythematodes chronicus discoides and indurative tuberculosis), X-ray studies revealed sacroiliac-changes consistent with a low-grade inflammation. This oteoarthritis slightly differs from the patterns disclosed earlier by the same authors in psoriatic patients, lacking any clinical bone or joint symptoms. So far, it has not been possible to explain their nature, cause or development.

[Chronic erythematous oedema of the upper face (Degos). Morbihan disease (author's transl)].

We studied 5 patients presenting with erythematous superior oedema of the face and with oedema of the eyelids. The first case, described by Degos 20 years ago, is reviewed. The first of our 5 patients had clinical lesions similar to those of Degos' case, with true pachydermal features of the integument. In all cases the clinical features are typical but the histology was not characteristic: epidermal atrophy, elastosis, discrete lymphocytic infiltrates. The diagnoses of LE, rosacea, sarcoidosis were excluded. No medication appears to be effective except thalidomide provided that it is given early enough. This disease ought to be recognized as a distinct clinical entity.

[Histopathologic study of perioral dermatitis].

38 biopsies of perioral dermatitis have been examined. The following has consistently been observed: follicular hyperkeratosis; vasodilatation and oedema (sometimes very pronounced) of the papillary dermis; perivascular and parafollicular infiltrates consisting predominantly of lymphocytes, histiocytes and polymorphonuclear leucocytes. Occasionally, there have been epithelioid or giant cells, sarcoid-like infiltrates or vasculitis of the leucoclastic type. Demodex folliculorum was observed in only two sections and the follicles that contained them were not inflamed. This histological picture is very similar to that of acne rosacea.

[Psyche and skin (author's transl)].

The important psychological functions of the human skin - the skin functions as a border, contact and sense organ and as an organ of expression - as well as causalgenetic aspects of psychodermatology and the most important psychodermatological diseases are reported. Frequent psychosomatic dermatoses like chronic urticaria (bibliography), pruritus (case report, perioral dermatitis and alopecia areata (case reports) are described. In atopic dermatitis, psoriasis vulgaris and acne rosacea psychological factors are involved in addition to constitutional factors. Psychovegetative dermatoses can be due to situational problems (stress), but dermatologic symptoms may also have a mere appellant character (e.g. slight effluvium). Dermatologic symptoms may occur in connection with psychoses (e.g. parasitophobia). Psychic dermatologic symptoms may also arise secundary to severe chronic dermatoses. In psychoneurotic diseases such as dermatitis arterficialis skin is the target organ. Therapeutical aspects are considered.

Myocardial effects of flavonoids from Crataegus species.

The influence of the main flavonoids from Crataegus species (hawthorn, Rosaceae) on coronary flow, heart rate and left ventricular pressure as well as on the velocity of contraction and relaxation was investigated in Langendorff perfused isolated guinea pig hearts at a constant pressure of 70 cmH2O. Drug action was evaluated in a concentration range of 10(-7) to 5 x 10(-4) mol/l. An increase of coronary flow caused by the O-glycosides luteolin-7-glucoside (186%), hyperoside (66%) and rutin (66%) as well as an increase of the relaxation velocity (positive lusitropism) by luteolin-7-glucoside (104%), hyperoside (62%) and rutin (73%) were the major effects observed at a maximum concentration of 0.5 mmol/l. Furthermore, slight positive inotropic effects and a rise in heart rate were seen. Similar but less intensive actions were found with the C-glycosides vitexin, vitexin-rhamnoside and monoacetyl-vitexin-rhamnoside. Possible beta-adrenergic activities of the flavonoids could be excluded by the addition of propranolol in fixed concentrations of 10(-8) to 10(-5) mol/l. Moreover, pretreatment of the animals with reserpine (7 mg/kg) did not influence myocardial activity of hyperoside (10(-4) mol/l). As previous experiments showed an inhibition of the 3',5'-cyclic adenosine monophosphate phosphodiesterase, the results suggest an inhibition of this enzyme as the possible underlying mechanism of cardiac action of flavonoids from Crataegus species.

Cosmetic use of lasers.

During the 35 years of their existence, lasers have assumed an ever-expanding role in cosmetic surgery due to their reproducible precision in effectively managing a number of cutaneous conditions and disorders for which no acceptable form of treatment has been developed. Current medical lasers can be used to fade or remove many vascular and pigmented birthmarks, decorative and traumatic tattoos, solar and rosacea telangiectasia, and many acquired pigmentary disorders, without a visible scar or change in texture. In order to provide their patients with the best possible aesthetic result, cosmetic surgeons should be familiar with the advantages, disadvantages, and potential complications of lasers.

Minocycline-induced hyperpigmentation.

A 70-year-old man developed hyperpigmentation of his forearms, hands, fingernails, sclerae, ears, and teeth after 9 years of therapy with minocycline for acne rosacea. Minocycline is widely used in the treatment of acne vulgaris and uncommonly produces the side effect of hyperpigmentation. This effect does not appear to be dose-dependent and usually resolves within months to years after discontinuation of therapy. Discoloration of adult teeth, however, is generally permanent.

[Dermatology meeting of Paris. 7-10 December 1994. Abstracts].

In 1994 acne should be a very treatable disorder. However there are a significant number of patients who do not respond as well to therapy as they ought. Compliance is an important reason for poor response. Inappropriate therapy is another common reason. An increasing problem are patients with resistance of P. acnes to commonly used antibiotics; 39% of our patients have such resistance. Within this sub-group the commonest antibiotics causing problems are Erythromycin and Clindamycin (69%). Resistance is seen in 20% of such subjects to Tetracycline and Vibramycin and resistance to Trimethoprim is 12%. Multiple resistance is 14%. This is an increasing problem and unless attention is given to this problem by the year 2,000 we may have very few safe and successful antibiotics. Minocycline is rarely associated with resistance. Some patients have no resistance and may have a high sebum excretion; this will dilute what otherwise could be effective therapy. Such patients ought to be given high dose antibiotics such as Minocycline 200 mgs per day, plus appropriate topical therapy. Dianette plus 50- 100 mg cyproterone remains an alternative therapy in females. Some patients will fail to respond with these regimes and require Isotretinoin. Patients who require oral Isotretinoin are those with non-responding moderate acne, patients who scar, plus patients with unusual but specific problems. These include Acne Fulminans, Rosacea Fulminans, Acne Conglobata, Gram-negative folliculitis, Gross Seborrhoea, Dysmorphophobia, Mature Acne and Aperts Syndrome. The choice of dose varies, often according to personal experience and cost. Most physicians prescribe 0.5-1.0 mg/day for 4 months. Fifteen percent require 5-12 months therapy. Frequent reasons for prolonged therapy are macrocomedones and polycystic ovary syndrome. Relapse post therapy occurs in 30%; relapse after 3 years is unusual. Repeat doses can be given. The results are predictive and successful. The treatment of certain troublesome side effects also will be discussed as will the use of Isotretinoin in systemic disease. Isotretinoin orally is so effective that it ought to be given sooner rather than later to minimise scarring about which still only a little can be done to help the patient.

Topical metronidazole for arterial insufficiency ulcers.

The use of topical metronidazole has been limited to the treatment of acne rosacea, infected foot ulcers associated with diabetes mellitus, varicose veins, postirradiation ulcers, and dental conditions since the Food and Drug Administration approved the drug in 1988. Because of this agent's apparent effectiveness in treating anaerobic bacterial infections in such ulcers, the authors believed that treatment of arterial insufficiency ulcers with a solution of topical metronidazole would be a rational approach. They describe a 30-year-old man in whom bilateral lower extremity cellulitis developed as a result of arterial insufficiency. The patient's ulcers were unresponsive to intravenously administered antibiotics and whirlpool therapy. However, when a topical solution of metronidazole was administered, the ulcers began to heal and epithelialization at the ulcer sites occurred. The authors review others' studies concerning clinical use of topical metronidazole and suggest that further study is warranted. To the authors' knowledge, topical metronidazole solution for the treatment of arterial insufficiency and venous stasis ulcers has not been previously reported.

[Plant poisoning cases in Turkey].

In Turkey, the majority of the population live in rural areas where they use wild plants as food and medicine. The confusion of an edible plant with a poisonous one give rise to serious poisoning which may even result in death. The incidence of plant poisoning in Turkey is about 6% and especially high among children between ages of 2 and 11 living in rural areas. The number of species that cause poisoning is around twenty and Hyoscyamus niger (Solanaceae), Colchicum species (Liliaceae), Conium maculatum (Umbelliferae) and Prunus species (Rosaceae) are the most important. Mushroom poisoning is more frequent in spring and fall. The main reasons are their widespread usage as food and the inexperience of the gatherers in distinguishing the edibles from the poisonous. Amanita phalloides, A. verna, A. muscaria, A. pantherina are responsible for severe cases of poisoning.

[Relationship between the degree of specific IgE sensitization to food allergens and the appearance of clinical manifestations].

AIMS: To assess distribution and correlation between serum concentrations of specific IgE for food allergens and clinical symptoms after exposure. PATIENTS AND METHODS: Four hundred and thirty seven allergic children with 1097 sensitizations to food allergens, confirmed by prick-test and/or Phadezym RAST, were evaluated. From these sensitizations, 558 were associated to clinical symptoms after food intake (SFS), demonstrated by clinical history and/or open oral provocation, and 539 were asymptomatic food sensitizations (AFS). RESULTS: In these patients, the most frequent values of serum specific IgE (44%) were found in the 0.7 and 3.5 PRU/ml range, although in 18% of the cases no specific IgE was detected. Some differences in this distribution exist for different groups of food allergen, with predominance of negative results for food of the rosaceae group. On the contrary, specific IgE values higher than 17.5 PRU/ml are predominant for fish allergens. There is a statistically significant correlation (chi 2 p < 0.001) between anti-food allergen specific IgE concentrations and the clinical symptoms occurred after intake, so the serum specific IgE values between 0.35 and 3.5 PRU/ml are present with higher frequency in the AFS, while values higher than 17.5 PRU/ml are found with a statistically significant higher frequency in the SFS. When different food groups are evaluated separately, some mild differences exist. CONCLUSIONS: A statistically significant correlation between anti-food allergen specific IgE concentrations and the clinical symptoms ocurred after intake. Therefore, specific IgE quantitative assessment and its relationship to clinical symptoms may be of great interest in the management of food allergy.

[Differentiation alcoholic liver cirrhosis from viral liver cirrhosis].

Liver cirrhosis (LC) in habitual drinkers is divided into three categories: (1) alcoholic LC, (2) LC due to hepatitis C virus and alcohol, and (3) LC due to hepatitis C virus. In Japan, the frequency of LC related to hepatitis B virus in habitual drinkers is comparatively low. Although making a distinct differentiation is very difficult, it is possible to point out some characteristics which ars due to either alcohol or hepatitis C virus: (1) multiple spider angioma, acne rosacea, and palmar erythema are more frequently found in categories 1 and 2 than in 3, (2) levels of AST/ALT, gamma-GTP, TG, ALP, lactate, and UA are higher in category 1 than in 3, (3) enlargement of both lobes is observed in category 1, and (4) abnormality due to alcohol improves relatively soon after abstinence of alcohol.

Contact Nd:YAG laser excision of rhinophyma.

Rhinophyma, an excessive hyperplasia of the sebaceous glands of the nose, results in disfigurement, occasional nasal blockage and recurrent infection. This condition is considered to be the culmination of acne rosacea and can degenerate to a basal cell or squamous cell carcinoma. Various surgical methods have been used to treat rhinophyma, but most have resulted in complications and poor cosmetic outcomes. Wenig and Weingarten reported four cases of rhinophyma which were successfully treated with the contact Nd: YAG laser, and this article describes my experience in use of this modality to treat a 71-year-old patient at the Eye and Ear Clinic of Charleston.

[Action potential changes induced by a polyflavane on normal or hypoxic guinea pig myocardial strips].

On normal isolated or stimulated Guinea-Pig myocardial strips, a polyflavane extracted from Poterium spinosum (Rosaceae) increased the duration of the action potential plateau phase. Such a modification was also elicited on hypoxic strips; in this case it did not depend on the glucose concentration in the medium; it could be related to an effect on the slow calcium-sodium channel.

[Positive corneal findings of Propioni bacterium acnes and keratitis].

PURPOSE: Propioni bacterium Acnes may be isolated from corneal specimens. Its presence usually evokes a neighbouring tissue contamination. Nevertheless we have tried to find arguments in favour of a possible pathological responsibility of this microorganism in corneal affections. MATERIAL AND METHODS: A retrospective study was conducted in thirty patients with proved P. acnes corneal infection. The specimens were collected immediately during the first examination in the emergency ward. The research of anaerobic bacteria was made for each corneal specimen. RESULTS: We studied nineteen corneal abscesses and eleven corneal ulcers. Ninety-one percent of the cases presented an associated irritating factor. In decreasing order we found: wearing of soft contact lenses (36%), foreign corneal body still in situ or recently removed (21%), recurrent corneal erosions (9%), palpebral surgery (6%), herpetic keratitis (6%) or rosacea (3%). P. acnes was the only microorganism identified in 97% of the cases. Starting treatment before bacteriological results were known to be effective in each case. CONCLUSIONS: These elements are an argument for the pathological role of P. acnes in corneal infections when combined with favourable local conditions. Its presence does not always mean actual contamination.
